Abstract

Background: Atopic dermatitis (AD), also known as Eczema, is an inflammatory skin disease that causes dry, itchy skin. several risk factors for AD have been reported including demographics, socioeconomics, active and passive smoking, urbanization, diet, breastfeeding and timing of introduction of solid foods, obesity, physical exercise, fast food, and environmental air pollution. This study aims to better understand the effect of smoking, obesity, and fast food consumption as one of the risk factors of AD. Subjects and Method: Systematic review with the help of PICOs to search for eligible articles. Articles were searched from 3 different databases (PubMed, ScienceDirect, and Google Scholar) with the help of a search engine application, Publish or Perish (PoP), using the following keyword: “smoking AND atopic dermatitis”, “fast food AND atopic dermatitis”, “obesity AND atopic dermatitis”. Results: 10 articles matched the specified inclusion and exclusion criteria for this study. Whether or not smoking causes AD is still conflicting, few studies reported there is no correlation between smoking and AD, whereas other studies said otherwise. In contrast to that, second-hand smoke was found to be affecting AD conditions more (up to 2.320). Most of our eligible articles reported that obesity did cause severe reactions of AD up to 1.62 times. Frequent fast food consumption was also found to be causing severe AD conditions (OR=1.580). Conclusion: Obesity and fast food consumption are one of the causes of AD development and AD severity. The risk factor of cigarette smoking still conflicts with whether or not it affects AD conditions.

Abstract

Background: The COVID-19 pandemic provides a unique opportunity to reinvigorate efforts to achieve the Triple Zero HIV goal by 2030-zero new cases, zero deaths from AIDS, and zero stigma. This scoping review explores technology, including social media and big data, in increasing HIV literacy and community engagement. It is crucial to prioritize the Triple Zero HIV targets, which involve the eradication of new HIV infections, AIDS-related deaths, and the stigma associated with HIV, amidst the COVID-19 epidemic. This study aimed to rigorously investigate and identify new technology-driven therapeutics to eliminate HIV, AIDS-related fatalities, and social discrimination by 2030. Subjects and Method: A systematic review was conducted by searching articles from PubMed and ResearchGate is an article published from 2015 to present. The keywords were “HIV education technology” and “community engagement.” Articles covering technology-based HIV literacy and community engagement were selected, and other databases were searched extensively for this scoping review. The search included "HIV education technology” “Community engagement” and "Big data in HIV prevention”. The articles were selected by PRISMA flow diagram method. Results: A total of 8 studies was included in this review. This study revealed innovative approaches to HIV education through technology, including mobile applications and social media campaigns. These tools provide easily accessible information, reducing stigma and discrimination. Big data and predictive analytics can target interventions effectively. The post-COVID-19 era allows for community mobilization through social media campaigns and virtual support groups, fostering solidarity and access to care. According to our analysis, mobile apps and social media campaigns are novel HIV education methods. These tools make knowledge accessible, decreasing stigma and discrimination. Big data and predictive analytics target interventions. Community mobilization through social media campaigns and virtual support groups promotes solidarity and care post-COVID-19. Conclusion: Technology, heightened health awareness, and community engagement can drive progress toward the Triple Zero goal of HIV by 2030, creating a world with no new cases, no AIDS deaths, and no stigma. This opportunity arises in the wake of the COVID-19 pandemic, which offers new avenues for HIV prevention and care.

Abstract

Introduction: Pulmonary tuberculosis (TB) remains one of the most significant global health challenges, especially complicated by cases with incomplete treatment histories. The recurrence of TB poses diagnostic and therapeutic dilemmas that significantly impact patient outcomes. Case Presentation: We report a case of a 57-year-old female patient presenting with recurrent pulmonary TB after an incomplete treatment course. The patient's history revealed gaps in adherence to the prescribed TB regimen, leading to multiple relapses. In this study, we found that patients undergoing TB treatment may experience liver damage as a result of the TB medication, also known as DIH. Comprehensive diagnostic procedures, including chest radiography, sputum culture, and molecular testing, confirmed recurrent TB. The patient's clinical manifestations, treatment plan, and response to therapy are detailed. Conclusion: Recurrent TB is common, particularly in older adults, and is influenced more by environmental factors than occupation. Additionally, ATT poses risks of hepatotoxicity, especially in malnourished patients, underscoring the importance of managing side effects and addressing factors like malnutrition to prevent complications and improve outcomes. This case underscores the importance of complete adherence to TB treatment regimens and highlights the challenges in managing recurrent TB. It emphasizes the need for robust follow-up, patient education strategies, early diagnosis, and prompt intervention to prevent recurrence and enhance patient outcomes.

