Abstract

The study of Premature Rupture of Membranes (PROM) in the classic mid-trimester preterm pregnancy is characterized as the breaking of the fetal layers before 28 weeks of incubation (BB) with oligo/anhydramnios. This complicates around 0.4-0.7% of all pregnancies and is related with very high neonatal mortality and morbidity rates. Concurring to the World Health Organization (WHO), the frequency of PROM globally was evaluated to be 50-60% in 2014 (WHO, 2015). In Indonesia, the rate of PROM ranges from 4.4-7.6% of all pregnancies. The rate of PROM in preterm pregnancy ranges from 3-18%, while in term pregnancies, it is estimated to be around 8-10%. In 2018, the rate of PROM in Indonesia was estimated to be 4.5% -6% of all pregnancies. Another study conducted in Indonesia found that 5-10% of all pregnancies experience PROM, and a third of deliveries that occur within less than a month are affected by PROM. 60% of PROM cases occur during full-term pregnancy, with most cases being discovered during this stage. The objective of this literature review is to survey the prove related to the experiences of mothers giving birth with PROM. This research was conducted by selecting journals concurring to consideration and prohibition criteria and using the Prisma Literature Review Flow Diagram. The experiences of mothers giving birth with PROM are described in two aspects: the characteristics of mothers giving birth with PROM (such as age, parity, work activity, gestational age) and factors related to PROM (sexual patterns, maternal and fetal genetic variations, infections, exposure to PM.25 ambient, and history of PROM). Abstrak: Ketuban Pecah dini (KPD) klasik mid-trimester preterm dideskripsikan sebagai hancurnya selaput ketuban janin sebelum usia masa hamil 28 minggu (BB) dengan oligo/anhidramnion; itu mempersulit sekitar 0,4-0,7% dari semua masa hamil dan berhubungan dengan kematian dan morbiditas neonatal yang sangat tinggi. Menurut Organisasi Kesehatan Dunia (WHO) angka terjadinya Ketuban Pecah dini (KPD) di dunia tahun 2014 sebanyak 50-60% (WHO, 2015). KPD di Indonesia berkisar 4,4 –7,6% dari keseluruhan masa hamil. Angka terjadinya KPD berada diantara 3-18% yang terjadi pada masa hamil preterm, sedangkan pada masa hamil aterm sekitar 8-10%. Insiden KPD di Indonesia berada diantara 4,5%-6% dari seluruh masa hamil tahun 2018. Hasil pengkajian lain di Indonesia bahwa dari seluruh masa hamil, 5–10% mengalami KPD. Pada persalinan kurang bulan, sepertiga diantaranya mengalami KPD. 60% KPD di antaranya terjadi pada masa hamil cukup bulan. Pada masa hamil dengan KPD, sebagian besar kasus ditemukan. Tujuan : Literatur review ini bertujuan untuk Untuk mereview evidence terkait dengan Pengalaman Ibu melahirkan dengan Ketuban Pecah dini. Method pengkajian : Method penelitan ini dilakukan dengan cara menyeleksi jurnal sesuai dengan kriteria inklusi dan eksklusi dan dilakukan dengan menggunakan Diagram Flow Literature Review PRISMA. Pengalaman ibu melahirkan dengan KPD digambarkan dalam dua aspek yakni sifat ibu melahirkan dengan Ketuban Pecah dini (usia, paritas, aktifitas kerja, umur masa hamil) dan faktor yang berhubungan dengan kpd (pola seksual, variasi genetik ibu dan janin, infeksi, paparan PM.25 ambien, dan riwayat KPD).

