Abstract

Background: The maternal mortality rate is an indicator of national health status. To reduce the Maternal Mortality Rate (MMR), especially in Karanganyar Regency, the risk factors that affect maternal mortality must be known.Subjects and Method:  This was a case-control study conducted in Karanganyar, Central Java, from 2016 to 2017. A sample of 57 study subjects was selected by fixed disease sampling. The dependent variable was maternal mor­tality. The independent variables were pregnancy complica­tions, labor complications, post­partum complications, maternal age, a distance of pregnancy, parity, maternal nutritional status during pregnancy, number of antenatal care visits, delay in making decisions, late referrals, late handling of staff, maternal education, maternal work, and income family. Data collected using questionnaires were then analyzed using bivariate analysis and multivariate analysis.Results: Maternal mortality was affected by pregnancy complications (OR= 6.98; 95% CI= 2.06 to 23.71; p= 0.001), labor complication (OR= 7.59; 95% CI= 2.19 to 26.28; p= 0.001), delayed labor (OR = 5.39; 95% CI= 1.17 to 24.75; p= 0.021), and family income (OR= 4.29; 95% CI= 1.28 to 14.41; p = 0.015).Conclusion: Maternal mortality was affected by pregnancy complications, labor complication, delayed labor, and family income.Keywords: risk factors, maternal mortality, Karanganyar RegencyCorrespondence: Noferi Kusnadi. Abstract

Anti-angiogenic Soluble FMS-Like Tyrosine Kinase-1 (sFlt-1) and proangiogenic Endhothelial Vascular Growth Factor (VEGF) factors can be used as an early detection of preeclampsia due to itsrole in the pathogenesis of preeclampsia, so it can be used as one effort to reduce maternal or perinatal morbidity and mortality.Objective: To analyze sFlt-1 and VEGF levels in the serum of normal pregnancy and pregnancy with preeclampsia risk in 8 – 20 weeks gestation.Method: Observational analytic with cross sectional method performed at the Obstetrics and Gynecology, Dr. Moewardi Hospital/ Medical Faculty Sebelas Maret University Surakarta and Prodia laboratory since November – December 2013. Number of samples studied was 30 samples, comprising 15 samples of normal pregnancy and pregnancy with preeclampsia risk with 8 – 20 weeks of gestational age. The sFlt-1 and VEGF serum levels was assessed using ELISA and analyzed using t-test.Result & Discussion: Serum level of sFlt-1 in normal pregnancy is (1252,17±564,65 ng/ml), and in pregnancy with preeclampsia risk is (1741,90±640,97 ng/ml) with p=0,023 serum level of VEGF in normal pregnancy was 96,88±144,29 ng/ml and in pregnancy with preeclampsia risk was 14,24±8,73 ng/ ml with p=0,044.Conclusion: sFlt-1 level is higher and VEGF level is lower in pregnant women with preeclampsia risk than normal pregnancy on 8 – 20 weeks gestational age.Keywords: Pregnancy, Preeclampsia Risk, sFlt-1, VEGF.ABSTRAKLatar belakang: Preeklampsia saat ini masih merupakan masalah pada ibu hamil yang berhubungan dengan mortalitas dan morbiditas maternal dan perinatal. Faktor anti-angiogenik Soluble FMS-Like Tyrosine Kinase-1 (sFLt-1) dan proangiogenik Vascular Endhothelial Growth Factor (VEGF) diduga dapat digunakan sebagai deteksi dini karena perannya dalam patogenesis preeklampsia, sehingga dapat digunakan sebagai upaya untuk mengurangi angka morbiditas dan mortalitas maternal dan perinatal. Tujuan: Untuk menganalisis kadar sFlt-1 dan VEGF pada serum ibu hamil normal dan ibu hamil dengan risiko preeklampsia pada usia kehamilan 8 – 20 minggu.Metode: Penelitian observasional analitik dengan metode Cross Sectional yang dilakukan di bagian Obstetri dan Ginekologi, RSUD Dr. Moewardi/ FK UNS, Surakarta dan Laboratorium Prodia mulai Nopember – Desember 2013. Jumlah sampel terdiri 30 yang terbagi menjadi 15 sampel ibu hamil normal dan dan 15 sampel ibu hamil dengan risiko preeklampsia usia gestasi 8 – 20 minggu. Abstract

