Abstract

Introduction: The Cleft Lip and Palate (CLP) is a congenital malformation involving the orofacial region. The failure of frontonasal and maxillary processes to fuse causes a gap through the lips, alveoli, base of the nose, and in the hard and/or soft palate. Case Presentation: This case report illustrates a 2-year-3-month-old female patient diagnosed with bilateral cleft lip and palate at birth. The patient underwent cheilorrhaphy at 4 months old and currently plans to have scar revision and columellar lengthening surgery at Dr. Ramelan Naval Hospital, Surabaya. The patient presents with a flat nose, a hypertrophic scar above the lip, and speech articulation impairment. Her medical history includes prenatal maternal hypertension, neonatal pneumonia, and exposure to cigar smoke. Additionally, there is a family history of cleft lip and palate. This case highlights the vital role of columellar lengthening in enhancing nasal appearance and airway patency for optimal outcomes without complications. Conclusion: Cleft lip and palate (CLP) require multidisciplinary care. Columellar lengthening and scar revision have been proven effective in enhancing nasal functions and aesthetics. The presence of prenatal maternal hypertension, exposure to cigar smoke, and familial history of CLP accentuates etiological complexity. The case shows successful intervention balancing function, aesthetics, and craniofacial growth in CLP repair. This study aims to increase insight for future management concerning nasal deformities in CLP patients.

Abstract

Background: Hemangiomas are the most common vascular tumors in infants and children, they typically appear after birth, grow rapidly, and then gradually involute. Early diagnosis is crucial, but the clinical presentation of hemangiomas can resemble other vascular anomalies, necessi­tating the use of imaging studies like ultrasonography (US) in order to establish accurate diag­nosis. This study aims to present the ultrasonographic characteristics of a rare case of vulvar hemangioma, with the objective of aiding in its differentiation from other vulvar vascular malformations. Case Presentation: A 9-year-old girl presented with a painless vulvar mass that had been enlarging over 3 months. Physical examination revealed a 3 x 2.5 x 1.5 cm, well-defined, skin-colored, non-tender, and immobile mass. Greyscale ultrasonography showed a septated cystic lesion with internal echoes. Doppler ultrasonography showed low flow vascular patterns, suggestive of a hemangioma.  Surgical excision was performed, and histopathology confirmed the diagnosis of a  hemangioma Results: Although vulvar hemangioma is rare, it can mimic other vascular anomalies, making imaging essential for accurate diagnosis and management. Ultrasonography, as a inexpensive and non-invasive imaging modality, plays a crucial role in differentiating hemangiomas from other vascular malformations. Conclusion: Hemangioma is a form of vascular mass and the most common one, at early phase the clinical presentation might resemble other vascular anomalies. Thereby diagnostic modalities such as ultrasonography might be able to help to establish the diagnosis

Abstract

Alzheimer's Indonesia (ALZI) reports a significant and growing prevalence of Orang Dengan Demensia Dementia (ODD), which is projected to reach 2 to 4 million individuals by 2050. Dementia is often attributed to cognitive deficits, prompting this study to investigate and compare the Community Mini Mental State Examination (MMSE) scores among elderly professionals, specifically fishermen and non-fishermen, residing in Socah Village, Bangkalan, East Java. Objective: This research aims to elucidate the extent of cognitive impairment in this demographic. Method: An analytical observational study employed a purposive sampling approach, with a total sample size of 96 individuals. The age categories ranged from 45 to 89 years, adhering to the Badan Pusat Statistik 2015 guidelines. Data collection took place during August to September 2022. Results: The results of the Mann-Whitney's test indicate a significant difference in Mini Mental State Examination (MMSE) scores between elderly fishermen and non-fishermen in Socah Village, with a p-value of 0.001 (p < α, α = 0.05). Conclusion: This finding leads to the conclusion that there is a notable disparity in MMSE scores among elderly professionals, highlighting the substantial impact of physical activity on cognitive function.

