Abstract

This study aims to determine the magnitude of the effect of pursed-lip breathing (PLB) exercises on increasing the highest flow achieved by maximal expiration peak expiratory flow (APE) in COPD patients at Jakarta Islamic Hospital Cempaka Putih based on the latest research results (evidence-based Nursing Practice). This study uses a quantitative method with a type of quantitative experimental research with a quasi-experimental approach. The results showed that the results of APE measurements before and after PLB breathing exercises to male COPD sufferers at the Jakarta Islamic Hospital showed that, in general, the effects of APE before and after PLB breathing exercises on Day-1 were 278.3 liters or minutes and general after PLB Day-1 breathing exercise was 290.0 greater. From the results of the T-Test experiment, it can be interpreted that there is a significant comparison between APE before and after PLB with the number P = 0.000 or statistically in general APE before and after PLB is smaller than in general in male respondents by (290.0– 278.3 =11.7). In conclusion, age is related to the increased value of pulmonary ventilation function in COPD patients. Age may explain the increased peak expiratory pulmonary flow in COPD patients. Thus there is a significant relationship between age and an increase in the height expiratory flow (APE) value of COPD patients after receiving pursed-lips breathing intervention. Abstract

Background: Human immunodeficiency virus (HIV) is a significant concern in the field of global health. However, studies on the relationship between viral load, opportunistic infections, and quality-of-life among them are limited. Objectives: This study aims to determine the relationship between viral load count, the presence of opportunistic infection, and the quality-of-life among people living with HIV. Methods: A cross-sectional study was used involving 35 respondents recruited through convenience sampling. The quality-of-life of individuals living with HIV was assessed using the World Health Organization Quality-of-Life instrument for HIV. Statistical analysis using descriptive statistics, including mean, standard deviation, and proportion, and bivariate analysis with p-value<0.05. Results: Of the 35 participants, the majority were male (57.1%), with a mean age of 41.63 (9.09), and had a low education level (51.4%). The mean quality-of-life score was 86.88 (12.27), with a mean duration of ARV treatment of 7.23 years (5.36), undetectable viral load (68.6%), living with opportunistic infections (80%), and with tuberculosis being the most common (52.63%). There was a significant correlation between viral load and overall quality-of-life (p-value 0.04). Conclusion: The results of viral load testing and the identification of opportunistic infections can serve as indicators to assess the quality-of-life among people living with HIV. The outcomes of this study have practical implications for enhancing healthcare services in clinical settings. This information can guide the implementation of targeted interventions to strengthen social relationships and these individuals' environmental quality-of-life.

Abstract

Good sleep quality in congestive heart failure (CHF) patients helps repair heart muscle cells. Sleep hygiene is one of the non-pharmacological interventions to overcome sleep disorders. This study aims to determine the effectiveness of sleep hygiene on sleep quality in CHF patients. A Randomized Controlled Trial (RCT) with two groups was taken using probability sampling with block random sampling method. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI) and sleep habits using the Sleep Hygiene Index (SHI). Outputs were measured based on the change in the mean between the measurement time using the paired t-test and the difference in the mean between groups using the independent t-test. A total of 30 respondents were involved in this study. The intervention group showed a significant improvement in sleep quality after sleep hygiene therapy with a mean difference (MD) of 4.20 (SE=0.70; p-value=<0.01). Sleep quality between the two groups showed a significantly greater difference in the intervention group than in the control group (MD=-2.13, SE=1.00, p-value=0.04, effect size (Cohens'd)=-0.77). Sleep hygiene is a non-pharmacological intervention that effectively improves sleep quality in patients with CHF.

Abstract

Fatigue is a lack of physical or mental energy that can be felt by individuals and can disrupt daily activities. Fatigue causes a constant decrease in vitality, a lack of energy, and prevents sleep gaps from being filled, ultimately leading to a decline in productivity and quality of life. The prevalence of fatigue in chronic patients varies, as there is no specific data available. However, the prevalence of fatigue depends on the chronic disease in question, such as chronic kidney disease, diabetes mellitus, and heart disease. To obtain a valid and reliable Indonesian version of the MFIS instrument that can measure fatigue in patients with chronic diseases. The sampling technique used was based on the population, using a non-probability sampling method with purposive sampling. The population in this study consisted of patients with chronic diseases (Diabetes Mellitus, Congestive Heart Failure, Stroke). The population was 244 respondents (4 respondents for language comprehension, 30 respondents for validity and reliability testing, and 210 respondents for the final instrument test). The validity test results, with a sample of (n=210 respondents), showed a loading factor value greater than 0.135, indicating that all MFIS items were valid and highly correlated. The reliability test (n=210 respondents) yielded a Cronbach’s Alpha coefficient of 0.869. This study demonstrates that the Indonesian version of the MFIS instrument is valid and highly correlated. This instrument can be used to identify fatigue in chronic diseases and can serve as a guideline for nurses to implement fatigue interventions, thus helping to reduce and address fatigue issues in patients with chronic diseases.

