Abstract

BPS data for Mempawah Regency shows that aquaculture production in the Anjongan District is 163 tons/year. This production result is still lower compared to other sub-districts. One of the factors causing the not-yet-optimal effects of aquaculture production in the Anjongan District is the limited mastery of aquaculture technology. Biotechnology in the field of fisheries offers sustainable technology to increase fishery resources. This Community Service aims to conduct a Focus Group Discussion on plans to apply biotechnology in fish farming. In this activity, several applications of biotechnology in aquaculture were presented. The results of the discussion activities revealed several problems, including limited sources of clean water for fish farming, high costs for feed, and limited resources for implementation in the field of biotechnology. Feed is a top priority and an essential factor because fish can grow in a healthy, optimal, and quality manner.

Abstract

Assessment and care of diabetic feet should always be provided to patients periodically to optimally prevent complications. This necessitates a practical guide that nurses can use to evaluate the risk of wounds in diabetic patients. Previous research has developed protocols for assessing the risk of diabetic foot ulcers; however, these protocols are manual, requiring nurses to spend time determining the risk level of wounds. The aim of this study is to develop a Web-Based Diabetic Foot Infection Risk Scale Application using the Forward Chaining Method. This research utilizes the Stetler model for evidence-based application development. The development process involves three stages: preparation, validation, and analysis. In the first stage, researchers conduct a literature review using literature review protocols and source scientific articles and books, followed by the development of a web-based application using the forward chaining method. There were 19 research articles recruited from three data base scientific (PubMed, Scopus, and Wiley online Library) from 2014-2024. In the second stage, experts will evaluate the content of the application concerning infection risk assessment. Finally, the application is tested. The results of this study have shown that, after testing, the Web-Based Expert System for Diabetic Foot Infection Risk Determination using the forward chaining method can be used and implemented directly by nurses working in diabetic foot care specialty clinics. This research represents an initial stage focusing on assessing infection risk levels, and it is hoped that future research will build upon this study, not only assessing infection risk but also evaluating the potential for diabetic foot ulcers based on assessed risk factors.

Abstract

A web-based expert system application using the Forward Chaining method has been developed to assist medical professionals in determining the risk scale of diabetic foot infection. Diabetic foot ulcers are a serious complication that requires early detection and accurate risk assessment to prevent further complications such as amputation. The reliability of this application needs to be evaluated to ensure its consistency and dependability in providing medical recommendations. This study aims to analyze the reliability of a web-based expert system application using the Forward Chaining method in determining the risk scale of diabetic foot infection. This research employs a quantitative approach, collecting data from 50 respondents who evaluated four main aspects of the application: Usefulness, Ease of Use, Ease of Learning, and Satisfaction. Reliability analysis was conducted using Cronbach's Alpha with the assistance of SPSS software. The analysis results show a Cronbach's Alpha value of 0.950, indicating that the application has excellent reliability. All four measured aspects are consistent in assessing the application's quality. The application can be relied upon to provide consistent and accurate recommendations in medical risk assessment. Abstract

Background: Diabetes mellitus (DM) is a chronic disease caused by the pancreas' inability to produce or use insulin effectively, typically characterized by hyperglycemia. Hyperglycemia increases the risk of diabetic foot ulcers. Prevention of diabetic foot ulcers heavily relies on proper foot care practices, which are influenced by the patient's level of knowledge. Insufficient knowledge can lead to suboptimal preventive behaviors, thereby increasing the risk of complications. Objective: This study aims to investigate the relationship between knowledge levels and preventive behaviors regarding the occurrence of diabetic foot ulcers among patients with type II diabetes mellitus at the 'Aisyiyah Singkawang Clinic. Methods: This study used a quantitative approach with a cross-sectional design. Cluster sampling was used to select 192 respondents. The research instrument consisted of a questionnaire to measure knowledge levels and preventive behaviors. Data analysis was performed using the chi-square statistical test. Results: A p-value of <0.05 was obtained, indicating a significant association between knowledge levels and preventive behavior regarding diabetic foot ulcers among diabetes mellitus patients at the ‘Aisyiyah Singkawang Clinic. Conclusion: The higher the patients’ knowledge levels, the better their preventive behavior regarding diabetic foot ulcers. Therefore, education and health counseling are essential.

