Abstract

Highlights: No relationship between the frequency of fast-food consumption as a risk factor for obesity in elementary school children. This result was based on interviews with students of Rangkah VII Elementary School during their daily life activities and their tendencies. This result was based on interviews with students of Rangkah VII Elementary School during their daily life activities and their tendencies.   Abstract Introduction: Obesity is defined as the result of excessive fat accumulation caused by an energy imbalance, and it poses significant health risks. The contributing factors to be considered in this instance are the frequent consumption of fast food and reduced physical activity, which result from the increased use of gadgets and games. This study aimed to investigate the correlation between the aforementioned factors and the prevalence of obesity among elementary school children. Methods: The heights and weights of 112 participants were measured to calculate their body mass index (BMI). The respondents were interviewed directly about their fast-food consumption habits over a one-week period and their daily use of gadgets and games. This study employed bivariate analysis to examine the relationships between variables in a sample, using the Chi-Square test with odds ratio determination, a 95% confidence interval, and a significance level of 0.05, as implemented in the International Business Machines Corporation (IBM) Statistical Package for the Social Sciences (SPSS) version 29. Results: The chi-square test showed p>0.05, indicating no significant relationship between fast-food consumption or gadget use and obesity in elementary school children. Conclusion: There was no association between the frequency of fast-food consumption and the habit of playing gadgets and games as a risk factor for obesity in elementary school children.

Abstract

Background: The Tenggilis Public Health Center (Puskesmas Tenggilis) is devoted to implementing Community Health Efforts (UKM) to ensure the provision of high-quality healthcare services, focusing on the measurement, monitoring, and supervision of local health problems. A Self-Assessment Survey (Survey Mawas Diri” (SMD) in Indonesian language) was conducted in Panjang Jiwo Village, Surabaya, revealing hyperten”sion as a primary health issue. Characterized by a systolic blood pressure ?140 mmHg and a diastolic pressure ?90 mmHg, hypertension remains widespread, with many residents unaware of the necessity for lifelong medication adherence and routine blood pressure monitoring. Methods: To address this health challenge, a thorough analysis was conducted, examining the root causes through the "man, method, machine, and material" framework. This approach informed the development of a targeted intervention strategy. Results:The intervention involved designing and distributing a Hypertension Calendar, providing an accessible means for residents to record their blood pressure readings and track medication adherence throughout the year. The calendar serves as a useful tool during healthcare visits, fostering enhanced dialogue between patients and healthcare providers regarding hypertension management. Conclusions: The introduction of the Hypertension Calendar aims to empower individuals with essential knowledge and tools for effective hypertension management, ultimately contributing to improved health outcomes in the Panjang Jiwo community. Abstract

Introduction: Congenital Rubella Syndrome (CRS) is characterized by congenital cataracts, congenital heart disease (CHD), hearing loss, and developmental delay. It is caused by maternal rubella infection during pregnancy, transmitted transplacentally or via respiratory droplets. CRS carries a high risk of morbidity and mortality, with approximately 10–20% of affected infants dying within the first year of life. This case report describes a 15-year-old boy with CRS who developed pulmonary hypertension (PH) due to a persistent patent ductus arteriosus (PDA). Case Description: A 15-year-old boy presented with progressive abdominal distension over one week. Initially suspected of having nephritic syndrome, further evaluation revealed bilateral congenital cataracts, non-cyanotic CHD in the form of PDA, sensorineural hearing loss, and developmental delay, fulfilling criteria for CRS. The patient also exhibited delayed motor milestones (walking at age seven) and limb rigidity suggestive of cerebral palsy (CP). Echocardiography confirmed PDA (0.4 cm) with severe tricuspid and aortic regurgitation, and chest X-ray demonstrated cardiomegaly with PH. The PDA was successfully closed using an Amplatzer Duct Occluder (ADO) via catheterization. Conclusion: This case underscores the importance of early diagnosis and intervention in CRS patients with PDA to prevent irreversible pulmonary vascular disease. Despite a very late PDA closure at age 15, the patient achieved hemodynamic improvement and maintained functional capacity, highlighting that catheter-based closure remains feasible and beneficial even in adolescence. Multidisciplinary care, including timely cardiac intervention and neurodevelopmental support, can improve quality of life in CRS survivors.