Abstract

The transition to parenthood is a difficult and stressful time for many couples, and those who become parents in their teens will most likely experience stress and challenges. The objective of this review is to scrutinize evidence related to the experiences of the transition to parenthood in teenage married couples. The scoping review method used was the Arksey and O'Malley framework consisting of 5 stages including identification of the PICO framework, identification of relevant articles, searching for articles from databases such as Science Direct, Pubmed, and Willey, selecting in Prisma Flow Charts, conducting Critical Appraisal on articles, doing data charting, compiling, summarizing, and reporting the results. Based on the search results from the databases, 11 articles were obtained. Of the 11 articles, an assessment was carried out and all articles got a grade of A, 4 articles with the quantitative method and 7 articles with the qualitative method. Furthermore, 3 themes were identified, namely social support, psychological impact, and barriers. The conclusion of the study found a gap in the results of review articles in developed countries and developing countries. In developed countries, the results of review articles tend to focus on psychological problems in the transition to parenthood. The adolescent parents feel pressured and are not ready to become new parents, causing stress and depression when becoming new parents. In contrast to developing countries, psychological problems in the transition to parenthood tend to focus on social support and barriers to becoming new parents. Abstrak: Transisi menjadi orang tua adalah masa yang sulit dan penuh tekanan bagi banyak pasangan apalagi pasangan yang menjadi orangtua pada usia remaja akan berpotensi mengalami stress dan tantangan. Tujuan Review untuk mereview evidence terkait pengalaman transisi menjadi orang tua pada pasangan yang menikah pada usia remaja. Metode scoping review yang digunakan adalah framework Arksey and O’Malley terdiri atas 5 tahap meliputi identifikasi  framework PICO, identifikasi artikel relevan, melakukan pencarian artikel dari database yaitu Science Direct, Pubmed, dan Willey, menyeleksi dalam Prisma Flow Chart, Critical Appraisal pada artikel, melakukan data Charting, menyusun meringkas kemudian melaporkan hasil. Berdasarkan hasil pencarian dari database didapatkan 11 artikel. Dari 11 artikel tersebut dilakukan penilaian dan didapatkan semua artikel mendapat grade A dengan metode penelitian kuantitatif dengan rincian 4 artikel dan metode qualitative study dengan rincian 7 artikel. Selanjutnya didapatkan 3 tema yaitu dukungan sosial,dampak psikologis, dan hambatan. Kesimpulan hasil penelitian ditemukan gap antara hasil review artikel pada negara maju cenderung fokus pada masalah psikologis pada transisi menjadi orangtua, orang tua remaja merasa tertekan dan belum siap untuk menjadi orangtua baru sehingga menyebabkan stress dan depresi saat menjadi orang tua baru, berbeda dengan di negara berkembang masalah psikologis pada transisi menjadi orangtua remaja cenderung fokus pada dukungan sosial dan hambatan menjadi orang tua baru.

Abstract

The safety aspect of health workers including midwives as the frontline in handling patients during the COVID-19 pandemic is at risk of being very vulnerable to being exposed to COVID-19 which threatens their lives and safety. Physical and mental fatigue in health workers during a pandemic increases the risk of burnout which can reduce the quality of service to patients. The purpose of this study was to review evidence related to the psychosocial experience of midwives in pregnancy and childbirth services during the COVID-19 pandemic. The method used was a scoping review beginning with the identification of scoping review questions using the PEO framework; identifying relevant articles to be determined based on inclusion and exclusion criteria; searching for articles using databases, namely Proquest, PubMed, Science Direct, and Wiley Online Library; selecting articles whose search flow is described through the PRISMA Flowchart; performing data charting and critical appraisal; compiling and reporting results. The results obtained based on 8 articles that had been critically appraised showed that 6 articles used qualitative studies, 1 article used mixed-method quantitative study, and 1 article used quantitative descriptive study. A total of 3 themes emerged, namely about the psychological impact, challenges and needs of midwives in pregnancy and childbirth services during the COVID-19 pandemic which affected the experience felt by midwives in pregnancy and childbirth services with psychological impacts such as fear and burnout. In addition, there were challenges faced by midwives and things that were needed by midwives in pregnancy and childbirth services during the COVID-19 pandemic. Abstrak: Aspek keselamatan tenaga kesehatan termasuk bidan sebagai garda terdepan dalam menangani pasien selama pandemic COVID-19 berisiko sangat rentan untuk terpapar COVID-19 yang mengancam jiwa dan keselamatannya. Kelelahan fisik dan mental pada tenaga kesehatan di masa pandemic meningkatkan resiko terjadinya burnout yang dapat mengurangi kualitas dalam pelayanan pada pasien. Tujuan penelitian ini untuk mereview evidence terkait dengan pengalaman psikososial bidan dalam pelayanan kehamilan dan persalinan di masa pandemic COVID-19. Metode yang digunakan ialah scoping review diawali dengan identifikasi pertanyaan scoping review menggunakan framework PEO; mengidentifikasi artikel yang relevan dengan ditentukan berdasarkan kriteria inklusi dan eksklusi; melakukan pencarian artikel menggunakan database yakni Proquest, PubMed, Science Direct, dan Wiley Online Library; melakukan seleksi artikel yang alur pencariannya digambarkan melalui PRISMA Flowchart; melakukan data charting dan critical appraisal; menyusun dan melaporkan hasil. Hasil yang diperoleh berdasarkan 8 artikel yang telah dilakukan critical appraisal, sebanyak 6 artikel menggunakan studi kualitatif, 1 artikel menggunakan kuantitatif mixed-method, dan 1 artikel menggunakan studi deskriptif kuantitatif. Sebanyak 3 tema yang muncul yakni tentang dampak psikologis, tantangan dan kebutuhan bidan dalam pelayanan kehamilan dan persalinan dimasa pandemic COVID-19 yang mempengaruhi pengalaman yang dirasakan oleh bidan dalam pelayanan kehamilan dan persalinan dengan adanya dampak psikologis seperti ketakutan dan burnout. Selain itu terdapat tantangan yang dihadapi oleh bidan dan  hal-hal yang menjadi kebutuhan bidan dalam pelayanan kehamilan dan persalinan di masa pandemic COVID-19.