Abstract

The increasing emergence of zoonotic diseases originating from vectors, rodents, mammals, and others has increased the potential for outbreaks of pandemics in the Indonesian territory. Although control and prevention have been implemented, these efforts have not yet revealed a bright spot; therefore, elaboration with advanced CRISPR-Cas9 technology is a way to accelerate efforts to control zoonotic diseases in Indonesia. However, there is limited literature on this topic. This review aims to comprehensively describe, identify, and summarize the application of CRISPR-Cas9 genome-editing technology in zoonotic disease control in Indonesia. Our findings show that CRISPR-Cas9 genome editing technology offers an innovative approach for zoonotic disease control by targeting disease vectors, modifying animal reservoirs, improving disease surveillance, enhancing vaccine development, and exploring traditional medicine candidates and immunotherapy. The high level of precision, efficiency, and versatility in targeting genomes capable of disrupting, damaging, and disrupting the disease transmission cycle in pathogens makes CRISPR-Cas9 highly effective. However, challenges such as off-target impacts, regulatory complexity, and ethical considerations must be overcome with inter- and multidisciplinary collaboration to promote transparency, equity, and public engagement throughout the process of implementing this technology in the field, especially in Indonesia.

Abstract

Nephrotic Syndrome is a glomerular disease characterized by edema, massive proteinuria >3.5 grams/day, hypoalbuminemia <3.5 grams/day, hypercholesterolemia, and lipiduria. Nephrotic syndrome has a variety of metabolic effects that impact the individual. Some episodes of nephrotic syndrome are self-limited, and some respond to specific therapy, while others are chronic. A 49 year old female patient complained of frequent sweats at night and chills in the morning. Then, the patient complained that his legs had been swollen since he left the hospital until now. Apart from that, edema also occurs in the lower extremities. The management given is general management such as rest, low cholesterol, low salt, fluid restriction <1500ml, and a protein diet. Treatment of edema with diuretics, treatment of proteinuria, treatment of hypertension and history of stroke, as well as causal treatment according to the etiology of nephrotic syndrome. The patient has undergone management planning and treatment therapy for nephrotic syndrome.

Abstract

Zoonotic neglected tropical diseases (zNTDs) pose significant global public health challenges, particularly in low-income and middle-income countries. Traditional siloed approaches to zNTD management have proven inadequate for addressing the complex interplay between human, animal, and environmental health factors. This review examined the limitations of fragmented strategies and explored the potential of integrated one-health approaches for more effective zNTD control. This narrative review covered publications from 2000 to 2023, sourced from databases including PubMed and Scopus, emphasizing studies that met inclusion criteria of peer-reviewed research focused on zNTDs through a One Health lens. A comprehensive analysis of the current literature and case studies was conducted to evaluate the multifaceted drivers of zoonotic pathogen spillover, assess the economic and health impacts of zNTDs, and identify successful implementation of the One Health strategy in endemic regions. This review highlights the inadequacies of siloed disease management approaches, which often lead to inefficient resource allocation and missed opportunities for synergistic intervention. Successful case studies have demonstrated the effectiveness of the One Health strategy in improving disease surveillance, enhancing cross-sectoral collaboration, and achieving sustainable health outcomes. The potential of emerging technologies and innovations to support integrated approaches has also been explored. Transitioning from siloed to systemic approaches in zNTD management requires overcoming institutional barriers, fostering cross-sector collaboration, and implementing multitiered frameworks that operate at the local, national, and global levels. Investing in cross-sectoral capacity development and leveraging technological innovations are crucial for building a workforce capable of addressing the complex challenges posed by zNTDs. Adopting the One Health principle offers a promising path toward more comprehensive, efficient, and sustainable strategies for zNTD prevention.

Abstract

In the wake of the COVID-19 pandemic, the world has become acutely aware of the devastating impact of infectious diseases on public health, economies, and societies. As we gradually recover from the aftermath of COVID-19, another potential threat looms on the horizon, mpox, formerly known as monkeypox. The recent surge in mpox cases worldwide has prompted health authorities to sound an alarm, and Indonesia, as a populous nation with a complex healthcare system, must ask itself: Are we prepared for this emerging challenge?. The question "Are we prepared?" may not have a simple yes or no answer. Instead, it should serve as a reminder of the need for vigilance, adaptation, and continuous improvement in public health systems. As we navigate this new health challenge, the true measure of Indonesia's preparedness is its ability to respond swiftly, effectively, and equitably to protect the health and well-being of all citizens.