LatarBelakang: Kematian ibu di kabupaten Sukoharjo masih cukup tinggi. Hal ini disebabkan oleh beberapa faktor seperti 4 “terlalu” ,komplikasi kehamilan, persalinan dan nifas,terlambat mengambil keputusan, merujuk dan mendapat pelayanan kesehatanserta sosioal ekonomi yang rendah.Metode: Observasional analitik dengan case control study. Jumlah sampel 16 kasus dan 32 kontrol dengan teknik simple random sampling. Analisis dilakukan secara univariat, bivariat dengan chi square test, multivariat dengan metode regresi logistik.Hasil: Terdapat hubungan yang bermakna antara komplikasi kehamilan (p<0.034) (OR=4,200; 95% CI : 1,064 – 16,584), komplikasi persalinan (p<0.001) (OR=9,533; 95% CI : 2,397 – 37,909), komplikasi nifas (p<0.000), keterlambatan penanganan petugas (p=0,011),paritas (OR=0,035; 95% CI : 0,004 – 0,300; p=0,000) dan ibu bekerja (p=0,017)(OR=4.592; 95% CI : 1.257 – 16.771) terhadap kematian maternal. Dengan faktor  risiko di atas kemungkinan kematian maternal meningkat sebanayak 88,9 %.Kesimpulan: Komplikasi kehamilan, persalinan, nifas ,keterlambatan penanganan petugas, paritas dan ibu bekerja meningkatkan risiko kematian maternal

Abstract

Preeklampsia Berat (PEB) masih merupakan penyebab utama morbiditas dan mortalitas maternal maupun perinatal. Kejadian preeklampsia berkisar antara 4,4-17,5% dari ibu hamil. Human Leukocyte Antigen-C (HLA-C) dan Natural Killer cell (sel NK) diduga memegang peranan penting terhadap proses terjadinya preeklampsia. Penelitian ini bertujuan untuk mengetahui ekspresi HLA-C dan sel NK pada PEB, dengan menggunakan metode observasional analitik dan potong lintang. Penelitian dilakukan pada bulan Desember 2014-Januari 2015 di bagian Obstetri dan Ginekologi RSUD Dr. Moewardi Surakarta dan Laboratorium Patologi Anatomi Fakultas Kedokteran Universitas Sebelas Maret Surakarta. Jumlah sampel 40 yang telah memenuhi kriteria inklusi dan eksklusi dibagi menjadi 2 kelompok, yaitu 20 PEB dan 20 hamil normal. Semua sampel dilakukan pemeriksaan ekspresi HLA-C pada trofoblasnya dan sel NK pada desidua dengan metode imunohistokimia dan dilakukan analisis statistik dengan uji t. Rerata ekspresi HLA-C di trofoblas pada kelompok PEB : 15,39±3,45 dan kehamilan normal 8,07±3,70, dengan nilai  p=0,00 (p<0,05).  Rerata ekspresi sel NK di desidua pada PEB 14,78±3,57 dan kehamilan normal 8,48±3,35, dengan nilai p=0,00 (p<0,05). Simpulan, ekspresi HLA-C di trofoblas dan sel NK di desidua pada PEB lebih tinggi dibandingkan dengan kehamilan normal.

