Abstract

Objective: analyzed the relationship between NGF mRNA expression and pain levels and severity of endometriosis. Methods: This research involved 30 endometriosis patients undergoing surgery from November 2024 to April 2025. NGF mRNA expression was measured in blood plasma and ectopic tissue. Pain history was assessed retrospectively, and disease stage determined intraoperatively. Results: No significant relationship was found between NGF mRNA expression and pain levels or severity of endometriosis in either plasma or lesion tissue. Based on the Livak method analysis, the average value of relative expression of NGF gene mRNA increased 6.943-fold in blood plasma and 17.319-fold in endometriosis lesions of women with endometriosis. In the Mann Whitney test, there was a significant difference in the level of NGF gene mRNA expression between blood plasma and with p < 0.0001. NGF expression may not always correlate with disease severity because it is influenced by many factors. Conclusion: No significant correlation was found between NGF mRNA expression and endometriosis severity or pain intensity, but expression levels differed significantly between plasma and lesion tissue.

Abstract

Breast cancer is the leading cause of cancer-related death in women globally. Progesterone receptor (PR) is known as the prime example of receptors amenable to targeted breast cancer drug therapy. Etlingera elatior is an herbal plant that has been renowned to have anticancer effect. This study aimed to identify the potential compounds derived from Etlingera elatior as anticancer agents of PR in breast cancer using molecular docking method. This study used fifteen compounds from Etlingera elatior along with lonaprisan as the comparative drug. The PR was downloaded from RCSB, whereas compounds and lonaprisan were from Pubchem. The drug-likeness test based on Lipinski’s rule of five was conducted using SwissADME. Toxicity analysis using admetSAR 2.0 was used to predict toxicological profile of the compounds. Compounds and lonaprisan were docked on PR using AutoDock tools 1.5.6 and AutoDock Vina 1.1.2. Molecular interactions were visualized by Discovery Studio v16. A total of nine compounds met the criteria as drugs based on drug-likeness and toxicity tests. All nine compounds except caffeic acid and vanillic acid had higher binding affinities on PR compared with lonaprisan. Ergosterol peroxide exhibited the highest binding affinity on PR with values of -9.8 kcal/mol. Moreover, ergosterol peroxide-PR interaction had thirteen hydrophobic bonds and a hydrogen bond with amino acid residues were found in the active site of PR. Most of the compounds found in Etlingera elatior have the potential to be anticancer agents of PR in breast cancer with ergosterol peroxide being the most potential compound. Further in vitro and in vivo research are needed. Keywords: breast cancer, ergosterol peroxide, Etlingera elatior, progesterone receptor, in silico.

Abstract

Leadership is a familiar topic of discussion. In this era of globalization, leadership has increasingly become a subject of human attention. Numerous pieces of literature on leadership can be found, and seminars, workshops, and meetings discussing leadership are held everywhere. This shows that today, people place significant emphasis on leadership. Leadership is a determining factor in the success or failure of achieving goals in any community. The church, as a community, also recognizes the importance of leadership in achieving its objectives. The accomplishment of goals is closely tied to the role of a leader. Therefore, specific qualifications are needed for a Christian leader. One essential qualification for a Christian leader is spiritual qualification. This journal discusses the spiritual qualifications of a Christian leader, drawing implications from the Book of Nehemiah. One of the key factors contributing to Nehemiah's success in leadership was his spirituality.

Abstract

The use of IT-based media in education systems has continuously risen, and the digital era has provided a great opportunity to integrate any media in the process of learning. This research will present an assessment of the role of optimized IT media in CRE at SMP N 1 Ledo. A qualitative approach with the case study method is used to gain depth into how IT media influences the learning process in this particular context. The findings of the case study reveal that IT media implementation increased both student activity and enhanced learning achievements of students in the subject of CRE. Furthermore, IT-based media make learning more interactive and fun for students; in this way, students can manage to retain concentration and sustain motivation throughout the entire learning process. Secondly, it has been indicated that IT media has to be duly developed and maintained if it is ever going to serve its full purpose in the classroom. If no support is given from time to time and technical developments are not made, the effectiveness of IT media might be rendered gradually useless. It also confirms that teachers need sufficient training and support so that they would feel confident in their contribution to students via the integration of technology into teaching. Capability of easily combining traditional approaches with the aid of digital tools generally increases the level of learning. The results of this research then demonstrate that IT media is capable of enhancing the effectiveness of Christian Religious Education, especially in making learning more interactive and fun for students. They also indicate the necessity for continuous professional development of teachers themselves so that teachers are able to keep up with the times and make use of these facilities appropriately in teaching

