Abstract

Salah satu Teknik edukasi yang tepat di berikan kepada keluarga yaitu berupa edukasi berbasis video dengan pendekatan family centered nursing. Tujuan penelitian untuk menganalisi pengaruh edukasi berbasis video dengan pendekatan family centered nursing terhadap kesiapan keluarga merawat klien stroke. Menggunakan desain quasi eksperimen terhadap 21 responden kelompok intervensi dan 21 responden kelompok kontrol. Hasil penelitian ini didapatkan bahwa terdapat perbedaan kesiapan keluarga dalam merawat pasien stroke sebelum dan sesudah dilakukan edukasi berbasis video dengan pendekatan family centered nursing pada kelompok intervensi dengan p value 0,000 sedangkan pada kelompok control tidak terdapat perbedaan kesiapan keluarga dalam merawat pasien stroke sebelum dan sesudah dilakukan edukasi berdasarkan SOP rumah sakit dengan p-value  0,955. Abstract

Diabetic foot ulcers are one of the complications of diabetes mellitus that are often suffered by patients with a history of diabetes mellitus in addition to complications in the form of retinopathy. The purpose of this study was to collect appropriate types of therapy for wound care in diabetic foot ulcer patients. This study uses a systematic review approach. Article search was limited to inclusion criteria and exclusion criteria. The inclusion criteria in this study were diabetic patients with diabetic foot ulcers. The conclusion in this study is that honey is very effective for wound healing in diabetic foot ulcer patients, although there are several methods that are quite effective besides honey, namely the Vacuum Closing package (negative pressure wound care). Honey has been shown to reduce wound size and heal wounds

Abstract

Introduction: Suicide remains a significant global health and social problem, with Indonesia showing a lower suicide rate compared to other Asian countries. However, the underreporting of cases in Indonesia suggests that the incidence rate of suicide is higher than documented. This study aimed to identify the risk of suicide among Indonesian college students. Methods: A cross-sectional study was conducted involving 557 students from three universities across Java Island, recruited using the purposive sampling method. The Beck Scale for Suicide Ideation (BSSI) was used to assess suicide risk.  The data was analyzed using descriptive statistics to determine the prevalence of suicide-related outcomes. Additionally, linear regression analysis was conducted to examine the associations between suicide risk, suicidal ideation, suicide planning, and self-control in relation to age, sex, and year of study. Results: The average suicide risk among college students was 4.04 (SD= 6.60; 95%CI = 3.49 – 4.59) out of 38, indicating a generally low risk. However, 34.6% of students experienced the desire to die at least once, and 12.7% had a strong desire to make active suicide attempts. Furthermore, 15.8% of students found suicide an acceptable solution to their problems. Gender was found to be significant predictor of overall suicide risk, ideation and self-control. Conclusions: These findings highlight the need for targeted interventions and support for college students to prevent suicide and promote mental well-being.