Abstract

Prematuritas menyebabkan mortalitas dan morbiditas neonatus yaitu cerebral palsy (CP), gangguan kognitif dan gangguan tingkah laku. Sitokin proinflamasi TNF-α dan IL-1β diduga secara signifikan meningkat pada bayi prematur. Magnesium sulfat (MgSO4) diduga dapat sebagai neuroprotektor terhadap otak janin, melindungi jaringan terhadap aktivitas radikal bebas, sebagai vasodilator dari vasculature otak dan menurunkan sitokin proinflamasi. Penelitian ini bertujuan untuk mengetahui peran MgSO4 terhadap kadar TNF-α dan IL-1β pada bayi prematur, menggunakan metode observasional analitik, cross sectional yang dilakukan pada bulan Januari-Desember 2015 di RSUD Dr. Moewardi Surakarta dan laboratorium Prodia Jakarta. Subjek penelitian berjumlah 40 yang memenuhi kriteria inklusi dan eksklusi dibagi menjadi 2 kelompok yaitu 20 subjek kelompok persalinan prematur yang mendapat MgSO4 dan 20 subjek kelompok persalinan prematur tidak mendapat MgSO4. Semua subjek dilakukan pemeriksaan kadar TNF-α dan IL-1β pada serum dari darah tali pusat bayi pada saat lahir dengan menggunakan metode ELISA. Data dianalisis menggunakan uji t. Rerata kadar serum TNF-α pada kelompok persalinan prematur yang diberikan MgSO4 (2,24±0,56ng/mL) dan rerata kadar serum TNF-α pada persalinan prematur yang tidak diberikan MgSO4 (2,80±0,85ng/mL) dengan p=0,01 (p&lt;0,05). Rerata kadar serum IL-1β pada kelompok persalinan prematur yang diberikan MgSO4 (0,49±0,22ng/mL) dan rerata kadar serum IL-1β pada persalinan prematur yang tidak diberikan MgSO4 (0,71±0,28ng/mL) dengan p=0,01 (p&lt;0,05). Magnesium Sulfat berperan menurunkan kadar serum TNF-α dan IL-1β pada bayi prematur.

Abstract

Pertumbuhan janin terhambat (PJT) merupakan salah satu penyebab utama tingginya morbiditas dan mortalitas perinatal. Maladaptasi imun berakibat gangguan invasi trofoblas dan remodeling arteri spiralis yang akan menyebabkan hipoksia pada jaringan plasenta. Penelitian ini bertujuan untuk menganalisis ekspresi human leukocyte antigen-G (HLA-G) dan heat-shock protein-7 (Hsp-70) pada trofoblas PJT dan hamil normal, dengan menggunakan metode observasional analitik dan pendekatan potong lintang. Penelitian dilakukan di Bagian Obstetri dan Ginekologi Rumah Sakit Umum Daerah Dr. Moewardi Surakarta, mulai bulan November‒Desember 2011. Jumlah sampel 30, terdiri atas 15 sampel trofoblas pada PJT dan 15 sampel trofoblas pada kehamilan normal. Pada semua sampel dilakukan pemeriksaan ekspresi HLA-G dan Hsp-70 dengan imunohistokimia. Analisis data menggunakan uji-t. Ekspresi HLA-G rata-rata pada kelompok PJT sebesar 32,42±8,90, sedangkan ekspresi HLA-G rata-rata pada kelompok kehamilan normal 43,92±14,91 (p=0,016). Ekspresi Hsp-70 pada kelompok PJT 2,4355+0,26647 dan kelompok kehamilan normal 1,5920+0,17142 (p=0,008). Simpulan, pada PJT ekspresi HLA-G lebih rendah dan ekspresi Hsp-70 lebih tinggi dibandingkan dengan kehamilan normal. Immune maladaptation affects trophoblast invasion and spiralis arteria remodeling that will cause placental tissue hypoxia. This research aimed to analyze human leukocyte antigen-G (HLA-G) and heat-shock protein-70 (Hsp-70) expression on the IUGR trophoblast and normal pregnancy, by applying analytical observational method and cross sectional approach. This research was conducted at the Obstetric and Gynecology Department of Dr. Moewardi Hospital Surakarta from November to December 2011. The total samples were 30, divided into two groups. There were 15 samples trophoblast on IUGR and 15 samples trophoblast from normal pregnancy. All samples were tested for HLA-G and Hsp-70 using immunohistochemistry. The data were analyzed by using t-test. The mean of HLA-G expression on the IUGR groups was 32.42±8.90 and on the normal pregnancy groups was 43.92±14.91 (p=0.016). Heat-shock protein70 expression on the IUGR groups was 2.4355+0.26647 and on the normal pregnancy groups was 1.5920+0.17142 with p=0.008. In conclusion, in IUGR, the HLA-G expression is lower and the Hsp-70 expression is higher than in normal pregnancy. Abstract

