Abstract

Low back pain (LBP) is a leading cause of disability throughout the world. The prevalence of low back pain is reported to be as high as 84%, with 12% of the population being disabled by low back pain. The prevalence in Indonesia is 24,7%, most of them are related with occupation that involves with the strength of spine, resulting in overload and trigger the accelerated degenerative articular. Based on the duration of pain, LBP is divided into acute, subacute, and chronic. Low back pain that persists for more than 12 weeks is defined as chronic LBP. Chronic LBP requires multimodal therapy, but seems to follow the main principle, such as to reduce the pain, to prevent muscle stiffness, and to enhance patients’ quality of life. The most common drugs used to treat pain in chronic LBP are NSAID, gabapentinoid, and muscle relaxant. NSAID, as the first modality to treat chronic LBP, works by inhibiting prostaglandin through the inhibition of both COX 1 and 2 (non-selective) or directly of COX 2 (selective). The use of NSAID is limited in patients with gastrointestinal and cardiovascular symptoms. Gabapentinoid is an anti-convulsant drug which involves in neurotransmitter modulation in afferent neuron presynaptic receptors, but this drug requires titration and it should be taken for considerations. Lastly, muscle relaxant is categorized into two different mechanisms, which directly works in spinal cords and skeletal muscles (antispastic) and in central nervous system conduction (antispasmodic) to prevent muscle spasms, but still considering about the risk of drug abuse. By taking all side effects of all therapy modalities into consideration, NSAID is chosen as first-line therapy to treat chronic LBP.

Abstract

In 2016 Tanggamus Regency had 70 cases of infant mortality, under-fives with malnutrition 1,371 cases (3.31%), and 1 case of malnutrition, and the coverage of community participation in under-fives growth monitoring was 75.6% . However , uneven distribution of health workers and low knowledge of health cadres about measuring the nutritional status of children, so the coverage of nutritional status is still low. This study uses the quassy experimental method with the design of one group pretest-posttest design. The sampling technique is total sampling. The research was conducted from April to July 2018, located in Gunungtiga Village, Ulubelu District, Tanggamus Regency, Lampung. The subjects that were obtained were 18 Posyandu cadres. The result of the median value of the knowledge pre-test is 30, while the median value of the post-test knowledge is 80. The results of the bivariate analysis p = 0,000. There were significant differences between before and after training in measuring the nutritional status of children under five to the knowledge of Posyandu cadres in Gunungtiga Village.

Abstract

Coronavirus disease 2019 (COVID-19) is the first viral outbreak pneumonia in Wuhan, China, in December 2019, caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-Cov-2). WHO reported 2.241.359 positive cases of COVID-19 patients worldwide, with a cumulative death toll of 152.551 men by April 19, 2020. As in 18 April 2020, Indonesia itself hit 6.248 positive cases with a total of 535 deaths, according to the Indonesian Health Ministry. SARS-Cov-2 is a coronaviral (β) beta group virus. In order to enter the target cell, SARS-CoV 2 uses the ACE Angiotensin conversion enzyme (ACE) 2. This can reduce the function of ACE2 which changes Angiotensin 2 into Angiotensin (1-7). The disrupted production of Angiotensin (1-7) can cause a vasodilator effect that is not optimal, so the condition of people with hypertension can be worsen. The risk of SARS-Cov-2 infection is higher among those suffering from hypertension with the drug Renin-Angiotensin-Aldosterone System inhibitor, due to the expression of ACE2. Overexpression of ACE2 may make it easier for SARS-CoV-2 to penetrate the body.