Abstract

Background: The Intrauterine device (IUD) is one of the most widely used and effective long-term contraceptive methods, with a success rate exceeding 99%. However, prolonged copper IUD use has been associated with an increased risk of bacterial vaginosis (BV), with users reportedly 1.28 times more likely to develop BV than users of other contraceptive methods.This study aims to assess the association between IUD usage and the incidence of BV.  Methods: This cross-sectional analytic observational study included 50 participants using IUDs, recruited from three public health centers in Surabaya between May and December 2023. Vaginal swab samples from suspected BV cases were examined independently by two observers. Data were analyzed using the Chi-square test to assess associations and Cronbach's Alpha to evaluate inter-observer reliability, with significance set at p < 0.05.  Results: Most BV cases were found among women aged 30–39 years (40%) and among users of non-hormonal IUDs (76%). Based on Nugent scoring, 26 participants (52%) were diagnosed with definitive BV. The association between IUD use and BV incidence was not statistically significant (p = 0.090). Inter-observer reliability was high, with a Cronbach’s Alpha of 0.963. Conclusion: The study concludes that IUD use is not significantly associated with the occurrence of bacterial vaginosis, based on Nugent score evaluation.

Abstract

Introduction: The Cleft Lip and Palate (CLP) is a congenital malformation involving the orofacial region. The failure of frontonasal and maxillary processes to fuse causes a gap through the lips, alveoli, base of the nose, and in the hard and/or soft palate. Case Presentation: This case report illustrates a 2-year-3-month-old female patient diagnosed with bilateral cleft lip and palate at birth. The patient underwent cheilorrhaphy at 4 months old and currently plans to have scar revision and columellar lengthening surgery at Dr. Ramelan Naval Hospital, Surabaya. The patient presents with a flat nose, a hypertrophic scar above the lip, and speech articulation impairment. Her medical history includes prenatal maternal hypertension, neonatal pneumonia, and exposure to cigar smoke. Additionally, there is a family history of cleft lip and palate. This case highlights the vital role of columellar lengthening in enhancing nasal appearance and airway patency for optimal outcomes without complications. Conclusion: Cleft lip and palate (CLP) require multidisciplinary care. Columellar lengthening and scar revision have been proven effective in enhancing nasal functions and aesthetics. The presence of prenatal maternal hypertension, exposure to cigar smoke, and familial history of CLP accentuates etiological complexity. The case shows successful intervention balancing function, aesthetics, and craniofacial growth in CLP repair. This study aims to increase insight for future management concerning nasal deformities in CLP patients.

Abstract

Background: Hemangiomas are the most common vascular tumors in infants and children, they typically appear after birth, grow rapidly, and then gradually involute. Early diagnosis is crucial, but the clinical presentation of hemangiomas can resemble other vascular anomalies, necessi­tating the use of imaging studies like ultrasonography (US) in order to establish accurate diag­nosis. This study aims to present the ultrasonographic characteristics of a rare case of vulvar hemangioma, with the objective of aiding in its differentiation from other vulvar vascular malformations. Case Presentation: A 9-year-old girl presented with a painless vulvar mass that had been enlarging over 3 months. Physical examination revealed a 3 x 2.5 x 1.5 cm, well-defined, skin-colored, non-tender, and immobile mass. Greyscale ultrasonography showed a septated cystic lesion with internal echoes. Doppler ultrasonography showed low flow vascular patterns, suggestive of a hemangioma.  Surgical excision was performed, and histopathology confirmed the diagnosis of a  hemangioma Results: Although vulvar hemangioma is rare, it can mimic other vascular anomalies, making imaging essential for accurate diagnosis and management. Ultrasonography, as a inexpensive and non-invasive imaging modality, plays a crucial role in differentiating hemangiomas from other vascular malformations. Conclusion: Hemangioma is a form of vascular mass and the most common one, at early phase the clinical presentation might resemble other vascular anomalies. Thereby diagnostic modalities such as ultrasonography might be able to help to establish the diagnosis

Abstract

Alzheimer's Indonesia (ALZI) reports a significant and growing prevalence of Orang Dengan Demensia Dementia (ODD), which is projected to reach 2 to 4 million individuals by 2050. Dementia is often attributed to cognitive deficits, prompting this study to investigate and compare the Community Mini Mental State Examination (MMSE) scores among elderly professionals, specifically fishermen and non-fishermen, residing in Socah Village, Bangkalan, East Java. Objective: This research aims to elucidate the extent of cognitive impairment in this demographic. Method: An analytical observational study employed a purposive sampling approach, with a total sample size of 96 individuals. The age categories ranged from 45 to 89 years, adhering to the Badan Pusat Statistik 2015 guidelines. Data collection took place during August to September 2022. Results: The results of the Mann-Whitney's test indicate a significant difference in Mini Mental State Examination (MMSE) scores between elderly fishermen and non-fishermen in Socah Village, with a p-value of 0.001 (p < α, α = 0.05). Conclusion: This finding leads to the conclusion that there is a notable disparity in MMSE scores among elderly professionals, highlighting the substantial impact of physical activity on cognitive function.