Background: Decreased endometrial recep­ti­vity is suspected to be one of the factors causing in­fertility in women with Polycystic Ovary Syn­dro­me (PCOS). Endometrial abnormalities in PC­OS­ due to impaired balance include estrogen, pro­­­­gesterone, growth factors, cytokines and cell at­tach­­­ment modulators including leptin. This study aims to determine the expression of leptin in the endometrium which is suspected to be more common in PCOS patients and related factors.Subjects and Method: A retrospective-case control study was conducted at Dr. Moe­wardi general hospital, Surakarta. A total of 60 women were involved; 30 infertile women with PCOS based on Rotterdam criteria (if found two of the three symptoms, including hyperandrogenism, ovu­la­tion dysfunction and/or polycystic ovaries from ultrasonography) and 30 normal women as con­trol group. Demographic data such as age, family history of PCOS, menarche, BMI, occupation, and edu­cation were recorded. Leptin expression was acquired from endometrium biopsy with IHC examination on day 19-24 of the menstrual cycle; under the condition that inclusion and exclusion cri­teria are fulfilled. The statistical analysis was per­formed using the Mann-Whitney test.Results: Mean of leptin expression was found significantly higher in PCOS group (Mean= 59.16; SD= 49.34) compared to control group (Mean= 6.00; SD= 22.98, with p= 0.001. PCOS (b= 0.42; 95% CI= 0.10 to 0.73; p= 0.010) and obesity (b= 0.31; 95% CI= 0.41 to 0.58; p= 0.025) increased leptin expression.Conclusion: There is a difference in leptin ex­pres­­sion in the endometrium of PCOS compared to nor­mal.  Leptin expression is higher in the en­do­metrium of women with PCOS and obesity compared to nor­mal.Keywords: Leptin expressions, endometrium, polycystic ovarian syndrome.Correspondence: Dwi Sakti Berlian Besar. Abstract

BACKGROUNDCoronavirus Infection 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is an emergency condition for global public health. Early detection of COVID-19 in pregnant women is needed. The neutrophil-to-lymphocyte ratio (NLR), as a marker of viral inflammatory response, is used to determine the presence of a viral or bacterial infection, both acute and chronic. The objective of this study was to determine the relationship between NLR and the polymerase chain reaction (PCR) swab test results in pregnant women with suspected coronavirus disease 2019.METHODSA cross-sectional study was conducted on 9 pregnant women with suspected COVID-19. The subjects were inpatients at Moewardi Hospital Surakarta from 19 April-19 May 2020, who had rapid tests, complete blood examinations, and PCR swab tests. The NLR was categorized based on early warning scores according to research developed at Zhejiang University, with cut-off point 5.8. Diagnosis of COVID-19 was confirmed by PCR swab tests. Relation between NLR   and PCR swab results was analyzed by the prevalence ratio.RESULTSTwo patients (22.2%) had NLR &gt;5.8, with positive swab results in both (100%). Seven patients with NLR &lt;5.8 had positive swabs in only one (33.3%). The relationship between NLR and PCR swab test results showed a prevalence ratio of 0.143 (95% CI 0.023-0.877).CONCLUSIONPregnant women with NLR &lt; 5.8 had a decreased risk of COVID-19. Routine blood examination is more suitable for finding pregnant women with suspected COVID-19.

