Abstract

Diabetic Foot Care is primary prevention in diabetic foot management to prevent injury to the occurrence of wounds on the feet of people with diabetes mellitus. Objective: To prove that educational interventions can prevent chronic complications, namely diabetic neuropathy (motor, sensory and autonomic) or death of the nerves in the feet, often exacerbated by infection. Method: This study used the Quasi Experiment method with the One Group pretest and posttest design method . Data collection was carried out using NAFF (Nottingham assessment of Functional Footcare) and the Attitude instrument modified by the Knowledge, Attitude and Practice (KAP) questionnaire. Determination of the sample using the accidental sample method of 24 people in Type 2 Diabetes Mellitus patients. Results: This study showed an increase in value of 11.12 points on NAFF and an increase of 1.41 points on attitudes after educational interventions related to diabetic foot care were carried out from May 30, 2024 to June 7, 2024. Conclusion: Diabetic Foot Care Education Intervention has been proven effective in reducing complications in diabetic feet and can be used as an evidence-based intervention in preventive care of diabetic feet in patients with Diabetes Mellitus.

Abstract

Diabetic retinopathy (DR) is a leading microvascular complication of diabetes mellitus (DM), primarily caused by chronic hyperglycemia. It leads to vascular occlusion, increased vascular permeability, retinal abnormalities, and detachment, ultimately resulting in visual impairment and blindness. Implementing Evidence Based Nursing Practice using diagnostic tests to determine the sensitivity and specificity of the assessment instrument for Diabetic Retinopathy Risk Factors in Type II DM Diagnostic tests are descriptive observational studies with a cross-sectional study design. This application is classified as descriptive observational because only observations are made without any intervention (treatment). The test is carried out with ROC where to see the sensitivity and specificity of the application instrument. The results of the analysis showed that the sensitivity of the application of the assessment of diabetic retinopathy risk factors using the ROC Curve Test was 100%. Meanwhile, the specificity of the instrument for the application of the assessment of diabetic retinopathy risk factors was 81.3%. The application of diabetic retinopathy risk factor assessment can be used to assess the risk of diabetic retinopathy because it has good sensitivity (%) and specificity (%) values. However, when applying the diabetic retinopathy risk factor assessment, only the risk value of diabetic retinopathy was obtained where the application patient did not experience complaints and decreased visual acuity.

Abstract

Nausea and vomiting are common side effects experienced by breast cancer patients undergoing chemotherapy. Complementary therapies, such as peppermint and ginger aromatherapy, have been widely studied as alternatives to relieve these symptoms. However, the effectiveness of each type of aromatherapy needs further evaluation. Objective: This study aims to analyze the effectiveness of peppermint and ginger aromatherapy in reducing nausea and vomiting in breast cancer patients undergoing chemotherapy. Methods: A Quasi-Experimental design was used with 28 breast cancer patients at Husada Hospital Jakarta, divided into two groups: peppermint and ginger. Nausea and vomiting levels were measured before, 2 hours, and 6 hours after chemotherapy and analyzed using ANOVA Repeated Measures and Independent T-Test. Results: The results showed that both peppermint and ginger aromatherapy significantly reduced nausea and vomiting after chemotherapy (p < 0.05). However, at 6 hours after chemotherapy, the peppermint group had lower average nausea and vomiting scores compared to the ginger group (6.57 vs. 10.29, p = 0.036). This indicates that peppermint is more effective than ginger in reducing nausea and vomiting after chemotherapy. Conclusion: Both peppermint and ginger aromatherapy are effective in reducing nausea and vomiting in breast cancer patients undergoing chemotherapy. However, peppermint showed higher effectiveness compared to ginger at the 6-hour post-chemotherapy time point. Future research could explore the combination of peppermint and ginger aromatherapy to enhance effectiveness in reducing nausea and vomiting in breast cancer patients.