Abstract

One of the needs of children is play, but some parents are not optimal in utilizing play time with children at home. Although children have special needs such as autism spectrum disorders and intellectual disabilities, the desire to play in children still exists and must be fulfilled by parents. The community service program to play with children, which is called the “Program Beranak”, is one of the programs that aims to bring closer the physical and emotional relationship between mothers and children so that it can increase self-confidence, stimulate cognitive development, social and emotional skills of children with special needs. In addition, playing with children with special needs is a form of therapy so that children can express themselves and train children's concentration. The method of this community service activity was carried out through lectures, small group discussions and simulations. The results of this activity show that there is an increase in the knowledge and skills of mothers who have children with special needs in playing with children at home. Some activities that mothers can do when playing with children at home such as reading fairy tales or storybooks, arranging puzzle toys, painting and coloring. It is expected that mothers can spend time with their children at least 30-45 minutes a day to play and the type of game is adjusted to the age and development of the child.

Abstract

Background: Diabetes Mellitus (DM) is a chronic metabolic disease characterized by elevated blood glucose levels, which over time causes serious damage to the heart, blood vessels, eyes, kidneys, and nerves. Vitamin D is thought to affect glucose metabolism, reduce ß-cell function, and increase insulin resistance and glucose intolerance. Objective: This study aimed to determine the relationship between vitamin D levels and the degree of wounds in patients with diabetes mellitus at the Kitamura Clinic, Pontianak. Methods: This research method is a quantitative descriptive method. Results: This study showed that the majority of patients with diabetic foot ulcers at the Kitamura Clinic Pontianak had normal vitamin D levels (86.7%), while 10% had low levels, and 3.3% had high levels. Most of the wounds were at a severe level, namely grade 3 (33.3%) and 4 (30%). Spearman correlation analysis showed a negative relationship between vitamin D levels and wound severity (r = -0.267), but this relationship was not statistically significant (p = 0.153). Conclusion: This study concluded that vitamin D levels were not significantly associated with the severity of diabetic foot ulcers.

Abstract

Introduction: Children with Special Needs (CSN) often have sensory and motor limitations, which increase their risk of experiencing household accidents, including burn injuries. Parents caring for CSN frequently lack adequate knowledge and skills in initial burn management, which can potentially worsen the child's condition and impede the healing process. This DAMKAR (Burn Injury Management Assistance) Program aims to enhance the competence of CSN parents at the Special School (SLB Kinarsih), located at Jl. Husein Hamzah Gg. Bhineka No. 27A, in providing prompt and appropriate first aid for burn injuries.Methodology: This community service activity (PkM) was implemented using the Participatory Action Research (PAR) approach, which emphasizes active collaboration among the implementation team, the target partner SLB Kinarsih, and the participants (parents, teachers, and caregivers of CSN). The implementation consisted of 5 stages: Preparation and Assessment (Weeks 1 to 3), Socialization and Education (Weeks 4 to 6), Practical Training and Simulation (Weeks 7 to 9), Implementation and Monitoring (Weeks 10 to 11), and Evaluation and Reporting (Week 12). Results: A significant increase was observed in the parents' average knowledge score, rising from 59.5 to 88.89. Furthermore, more than 90% of participants were able to correctly demonstrate the techniques for burn management and dressing during the simulation. This enhancement in skills provided greater confidence to the parents in handling burn emergency situations. Conclusion: The DAMKAR Program successfully improved the knowledge and practical skills of CSN parents in initial burn management. This program is recommended to be integrated as a routine module in CSN care training to minimize the risk of complications and enhance the quality of life for the child and family.

