Abstract

Background: Unintended pregnancy (UP) among adolescents represents a global issue with profound psychological, social, and emotional consequences. Adolescents experiencing UP often face social stigma, family pressure, and uncertainty about their future, which can lead to prolonged psychological distress. This study aims to explore the experiences of stress and the resilience strategies employed by adolescents dealing with UP.Subjects and Method: This research employs a scoping review methodology based on the Arksey and O’Malley framework, with the population consisting of adolescents, teenage girls, and young mothers. The concepts included stress, stress experience, resilience strategies, and coping mechanisms, and the context was applicable across various settings. Literature was sourced from databases such as PubMed and ScienceDirect. Inclusion criteria comprised scholarly publications in English or Indonesian, primary research articles or relevant reviews addressing similar themes, and studies published within the last 10 years to ensure relevance. The PRISMA Flowchart illustrated the selection process, while the critical appraisal of the selected articles followed the Joanna Briggs Institute (JBI) guidelines. Results: UP is a significant contributor to psychological stress among adolescents, driven by social stigma, isolation, and a lack of emotional support. Nevertheless, resilience emerges as a crucial factor in managing such stress. Protective factors such as support from family and peers and emotional regulation training are pivotal in enhancing adolescent resilience. Conclusion: Community-based interventions focusing on fostering resilience through the development of psychological skills and stress management strategies are essential for supporting the mental health of pregnant adolescents.

Abstract

The father is the closest person after the baby's mother, who plays a role in exclusive breastfeeding. The interventions of dads attempt to improve breastfeeding knowledge, positive attitudes, and fathers' engagement in providing practical, physical, and emotional support to mothers, which can improve breastfeeding behaviors. The purpose of this study is to map current information about the role of fathers in exclusive breastfeeding in developing countries by including relevant sources of evidence from each article that has been found. The research method uses a Scoping review with the Arkey and O'Malley approach. It consists of 5 steps, namely: (1) identifying each scoping review question, (2) identifying relevant articles, (3) selecting articles, (4) charting data (5) compiling, summarizing and reporting results. Article search uses three databases, PubMed, ScienceDirect and Wiley, published between 2018-2022. Keywords used in the search process are (Father Role* OR Partner Role*) AND (Exclusive Breastfeeding* OR Lactation* OR Breast Milk*).  There are 14 articles found, including several developing countries from various continents, nine articles from Thailand, Vietnam, Iran, India, Philippines, and China from the Asian continent, and five articles from the countries Tanzania, Ethiopia, and South Africa from the African continent. The results of the study show that the father's role in exclusive breastfeeding in developing countries is to provide emotional support, practical support and informational support so that mothers have a greater chance of producing breast milk more effectively. Husband’s support towards breastfeeding mothers positively affects the mother's psychological condition and success, especially in exclusive breastfeeding. The next author is expected to help make the policy if health workers are required, especially the policy for husbands to be involved in giving counseling while accompanying their wives in health facilities.

Abstract

Family planning (FP) coverage in Pangarengan District declined from 84% to 74%, partly due to cultural beliefs that contraception is solely a woman’s responsibility. This study explores the role of husbands in contraceptive selection among couples at the Gulbung Village Sub-Health Center using a qualitative phenomenological approach. Ten husbands of FP acceptors were interviewed using purposive sampling. Thematic analysis revealed that husbands’ roles were influenced by their initial knowledge, information sources (midwives, social media, environment), and cultural norms. Their support included financial aid, accompanying wives to FP services, and decision-making discussions. Barriers included limited knowledge, rigid gender roles, and sociocultural pressures. Health workers acted as communicators, facilitators, and counselors, but often did not involve husbands actively in counseling. Husbands’ expectations centered on supporting their partners and ensuring family welfare. Overall, the study highlights the need to improve husband involvement in FP through inclusive health education and culturally sensitive counseling approaches.