Abstract

Acute exacerbation in chronic obstructive pulmonary disease (COPD) is characterized by increased production and accumulation of mucus which aggravates clinical symptoms. One of interventions well-known effectively overcoming airway clearance problem is active cycle of breathing technique. The aim of implementing evidence-based nursing practice was to determine the effectiveness of the active cycle of breathing technique on sputum production in COPD patients. The search for evidence was carried out by PICO (problem, intervention, comparison, and outcome) analysis. 18 COPD inpatients and outpatients were selected based on inclusion and exclusion criteria and were given the intervention. Sputum production was measured before and after it. The data analysis was carried out using Wilcoxon with a p value of 0.014 which showed that there was a difference in the amount of sputum production before and after the intervention. The researchers concluded that active cycle of breathing technique is an effective intervention in overcoming airway clearance problems in COPD patients.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a chronic lung disease with progressive, irreversible and partially reversible airway obstruction. COPD includes chronic bronchitis and emphysema, or a combination. COPD patients generally complain of continuous shortness of breath and this will result in a decrease in the patient's quality of life and as a result the patient will go back and forth to the hospital. To prevent this, it is necessary to do breathing exercises. The aim of implementing EBNP is to implement Progressive Muscle Relaxation (PMR) and Deep Breathing Exercises (DBE) interventions to change the shortness of breath response in COPD patients in the form of a randomized controlled trial with a pre-post-test design. The sampling technique uses G-Power version 3.1.9.7 with T-test, one tail, effect size 0.53, alpha error 0.05, and power 0.80, a minimum of 18 is required plus a possibility of error of 10%, so the number of patients needed in this study is 20 at Persahabatan Hospital, East Jakarta. The implementation of PMR actions with 13 (thirteen) movements and DBE with 6 (six) movements is carried out every day for 5 weeks, from May 15 to June 16 2023. Method: The study design in this research is a case study involving 40 respondents. Respondents were divided into two groups, namely 20 people in the PMR group and 20 people in the DBE group. Evaluation is carried out every week using the dyspnoe-12 instrument. Results: This application shows that there is a difference in the response to shortness of breath between before and after the PMR procedure (P value 0.001) and the DBE procedure (P value = 0.001). Conclusion: The application of EBNP recommends the need for nurses to carry out PMR and DBE interventions to change the shortness of breath response in COPD patients.

Abstract

HIV infection continues to be a significant public health problem worldwide, including Indonesia, with impacts spanning social, economic, and cultural aspects. People living with HIV (PLHIV) are faced with various pressures and are in dire need of coping strategies. This study aimed to determine the relationship between self-efficacy and spiritual well-being with the use of coping strategies in PLHIV. This study is an analytic study with a cross sectional approach. A sample of 39 ODHIV was selected using purposive sampling method. Data were collected by filling out the GSES questionnaire to assess self-efficacy, the SWBS questionnaire to measure spiritual well-being, and the BRIEF RCOPE questionnaire to determine coping strategies. Logistic regression multinomial inferencial test found no association between self-efficacy and spiritual well-being with the use of coping strategies. Most were male (76.9%), high school education level (64.1%), and married (59%). Descriptive analysis obtained respondents who used problem-oriented coping strategies (28.2%); emotion-oriented (38.5%); and religious-oriented (33.3%). Multinomial logistic regression test showed no significant association between self-efficacy and the use of coping strategies and no association between spiritual well-being and coping strategies (p-value > 0.05). Spiritual aspects are important to consider as they can be a source of coping and motivation to help patients cope with sudden illness and reduce the level of pain due to the perceived condition so as to quickly move towards healing. Nurses need to apply nursing care using coping instruments, comprehensively from the assessment process to the evaluation process.

Abstract

Aims: This study was to determine the prevalence of metabolic syndrome and its associated factors among nurses in Banten. Methods: The cross-sectional design of this particular type of quantitative correlation study. A total of 102 respondents were comprised of samples selected using random sampling. Variabel aktivitas fisik diidentifikasi menggunakan APARQ (Adolescent Physical Activity Recall Questionnaire) dan Stres diidentifikasi menggunakan DASS 21. Metabolic indicators include hypertension, Obesity, cholesterol, and diabetes. Data analysis used logistic regression.   Result:  The prevalence of metabolic syndrome among nurses was 16.7%. Older age (p=0.006), smoking (p=0.008), and stress (p=0.000) were significantly associated with metabolic syndrome. While gender and exercise were not significantly associated with metabolic syndrome.  Conclusion: smoking and stress were the major modifiable risk factors of metabolic syndrome among nurses. Campaigns and prevention of healthy behavior are crucial even among healthcare professionals.