Abstract

ABSTRACT Stroke is a multi-complex neurological deficit disease that impacts not only the physical condition but also the psychological and social condition of stroke patients which can affect all aspects of their life. Sleep disorders are a group of conditions characterized by disturbances in the quality and quantity of a person's sleep. Sleep quality is related to sleep interruption due to frequent and repeated brief periods of nighttime wakefulness while sleep quantity is related to difficulty in initiating and maintaining sleep. This Evidance Based Nursing application aims to get an idea of the effect of white noise on sleep disorders in hospitalized stroke patients. This research is cross-sectional. The number of samples determined by the researcher, using the G*Power 3.1 application, obtained results of 5 people with 1 additional person as a respondent in preparation for the drop out of 1 person with a final total of 7 people. White noise music therapy was effective in reducing sleep disturbances in stroke patients before the intervention, the average sleep disturbance was 8.57, decreased to 4.14 after the intervention, with a significant effect of -3.91 and a p-value <0.001, even though the EBNP journal recommended therapy using Fitbit, in the field, the use of Fitbit is not recommended because it can interfere with the patient's sleep comfort with the device worn on the wrist throughout sleep. Keywords: Evidance Based Practice, White Noise Music Therapy, Sleep Disorders, Stroke Patients.  ABSTRAK Stroke merupakan penyakit defisit neurologis multi kompleks yang berdampak bukan hanya pada kondisi fisik namun juga psikologis dan sosial pasien stroke yang dapat mempengaruhi semua sendi kehidupannya. Gangguan tidur merupakan suatu kumpulan kondisi yang dicirikan dengan adanya gangguan dalam kualitas dan kuantitas tidur seseorang. Kualitas tidur terkait dengan terputusnya tidur akibat periode singkat terjaga di malam hari yang sering dan berulang sedangkan kuantitas tidur terkait dengan kesulitan untuk memulai dan mempertahankan tidur. Aplikasi Evidance Based Nursing ini bertujuan untuk mendapatkan gambaran pengaruh white noise  terhadap gangguan tidur  pasien stroke yang menjalani rawat inap.  Penelitian ini bersifat potong lintang (cross-sectional). Jumlah sampel yang ditentukan oleh peneliti, menggunakan aplikasi G*Power 3.1 dengan mendapatkan hasil 5 orang dengan 1 orang tambahan sebagai responden sebagai persiapan droop out 1 orang dengan total akhir 7 orang. Terapi musik white noise efektif dalam mengurangi gangguan tidur pasien stroke sebelum intervensi, rata-rata gangguan tidur adalah 8.57, turun menjadi 4.14 setelah intervensi, dengan efek signifikan sebesar -3.91 dan nilai p-value <0.001, meskipun jurnal EBNP merekomendasikan terapi menggunakan Fitbit, di lapangan, penggunaan Fitbit tidak disarankan karena dapat mengganggu kenyamanan tidur pasien dengan perangkat terpasang di pergelangan tangan sepanjang tidur. Kata Kunci: Evidance Based Practice Therapy Music White Noise, Gangguan Tidur, Pasien Stroke.

Abstract

Background: Anxiety is a common problem among patients in the Intensive Care Unit (ICU) and can result in hemodynamic instability. One innovative solution to reduce anxiety is to use video calls as a substitute for limited in-person family visits. Purpose: To determine the effect of virtual interaction as therapy in reducing anxiety and improving hemodynamics in ICU patients. Method: Quasi-experimental research with pre-test and post-test on one intervention group, conducted in the ICU of An-nisa Hospital Tangerang in October-December 2024. The sampling technique used purposive sampling with a sample size of 60 participants. The independent variable in this study is virtual interaction via video call, while the dependent variable is the level of anxiety of ICU patients. Data analysis used univariate in the form of frequency distribution and bivariate using the Wilcoxon Signed Rank test. Results: Video call therapy was effective in significantly reducing the anxiety level of ICU patients (pretest score 17.07±0.583 to 12.43±0.898; p=0.000). Video calls had a significant impact on hemodynamic parameters, namely a decrease in systolic pressure (166.00±7.376 to 1125.70±7.728), diastolic (111.80±9.876 to 89.33±8.821), heart rate (113. 00±9.927 to 78.533±6.981), and respiration rate (22.766±1.165 to 16.766±1.381) with p=0.000. This intervention is effective to reduce anxiety and improve hemodynamic stability. Conclusion: The video call intervention was effective in reducing anxiety levels and improving hemodynamic parameters.   Keywords: Anxiety; Hemodynamics; Intensive Care Unit (ICU); Therapy; Virtual Interaction.   Pendahuluan: Kecemasan merupakan masalah umum yang dihadapi oleh pasien di Intensive Care Unit (ICU) dan dapat berdampak pada ketidakstabilan hemodinamik. Salah satu solusi inovatif untuk mengurangi kecemasan adalah dengan menggunakan video call sebagai pengganti kunjungan langsung keluarga yang terbatas. Tujuan: Untuk mengetahui pengaruh interaksi virtual sebagai terapi untuk mengurangi kecemasan dan meningkatkan hemodinamik pasien ICU. Metode: Penelitian kuasi-eksperimental dengan desain pre-test dan post-test pada satu kelompok intervensi, dilaksanakan di ICU Rumah Sakit An-nisa Tangerang, pada bulan Oktober-Desember 2024. Teknik pengambilan sampel menggunakan purposive sampling dengan jumlah sampel sebanyak 60 partisipan. Variabel independen dalam penelitian ini adalah interaksi virtual melalui video call, sedangkan variabel dependen adalah tingkat kecemasan pasien ICU. Analisis data yang digunakan univariat dalam bentuk distribusi frekuensi dan bivariat menggunakan uji Wilcoxon Signed Rank. Hasil: Terapi video call efektif menurunkan tingkat kecemasan pasien ICU secara signifikan (skor pretest 17.07±0.583 menjadi 12.43±0.898 ketika posttest; p=0.000). Video call memberikan dampak signifikan terhadap parameter hemodinamik, yaitu penurunan tekanan sistolik (166.00±7.376 menjadi 1125.70±7.728), diastolik (111.80±9.876 menjadi 89.33±8.821), denyut jantung (113.00±9.927 menjadi 78.533±6.981), dan laju respirasi (22.766±1.165 menjadi 16.766±1.381) dengan nilai p=0.000. Intervensi ini efektif untuk menurunkan kecemasan dan memperbaiki stabilitas hemodinamik pasien. Simpulan: Intervensi video call terbukti efektif dalam menurunkan tingkat kecemasan dan memperbaiki parameter hemodinamik pasien.   Kata Kunci: Hemodinamik; Intensive Care Unit (ICU); Interaksi Virtual; Kecemasan; Terapi.