Abstract

One of the pharmacologically controlled blood sugar levels in T2DM patients is by giving the correct insulin injection including the dose, method, time, and location, the success parameters of insulin injection include the injection location, injection procedure, and insulin management where everything will depend on glycemic control. Complications that appear at the injection site include bruising, pain, and swelling that indicate the occurrence of Lipohypertrophy (LH) Lipodystrophy is a form of lipohypertrophy (LH) which is a common complication that often occurs in T2DM patients with insulin injection therapy. This lipodystrophy will also be one of the factors that cause other T2DM complications. Method: the design in this study is One-Group Pre-test-Post-test Design, with a sampling of 14 DM patients who receive insulin injection therapy. The research period is from May to July 2024. Education using injection pad sets and educational videos on insulin injection techniques, observation of skills, and signs of lipodystrophy using questionnaires. Results: The results showed a decrease in the average blood sugar level before and after the educational intervention with a P-value of <0.001, the average patient's skill insulin injection showed an increase with a P-value of <0.001, the incidence of lipodystrophy showed that out of three areas of insulin injection, there was one location where lipodystrophy was found, namely in the arm area. With a p-value of 0.041. Conclusion: in this study, modifications are needed in the use of media or techniques in educating DM patients on insulin injection. Education can be repeated in both new and old patients on insulin. Patients need to be explained how to independently check for signs of lipodystrophy or changes in the injection site area

Abstract

Diabetes mellitus (DM) merupakan penyakit kronis dengan prevalensi yang terus meningkat, baik di dunia maupun di Indonesia. Pada tahun 2019, terdapat 463 juta penderita diabetes secara global, dengan estimasi peningkatan hingga 578 juta pada tahun 2030 (International Diabetes Federation, 2019). Di Indonesia, penderita DM mencapai 10,7 juta jiwa dan menduduki urutan keempat penyebab kematian akibat Penyakit Tidak Menular (PTM). Pengelolaan DM, terutama T2DM (diabetes mellitus tipe 2), menekankan pentingnya perawatan mandiri, termasuk rotasi injeksi insulin untuk menghindari komplikasi seperti lipohipertrofi.Kegiatan Pengabdian kepada Masyarakat (PkM) di RS PKU Muhammadiyah Gamping Yogyakarta bertujuan meningkatkan kompetensi edukasi perawat dalam penggunaan media edukasi rotasi injeksi insulin pada pasien DM. Kegiatan ini melibatkan 24 peserta yang terdiri dari perawat rawat inap, rawat jalan, dan bagian PKMRS, dengan metode klasikal yang mencakup pemaparan materi, demonstrasi, dan diskusi. Media yang digunakan meliputi set bantalan injeksi dan video teknik injeksi yang diunggah ke YoTube. Hasil PkM menunjukkan peningkatan signifikan pada pengetahuan, keterampilan, dan kepercayaan diri perawat dalam memberikan edukasi rotasi injeksi insulin kepada pasien. Peningkatan ini mencapai 85% untuk pengetahuan dan 95% untuk kepercayaan diri. Hasil ini sejalan dengan penelitian yang menunjukkan bahwa penggunaan media edukasi visual, seperti video dan demonstrasi, secara efektif meningkatkan keterampilan dan pemahaman pasien dalam melakukan injeksi insulin. Kompetensi edukasi perawat dapat ditingkatkan lebih lanjut melalui pelatihan rutin, pembaruan pengetahuan, dan peningkatan keterampilan komunikasi.

Abstract

Diabetes Melitus (DM) tipe 2 adalah gangguan metabolik yang membutuhkan manajemendan perawatan secara mandiri guna untuk mencegah terjadinya komplikasi mikrovaskulerdan makrovaskuler. Gangguan perfusi pada kaki kerap muncul sebagai tanda gejala awalterjadinya luka pada ekstremitas bawah. Buerger Allen Exercise disebut sebagai sistemlatihan untuk insufisiensi aliran darah arteri pada ekstremitas bawah dengan mengandalkanmuscle pump dan gaya gravitasi untuk meningkatkan perfusi perifer. Tujuan penelitianini adalah mengetahui pengaruh Buerger Allen Exercise terhadap peningkatan perfusiekstremitas bawah pada neuropathy perifer diabetes melitus tipe 2. Metode penelitianyang dilakukan yakni pre eksperimen dengan dengan one group pretest dan posttest.Teknik pemilihan sampel yang digunakan yakni purposive sampling dengan jumlah 15responden. Uji statistik menggunakan uji Wilcoxon dan Paired T Test. Instrumen yangdigunakan yakni lembar observasi, sphygmomanometer dan doppler vaskuler. Hasil ujiunivariat didapatkan nilai rata-rata ABI sebelum intervensi pada kaki kiri 0,96 dan kakikanan 1,00, sementara itu setelah diberikan intervensi didapatkan pada kaki kiri 0,96dan kaki kanan 1,11. Hasil uji bivariat didapatkan terdapat nilai ABI kaki kiri adalahp=0,000< 0,05 dan Kaki kanan adalah p=0,003 < 0,05, hal ini menunjukan adanyaperbedaan nilai ABI pada ekstremitas bawah sebelum dan sesudah intervensi (p-value<0.05). Pelaksanaan Buerger Allen Exercise dapat dilakukan di rumah sakit denganinterval waktu yang sudah dijadwalkan oleh perawat sebagai pencegahan komplikasiakibat penurunan perfusi pada ekstremitas bawah.