Abstract

Background: Diabetic neuropathy is a complication that develops slowly and often goes undiagnosed. Guttormsen explained that neuropathy usually appears after years of diabetes and is often the initial cause of wounds and amputations. Therefore, early detection is crucial to prevent further complications. One recommended approach is non-invasive screening, which allows for early identification before severe symptoms appear. Purpose: To evaluate the effectiveness of non-invasive screening for early detection of foot disorders in people with diabetes. Method: The implementation of evidence-based nursing practice (EBNP) at Al Ihsan Regional Hospital, West Java Province, from June to July 2025, involved a non-invasive screening intervention for each patient for early detection of foot disorders in diabetic patients. This study was preceded by a systematic review and meta-analysis that compiled data from various previous studies. Results: Participants with the ROC curve test showed screening results on BDRFS with a sensitivity value of 1.000; on monofilament screening the sensitivity value was 0.545; vibration screening test sensitivity value was 0.818; temperature screening test sensitivity value was 0.864. In the QST screening test of touch sensation, it showed that the temperature screening test sensitivity value was 0.909. The QST screening test of vibration sensation showed that the temperature screening test sensitivity value was 0.909. The QST screening test of temperature sensation showed that the temperature screening test sensitivity value was 0.773. Conclusion: The implementation of EBNP “Evaluation of the Effectiveness of Non-Invasive Screening for Early Detection of Foot Disorders in Diabetics” has been proven to significantly increase early detection of diabetic neuropathy through a method that is safe, inexpensive, and can be performed by nurses.   Keywords: Diabetes; Early Detection of Foot Disorders; Non-Invasive Screening.   Pendahuluan: Neuropati diabetik merupakan komplikasi yang berkembang secara perlahan dan sering kali tidak terdiagnosis secara dini. Guttormsen menjelaskan bahwa neuropati biasanya muncul setelah bertahun-tahun menderita DM dan sering kali menjadi penyebab awal terjadinya luka hingga amputasi. Untuk itu, deteksi dini sangat penting guna mencegah komplikasi lebih lanjut. Salah satu pendekatan yang direkomendasikan adalah skrining non-invasif, yang memungkinkan identifikasi dini sebelum muncul gejala berat. Tujuan: Untuk mengevaluasi efektivitas skrining non-​​invasif untuk deteksi dini gangguan kaki pada diabetisi. Metode: Penerapan praktik keperawatan berbasis bukti (Evidence Based Nursing Practice/EBNP) yang dilaksanakan di RSUD Al Ihsan Provinsi Jawa Barat pada bulan Juni-Juli 2025. Setiap pasien dilakukan intervensi berupa pemeriksaan skrining non-invasif untuk deteksi dini gangguan kaki pada pasien diabetes. Hasil: Partisipan dengan uji ROC curve menunjukkan hasil skrining pada BDRFS dengan nilai sensitivity 1.000; pada skrining monofilamin nilai sensitivit 0.545; skrining vibrasi test nilai sensitivity 0.818; skrining temperatur test nilai sensitivit 0.864. Pada uji skrining QST sensasi sentuhan menunjukkan bahwa skrining temperatur test nilai sensitivit 0.909. Uji skrining QST sensasi getaran menunjukkan bahwa skrining temperatur test nilai sensitivit 0.909. Uji skrining QST sensasi suhu menunjukkan bahwa skrining temperatur test nilai sensitivit 0.773. Simpulan: Penerapan EBNP “evaluasi efektivitas skrining non-invasif untuk deteksi dini gangguan kaki pada diabetisi” terbukti dapat meningkatkan deteksi dini neuropati diabetik secara signifikan melalui metode yang aman, murah, dan dapat dilakukan oleh perawat.   Kata Kunci: Diabetisi; Deteksi Dini Gangguan Kaki; Skrining Non-​​Invasif.