Abstract

Introduction: Rapid bedside risk stratification of burn-injured patients informs intensive-care admission, fluid-resuscitation intensity, and timing of early surgical excision and grafting. The Modified Abbreviated Burn Severity Index (mABSI) and the Revised Baux (R-Baux) score are widely cited but have not previously been compared head-to-head in a Sumatran tertiary burn unit. Methods: STARD-aligned retrospective diagnostic-accuracy study of consecutive thermal-burn patients admitted to the Burn Unit, Dr. Mohammad Hoesin General Hospital Palembang (January–December 2025). Patients with concurrent major trauma or incomplete records were excluded. Both scores were calculated from medical records by an investigator blinded to the outcome. In-hospital mortality was the reference standard. Receiver-operating-characteristic, Wilson-CI sensitivity/specificity, DeLong AUC comparison, McNemar paired test, and Cohen's κ analyses were performed in SPSS v25 (α = 0.05). Results: Of 45 included patients (mean age 40.8 years; 77.8% male; 64.4% fire burns), 20 (44.4%) died. Optimal cutoffs were mABSI ≥ 5 (AUC 0.811, 95% CI 0.688–0.934; sensitivity 72.0%, specificity 75.0%, Youden 0.47) and R-Baux > 60 (AUC 0.808, 95% CI 0.678–0.938; sensitivity 52.0%, specificity 82.0%, Youden 0.34). Inter-test agreement was substantial (κ = 0.69, 95% CI 0.48–0.90; p < 0.001), with overall accuracy of 84.4% when R-Baux was treated as the comparator. Conclusion: Both scores discriminated burn mortality well; the Modified ABSI offered higher sensitivity and Youden index, supporting its evaluation as a primary triage tool in similar Indonesian tertiary burn units, while the Revised Baux retained complementary specificity for confirmatory risk classification.

Abstract

Introduction: Computed tomography (CT) classification anchors surgical triage in traumatic brain injury (TBI), but contemporary Indonesian data on the prognostic performance of the Marshall CT score in surgically managed patients are scarce. Methods: An observational retrospective cohort study was conducted at Dr. Mohammad Hoesin Central General Hospital, Palembang. Consecutive adult surgical TBI patients operated on between January 2023 and December 2025 were enrolled. Marshall CT categories were assigned from non-contrast head CT scans by two independent raters (κ = 0.87). The primary outcome was in-hospital mortality. We quantified the association between Marshall score and in-hospital mortality using Spearman rank correlation, point-biserial correlation, penalised L2 logistic regression, and ROC analysis. Results: Thirty-six patients met the inclusion criteria (75.0% male; median age 25 years). In-hospital mortality was 19.4% (7/36; 95% CI 9.8–35.0%). Spearman correlation between Marshall score and mortality was ρ = 0.532 (95% CI 0.245–0.731, p = 0.001). Marshall category was an independent predictor in penalised logistic regression (adjusted OR 3.12, 95% CI 1.63–4.41; p = 0.013). ROC AUC was 0.852 (95% CI 0.724–0.960), numerically superior to admission GCS AUC of 0.778 (DeLong p = 0.298). Conclusion: Marshall CT classification is an independent, point-of-CT predictor of in-hospital mortality after surgery for TBI in an Indonesian tertiary cohort. Integration into structured operative triage may enhance early risk stratification.

Abstract

Introduction: Stage III breast carcinoma dominates surgical-oncology practice in Indonesian referral hospitals. Tumour-secreted PTHrP and dysregulated calcium signalling link tumour mass to systemic calcium, yet routinely available serum calcium is rarely quantified as a preoperative surrogate of tumour burden. Methods: In this cross-sectional study, 35 women with Stage III breast carcinoma at Dr. Mohammad Hoesin General Hospital Palembang underwent preoperative serum calcium measurement. Tumour size was dichotomised (≤5 cm vs >5 cm). Associations were tested by Spearman correlation and the Mann–Whitney U test; serum calcium was assessed as a classifier of large tumours by ROC analysis, with multivariable logistic regression, effect sizes and 95% confidence intervals (CI). Results: Mean serum calcium was 9.34 ± 0.82 mg/dL, and 27 patients (77.1%) had tumours >5 cm. Calcium was higher in tumours >5 cm (9.58 ± 0.76 mg/dL; 95% CI 9.28–9.88) than ≤5 cm (8.54 ± 0.37 mg/dL; 95% CI 8.23–8.85), a difference of 1.04 mg/dL (95% CI 0.66–1.43; Cohen d = 1.50; Mann–Whitney U = 0.000; p < 0.001; r = 0.72). Calcium correlated with size (Spearman rho = 0.731; p < 0.001) and discriminated tumours >5 cm (area under the curve 1.000; cut-off 8.95 mg/dL; sensitivity and specificity 100%). Immunohistochemical subtype was the only independent predictor (adjusted odds ratio 71.37; 95% CI 2.95–1728; p = 0.009). Conclusion: Preoperative serum calcium rose in proportion to tumour size in Stage III breast carcinoma, acting as a low-cost surrogate of tumour burden that may aid risk stratification in resource-limited centres, pending validation.