Abstract

Contraceptives (KB) have various types of methods with advantages and disadvantages in their use. Husband's support is needed because it can provide motivation and comfort in choosing contraception. The results of the BKKBN mini survey show that men's participation in family planning has decreased, namely 11.1% in 2018, down to 8.9%. in 2017 husband's participation in selecting and using contraceptives was still low at 1.3%. Purpose: to map available evidence related to the husband's role in choosing contraception. Scoping this review follows the framework of Arksey and O'Malley, which consists of identifying review questions; looking for relevant evidence; selecting relevant studies; map data; and discuss, conclude and report results. PRISMA flowchart is used to display evidence search flow. The results of the review show that there are 10 articles obtained from the search process. This review raises 4 themes, namely the role of the husband in choosing contraception; factors influencing husband's involvement in contraception selection; cause of the lack of participation of husbands and the health system. The husband's role in contraception selection is still not carried out optimally, as a result of the division of roles between husband and wife due to gender bias, therefore innovative efforts are needed to involve husbands in contraception selection.

Abstract

Maternal health is a topical issue of Population, Family Planning, and Family Development Programs in the 2030 SDGs indicators, namely by ensuring universal access to sexual health and reproductive rights. The Unmet need contraception is one of the main barriers to achieving SDGs goals in low-and middle-income countries. This study aims to explore and map the scientific evidence related to family planning in women of childbearing age with Unmet need contraception. Scoping review refers to the framework of Arksey O'Malley and is documented based on the PRISMA-ScR Checklist guide. Article search using 3 databases Pubmed, Sciencedirect, Ebsco host and 1 Gray literature Google Scholar by filtering the year of publication from 2019-2022. Obtained 15 articles are eligible and 3 findings of the theme are socio-demographic aspects, socio-psychology, and health. The results of the review found that the motivation of women of childbearing age in deciding Unmet need contraception is to exclude children and limit the number of children. Women of childbearing age with low decision-making autonomy within a family are most influential in determining the Unmet need contraception. Planning related to contraceptive use in the future is expressed by the motivation of contraceptive use plans at one time. Abstrak: Kesehatan ibu menjadi Isu Topik Program Kependudukan, Keluarga Berencana dan Pembangunan Keluarga dalam Indikator SDGs 2030 yaitu dengan menjamin akses universal terhadap kesehatan seksual dan hak reproduksi. Unmet need kontrasepsi merupakan salah satu hambatan utama dalam mencapai tujuan SDGs di negara berpenghasilan rendah dan menengah. Penelitian ini bertujuan untuk menggali dan memetakan bukti ilmiah terkait dengan perencanaan keluarga pada Wanita Usia Subur dengan Unmet need kontrasepsi. Scoping review ini mengacu pada Framework dari Arksey O’Malley dan didokumentasikan berdasarkan panduan PRISMA-ScR Checklist. Pencarian artikel menggunakan 3 Database Pubmed, Sciencedirect, Ebsco host serta 1 Grey literature Google Scholar dengan memfilter tahun terbit artikel sejak 2019-2022. Didapatkan 15 artikel yang eligible serta 3 Temuan tema yaitu Aspek Sosio-Demografic, Sosio-Psikologi, dan Kesehatan. Hasil review didapatkan bahwa motivasi wanita usia subur dalam memutuskan Unmet need kontrasepsi yaitu untuk menjarangkan anak dan untuk membatasi jumlah anak. Wanita usia subur dengan otonomi pengambilan keputusan yang rendah didalam suatu keluarga paling berpengaruh terhadap penentuan Unmet need kontrasepsi. Perencanaan terkait dengan penggunaan kontrasepsi dimasa depan dinyatakan dengan adanya motivasi rencana penggunaan kontrasepsi disuatu waktu.

