Abstract

Background: In Indonesia, the study about effectivity and side effect of colloid and crystalloid fluid have been completed, but the study about standard operating procedures (SOP) never have been done. The aim of this research was to describe and evaluate the SOP for the use of crystalloid and colloid fluids on blood pressure in sectio caesarea by using spinal anesthesia techniques. Method: The research employed a descriptive quantitative method by using a questionnaire. The basic features of crystalloid and colloid fluid selection were analyzed by multiple response tests. This research was conducted in type A hospitals in the Denpasar area by involving 30 anesthesiologist as a respondent. Result: The results indicated that 76.7% of respondents stated that there was no SOP for crystalloid and colloid fluids to prevent maternal hypotension in sectio caesarea by using spinal anesthesia techniques. In the multiple response tests, the effectiveness and side effects (21.3%) were the factors that had to be considered in the crystalloid and colloids fluid selection to prevent maternal hypotension in sectio caesarea by using spinal anesthesia techniques. The National Guideline of Medical Services for Anesthesiology and Incentive Therapy have been followed in determining the use of crystalloid and colloid fluids at section Caesarea. However, the guideline did not explain the basis for selecting crystalloid and colloid fluids to prevent maternal hypotension in sectio caesarea by using spinal anesthesia techniques in more detail, therefore the factors of effectiveness and risk of side effects are the main consideration.

Abstract

Background: The digital era has accelerated the digitalization of organizational processes, including leadership communication. The emergence of digital leadership requires leaders to adapt their communication and leadership styles to dynamic, technology-mediated work environments. In hospital settings, effective digital leadership communication is essential to support coordination, decision-making, and quality nursing care. Purpose: This study aimed to identify determinants associated with the digitalization of leadership communication in hospitals. Methods: A cross-sectional study design was employed involving 131 nurses recruited through purposive sampling based on predefined inclusion criteria. Data were collected using structured questionnaires measuring leadership style, communication style, leadership skills, leader enthusiasm, and the implementation of digital leadership communication. Statistical analyses included the Spearman rho test, contingency coefficient, and multiple logistic regression to examine associations between variables. Results: The findings indicated that 94.7% of respondents perceived the implementation of digital leadership communication as good. Leadership style showed a moderate positive correlation with digital leadership communication (p = 0.000; r = 0.512). Communication style was not significantly associated with digital leadership communication (p = 0.434; r = 0.068). Leadership skills demonstrated a strong positive relationship (p = 0.000; r = 0.589), while leader enthusiasm showed the strongest association (p = 0.000; r = 0.698). Multivariate analysis confirmed that leadership skills and enthusiasm were the most influential determinants. Conclusion: Leader enthusiasm and leadership skills are key determinants of effective digitalization of leadership communication in hospital settings. Strengthening these competencies may enhance digital leadership practices and improve organizational communication in healthcare institutions.

Abstract

Aims: To review the existing evidence on the assessment of spiritual needs and family support in palliative end-of-life nursing care. Methods: A narrative review was conducted using databases, including ScienceDirect, EBSCOhost, ProQuest, and PubMed. Keywords included “palliative care,” “assessment tools,” “family,” and “spiritual.” After applying the inclusion criteria, 12 relevant articles were analyzed. Results: Two major assessment domains were identified. First, spiritual needs encompass perceptions of death, hopelessness, dependency, fear, avoidance, anxiety, stress, depression, Quality of Life (QoL), and religious barriers. Second, family support includes caregiver burden, fatigue, depression, declining physical health, anxiety, and financial stress. These factors influence both patient well-being and the family’s capacity to provide end of life care. Conclusion: Spiritual needs and family support are interconnected and critical components of a comprehensive palliative nursing assessment. Integrating structured spiritual and family assessment tools into clinical practice is essential for developing culturally sensitive holistic care plans that enhance quality of life and death. Future nursing education, clinical training, and policies should prioritize systematic assessment approaches and interdisciplinary collaboration to strengthen end-of-life care.

Abstract

One key aspect of elderly care is personal hygiene management, which directly influences older adults' physical health, psychological well-being, and dignity. Our observation indicates that 6% of older people experience difficulties in maintaining personal hygiene independently. These older people depend on caregivers to maintain their hygiene. However, many caregivers lack adequate knowledge and skills, highlighting the need for practical training and mentoring interventions. This community service activity aimed to enhance knowledge related to personal hygiene management in elderly care by conducting a structured training and mentoring program for caregivers. This activity, carried out in March 2025, included interactive training sessions and one-on-one mentoring involving 41 caregivers from Pedungan and Melinggih villages. Both villages were chosen because our previous studies were carried out in these places. They were representatives of rural and urban areas. To measure the impact of our program, we conducted a pre-post test experimental design without control groups. Knowledge was measured with a self-developed, validated questionnaire. A Wilcoxon Signed Rank Test revealed a statistically significant increase in the knowledge after training, z = -3.829, P < 0.001, with a large effect size (r = 0.6). The median score on the knowledge increased from pre-program (Md = 13) to post-program (Md = 14). Training and mentoring empower caregivers by enhancing their knowledge to better manage the personal hygiene of older people. This approach not only improves the quality of care provided but also contributes to the dignity, comfort, and overall health outcomes of the elderly population. Future programs need to be expanded to include a broader range of caregivers from different areas.

Abstract

Introduction: The Emergency Department (ER) is a vital hospital unit that handles critically ill patients. The triage process determines treatment priorities, so speed and accuracy significantly impact patient safety. The implementation of Electronic Medical Records (EMR) is expected to improve triage quality. However, usability aspects such as ease of use, efficiency, error prevention, user satisfaction, and workflow compatibility still require a comprehensive review. This review aimed to explore current evidence on the usability of EMR-based triage systems in Emergency Departments and the factors influencing successful implementation. Methods: This literature review followed the PRISMA 2020 guidelines. Articles were searched through PubMed, ScienceDirect, and Google Scholar using the terms “usability,” “triage,” “electronic medical record,” and “emergency room.” Included articles were original studies published in English between 2020 and 2025 that addressed the validity, reliability, or usability aspects of an EMR-based triage system in the ED. Review articles, conference abstracts, and irrelevant studies were excluded. From the selection process, 15 articles were selected for analysis. Results: EMR-based triage instruments, such as EMOnco (Brazil) and CETS (China), are valid and reliable. Digital systems such as eResus, MUST-Plus, and CDSS improve documentation, accuracy, and patient safety. Mobile applications have yielded mixed results: MEDIC faces learnability challenges. At the same time, CoSMoS is easy to use despite limited EMR integration. User satisfaction increases when systems support team communication and transparency, but barriers such as low computer literacy, alert fatigue, resistance, and documentation burden persist. Conclusion: EMR-based triage technology in the ER has the potential to improve efficiency, accuracy, and patient safety, but its effectiveness depends heavily on usability. Successful implementation requires a user-centered design approach, ongoing training, and periodic usability evaluation.
