Abstract

TB is a disease that is still difficult to complete. The estimated of the number of TB cases in children reaches 99,000 cases or 12% of the total case. The analysis of this study is case report. Assessment is carried out based on a holistic diagnosis from the start, process, and end of the study quantitatively and qualitatively. An. L, 9 years old, has a main complaint of coughing up phlegm. The patient also experienced complaints of not gaining weight, looking thin, and the patient's mother said her child had become less active since she was sick. The patient is a TB patient who has been undergoing OAT treatment for almost 4 months. Some of the factors that can influence the patient's condition are internal and external risk factors. In this case, a diagnosis of tuberculosis was carried out in accordance with current theories and research. After the intervention, there was a decrease in clinical symptoms and changes in the behavior of the patient and his family. The management given is in accordance with the guidelines, it can be seen that changes in behavior and knowledge of the patient and the patient's family are seen after intervention based on evidence-based medicine which is patient-centered, family approach.

Abstract

Malnutrition in Pregnancy (ICD 10-O25) is a malnutrition problem cause by the lack of food or nourishment intake in a long period of time, up to years. One of the potential problem in long term macro malnutrition, specifically in pregnant women with ICD 10-O25 is give birth to Low Weight Baby (LWB), anemia and decreased immunity. In Indonesia, there are 17.3% pregnant women with ICD 10-O25. Lampung in particular, there are 13.6% pregnant women with ICD 10-O25. Therefor, it is crucial to comprehensively manage nutrition status, especially in pregnant women. Identify internal and external risk factors and clinical problems present in the patient. Apply a holistic and comprehensive family doctor approach according to the problems found in the patient and carry out medicine-based management. This study is descriptive study about case report. The primary data obtained through anamnesis, physical examination and a visit to the home. Secondary data were obtained from medical records of patients. The patient is a pregnant women with ICD 10-O25. The internal risk factor aspect in the patient is the patient's unhealthy eating and drinking patterns. Aspects of the patient's external factors due to lack of support and information from the family and the environment. Patients are given information about CED and healthy eating patterns. In addition, patients are assisted in applying their knowledge. The diagnosis enforcement and management with this patient has been done with family medical approach, several theories and latest researches. After intervention to the patient, the patient has better understanding and attempted to apply the knowledge. Furthermore, the patient’s Mid Upper Arm Circumference (MUAC) has been increased in the last evaluation visit.

Abstract

Dementia is the 4th cause of death after heart disease, cancer and stroke. To prevent dementia, 6 pillars of dementia prevention are needed. This service aims to screen for dementia and health education about preventing dementia. The benefit of this activity is to improve the quality of life for the elderly. Participatory Impact Monitoring (PIM) activity methods include involvement, active participation and community empowerment. The instrument uses the Mini Mental Status Examination (MMSE) questionnaire, sphygmomanometer, scales and microtoise. Indicators of achievement are dementia screening, health services and increasing knowledge for the elderly. The partners' targets are 34 elderly people at the Tresna Nursing Home Bakti Yuswa Natar. South Lampung. The results of the activity showed that blood pressure was hypertension stage 1 (35.29%), hypertension stage 2 (8.82%), hypertensive crisis (2.94%). Examination of nutritional status: overweight (26.47%), obesity I (8.82%), obesity II (11.76%). Dementia screening showed probable dementia (44.12%), definitive dementia (23.53%). The results of health education showed that there was a difference in the mean knowledge about dementia before and after being given health education (p=0.02). and the solutions offered are health screening, health education and the SMART Elderly program. The results disseminated are the SMART Elderly Program and health education. It requires collaboration and joint commitment from all parties to prevent dementia. The continuation of this activity is in the form of the SMART (Healthy, Independent and Active) Elderly program in collaboration with the Natar Community Health Center, South Lampung.

Abstract

Chronic Energy Deficiency in pregnancy is a condition where pregnant women suffer from a lack of food that lasts for a long time. Pregnant women with chronic energy deficiency have poor eating behavior, which is influenced by knowledge, perception, beliefs and health education. Health education provided by health workers can increase knowledge and change eating behavior and can be done better if assisted by an expert system. The aim of this activity is to train health workers (doctors, midwives, nutritionists) to carry out early detection of chronic energy deficiency in pregnancy using an expert system, increasing knowledge, attitudes and eating behavior of pregnant women. The method that will be carried out is training using the web-based expert system modelangraini.com for health workers, health education for pregnant women, and increasing knowledge and changing eating behavior of pregnant women. This activity was carried out for 1 day and took place in Faculty of Medicine, University of Lampung on Saturday 30 September 2023 with 45 family doctors, midwives and nutritionists in Bandar Lampung City participating. The results of the evaluation of the implementation of the service showed that there was an increase in participants' understanding by 91.1% to good understanding, 8.9% of participants had a fairly good understanding, and no participants had a poor understanding of the causes, risk factors, impacts, prevention methods, and early detection of chronic energy deficiency in pregnancy with techniques that are fast, easy and can help health workers in providing education and interventions on the eating behavior of pregnant women. This training activity can improve the skills of health workers in an effort to increase knowledge, attitudes and eating behavior of pregnant women so that they can prevent, overcome and help reduce the prevalence of chronic energy deficiency in pregnant women and prevent stunting in toddlers. Abstract