Abstract

BackgroundThe polycystic ovary syndrome (PCOS) is caused by endocrine system dysfunction in women. MUCIN-1 (MUC-1) expression is found in endometrial tissues, which leads to implantation process dysfunction because of imbalance of trophoblast adhesion process. This study was conducted to compare endometrial MUC-1 expression between PCOS and normal women considering all existing external variables.MethodsThis cross-sectional study was conducted in General Hospital Dr. Moewardi Surakarta. Endometrial samples were obtained from 30 infertile PCOS women based on Rotterdam criteria, and 30 normal women. Life style and reproductive data such as age, menstrual problems, menstrual cycle, age at menarche, and BMI were collected. Subjects underwent endometrial biopsy in luteinizing hormone (LH) secretion phase LH + 5 days to LH + 10 days for immunohistochemistry (IHC) of MUC-1 expression. An independent-t and multiple linear regression test were used to analyze the data at significance level of p&lt;0.05. ResultsMean MUC-1 expression in the PCOS endometrium (49.66 ± 47.79) was significantly higher than in normal women (7.66 ± 14.55) (p=0.03). Multivariate linear regression model of life style and reproductive variables with MUC-1 showed that PCOS (b=29.54; 95% CI 9.57-49.49; p=0.004) and BMI (b= 29.99; 95% CI 5.91-54.07; p=0.001) significantly increase MUC-1 expression. PCOS (Beta=0.37) was more important than BMI (Beta=0.30) in increasing the MUC-1 expression. ConclusionExpression of MUC-1 levels in the PCOS endometrium was higher than in normal women. This suggests that MUC-1 contributes to the unexplained reproductive failure in PCOS.

Abstract

Preeklamsi berat merupakan penyebab kedua terbesar kematian ibu di seluruh dunia. Radikal bebas menstimulir kadar heat shock protein 90 (HSP90), diduga menentukan terjadinya preeklamsi berat. HSP90 merupakan protein penting untuk membantu pembentukan dan pemeliharaan protein lain serta untuk memacu kelangsungan hidup sel setelah terjadi berbagai macam keadaan patologis (fungsi chaperone). Fungsi chaperone merupakan faktor kunci adaptasi terhadap stres endogen di beberapa jaringan. Dengan mengetahui kadar HSP90, sebagai deteksi dini preeklamsi berat, maka dapat dilakukan upaya pencegahan dan penanganan secara dini. Penelitian ini bertujuan membuktikan bahwa kadar HSP90 pada kehamilan dengan preeklamsi berat lebih tinggi dibanding dengan kehamilan normal. Metode penelitian studi kuantitatif observasional pendekatan potong lintang dengan pendekatan uji laboratoris kadar HSP90. Studi dilakukan bulan September–November 2013 di Bagian Obstetri dan Ginekologi RS Dr. Moewardi Surakarta dan Prodia Jakarta. Subjek 30 pasien terdiri atas 15 pasien hamil dengan preeklamsi berat dan 15 pasien normal. Dilakukan pemeriksaan kadar HSP90 serum dengan metode enzyme-linked immunosorbent assay (ELISA). Data dianalisis menggunakan uji-t dengan SPSS versi 17.00 for Windows. Didapatkan kadar  HSP90 rata-rata kelompok preeklamsi berat sebesar 131,91±26,66, sedangkan kelompok kehamilan normal 80,28±13,39 dengan p=0,00 (p<0,05). Kadar HSP90 serum preeklamsi berat lebih tinggi dibanding dengan kehamilan normal karena pada preeklamsi berat terjadi stres oksidatif. Free radicals that stimulate heat shock protein 90 (HSP 90) are believed to determine severe pre-eclampsia. HSP90 is an important protein that helps the establishment and maintenance of other proteins. It also increases the life time of cells after various pathological conditions (chaperone function). The chaperone function is the adaptation key factor to endogenous stress in tissues. By recognizing HSP90 level in early detection of severe pre-eclampsia, prevention and management can be started early. This study aimed to prove that the HSP90 level in pregnancy with severe pre-eclampsia is higher than normal pregnancy. This was a quantitative study using cross sectional approach by testing the HSP90 level.  The study was conducted during the period of September to November 2013, at the Obstetrics and Gynecological Unit, Moewardi Hospital Surakarta and Prodia Laboratory Jakarta. The number of subjects was 30 patients, consisting of 15 normal pregnant mothers and 15 pregnant  mothers with pre-eclampsia . The calculation of serum HSP90 level was conducted using enzyme-linked immunosorbent assay (ELISA). Data were analyzed using t-test using SPSS for Windows version 17 for Windows. The mean of HSP90 in the severe pre-eclampsia group was 131.91±26.66 while the mean in the normal pregnancy group was 80.28±13.39 with p=0.00 (p<0.05). Level of HSP90 serum in severe pre-eclampsia is higher than in normal pregnancy, due to the occurrence of oxidative stress in severe pre-eclampsia.