Abstract

Background: Tuberculosis (TB) remains among the top ten global causes of mortality, with approximately 1.3 million deaths annually. Diabetes elevates the risk of active TB and treatment failure, potentially increasing drug-resistant TB (DR-TB). This study aimed to compare treatment success rates between TB patients with and without diabetes mellitus (DM) at Dr. Ramelan Central Naval Hospital, Surabaya.Subjects and Method: This cross-sectional study was conducted from January 2019 to December 2023 at Dr. Ramelan Central Naval Hospital Surabaya. A total of 158 patients with TB-DM and TB-NonDM were selected using total sampling. The independent variables were the Presence of Diabetes Mellitus in TB patients (TB-DM vs. Non-TB-DM). The dependent variable was the treatment success rate. The data were collected from patient medical records and analyzed using a chi-square test to compare treatment outcomes between TB-DM and TB-Non-DM patients.Results: The analysis included 158 medical records. Predominantly affecting those over 45 years, both TB-DM and TB-Non-DM patients commonly underwent six months of treatment, with success rates of 78% in TB-DM and 82.4% in TB-Non-DM cases. The chi-square test yielded a p-value of 0.511, indicating no significant difference in treatment success between the groups. However, older age and HIV-positive status were associated with lower odds of treatment success.Conclusion: Success rates were similar between the groups, showing no significant difference based on DM status. Despite similar success rates, older age and HIV-positive status were associated with lower odds of treatment success.

Abstract

Background: The COVID-19 pandemic provides a unique opportunity to reinvigorate efforts to achieve the Triple Zero HIV goal by 2030-zero new cases, zero deaths from AIDS, and zero stigma. This scoping review explores technology, including social media and big data, in increasing HIV literacy and community engagement. It is crucial to prioritize the Triple Zero HIV targets, which involve the eradication of new HIV infections, AIDS-related deaths, and the stigma associated with HIV, amidst the COVID-19 epidemic. This study aimed to rigorously investigate and identify new technology-driven therapeutics to eliminate HIV, AIDS-related fatalities, and social discrimination by 2030. Subjects and Method: A systematic review was conducted by searching articles from PubMed and ResearchGate is an article published from 2015 to present. The keywords were “HIV education technology” and “community engagement.” Articles covering technology-based HIV literacy and community engagement were selected, and other databases were searched extensively for this scoping review. The search included "HIV education technology” “Community engagement” and "Big data in HIV prevention”. The articles were selected by PRISMA flow diagram method. Results: A total of 8 studies was included in this review. This study revealed innovative approaches to HIV education through technology, including mobile applications and social media campaigns. These tools provide easily accessible information, reducing stigma and discrimination. Big data and predictive analytics can target interventions effectively. The post-COVID-19 era allows for community mobilization through social media campaigns and virtual support groups, fostering solidarity and access to care. According to our analysis, mobile apps and social media campaigns are novel HIV education methods. These tools make knowledge accessible, decreasing stigma and discrimination. Big data and predictive analytics target interventions. Community mobilization through social media campaigns and virtual support groups promotes solidarity and care post-COVID-19. Conclusion: Technology, heightened health awareness, and community engagement can drive progress toward the Triple Zero goal of HIV by 2030, creating a world with no new cases, no AIDS deaths, and no stigma. This opportunity arises in the wake of the COVID-19 pandemic, which offers new avenues for HIV prevention and care.

Abstract

The increasing emergence of zoonotic diseases originating from vectors, rodents, mammals, and others has increased the potential for outbreaks of pandemics in the Indonesian territory. Although control and prevention have been implemented, these efforts have not yet revealed a bright spot; therefore, elaboration with advanced CRISPR-Cas9 technology is a way to accelerate efforts to control zoonotic diseases in Indonesia. However, there is limited literature on this topic. This review aims to comprehensively describe, identify, and summarize the application of CRISPR-Cas9 genome-editing technology in zoonotic disease control in Indonesia. Our findings show that CRISPR-Cas9 genome editing technology offers an innovative approach for zoonotic disease control by targeting disease vectors, modifying animal reservoirs, improving disease surveillance, enhancing vaccine development, and exploring traditional medicine candidates and immunotherapy. The high level of precision, efficiency, and versatility in targeting genomes capable of disrupting, damaging, and disrupting the disease transmission cycle in pathogens makes CRISPR-Cas9 highly effective. However, challenges such as off-target impacts, regulatory complexity, and ethical considerations must be overcome with inter- and multidisciplinary collaboration to promote transparency, equity, and public engagement throughout the process of implementing this technology in the field, especially in Indonesia.