Abstract

Background: The eating behavior of adolescent girls is often not concerned with the nutritional content of food, consuming more fast food, causing changes in eating behavior leading to unhealthy eating behavior. The impact of unhealthy eating behavior is the emergence of obesity and underweight. Adolescent eating behavior is influenced by predisposing, supporting, and reinforcing factors.Aims: This study aims to analyze the factors to predict the eating behavior of community-based adolescent girls in Indonesia.Method: This study uses a quantitative method with a cross-sectional approach. The sample consisted of 210 adolescent girls aged 15-18 years in the city of Bandar Lampung, Indonesia. Data were taken from July to October 2022. The sample was taken using the multistage random sampling method and analyzed using the chi-square test and logistic regression.Results: As many as 75 adolescent girls (35.7%) have unhealthy eating behavior, 164 people (78.1%) have poor nutritional knowledge, 6 people (2.9%) experience early menarche, 98 people (46.7%) were malnourished, 99 people (47.1%) had an eating disorder, 74 people (35.2%) had a risky personality, 109 people (51.9%) were dissatisfied with their body image, 100 people (47.6%) had a low allowance, 109 people (51.9%) influenced by social media, 98 people (46.7%) influenced by idols, 136 people (64.8%) influenced by family, 111 people (52.9%) influenced by teachers, and 76 people (36.2%) were influenced by their peers. Statistically, the factors related to the eating behavior of adolescent girls were knowledge (p=0.016), nutritional status (p=0.000), eating disorder (p=.0.008), personality (p=0.002), body image (p= 0.006), allowance (p=0.11), social media (p=0.000), idol figures (p=0.03), family (p=0.037), teachers (p=0.048) and peers (p=0.000 ), while menarche was not related (p=0,241). The most associated factors in predicting the eating behavior of adolescent girls are nutritional status, eating disorders, personality, body image, social media, idol figures, teachers, and peers.Conclusion: The prevalence of unhealthy eating behavior in adolescent girls is 35.7% and the most associated factors in predicting the eating behavior of adolescent girls are nutritional status, eating disorders, personality, body image, social media, idol figures, teachers, and peers.

Abstract

Indonesia menghadapi penyakit menular dan tidak menular (PTM). Salah satu PTM adalah Sindrom Metabolik (SM). Jumlah penderita SM tahun 2019 ± 12-37% dari total penduduk Asia di dunia, 21,66% di Indonesia, 20,59% di Lampung. Pendekatan keluarga membantu mengidentifikasi faktor klinis, individu, psikososial dalam keluarga sehingga penatalaksanaannya lebih global. Menerapkan pelayanan dokter keluarga berbasis evidence based medicine dengan menganalisis faktor risiko, permasalahan klinis, penatalaksanaan pasien menurut kerangka penyelesaian permasalahan pasien menggunakan patient centered dan family approach. Penelitian ini adalah laporan kasus. Data primer dari anamnesis, pemfis, pemeriksaan penunjang, kunjungan rumah. Penilaiannya dari hasil diagnosis holistik awal-akhir studi secarakualitatif dan kuantitatif. Pasien mengeluh pusing, lelah, nyeri leher-bahu selama 8 hari. Pasien mempunyai riwayat dislipidemia, hipertensi terkontrol, DM tipe 2 sejak tahun 2019. Pasien didiagnosis SM menurut anamnesis, pemfis, pemeriksaan penunjang. Pemahaman pasien tentang penyakit, kebiasaan pola makan buruk dan jarang olahraga. Setelah intervensi terjadi penurunan gejala klinis dan perubahan perilaku pasien dan keluarga. Diagnosis dan penatalaksanaan pasien dilakukan holistik dan komprehensif, patient center,  family appropried dengan pengobatan SM teratur menurut EBM dibutuhkan untuk mengendalikan tekanan darah, glukosa darah, kolesterol, penurunan berat badan yang lebih ideal, dan mengubah perilaku pasien menjadi lebih baik. mencegah komplikasi.

Abstract

Abstrak: Penatalaksanaan Holistik Pada Wanita Usia 72 Tahun dengan Hipertensi dan Dislipidemia Melalui Pendekatan Kedokteran Keluarga di Wilayah Kerja Puskesmas Rajabasa Indah. Sesuai dengan data yang dikeluarkan WHO, diperkirakan sekitar 15-20% penduduk dunia menderita hipertensi dan menurut data American Heart Association tahun 2021, lebih dari 100 juta orang Amerika mempunyai kadar kolesterol total >200 mg/dl. Data Indonesia dari Riset Kesahatan Dasar tahun 2018 menunjukkan bahwa 54.4% penduduk Indonesia dengan usia ≥15 tahun mempunyai kadar kolesterol ≥200 mg/dl.

Abstract

TB is a disease that is still difficult to complete. The estimated of the number of TB cases in children reaches 99,000 cases or 12% of the total case. The analysis of this study is case report. Assessment is carried out based on a holistic diagnosis from the start, process, and end of the study quantitatively and qualitatively. An. L, 9 years old, has a main complaint of coughing up phlegm. The patient also experienced complaints of not gaining weight, looking thin, and the patient's mother said her child had become less active since she was sick. The patient is a TB patient who has been undergoing OAT treatment for almost 4 months. Some of the factors that can influence the patient's condition are internal and external risk factors. In this case, a diagnosis of tuberculosis was carried out in accordance with current theories and research. After the intervention, there was a decrease in clinical symptoms and changes in the behavior of the patient and his family. The management given is in accordance with the guidelines, it can be seen that changes in behavior and knowledge of the patient and the patient's family are seen after intervention based on evidence-based medicine which is patient-centered, family approach.