Abstract

Stroke is the third highest level of disability in the world. The prevalence ofdisability in Indonesia is 13.88% (4,270 people) with dependency levelsincluding 36.33% independent, 33.25% mild dependency, 7.10% moderatedependency and 9.43% severe dependency. Basic Health Research in 2018showed that DKI Jakarta was in 11th place with the highest stroke prevalenceat 12.2%. This research aims to analyzed factors associated with self-care inpost-stroke patients at RSIJ Cempaka Putih. This research used a quantitativeresearch design using an analytical study with a cross sectional approach.The Bivariate results show that there is a relationship between independentvariables including age, gender, education, physical domain, emotionaldomain and social domain. Multivariate results show that the most dominantfactors related to self-care include the social domain, education and age. Thefactors most related to self-care in post-stroke patients in this study are thesocial domain, education and age. The findings suggest health workers to beable to carry out holistic assessments (bio, psycho, social and spiritual)including exploring sources of support for patients in carrying out self-care,carrying out therapeutic communication and providing innovative groupactivities that can be carried out in stroke clubs

Abstract

Multifactorial fatigue syndrome is a symptom often felt by people with type 2 diabetes mellitus. Fatigue can prevent someone from doing daily activities that support glucose control. Progressive muscle relaxation and Bensonspiritual careis a combination of relaxation to reduce fatigue. This study was to determine the effectiveness of the combination of progressive muscle relaxation and Bensonspiritual careon fatigue in patients with type 2 diabetes mellitus. Research design using designQuasy Experimental pre test – post test control group designand techniquespurposive sampling. The sample of this study consisted of 2 groups, namely intervention and control, each with 24 respondents. The sampling technique used is purposive sampling. Fatigue was measured using the Indonesian Multidimensional Fatigue Inventory (IMFI-20). There was a decrease in fatigue with the combination of progressive muscle relaxation and Bensonspiritual care with the average fatigue value decreasing significantly (-59.5) compared to the control group (-16.8). The results of the independent t-test obtained ap value <0.001, there was a difference in the average fatigue (difference) between the intervention group and the control group. The study concluded that there was a combined effect of progressive muscle relaxation and Bensonspiritual care on reducing fatigue in patients with type 2 diabetes mellitus. Interventions can be developed as independent nursing interventions in patients with type 2 diabetes mellitus with fatigue.