Abstract

Latar belakang: Laju kasus HIV/AIDS di Indonesia dari laporan 34 provinsi di Indonesia sudah sangat mengkhawatirkan. Kumulatif kasus HIV sampai dengan Maret 2023 sebanyak 377.650 orang, sedangkan kasus AIDS sebanyak 145.037 orang. Tujuan penelitian ini untuk mengetahui hubungan dukungan keluarga dengan kepatuhan terapi ARV. Metode: Penelitian observasional dengan desain potong lintang dengan jumlah sampel sebesar 37 partisipan yang dipilih dengan teknik purposive sampling. Variabel independen adalah dukungan keluarga dimensi informasional, instrumental, emosional, dan penghargaan. Variable dependentnya kepatuhan terapi ARV. Hasil: Rata-rata usia responden 42,8 tahun, sebagian besar laki-laki (81,1%), berpendidikan SMA (68,5%), menikah (70,3%), memiliki komorbid (67,6%), dan berada pada stadium dua (56,8%) dengan cara penularan Non IDU (83,8%). Analisis lanjut membuktikan bahwa tidak ada hubungan yang signifikan antara dimensi dukungan keluarga dan kepatuhan pasien HIV (p>0,05). Kesimpulan: Tidak ada hubungan antara dukungan keluarga dan kepatuhan pasien HIV. Kepatuhan terhadap terapi ARV juga dipengaruhi oleh sosial demografi, fasilitas kesehatan, karakteristik pengobatan, penyakit, teman, dan Kelompok Dukungan Sebaya, serta faktor internal dalam diri Orang Dengan HIV AIDS (ODHIV), yaitu motivasi diri untuk tetap hidup dan melakukan aktifitas yang baik.

Abstract

Background: Buerger Allen Exercise (BAE) and Self-Thai Foot Massage (STFM) are strongly recommended ameliorating Diabetes Peripheral Neuropathy (DPN) symptoms. Nevertheless, there is lack of evidence on the combination of both on alleviating DPN symptoms and Ankle-Brachial Index (ABI). Aims: This study aimed to examine the combination effect of BAE and STFM on symptoms of diabetic peripheral neuropathy and ABI among individuals with DPN. Methods: The research design used an experimental one-group with pre-test and post-test design. In total, 36 eligible individuals with DPN with a seven-day received combination of BAE and STFM implementing ankle exercise and foot massage. DPN scores were measured using the Michigan Screening Instrument (MNSI) questionnaire and foot perfusion was measured using the ABI. An analysis of the research data was conducted using the Paired t-test and effect size with Cohen’s d with a significance threshold of p >0.05. Results: There are a significant finding combination of BAE and STFM on MNSI subjective (mean diff=-2.78, p=>.001, d=0.71), MNSI objective (mean diff=-1.60, p=>.001, d=0.38), ABI left (mean diff= 0.01, p=>.001, d=0.51), and ABI right (mean diff=0.07, p=>.001, d=0.59). Conclusion: The finding of this study indicates that a combination of BAE and STFM is a feasible alternative therapy for ABI and MNSI in patients with DPN.

Abstract

Type 2 diabetes mellitus (T2DM) is often accompanied by sleep disturbances that can worsen glycemic control and increase the risk of complications. The combination of lavender aromatherapy and sleep hygiene is a non-pharmacological intervention that can potentially improve sleep quality in T2DM patients. This study aims to evaluate the effectiveness of lavender aromatherapy and sleep hygiene in improving sleep quality in T2DM patients. The study used a quasi-experimental design with a pre-test and post-test approach. The sample consisted of 15 inpatients with T2DM at PKU Muhammadiyah Gamping Hospital, who were selected using an accidental sampling method. Interventions included lavender aromatherapy inhalation and sleep hygiene implementation, including ablution, prayer, sleeping position, and lighting. Data were analyzed using paired t-tests to see changes in Pittsburgh Sleep Quality Index (PSQI) scores before and after the intervention. The mean PSQI score of the patients decreased significantly from 8.67 before the intervention to 5.40 after the intervention (p=0.001), indicating a significant improvement in sleep quality. The combination of lavender aromatherapy and sleep hygiene effectively improves sleep quality in T2DM patients. This intervention can be adopted in nursing practice to manage sleep disorders holistically.
