Abstract

Depression, one of the common mental disorders; generally identified by feelings of sadness or disappointment when experiencing a change, loss, or failure and becomes pathological when unable to adapt. According to WHO, Indonesians have a suicide ratio of 3,7 per 100.000 population. People with depression have some dysfunction in the hypothalamic-pituitary-adrenal (HPA) axis, which takes a part in the organism's reaction to depression. As a hormonal response to stress signals, thus the cortisol hormone will be released. The cortisol hormone mainly modulates emotions, especially fear and anxiety. Hence, patients with depression tend to have higher cortisol hormone levels. The most commonly used psychopharmacological medication is antidepressants. However, only about 40-60 out of 100 individuals are helped by taking antidepressants. Thereupon, as support for antidepressants, an adjuvant, i.e., omega-3 fatty acids in mackerel (Rastrelliger kanagurta) is needed. Therefore, the author uses Google Scholar, WHO, PubMed, and HealthLine as references for this literature review. After reviewing the literature, omega-3 fatty acids in mackerel can act as an adjuvant to antidepressants in lowering cortisol hormone levels. The dosage of omega-3 fatty acids needs to be analyzed to optimize the potential of adjuvant therapy for fluoxetine.

Abstract

Gastroesophageal Reflux Disease (GERD) is a pathological state due to stomach reflux within the esophagus, which causes necrosis of esophageal mucosa, resulting in erosions, ulcers, and other complications. GERD is still a growing global issue with 783.95 million cases worldwide in 2019. Although PPIs were used as medications to treat GERD, there were refractions and adverse effects to the patients. Azadiradione, terpenoids that can be found in A. indica and various plants, has potential as alternative H+/K+ ATPase inhibitor sources with its anti-secretory and cytoprotective activity. Some studies explain that there is also adverse effects of azadiradione. Further research are needed to determine alternative sources, safety dosage, usage duration, side effects, also drug-food or drug-drug interaction of azadiradione.

Abstract

Neuroplasticity pertains to the brain's ability to adjust functions or structure in response to events and is an important factor for skill-learning development as well as functional recovery from a neurological disorder. Numerous factors could influence neuroplasticity processes. This literature review aims to discuss the roles of genetic factors in neuroplasticity. The literature search was conducted using the keywords “neuroplasticity”, genetics”, “genes”, and polymorphism” in search engines like google scholar and PubMed, covering original articles, reviews, and text book both in Bahasa Indonesia and English for the last ten years. Genetic variation including gene polymorphism was responsible for the impact of BDNF, ApoE, and dopamine on the functional neural repair of the brain. Certain processes might directly influence neuroplasticity; others might interfere indirectly through the process. A deeper insight into genetic influence regarding neuroplasticity could lead to a better understanding and potential improvement of treatment.

Abstract

Placenta previa is a placenta that lies at the bottom of the uterus, above or very close to the internal cervical ostium. The exact cause of placenta previa is still unknown, but several factors have been associated with an increased risk of placenta previa. This study aims to determine the relationship between maternal age, parity, distance between pregnancies, history of placenta previa, history of cesarean delivery, history of curettage, assisted reproductive technology, multiple pregnancies, and fetal gender on the incidence of placenta previa at RSUP Dr. Mohammad Hoesin Palembang in 2020-2021. This study is an observational analytic study with a case control design. This study used secondary data from medical records of obstetric patients at RSUP Dr. Mohammad Hoesin Palembang in 2020-2021 that met the inclusion criteria and exclusion criteria. The study sample amounted to 100 people consisting of 50 cases and 50 controls. The results of bivariate analysis using the chi-square test and the fisher exact alternative test showed that maternal age (p=0.010), parity (p=0.038), history of cesarean delivery (p=0.000), and history of curettage (p=0.001) had a significant relationship with the incidence of placenta previa. Distance between pregnancies (p=0.204), history of placenta previa (p=0.242), and fetal sex (p=0.162) did not have a significant relationship with the incidence of placenta previa. The results of multivariate analysis using binary logistic regression test showed that the most influential risk factors for the incidence of placenta previa were history of cesarean delivery (OR=54.751) and history of curettage (OR=20.204). It can be concluded that a history of cesarean delivery and a history of curettage are the most influential risk factors for the incidence of placenta previa.

