Abstract

Intra Uterine Fetal Death (IUFD) is one contributor to infant mortality. Human Leukocyte Antigen-E (HLA-E) and Natural Killer Cells (NK cells) are believed to play an important role towards IUFD associated with immune maladaptation. This study aimed to determine the expression of HLA-E and NK cells on trophoblast on IUFD and normal pregnancy. The study used cross sectional approach undertaken at Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, networking hospitals, and Anatomic Pathology Laboratory of Faculty of Medicine, Sebelas Maret University. The number of samples were 32 subjects consisting of 16 subjects with normal pregnancy and 16 subjects with IUFD. The expression of HLA-E and NK cells on trophoblast of subjects who met the inclusion and exclusion criteria were examined using immunohistochemistry method and t-test statistical analysis. The mean value of HLA-E expression in the trophoblast of IUFD group was 17.30±6.69, in normal pregnancy was 57.06±32.04, with p=0.00 (p<0.05). The mean value of NK cell expression in trophoblast in IUFD group was 78.62±36.43, in normal pregnancy was 19.87±6.43, with p=0.00 (p<0.05). This study concluded that the expression of HLA-E was lower and NK cells wass higher in IUFD compared to those in normal pregnancy.

Abstract

Cervical cancer is the most common cause of death among gynaecological neoplasms. Management of advanced cervical cancer currently has not been able to improve the prognosis. Standard intervention therapy, chemotherapy and radiotherapy, as well as the emerging pain could lead to psychological stress till depression, thus reduce patients' quality of life. CBT for psychological stress and depression is expected to improve the fruitfulness of standard therapy. Serotonin is a neurotransmitter which plays a role in the patophysiology of depression. The aim of this study is to analyze the effect of cognitive behavioral therapy on serotonin level, depression score and quality of life in advanced cervical cancer patient with experimental quasi pre and post test design methods. The samples are 15 subjects in both intervention and control groups. Intervention group was given CBT and standard therapy, while control group was given standard therapy only. Study was held in Department of Obstetric and Gynecologic Dr. Moewardi Hospital Surakarta and Prodia Laboratory, in January to March 2015. Independent variable was advanced cervical cancer patients underwent CBT intervention and dependent variables were serotonin level, depression score and quality of life score. The result, serotonin level and quality of life scores were higher after intervention compared with control, (219.43±33,42 vs 89.57±23.23) and (85.13±14.62 vs 41.86±7.24), respectively. Depression score was lower after intervention than without CBT intervention (11.20±4.94 vs 17.00±4.86) and statistically significant (p < 0.05). So, there were effects of cognitive behavioural therapy on serotonin level, depression score and quality of life score in cervical cancer patients and were statistically significant.

Abstract

Keselamatan pasien adalah isu global krusial, di mana 1 dari 10 pasien mengalami kejadian tidak diinginkan di rumah sakit, dan 50% di antaranya dapat dicegah melalui kepatuhan terhadap standar. Di Indonesia, Sasaran Keselamatan Pasien (SKP) menjadi indikator utama mutu pelayanan, namun kepatuhan perawat masih bervariasi. Tujuan: Menganalisis faktor pengetahuan, sikap, motivasi, budaya keselamatan, dukungan pimpinan, supervisi, ketersediaan fasilitas, dan beban kerja terhadap kepatuhan perawat dalam implementasi SKP. Penelitian kuantitatif dengan desain deskriptif analitik dan pendekatan cross-sectional. Sampel berjumlah 122 perawat dan dianalisis menggunakan uji Chi-Square. Faktor yang berhubungan signifikan (p=0,000) dengan kepatuhan adalah sikap, budaya keselamatan pasien, dukungan pimpinan, supervisi, dan ketersediaan fasilitas.Beban kerja ditemukan tidak berhubungan signifikan (p=0,276).Seluruh responden (100%) memiliki pengetahuan baik, namun kepatuhan hanya mencapai 50% kategori baik, menunjukkan adanya kesenjangan antara pengetahuan dan praktik. Kepatuhan perawat lebih dipengaruhi oleh faktor sistem dan lingkungan kerja dibandingkan sekadar aspek kognitif. Diperlukan penguatan budaya keselamatan, supervisi, dan dukungan organisasi untuk meningkatkan keselamatan pasien secara berkelanjutan. Kata Kunci : Kepatuhan Perawat, Sasaran Keselamatan Pasien

Abstract

HIGHLIGHTS First comprehensive synthesis of human and mechanistic evidence linking placental micro- and nanoplastic (MNP) contamination to preterm birth and neonatal outcomes. Quantitative human studies demonstrate higher placental MNP burdens in preterm versus term pregnancies. Molecular pathways identified include oxidative stress, ferroptosis-driven syncytiotrophoblast senescence, trophoblast invasion impairment, inflammatory signaling, endocrine disruption, and epigenetic modifications. Clinical and policy relevance: Findings support the need for standardized biomonitoring, maternal exposure mitigation, and the integration of eco-exposome considerations into perinatal care.   ABSTRACT Objective: To systematically review emerging evidence on micro- and nanoplastic (MNP) contamination of the human placenta, explore molecular pathways underlying placental dysfunction, and evaluate associations with preterm birth and neonatal outcomes. Materials and Methods: Following PRISMA 2020 guidelines, literature searches (in PubMed, Web of Science, and Scopus) and grey sources were conducted through July 2025. Inclusion criteria comprised studies detecting MNPs in the human placenta or fetal compartments, mechanistic experiments using human placental models, or reviews addressing pregnancy outcomes. Methodological quality was assessed using AMSTAR-2, ROBIS, or the Newcastle–Ottawa Scale. Data were synthesized into three evidence domains: human biomonitoring, molecular pathways, and clinical implications. Results: Twenty studies met the inclusion criteria. MNPs were consistently detected in the human placenta, amniotic fluid, cord blood, and meconium, with higher burdens in preterm versus term placentae. Mechanistic studies demonstrated oxidative stress, ferroptosis-mediated syncytiotrophoblast senescence, impaired trophoblast invasion, inflammatory responses (IL-6, TNF-a, NLRP3 activation), endocrine disruption (altered ß-hCG and progesterone signaling), and epigenetic modifications. These pathways converge to impair nutrient and oxygen exchange and immune tolerance, increasing the risks of preterm birth, fetal growth restriction, low birth weight, and neonatal respiratory and metabolic vulnerability. Conclusion: Micro- and nanoplastic contamination of the human placenta is increasingly documented and biologically plausible as a contributor to preterm birth and neonatal morbidity. These findings support urgent investigation of exposure mitigation, standardized biomonitoring, and the integration of eco-exposome risks into perinatal clinical practice and policy.

Abstract

This study aims to analyze trends in corruption cases and the characteristics of defendants based on the level of education in the Corruption Court at the Medan District Court Class 1A specifically in the period 2020-2024. The method used is a quantitative descriptive approach by utilizing secondary data in the form of case decisions obtained from The Case Tracking Information System (SIPP). The results showed that the trend of corruption cases has increased significantly over the past five years, with a total of 596 cases and 611 defendants. From a demographic aspect, the majority of defendants are men, while women are relatively fewer. Based on the level of education, most of the defendants have a higher education background, especially Bachelor (S1), followed by master (S2) and doctorate (S3). These findings indicate that corruption is not directly influenced by the level of education, but rather related to internal factors such as greed and abuse of power. Therefore, efforts to eradicate corruption not only require a repressive approach through law enforcement, but also a preventive approach through anti-corruption education and strengthening moral integrity.