Abstract

Background: Stroke is one of the causes of death, most of which is caused by high blood pressure. There are still many patients who are less concerned about hypertension cases resulting in strokes when being treated, it is felt that continuous education is needed regarding knowledge and compliance. Purpose: To determine the effect of mobile phone educational intervention on reducing blood pressure in stroke patients. Method: A literature study of meta-analyze articles using the AMSTAR checklist Based on the scimago journal ranking, it entered Q1 with an impact factor of 3.593. With the G Power application, 6 samples were obtained and Jamovi 2.3.28 software was used for data processing. There were 12 educational themes based on SHEMA delivered via WhatsApp Messenger for 5 days. Results: This education was able to reduce systolic from 182 to 171, diastolic 98 to 91 and mean arterial pressure (MAP) 127 to 121. This education at least increased confidence and had productivity in controlling high blood pressure. Conclusion: The changes in systolic, diastolic and MAP reduction rates were not significant, but could control the occurrence of no increase or readmission.   Keywords: Hypertension; Mobile Phone; Stroke.   Pendahuluan: Stroke salah satu penyebab kematian yang sebagian besar disebabkan oleh tekanan darah tinggi. Masih banyaknya pasien kurang peduli kasus hipertensi berakibat stroke saat dirawat dirasa perlu edukasi terus menerus terkait pengetahuan maupun kepatuhan. Tujuan: Untuk mengetahui pengaruh intervensi edukasi mobile phone terhadap penurunan tekanan darah pada pasien stroke. Metode: Studi literatur dari artikel meta analysis menggunakan AMSTAR checklist, berdasarkan scimago journal ranking masuk Q1 dengan impact factor 3,593.  Melalui aplikasi G Power mendapat 6 sampel dan software jamovi 2.3.28 untuk olah data. Ada 12 tema edukasi berdasar SHEMA yang disampaikan melalui whatsapp messenger selama 5 hari. Hasil: Edukasi mobile phone mampu menurunkan sistolik dari 182 menjadi 171, diastolik 98 menjadi 91, dan mean arterial pressure (MAP) 127 menjadi 121. Edukasi ini setidaknya menambah keyakinan dan memiliki produktivitas dalam pengendalian tekanan darah tinggi. Simpulan: Perubahan angka penurunan sistolik, diastolik, dan MAP tidak signifikan, tetapi dapat mengendalikan untuk tidak terjadinya peningkatan atau readmisi.   Kata Kunci: Hipertensi; Mobile Phone; Stroke.

Abstract

One of the most complex chronic diseases is diabetes, which requires ongoing medical care aimed at reducing multifactorial risks beyond glycaemic control. Diabetes and stress are interrelated, where stress results in increased cortisol production, cortisol itself inhibits the work of insulin, this triggers an increase in blood sugar levels. One of the ways to control blood sugar levels by providing non-pharmacological therapy is to provide music therapy, relaxation and guided imagery. Progressive Muscle Relaxation (PMR) is one of the relaxations that can be applied to patients with diabetes mellitus. This relaxation technique can speed up the body's metabolism by physiologically increasing blood flow. This study aims to determine the effect of progressive muscle relaxation on blood sugar levels in patients with type 2 diabetes mellitus. This research is a quasi-experimental. The research design used is nonrandomised pretest - posttest with control group design. The research sample was divided into two groups, the intervention group and the control group. Blood sugar level measurements were carried out in the intervention group twice, namely before (pretest) and after (posttest) being given progressive relaxation therapy, while the control group did not receive any actions. The population in this study were patients with type 2 diabetes mellitus at Dr Moewardi Surakarta Hospital. Data analysis using the parametric paired t-test because the data is normally distributed. The blood sugar level in the control group before the relaxation intervention was an average of 237.90 mg/dl and after the relaxation intervention was an average of 218.57 mg/dl. The average blood sugar level in the control group before relaxation was 238.10 mg/dl and after relaxation was 231.43 mg/dl. The p-value for the intervention group based on the paired sample t-test statistic is 0.000 (p < 0.05) with an average decrease in blood sugar level of 19.33 mg/dl. These results indicate that progressive muscle relaxation exercises have an effect on changes in blood sugar levels in patients with type 2 diabetes mellitus. Based on the above description, it can be concluded that progressive muscle relaxation has an effect on changes in blood sugar levels in patients with type 2 diabetes mellitus.

Abstract

Introduction: Diabetic foot ulcers (wounds) are chronic ulcers that require special treatment, one of which is to ensure that the wound edge epithelialization process goes well. Epibole is a wound edge complication where the epithelium rolls downward, preventing wound closure. Methods: This study is a cross sectional observation with a conscusive sampling method in diabetikic foot ulcer patients. Results: Statistical analysis of this study showed the incidence of epibole in diabetic foot ulcers was 18.3% (N=93) and showed no significant relationship between the characteristics of respondents, ulcer characteristics with the incidence of epibole in diabetic foot ulcers. Multivariate analysis showed that the determinant factor of epibole formation in diabetic foot ulcer healing was wound age with an OR value of 5.566. Chronic wounds aged ≥ 21 days have the characteristic of keratinocytes (epithelialization) that slow down (do not migrate) so that epibole is formed and the wound is difficult to close. Recommendation: The occurrence of epibole should be identified as soon as posible, so that it can be treated immediately and accelerate wound re-epithelial or closure.