Working in a hot environment can disrupt the balance of body temperature so that the body tries hardly to control the heat which can cause a lack of fluids and is called dehydration. Dehydration can affect cognitive performance, causing decreased productivity and increased risk of work accidents, and in the long term can trigger some non-communicable diseases such as hypertension, urinary tract disease, obesity and others. Pak Rama Education (Active Workers Diligently Drink Water) is an educational and empowerment activity for the worker community, especially those who work in hot environments, in an effort to prevent dehydration and non-communicable diseases. The aim of this activity was to increase workers' knowledge about healthy hydration and increase workers' ability to assess hydration status independently. This activity was carried out using the education method about healthy hydration and a demonstration of how to assess hydration status using a urine color chart. Pak Rama's activity was held on 17th November 2022 at PTPN VII Unit Way Berulu South Lampung. The participants were 50 workers consisting of management officers, factory and plantation workers. From the evaluation results, it was found that the average pre-test score of participants was 66.7 and there was an increase of participants' knowledge about healthy hydration which was seen from the increase in the average score on the post-test, namely 86.6. Based on analysis using the Wilcoxon test, it was found that there was a statistically significant difference between average pretest and posttest scores. The results of the assessment of participants' ability to assess hydration status using a urine color chart were also found increase. Pak Rama's education has proven effective in increasing workers' knowledge about healthy hydration and their ability to assess hydration status independently. Abstract

Keberadaan UKBM TOGA sangat bermanfaat sebagai upaya pengobatan sendiri atau swamedikasi. Desa Karanganyar, Kecamatan Jati Agung, Kabupaten Lampung Selatan memiliki potensi untuk budi daya tanaman TOGA dengan memanfaatkan pekarangan warga namun animo masyarakat masih rendah dan masyarakat masih sedikit yang memahami cara budi daya, manfaat TOGA sebagai swamedikasi. Pengabdian ini bertujuan untuk meningkatkan partisipasi masyarakat untuk menggunakan TOGA sebagai swamedikasi melalui Program CINTA (Curahan Ibu pada Tanaman Obat Keluarga). Manfaat kegiatan ini untuk meningkatkan pengetahuan dan keterampilan ibu-ibu PKK tentang budi daya, asuhan mandiri (asman) TOGA, dan status kesehatan masyarakat di Desa Karanganyar. Sasarannya adalah 30 orang ibu-ibu PKK, solusi yang ditawarkan adalah Program CINTA dengan metode Participatory Impact Monitoring (PIM) dengan kegiatan edukasi kesehatan dan pembentukan kader CINTA. Hasil yang didapatkan, sebelum dilakukan edukasi, pengetahuan ibu-ibu PKK yang rendah sebanyak 22 orang (73%), tinggi sebanyak 8 orang (27%) dan setelah dilakukan edukasi pengetahuan ibu –ibu PKK yang rendah hanya 1 orang (7%) dan tinggi sebanyak 29 orang (97%). Dari hasil analisis McNemar didapatkan perbedaan bermakna antara pengetahuan sebelum dan sesudah edukasi kesehatan (p=0,001). Kegiatan ini perlu terus dilaksanakan secara berkelanjutan dan komitmen bersama dari pihak mitra dalam program CINTA dan Desa Karanganyar sebagai desa sehat dan mandiri. The existence of UKBM TOGA is very useful as a self-medication or self-medication effort. Karanganyar Village, Jati Agung District, South Lampung Regency has the potential to cultivate TOGA plants by utilizing residents' yards. However, public interest is still low and few people understand how to cultivate and the benefits of TOGA as self-medication. This service aims to increase community participation in using TOGA as self-medication through the CINTA Program (Mother's Outpouring of Family Medicinal Plants). The benefit of this activity is to increase the knowledge and skills of PKK women regarding cultivation, TOGA self-care (asman) and community health status in Karanganyar Village. The target audience is 30 PKK mothers, the solution offered is the CINTA Program using the Participatory Impact Monitoring (PIM) method with health education activities and the formation of CINTA cadres. The results obtained, before the education was carried out, the knowledge of PKK mothers was low as many as 22 people (73%), high as many as 8 people (27%) and after the education was carried out the knowledge of PKK mothers was low only 1 person (7%) and high as many as 29 people (97%). From the results of McNemar's analysis, a significant difference was found between knowledge before and after health education (p=0.001). This activity needs to continue to be carried out in a sustainable manner and with a joint commitment from partners in the CINTA program and Karanganyar Village as a healthy and independent village.