Abstract

Mental disorders posses significant obstacles both during and after pregnancy for many women. Birth weight, premature birth, perinatal and infant deaths, postpartum psychopathology, emotional difficulties, inappropriate behavior in children and adolescents, and adverse effects on other people are just a few of the negative impacts that mental disorders are linked to during pregnancy in that family. Women with mental disorders may experience issues after giving birth, affecting their health throughout pregnancy. Infections, including depression, frequently go undiagnosed and receive subpar treatment despite routine interactions with medical providers during pregnancy. Hormonal changes impact Mother's body movements; for example, she moves more slowly and gets exhausted more rapidly. Pregnant ladies who are in this scenario tend to become indolent. A systematic assessment of research findings on the effectiveness of economic evaluation of mental health screening instruments during pregnancy is frequently used to encourage early detection of mental health in pregnant women as maternal reactions become more sensitive, irritable, and irritable. These investigations mostly used the EPDS and PHQ-9. For the systematic review portion of this investigation, the PRISM ScR 2020 Flowchart was employed, and the A search of the literature was done using the databases Pubmed and Whiley. This article was assessed using the CHEC-Extended checklist. Two reports satisfied the inclusion criteria after an investigation of 890 chosen papers. The systematic review identified three themes: cost, effectiveness, and threshold. Given the variety of treatments used to treat mental health issues in pregnant women, it is clear that using two approaches rather than just one will result in greater cost-effectiveness

Abstract

The readiness of prospective parents plays a pivotal role in their journey toward parenthood, influencing their ability to navigate the transition smoothly. This readiness can be cultivated through comprehensive physical and mental health screenings, empowering them to embark on their parenthood journey with a well-prepared and healthier pregnancy plan. This scoping review aimed to identify previous research evidence regarding health service models for prospective parents. This research is a scoping review using Framework Arkshey & O’Malley and PRISMA-Scr Checklist, four journal databases: Pubmed, Wiley Online Library, EBSCO, and Proquest. Apart from that, this research also used Google Scholar and official sites that support it. The keywords used are models, health services, and future parents. Article quality assessment using MMAT (Mixed Methods Appraisal Tool). A total of 2,899 articles were checked for duplication and title screening, abstract screening. A total of 21 articles were screened for the whole article, and only 11 were deemed eligible. The results of this article is that health programs during the premarital and preconception periods are important in preparing a healthy generation. Each country has premarital and preconception health programs, including providing blood supplement tablets and folic acid, mental health screening, and genetic disease screening. It is expected that cross-sector collaboration could increase the use of health services during the premarital and preconception periods. These programs serve as crucial avenues for imparting essential knowledge and fostering positive behaviors that can significantly impact the health outcomes of both parents and their future offspring. By prioritizing proactive healthcare measures and education during these pivotal stages, society lays a robust foundation for promoting the well-being and vitality of future generations.