Abstract

Latar Belakang: Interaksi disosiatif, yang ditandai dengan adanya kompetisi, kontravensi, dan konflik, dapat memengaruhi kondisi psikologis seperti tingkat stres dan kesejahteraan mental pada mahasiswa keperawatan. Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan antara interaksi disosiatif dengan tingkat stres dan kesejahteraan mental pada mahasiswa keperawatan di Kalimantan Barat. Metode: Penelitian ini menggunakan desain potong lintang (cross-sectional) dengan jumlah sampel 91 mahasiswa keperawatan yang dipilih melalui total sampling. Instrumen penelitian meliputi Skala Interaksi Disosiatif (16 item), Perceived Stress Scale (PSS-10), dan WHO-5 Well-Being Index. Analisis data dilakukan menggunakan korelasi Spearman dan regresi linier sederhana. Hasil: Interaksi disosiatif berkorelasi negatif dengan kesejahteraan mental (ρ = –0,207; p = 0,049), tetapi tidak menunjukkan korelasi yang signifikan dengan tingkat stres (ρ = 0,161; p = 0,127). Analisis regresi menunjukkan bahwa interaksi disosiatif tidak secara signifikan memprediksi stres (R² = 0,024; p = 0,146) maupun kesejahteraan mental (R² = 0,033; p = 0,084). Namun, tingkat stres terbukti berkorelasi negatif secara signifikan dengan kesejahteraan mental (ρ = –0,372; p < 0,001). Kesimpulan: Interaksi disosiatif berhubungan dengan rendahnya kesejahteraan mental, namun tidak berhubungan dengan tingkat stres. Tingkat stres, pada gilirannya, secara signifikan menurunkan kesejahteraan mental. Intervensi yang berfokus pada peningkatan interaksi sosial positif dan manajemen stres direkomendasikan dalam pendidikan keperawatan.

Abstract

Background: Diabetic foot ulcers (DFUs) remain a significant clinical challenge, requiring precise classification systems to aid prognosis and treatment planning. The Wagner classification is widely used but offers limited detail on specific wound characteristics, while the SHID (Suriadi, Haryanto, Imran, Defa) system provides a more comprehensive evaluation but lacks validation. Purpose: This study was designed to evaluate and compare the predictive validity of the Wagner and SHID classification systems in forecasting DFU healing outcomes. Methods: A prospective cohort study was conducted at Kitamura Clinic and Doctor Soedarso Pontianak Hospital between August 2021 and July 2022, involving 89 DFU patients. Both systems were evaluated based on sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and likelihood ratios. Predictive validity was determined using receiver operating characteristic (ROC) curve analysis and the Youden index. Results: A cut-off grade of >2 provided optimal predictive value for both systems. The SHID classification demonstrated a sensitivity of 92%, specificity of 62%, PPV of 27.5%, and NPV of 98%, while the Wagner classification showed a sensitivity of 58%, specificity of 77%, PPV of 28%, and NPV of 92.2%. Positive likelihood ratios were 2.4 for SHID and 2.5 for Wagner. ROC analysis yielded an area under the curve (AUC) of 0.786 (95% CI: 0.69–0.87) for SHID and 0.703 (95% CI: 0.60–0.80) for Wagner. The Youden index was higher for SHID (0.540) compared to Wagner (0.349). Conclusion: Although both systems are effective for predicting DFU healing within 12 weeks, SHID's superior AUC and Youden index suggest greater clinical utility in screening and managing DFUs.

Abstract

Background: Diabetes-related foot ulcer (DFU) is a chronic complication of diabetes mellitus that is frequently accompanied by infection and carries a high risk of lower-limb amputation. Effective infection control and continuous monitoring of wound progression are critical determinants of therapeutic success. Trigona honey has been reported to possess antibacterial and anti-inflammatory properties that may support wound healing. Case Presentation: A 47-year-old man with a five-year history of type 2 diabetes mellitus presented with a Wagner grade 2 DFU on the left toe following trauma. The ulcer exhibited signs of severe infection, including erythema, pain, and purulent exudate. Laboratory findings revealed leukocytosis and elevated blood glucose levels. The patient received systemic antibiotic therapy in combination with standard wound care, including cleansing, debridement, and topical application of Trigona honey using gauze dressings. Dressings were changed every 24–96 hours depending on wound condition. Results: Over a 30-day treatment period, there was a gradual reduction in infection status and progressive improvement in wound healing, characterized by decreased inflammation and the formation of healthier granulation tissue. However, prolonged use of Trigona honey during the proliferative phase was associated with maceration and hypergranulation around the wound. Conclusion: Trigona honey, when used as an adjuvant therapy, is effective in controlling infection and promoting wound healing during the early phase of DFU. Continued application should be reassessed if signs of maceration or hypergranulation occur, and treatment strategies should be adjusted according to the wound-healing phase to optimize clinical outcomes.