Abstract

Diabetes melitus merupakan penyakit kronik yang berdampak terhadap kualitas hidup pasien. Penelitian ini bertujuan untuk mengukur kualitas hidup dan faktor-faktor yang memengaruhinya pada pasien DM tipe 2. Penelitian ini merupakan penelitian observasional analitik dengan desain cross-sectional. Penelitian ini dilakukan di Puskesmas Mulyorejo dan Puskesmas Keputih. Pengukuran kualitas hidup pasien DM tipe 2 dilakukan dengan menggunakan kuesioner short form-36 (SF-36). Analisis multivariat digunakan untuk menganalisis faktor-faktor yang memengaruhi kualitas hidup pasien DM tipe 2. Penelitian ini diperoleh 56 pasien DM tipe 2 yang memenuhi kriteria inklusi. Diantara 56 pasien tersebut, jenis kelamin perempuan lebih banyak ditemukan dengan rerata usia keseluruhan ialah 56,4 tahun. Secara umum, skor rerata kualitas hidup pasien DM tipe 2 tergolong baik, yaitu 60,5 ± 9,25. Berdasarkan uji multivariat, didapatkan hasil bahwa usia ≥ 56 tahun menjadi satu-satunya variabel yang secara berpengaruh terhadap kualitas hidup (P-value 0,011; aOR 5,614; CI 95% 1,216–12,314). Pasien DM tipe 2 dengan usia ≥ 56 tahun berpengaruh terhadap kualitas hidup pasien DM Tipe 2.

Abstract

ABSTRACT Background: Knee osteoarthritis (OA) affects mainly older individuals, causing joint degeneration and decreased mobility. While current treatments include medications and surgery, regenerative therapies like Platelet Rich Plasma (PRP) and Hypertonic Dextrose Prolotherapy (HDP) offer promising alternatives. This study aims to compare the effectiveness of PRP and HDP injections in knee OA patients, providing insights into optimal treatment choices. Methods: This experimental analytical study involved two groups of participants. Group 1 (N=20) received PRP injections three times into both knees with four-week intervals. Group 2 (N=20) received HDP injections with 20% concentration into both knees with the same intervals. Numeric Rating Scale (NRS) and Western Ontario and McMaster University Arthritis Index (WOMAC) scores were assessed before and after injections to evaluate changes in pain, stiffness, and functional limitation. Results: Both PRP and HDP injections significantly reduced NRS scores, pain, stiffness, functional limitation, and total WOMAC scores. The reduction in pain, functional limitation, and total WOMAC scores was higher with HDP, while PRP was more effective in reducing stiffness. Mild pain during injection and knee swelling were reported by some participants, but no systemic complaints were noted, and all side effects resolved within a week. Conclusion: Both PRP and HDP injections are equally effective in reducing pain and stiffness and improving function in knee OA patients. These procedures offer promising, cost-effective, and easily administered therapies for knee OA, with HDP being a more cost-efficient option compared to PRP. Abstract

The increasing emergence of zoonotic diseases originating from vectors, rodents, mammals, and others has increased the potential for outbreaks of pandemics in the Indonesian territory. Although control and prevention have been implemented, these efforts have not yet revealed a bright spot; therefore, elaboration with advanced CRISPR-Cas9 technology is a way to accelerate efforts to control zoonotic diseases in Indonesia. However, there is limited literature on this topic. This review aims to comprehensively describe, identify, and summarize the application of CRISPR-Cas9 genome-editing technology in zoonotic disease control in Indonesia. Our findings show that CRISPR-Cas9 genome editing technology offers an innovative approach for zoonotic disease control by targeting disease vectors, modifying animal reservoirs, improving disease surveillance, enhancing vaccine development, and exploring traditional medicine candidates and immunotherapy. The high level of precision, efficiency, and versatility in targeting genomes capable of disrupting, damaging, and disrupting the disease transmission cycle in pathogens makes CRISPR-Cas9 highly effective. However, challenges such as off-target impacts, regulatory complexity, and ethical considerations must be overcome with inter- and multidisciplinary collaboration to promote transparency, equity, and public engagement throughout the process of implementing this technology in the field, especially in Indonesia.

Abstract

Background: Menstrual cycle disorders are common problems among adolescent girls and are associated with reproductive health issues. Factors such as Body Mass Index (BMI) and stress are believed to influence menstrual irregularities. This study aims to examine the relationship between BMI, stress, and menstrual cycle irregularities as a preventive measure to maintain optimal menstrual health. Method: This observational analytical study utilized a non-probability sampling technique. Data analysis was conducted using Spearman’s Rho Correlation test and Cross Tabulation test at a significance level of 0.05. Results: The analysis showed a significant positive correlation between BMI and menstrual irregularities (p = 0.010, r = 0.258) and between stress and menstrual irregularities (p = 0.001, r = 0.480). Stress exhibited a stronger association with menstrual irregularities than BMI. Conclusion: This study highlights the relationship between BMI and stress with menstrual irregularities in female adolescents at SMA Hang Tuah 2 Sidoarjo. These findings emphasize the need for targeted interventions addressing nutritional and psychological support to improve menstrual health. Monitoring BMI and managing stress are essential strategies to enhance reproductive health in adolescents. Further research is recommended to explore effective interventions to address these factors and improve adolescent well-being.