Abstract

Background: Child marriage remains a global reproductive health concern, placing adolescents at high risk of early pregnancy and obstetric complications. The selection of effective and cost-efficient contraceptive methods is essential to prevent unintended pregnancies. This study aimed to identify and compare scientific evidence on the cost-effectiveness of long-acting reversible contraception (LARC) versus short-acting reversible contraception (SARC) among married adolescents. Subjects and Method: This study was a systematic review guided by the PRISMA 2020 statement, employing the PICO framework. The population comprised married adolescents aged 10–19 years; the intervention included long-acting reversible contraceptives (IUDs and implants); the comparison involved short-acting reversible contraceptives (oral pills, injectables, and condoms); and the outcome focused on economic evaluation in health care. Literature searches were conducted in PubMed, ScienceDirect, and Google Scholar using MeSH-based keywords and Boolean operators. Articles meeting the inclusion criteria were screened using Rayyan, and their quality was assessed using the Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist. Data were narratively synthesized based on parameters of cost-effectiveness, cost-utility, cost-benefit, Incremental Cost-Effectiveness Ratio (ICER), Average Cost-Effectiveness Ratio (ACER), and Quality-Adjusted Life Years (QALY). Results: Of the 471 articles identified, six met the inclusion criteria. Most studies employed Markov or decision tree models. The findings consistently indicated that LARC methods were more cost-saving compared to SARC. In Indonesia, IUDs had an ICER of USD 0.84 per pregnancy averted, compared to USD 3.76 for oral pills and USD 5.18 for injectables. In India, the etonogestrel implant achieved an Incremental Cost-Utility Ratio (ICUR) of USD 232 per QALY, well below the national willingness-to-pay threshold. In Kenya, the lowest cost per couple-year of protection was reported for IUDs at USD 4.87. Overall, LARC was shown to prevent more unintended pregnancies at a lower cost compared to SARC. Conclusion: Long-acting reversible contraceptives are more cost-effective than short-acting methods among married adolescents, supporting their prioritization in family planning policies to improve access and utilization.

Abstract

Introduction: Menarche is a key biopsychosocial milestone in early adolescence, yet many young girls experience it with limited knowledge and emotional readiness due to inadequate, age-appropriate reproductive health education. This lack of readiness stems from insufficient understanding, limited information provided by parents and teachers, and suboptimal reproductive health education in schools. Providing education that is both comprehensive and age-appropriate about the physical and emotional changes of puberty is essential to help reduce fear, correct misconceptions, and lessen the stigma associated with menstruation. Methods: The study aimed to analyze the effect of simulation-based video media on adolescent readiness to face menarche. This study employed a pre-experimental design with a one-group pretest-posttest approach. The sample comprised 54 fourth-grade female students within elementary school, selected via total sampling. The research instrument was a questionnaire assessing readiness to face menarche. The intervention video presented realistic scenarios depicting the physical and emotional experiences associated with menarche, including hygiene management and coping strategies for discomfort, to support both cognitive and affective learning. Results: The analysis revealed an increase in readiness scores following the intervention, with a pretest mean of 15.44 and a posttest mean of 18.40. The Wilcoxon Signed Rank Test yielded a p-value of 0.000 (p < 0.05), indicating a significant effect of simulation-based video media on adolescent readiness to face menarche. Simulation-based video media effectively enhances adolescent readiness to face menarche and serves as a viable alternative for reproductive health education in elementary schools. Conclusion: This media can be implemented as an effective and practical method for delivering reproductive health education in primary school settings. It also serves as a feasible tool to support adolescents’ preparation for menarche by improving their readiness and understanding of this developmental milestone.

Abstract

The high prevalence of adolescent pregnancy in Timor-Leste reflects vulnerabilities in adolescents’ reproductive health, particularly limited knowledge and unfavorable attitudes toward sexuality, which increase engagement in premarital sexual behavior and the risk of unintended pregnancy. However, empirical evidence explaining the role of reproductive health status in linking these outcomes, as well as the effectiveness of video-based reproductive health education in low-resource settings, remains limited. This study aimed to evaluate the effectiveness of video-based reproductive health education in improving knowledge and attitudes toward teenage pregnancy among female adolescents. A quasi-experimental one-group pretest–posttest design was conducted among 96 female students aged 17–18 years at ESGP de Suai, Covalima, Timor-Leste, selected through simple random sampling. The intervention consisted of a 15 minute reproductive health education video addressing adolescent pregnancy, related risks, and prevention strategies. Knowledge (20 items) and attitudes (10 items) were measured using a validated questionnaire (Cronbach’s α = 0.679 and 0.520, respectively). Effectiveness was defined as a statistically significant improvement in post-intervention knowledge and attitude scores. Data were analyzed using the Wilcoxon signed-rank test and the Mann–Whitney U test. Significant improvements were observed in knowledge (Z = −8.526; p < 0.001) and attitudes (Z = −8.532; p < 0.001) after the intervention. No significant difference was found between the magnitude of improvement in knowledge and attitudes (p = 0.850). Video based reproductive health education effectively improves adolescents’ knowledge and attitudes toward teenage pregnancy and represents a feasible intervention for adolescent reproductive health promotion in resource-limited settings.