Abstract

Background: Chest tube placement in post-thoracostomy patients often causes pain, which impacts the healing process. Pharmacological therapies such as analgesics are commonly used but have limitations. Therefore, non-pharmacological interventions such as Breathing Relaxation Exercise (BRE) are needed to help reduce pain. Purpose: To determine the effect of breathing relaxation exercises on pain reduction during chest tube placement and thoracotomy. Method: A pre-experimental one-group pretest-posttest design with a sample size of 13 participants determined using the G*Power calculation. The pain measurement instrument used was the Visual Analogue Scale (VAS). Data analysis was performed using the Shapiro-Wilk normality test and a paired sample t-test. Results: Before BRE, most patients experienced severe pain (69.2%). After the intervention, the pain scale decreased to moderate (38.5%) and mild (46.2%) pain categories. The paired sample t-test results showed a significant difference (p<0.05), with an average pain reduction of 1.615. Conclusion: There was a significant decrease in the pain scale before BRE was implemented.   Keywords: Breathing Relaxation Exercise; Chest Tube; Evidence Based Nursing In Practice; Painful; Thoracotomy.   Pendahuluan: Pemasangan chest tube pada pasien pasca torakostomi sering menimbulkan nyeri yang berdampak pada proses penyembuhan. Terapi farmakologis seperti analgetik umum digunakan, namun memiliki keterbatasan. Oleh karena itu, diperlukan intervensi nonfarmakologis seperti Breathing Relaxation Exercise (BRE) untuk membantu menurunkan nyeri. Tujuan: Mengetahui pengaruh penerapan breating relaxation exercise untuk mengurangi nyeri selama pemasangan chest tube dan thoracotomy. Metode: Desain pre-eksperimental one group pretest-posttest dengan jumlah sampel 13 partisipan yang ditentukan melalui perhitungan G*Power. Instrumen pengukuran nyeri menggunakan Visual Analogue Scale (VAS). Analisis data dilakukan dengan uji normalitas Shapiro-Wilk dan uji Paired Sample t-Test. Hasil: Sebelum diberikan BRE, sebagian besar pasien mengalami nyeri berat (69.2%). Setelah intervensi, skala nyeri menurun menjadi kategori nyeri sedang (38.5%) dan ringan (46.2%). Hasil uji Paired Sample t-Test menunjukkan terdapat perbedaan bermakna (p<0.05), dengan rata-rata penurunan nyeri sebesar 1.615. Abstract

Introduction: Diabetes mellitus (DM) is one of the major non-communicable diseases with a steadily increasing prevalence in Indonesia, including in island regions with limited access to healthcare services. This community service program was conducted on Lancang Island, Kepulauan Seribu. Objective: The purpose of this service was of improving elderly participants’ knowledge of diabetes management, conducting early detection through health screening, and providing education on healthy lifestyle practices. Method: The methods included interactive health education sessions and physical examinations covering random blood glucose, blood pressure, and cholesterol levels. Result: The results showed that the majority of the 101 elderly respondents exhibited significant chronic health risk factors. The average blood pressure was 152/88 mmHg, indicating a tendency toward hypertension. The mean random blood glucose level of 140 mg/dL suggested a predisposition to prediabetes and diabetes mellitus, while the mean cholesterol level reached 253 mg/dL, categorized as high. Conclusion These findings indicate that most elderly participants have not yet implemented optimal dietary management and physical activity, further exacerbated by limited health facilities and access in island areas. The educational intervention effectively increased participants awareness of disease risks and complications, as well as the importance of early detection. Overall, the promotive and preventive activities had a positive impact on improving knowledge, awareness, and motivation among elderly participants to adopt healthy lifestyles. This program is expected to serve as a foundation for sustainable community health development efforts in island regions.