Abstract

Systemic immune-inflammation index (SII) and prognostic nutritional index (PNI) serve as simple and practical tests that help indicate inflammatory and nutritional status to some extent. Lung cancer stands out as the most common contributor to cancer-related mortality globally. It is associated with an unfavorable prognosis. Most patients diagnosed with lung cancer have metastasis at the time of diagnosis. Prognostic heterogeneity of cancer patients causes the need for more prognostic biomarkers. This study aimed to evaluate the clinical value of SII and PNI in predicting metastasis in lung cancer patients. SII and PNI provides a prognostic value in lung cancer. Retrospective cross-sectional research was conducted in this study involving 138 data from medical records at the Inpatient and Outpatient Department of Pulmonology, RSPAL dr. Ramelan Surabaya in April 2019 to July 2023. Kolmogorov-Smirnov test, contingency coefficient test, and ROC analysis were done to analyze the data obtained. Patients with metastatic lung cancer had higher SII than those without metastasis. The group of patients with metastasis had an average SII of 5391.34 and a PNI of 40.11. The group of patients without metastasis had an average SII of 2849.52 and PNI of 43.05. Lung cancer metastasis was correlated significantly with SII but not with PNI. The cut-off value was determined using the ROC curve. The cut-off value for SII was 2198.54 (68.5% sensitivity and 58.7% specificity) and for PNI was 42.2 (62% sensitivity and 54.3% specificity). SII was correlated with lung cancer metastasis and may be a promising indicator predicting of metastasis. PNI showed no significant correlation with lung cancer metastasis.

Abstract

Seborrheic Dermatitis (DS) is a chronic skin disease that often occurs in areas rich in sebaceous glands, characterized by erythematous patches, plaques, and scales. Psychological factors, especially stress, will affect sebum production and colonization of Malassezia spp being the main cause of seborrheic dermatitis. Stress can increase skin inflammation and worsen DS symptoms. The severity of DS was assessed by the Seborrheic Dermatitis Area Severity Index (SDASI), while stress was measured by the Perceived Stress Scale (PSS). PSS is designed to measure the level of stress in everyday life. SDASI scale used to assess the severity of seborrheic dermatitis. The purpose of the study was to analyze the relationship between the level of stress measured using the Perceived Stress Scale (PSS) with the severity of seborrheic dermatitis evaluated through the Seborrheic Dermatitis Area Severity Index (SDASI). The study involved 51 patients who met the inclusion and exclusion criteria. Data collection was conducted through PSS questionnaires totaling 14 questions and SDASI assessment to measure the severity of DS. The results showed that most patients experienced very severe stress (88.2%) and the severity of severe SDASI (74.5%). The correlation between PSS and SDASI showed a p value of 0.010 (p < 0.05), which showed significant results between the level of stress with the severity of DS with correlation efficiency of 0.359..Stress can worsen the condition of seborrheic dermatitis in which an increase in PSS values is obtained.

Abstract

Background: Tuberculosis (TB) remains among the top ten global causes of mortality, with approximately 1.3 million deaths annually. Diabetes elevates the risk of active TB and treatment failure, potentially increasing drug-resistant TB (DR-TB). This study aimed to compare treatment success rates between TB patients with and without diabetes mellitus (DM) at Dr. Ramelan Central Naval Hospital, Surabaya.Subjects and Method: This cross-sectional study was conducted from January 2019 to December 2023 at Dr. Ramelan Central Naval Hospital Surabaya. A total of 158 patients with TB-DM and TB-NonDM were selected using total sampling. The independent variables were the Presence of Diabetes Mellitus in TB patients (TB-DM vs. Non-TB-DM). The dependent variable was the treatment success rate. The data were collected from patient medical records and analyzed using a chi-square test to compare treatment outcomes between TB-DM and TB-Non-DM patients.Results: The analysis included 158 medical records. Predominantly affecting those over 45 years, both TB-DM and TB-Non-DM patients commonly underwent six months of treatment, with success rates of 78% in TB-DM and 82.4% in TB-Non-DM cases. The chi-square test yielded a p-value of 0.511, indicating no significant difference in treatment success between the groups. However, older age and HIV-positive status were associated with lower odds of treatment success.Conclusion: Success rates were similar between the groups, showing no significant difference based on DM status. Despite similar success rates, older age and HIV-positive status were associated with lower odds of treatment success.