Abstract

Gastroesophageal Reflux Disease (GERD) is a medical condition where stomach acid rises into the esophagus, with primary symptoms such as heartburn, regurgitation, chest pain, chronic cough, and hoarseness. GERD occurs due to the weakness of the valve between the stomach and esophagus. According to Riskesdas 2018, the prevalence of GERD in Indonesia is 40.8%, making it a significant health issue. Family doctor services that are oriented towards evidence-based medicine focus on identifying risk factors, clinical issues, and patient management through an approach that actively involves the patient and their family. This study used primary data through anamnesis, physical examination, and home visits to assess the environmental conditions that may impact the patient’s health. The assessment was done holistically, covering diagnosis, process, and research results both qualitatively and quantitatively. Indicators evaluated include diet, stress levels, adherence to therapy, physical activity, and environmental factors such as home cleanliness and access to healthcare. Before the intervention, the patient and family had limited knowledge about GERD, with unhealthy eating habits, such as irregular meals, overeating in one sitting, minimal intake of fruits and vegetables, and lack of exercise. After the intervention, the knowledge score increased from 4 in the pre-test to 9 in the post-test, indicating improved understanding of GERD and preventive measures. The holistic approach proved effective in improving the patient’s understanding, attitude, and behavior, as evidenced by improvements in the diagnostic evaluation at the end of the study.

Abstract

Family doctors provide evidence-based medicine services with a patient-centered and family-focused approach to identify clinical problems, risk factors, and patient management. This article is compiled using a case report method according to research needs. The data used are primary data obtained through anamnesis, both auto and alloanamnesis, physical examination, home visits to obtain data related to family, psychosocial and environment. A holistic diagnosis of various factors is carried out from beginning to end to solve the problem holistically. A 60-year-old woman was diagnosed with Grade I Hypertension and Scabies. The patient has internal risk aspects, namely obesity, poor hygiene, unbalanced diet, low knowledge of the disease, and stress. The external risk in this case is the lack of family knowledge regarding the patient's disease. Then, comprehensive management is carried out for the patient and family. Through this approach, it is expected to be able to improve the patient's knowledge, perspective, and actions. The role of the family to change the patient's lifestyle to be healthier as support for the patient, such as reminding to take medication and inviting exercise greatly affects the results of holistic management. The complexity of this problem requires time and cooperation between family doctors, patients, and families.

Abstract

Chronic obstructive pulmonary disease (COPD) is defined as a lung disease with clinical symptoms as shortness of breath, coughing, phlegm production due to airway abnormalities that cause persistent airflow obstruction. This airflow obstruction in COPD is progressive and often associated with chronic-inflammation caused by exposure to noxious substances, such as cigarette smoke. Tuberculosis (TB) is a contagious-infectious disease caused by Mycobacterium-tuberculosis and most often manifests in the lungs (pulmonary TB), although-M.Tb can also infect other organs (extrapulmonary TB). History of pulmonary TB could cause permanent-damage to lung-anatomy and associated with loss of lung-function, such as bronchiectasis, fibrosis, or post-TB emphysema, which can increase the risk of COPD. Research by Fan H et al in 2021 stated that there was a risk of COPD in patients with a previous history of pulmonary TB. Providing family doctor services rooted in evidence-based medicine involves the identification of risk factors and clinical issues, along with the management of patients, all within a patient centered and family oriented problem solving framework. This study involves a case-report-analysis. Primary data were gathered through history taking, physical examinations, and home visits. The evaluation is conducted based on a comprehensive diagnosis-spanning the initial, process-oriented, and final phases of the study, using both quantitative and qualitative methods. Interventions for the patient-consisted of educational measures for patients and their families using posters and direct discussions. Following these interventions, there was an observed improvement in both knowledge and behavioral-changes both patient and family-behaviors, as evidenced by improvements in the final holistic-diagnostics.

Abstract

Hypertension is a condition when blood pressure in the blood vessels increases chronically. Data from the World Health Organization (WHO) in 2015 showed that the incidence of hypertension in the world reached 1.13 billion and in 2025 the number of hypertension sufferers will continue to increase to reach 1.5 billion individuals. Type 2 DM is a metabolic disease characterized by hyperglycemia due to insufficiency of insulin secretion by pancreatic beta cells or impaired insulin function. According to the International Diabetes Federation (IDF), it is estimated that one in 11 adults or 90 million people (aged 20-79 years) in Southeast Asia will suffer from DM in 2021. Hypercholesterolemia is a condition characterized by high levels of cholesterol in the blood. According to (Riskesdas) 2018, it shows 21.2% of the Indonesian population aged ≥ 15 years. Hypertension, type 2 diabetes mellitus (T2DM) and hypercholesterolemia are chronic disease disorders that are interrelated with each other due to overlapping risk factors such as endothelial dysfunction, vascular inflammation, arterial remodeling, atherosclerosis, dyslipidemia and obesity. There are many cardiovascular complications in diabetes and hypertension, especially related to microvascular and macrovascular diseases such as stroke, heart failure, kidney disease, retinopathy, neuropathy and amputation. To reduce morbidity and mortality rates, Evidence Based Medicine-based service management is carried out in patients by identifying risk factors, clinical problems, and patient management based on a patient problem-solving framework with a patient-centered, family-focused and community-oriented approach. This study is a case report. Primary data were obtained through anamnesis, physical examination and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively.