Abstract

Highlight: The potential role of hyperbaric oxygen therapy (HBO) as an adjunctive treatment for autism spectrum disorder (ASD), demonstrating significant improvements in clinical symptoms. By exploring the plausible mechanisms of HBO, the manuscript underscores the importance of further research and clinical trials to validate its efficacy and safety in the ASD population. This study adds to the literature advocating for innovative therapeutic strategies that address the complex interplay of oxidative stress, neuroinflammation, and mitochondrial dysfunction, inviting a personalized treatment approach.   ABSTRACT Introduction:  Autism spectrum disorder (ASD) is a multifaceted neurodevelopmental condition influenced by both genetic predispositions and environmental factors. Its underlying pathophysiology commonly involves oxidative stress, impaired cerebral perfusion, neuroinflammation, immune dysregulation, and mitochondrial dysfunction. Although conventional interventions, such as applied behavior analysis (ABA), are commonly employed, they may yield limited progress for some individuals. Consequently, emerging interventions, such as hyperbaric oxygen therapy (HBO), show promise for enhancing neurological outcomes and warrant further exploration. HBO, a well-established treatment for decompression sickness, is currently being investigated for its potential benefits in various neurological disorders, including ASD. Case: We present a case of ASD in a 12-year-old boy characterized by minimal eye contact, limited expressive language, and restricted, repetitive behaviors. On neurological examination, the patient was alert and capable of spontaneous eye opening, vocalizing incomprehensible words, and, at times, responding to simple commands. Despite undergoing ABA therapy, the patient exhibited limited developmental progress. The patient was subsequently undergoing hyperbaric oxygen therapy (HBO) at 1.3 ATA for 60 minutes for 10 days. Following the completion of HBO therapy, there was a decline in the CARS score from 41.5 (severe) to 29 (mild), the ATEC score from 115 to 68, the platelet count (51,000 / μL), and interleukin-1β levels (181.4 pg/mL). Conclusion: This case highlights the potential of HBO therapy as an adjunctive treatment modality for ASD, offering significant symptomatic improvements in a patient for whom traditional therapies were insufficient. The presumed mechanisms underlying these effects include anti-inflammatory effects and enhanced cerebral oxygen delivery, which may facilitate neuronal recovery and function. Given these findings, further research and clinical trials are warranted to confirm the therapeutic efficacy, long-term benefits, and safety profile of HBO therapy.

Abstract

Rhinosinusitis adalah suatu keadaan dimana terjadinya infeksi dan peradangan pada sinus mukosa paranasal, dimana setidaknya terjadi 12 minggu. Rhinosinusitis sendiri, merupakan salah satu masalah kesehatan dengan prevalensi yang besar di dunia. Penelitian ini dilakukan untuk mengetahui karakteristik pada pasien rhinosinusitis kronis yang ada di poli THT RSPAL dr. Ramelan Surabaya Periode Januari 2020 – Juli 2022. Penelitian deskriptrif ini dilakukan dengan desain cross-sectional dengan metode kuantitatif. Pengambilan data menggunakan data sekunder rekam medis dan kemudian dilakukan pencatatan untuk mengetahui karakteristik pada pasien. Pemilihan sampel menggunakan metode purposive sampling. Hasil penelitian yang dilakukan pada 30 pasien didapatkan jenis kelamin terbenyak adalah perempuan dengan jumlah 23 pasien (76,66%), kelompok usia terbanyak adalah 41-50 tahun dengan jumlah 11 pasien (36,7%), gejala terbanyak adalah hidung tersumbat dengan jumlah 24 pasien (80%), hasil pemeriksaan terbanyak adalah konka hiperemi dan udem dengan jumlah 23 pasien (76,66%), dan untuk tindakan manajemen terbanyak adalah medikamentosa dan tindakan operatif dengan jumlah 16 pasien (53,33%).

Abstract

Stunting is a prolonged state of chronic malnutrition. This disorder is characterized by having a body length-for-age or body height-for-age measurement that falls below the -2 standard deviation established by the World Health Organization (WHO). The etiology of stunting is multifaceted, encompassing factors such as nutrition, feeding practices, infectious illnesses, and household income. Maternal understanding of nutrition, including dietary factors, is also associated with the occurrence of stunting. This study aimed to establish the relationship between feeding habits and maternal knowledge of nutrition with the prevalence of stunting in the operational region of the Gedongan Health Center, located in Mojokerto City. This study is an observational analytical study employing a cross-sectional design. The study’s population consisted of all women with children residing in the working area of the Gedongan Health Center. This study encompassed 144 participants who completed the Child Feeding Questionnaire (CFQ) regarding eating practices and a questionnaire assessing maternal knowledge of nutrition. The objective was to examine the relationship between these factors to height and age. The chi-square test results indicate a statistically significant association between feeding practices and stunting (p = 0.0001) and maternal knowledge about nutrition and stunting (p = 0.0001). The survey revealed that most children had been provided with appropriate feeding patterns, and most moms had a commendable nutritional understanding. The study’s findings indicate a noteworthy relationship between feeding behaviors and mothers’ nutrition knowledge.