Abstract

Intellectual disability (ID) brings challenges to the affected individuals, their families, and the community at large. It is a multifactorial condition with many contributing factors, namely genetics. This review aims to briefly provide several related genetic conditions for ID in the Indonesian population. Literature studies search relevant articles using PubMed and Google Scholar using the terms ‘intellectual disability’, ‘genetics', ‘Indonesian population’. In Indonesia, the prevalence of children with ID is 1–3%, with 62,011 school-aged children affected with Intellectual Disability. Genetics plays a role in around 25–50% of cases. ID in the Indonesian population is associated with several genetic disorders, namely Duchenne muscular dystrophy (DMD), autism spectrum disorder (ASD), fragile X syndrome (FXS), Down syndrome (DS), Apert syndrome, and subtelomeric chromosomal rearrangements. There is limited research on genetics related to intellectual disability in Indonesia, implying further research is needed.

Abstract

Disorders of Sex Development (DSD) encompass rare congenital conditions involving atypical development of chromosomes, gonads, or sex anatomy. The 2006 global consensus led to a shift in the approach to handling DSD cases, emphasizing integrated treatment involving various scientific disciplines. DSD diagnosis involves a structured and multimodality approach, including history, pedigree analysis, physical examination, imaging, hormone and chromosome analysis, and genetic testing. An in-depth anamnesis and detailed pedigree analysis help trace family history and inheritance patterns of Mendelian traits in DSD cases. Thorough physical examination, including general and specific assessments, helps screen for abnormalities in sexual development. Karyotyping, hormone level investigation, and imaging examinations are crucial for supporting the diagnosis of DSD. However, genetic or molecular examination remains the modality for definitively confirming the genetic disorder underlying DSD, subsequently leading to a better understanding and management of the condition. Considering the limited resources available in the field in efforts to establish a molecular diagnosis of DSD, it is recommended to explore the role of anamnesis, pedigree analysis, physical examination, imaging examination, hormonal laboratory examination, and chromosome analysis in efforts to approach the diagnosis of DSD.

Abstract

Introduction. Accurate knowledge of mandibular thickness is necessary for performing surgical treatments on the mandible. The Champy procedure has become the predominant approach for treating fractures of the mandible. Placing plates along these lines is believed to offer the best possible fixing and stability. The objective of this study is to analyze and compare the thickness of the mandible in patients, specifically depending on their gender. The purpose is to gain a more comprehensive understanding of the differences in mandibular architecture across various populations in South Sumatra, Indonesia. Methods. This study is a cross-sectional analytical-observational study. The Department of Radiology of Dr. Mohammad Hoesin Central General Hospital, located in Palembang, conducted the study. We included a total of 103 research participants in this study, all of whom met the specified inclusion criteria. Results. There is a notable difference in the thickness of the lower jawbone in the front part (A and B) that runs parallel to Champy's line, depending on the person's gender. Males have a greater morphometric thickness of the mandibular symphysis in comparison to females. The thickness of the mandibular symphysis closely links to gender disparities. Conclusion. The thickness of symphysis A in men was substantially greater than in females, while the thickness of symphysis B in males was also significantly greater than in females, with a P value less than 0.001.

Abstract

Introduction: The maxilla is a crucial bone in the facial skeleton, contributing significantly to both aesthetics and function. Understanding the morphometric variations of the maxilla, particularly the intercanine width (ICW), is vital for successful maxillofacial reconstruction procedures. This study aimed to investigate the ICW in a South Sumatran population, considering its implications for reconstructive surgeries. Methods: This retrospective study analyzed computed tomography (CT) scans of 96 adult patients (60 males, 36 females) from South Sumatra. The ICW was measured using 3D reconstruction software, and the data were analyzed for gender-based differences. The findings were compared with existing literature to understand regional variations in maxillary morphology. Results: The mean ICW for males was 3.58 ± 0.27 cm and for females was 3.44 ± 0.18 cm. Males exhibited a significantly larger ICW than females (p < 0.05). These findings are consistent with previous studies indicating sexual dimorphism in maxillary morphology. Conclusion: This study provides valuable data on maxillary intercanine width in a South Sumatran population. The observed gender-based differences highlight the importance of considering patient demographics in maxillofacial reconstruction. This information can aid surgeons in achieving optimal functional and aesthetic outcomes.