Abstract

Latar belakang: Kelahiran Prematur merupakan salah satu faktor risiko yang berkontribusi terhadap kematian bayi. Penyebab kematian neonatal yang paling umum adalah Prematuritas dan Berat Bayi Lahir Rendah (BBLR) dan Tujuan penelitian: Untuk mengetahui efektivitas perawatan kanguru pada bayi prematur. Metode: Scoping review ini dilakukan dengan menggunakan kriteria inklusi dan eksklusi, termasuk melakukan penilaian kritis, ekstraksi data, menganalisis dan melaporkan hasil. Hasil: Ada empat tema yang diambil yaitu perawatan kangguru karena mudah dalam pelaksanaannya, manfaatnya bagi fisik bayi, manfaat bagi orang tua bayi, dan manfaat dalam proses menyusui bayi. Simpulan: Penerapan perawatan kanguru pada bayi prematur memberikan manfaat bagi bayi, terutama fisiknya.

Abstract

The majority of GDM occurs in low- and middle-income countries. GDM is associated with increased maternal and infant complications as well as an increased burden of health financing. Accurate and cost-effective screening is needed for prevention and further treatment. Unfortunately, there is a lack of research on the cost-effectiveness of GDM screening in middle-income countries. The objective of this study was to uncover scientific proof concerning the cost-effectiveness analysis of screening for gestational diabetes in middle-income countries. We conducted a systematic review using Pubmed and ScienceDirect. Four reviewers screened the title and abstract of each article, followed by the selection of the full text based on the inclusion criteria (cost-effectiveness analysis studies of GDM screening and original research). Quality articles were assessed using the CHEC-Extended tool. A total search of 1239 articles found 3 studies that met the inclusion criteria. A screening program is more cost-effective than no screening, this represents a savings of $1,329,671. The ICER value is less than 3x GDP per capita, which indicates that the intervention is cost-effective. Compared to two-step screening, one-step screening is more likely to be cost-effective since it detects more cases. As the prevention of DALYs is mainly due to the prevention of T2DM, middle-income countries should focus more on postnatal care for women with GDM in the future. It is advised that long-term follow-up studies be a major focus of future research in order to evaluate the follow-up GDM screening intervention's possible long-term health benefits and financial effects.

Abstract

Pola asuh orang tua adalah keseluruhan interaksi orang tua dengan anak, orang tua merangsang untuk mengubah sikap, tingkah laku, perhatian, aturan, disiplin, penghargaan dan punishment, pengetahuan dan tanggapan, serta nilai-nilai yang dianggap pantas oleh orang tua. Pola asuh anak di Jepang cukup berbeda apabila dibandingkan dengan pola asuh anak di negara lain. Keluarga Indonesia yang tinggal di Jepang mempunyai norma-norma agama, sosial dan budaya tertantu yang ingin di terapkan untuk anaknya namun dikarenakan mereka tinggal di negara lain menyebabkan beberapa tantangan yang tidak mudah untuk dijalani. Kegiatan ini bertujuan untuk meningkatnya pengetahuan mitra yaitu diaspora Indonesia (orang tua yang mempunyai anak remaja) tentang parenting style, pola asuh anak, dan mempersiapkan kesehatan reproduksi yang sehat untuk anak remaja. Langkah-langkah yang dilakukan yaitu observasi melalui data studi pendahuluan dengan literature dan wawancara dengan key person, penyuluhan dengan melakukan edukasi kesehatan terkait dengan topik dengan memberikan ceramah dan memberikan leaflet dan evaluasi dengan mengukur pengetahuan melalui pretest dan post test. Hasil dari kegiatan adalah meningkatnya pengetahuan mitra tentang parenting style, adanya peningkatan pengetahuan mitra tentang strategi persiapan kesehatan reproduksi yang sehat untuk remaja. Hasil pre-test dan post menunjukkan peningkatan pengethauan sasaran. Hasil pengabdian masyarakat dapat terus diterapkan oleh mitra dalam kehidupan sehari – hari dan untuk memastikan kegiatan berlanjut tim akan melakukan monitoring melalui online (via whatshapp, zoom, dll).