Abstract

Malnutrition in Pregnancy (ICD 10-O25) is a malnutrition problem cause by the lack of food or nourishment intake in a long period of time, up to years. One of the potential problem in long term macro malnutrition, specifically in pregnant women with ICD 10-O25 is give birth to Low Weight Baby (LWB), anemia and decreased immunity. In Indonesia, there are 17.3% pregnant women with ICD 10-O25. Lampung in particular, there are 13.6% pregnant women with ICD 10-O25. Therefor, it is crucial to comprehensively manage nutrition status, especially in pregnant women. Identify internal and external risk factors and clinical problems present in the patient. Apply a holistic and comprehensive family doctor approach according to the problems found in the patient and carry out medicine-based management. This study is descriptive study about case report. The primary data obtained through anamnesis, physical examination and a visit to the home. Secondary data were obtained from medical records of patients. The patient is a pregnant women with ICD 10-O25. The internal risk factor aspect in the patient is the patient's unhealthy eating and drinking patterns. Aspects of the patient's external factors due to lack of support and information from the family and the environment. Patients are given information about CED and healthy eating patterns. In addition, patients are assisted in applying their knowledge. The diagnosis enforcement and management with this patient has been done with family medical approach, several theories and latest researches. After intervention to the patient, the patient has better understanding and attempted to apply the knowledge. Furthermore, the patient’s Mid Upper Arm Circumference (MUAC) has been increased in the last evaluation visit.

Abstract

Dementia is the 4th cause of death after heart disease, cancer and stroke. To prevent dementia, 6 pillars of dementia prevention are needed. This service aims to screen for dementia and health education about preventing dementia. The benefit of this activity is to improve the quality of life for the elderly. Participatory Impact Monitoring (PIM) activity methods include involvement, active participation and community empowerment. The instrument uses the Mini Mental Status Examination (MMSE) questionnaire, sphygmomanometer, scales and microtoise. Indicators of achievement are dementia screening, health services and increasing knowledge for the elderly. The partners' targets are 34 elderly people at the Tresna Nursing Home Bakti Yuswa Natar. South Lampung. The results of the activity showed that blood pressure was hypertension stage 1 (35.29%), hypertension stage 2 (8.82%), hypertensive crisis (2.94%). Examination of nutritional status: overweight (26.47%), obesity I (8.82%), obesity II (11.76%). Dementia screening showed probable dementia (44.12%), definitive dementia (23.53%). The results of health education showed that there was a difference in the mean knowledge about dementia before and after being given health education (p=0.02). and the solutions offered are health screening, health education and the SMART Elderly program. The results disseminated are the SMART Elderly Program and health education. It requires collaboration and joint commitment from all parties to prevent dementia. The continuation of this activity is in the form of the SMART (Healthy, Independent and Active) Elderly program in collaboration with the Natar Community Health Center, South Lampung.

Abstract

Chronic Energy Deficiency in pregnancy is a condition where pregnant women suffer from a lack of food that lasts for a long time. Pregnant women with chronic energy deficiency have poor eating behavior, which is influenced by knowledge, perception, beliefs and health education. Health education provided by health workers can increase knowledge and change eating behavior and can be done better if assisted by an expert system. The aim of this activity is to train health workers (doctors, midwives, nutritionists) to carry out early detection of chronic energy deficiency in pregnancy using an expert system, increasing knowledge, attitudes and eating behavior of pregnant women. The method that will be carried out is training using the web-based expert system modelangraini.com for health workers, health education for pregnant women, and increasing knowledge and changing eating behavior of pregnant women. This activity was carried out for 1 day and took place in Faculty of Medicine, University of Lampung on Saturday 30 September 2023 with 45 family doctors, midwives and nutritionists in Bandar Lampung City participating. The results of the evaluation of the implementation of the service showed that there was an increase in participants' understanding by 91.1% to good understanding, 8.9% of participants had a fairly good understanding, and no participants had a poor understanding of the causes, risk factors, impacts, prevention methods, and early detection of chronic energy deficiency in pregnancy with techniques that are fast, easy and can help health workers in providing education and interventions on the eating behavior of pregnant women. This training activity can improve the skills of health workers in an effort to increase knowledge, attitudes and eating behavior of pregnant women so that they can prevent, overcome and help reduce the prevalence of chronic energy deficiency in pregnant women and prevent stunting in toddlers.
