Abstract

Tuberkulosis yang resistan terhadap obat merupakan masalah kesehatan masyarakat. TB yang resistan terhadap obat rifampisin dan isoniazid dikenal sebagai MDR-TB, sedangkan XDR-TB adalah MDR-TB yang juga resisten terhadap obat lini kedua, seperti fluoroquinolones (levofloxacin, ofloxacin, dan moxifloxacin). tuberkulosis yang resistan terhadap rifampisin (RR-TB), di mana 78 persen di antaranya menderita tuberkulosis yang resistan terhadap berbagai obat (MDR-TB) (MDR-TB). Fluoroquinolones adalah kelas antimikroba spektrum luas yang telah menjadi semakin populer dalam beberapa tahun terakhir. Fluoroquinolones memiliki aktivitas melawan Mycobacterium tuberculosis baik secara in vitro maupun in vivo. Fluoroquinolones dapat menyebabkan resistensi jika digunakan secara tidak tepat atau berlebihan. Menurut beberapa penyelidikan, mayoritas isolat M. tuberculosis yang resistan terhadap fluorokuinolon (sekitar 50-90 persen) memiliki mutasi pada gen gyrA Daerah Penentuan Resistensi Kuinolon QRDR. Namun, keterlibatan genetik dari berbagai mutasi gen gyrA pada isolat Mycobacterium TB yang resisten terhadap resistensi fluoroquinolone tetap menjadi pola mutasi gen gyrA yang tidak diketahui pada isolat Mycobacterium tuberculosis yang resisten. Pada penelitian sebelumnya, mutasi pada gen gyrA ditemukan pada kodon 90 dan 94.

Abstract

Long COVID is a term used for persistent symptoms after being infected with COVID-19 with an estimated point of onset ranging from 3-12 weeks after infection. The risk factors for the occurrence of Long COVID are influenced by age, gender, comorbidity, ethnicity, and the severity of the acute phase. It is known that central, peripheral, and psychological factors play an important role in chronic fatigue, which is one of the most common symptoms of Long COVID. The most common neuropsychiatric clinical manifestations in Long COVID are sleep disturbances, fatigue, depression, anxiety disorders, decreased cognitive function or decreased concentration, and post-traumatic stress disorder. Therapeutic management for Long COVID cases involves various aspects, such as physical rehabilitation, management of pre-existing co-morbidities, mental health support management, and social services.

Abstract

Cholesterol is a necessary component of numerous physiologic processes that must be strictly regulated. Multiple genetic disorders, such as Familial Hypercholesterolemia, frequently induce dysfunctional cholesterol metabolism. Familial hypercholesterolemia (FH) is a frequent hereditary autosomal co-dominant marked by disarray by increased plasma amounts of low-density lipoprotein (LDL) cholesterol and premature cardiovascular disease. Familial Hypercholesterolemia (FH) is increasing but often remains underdiagnosed. This review aims to outline existing information on Familial Hypercholesterolemia with a focus on genetics, diagnostic strategies, treatments, and guidelines for management.

Abstract

Abstract. Human Growth and human insulin were the first proteins to be produced on an industrial scale. Methods: The research method used in this study was a clinical literature review on recombinant DNA techniques from insulin in bacteria from the review literature taken from Pubmed and SCopus, for further search and analysis with other existing research. Recombinant DNA technology comprises altering genetic material outside an organism to obtain enhanced and desired characteristics in living organisms or as their products. Two biosynthetic insulin analogs have sufficiently long duration of action for use as once-daily basal insulins. Some biosimilars have been developed to human insulins as produced by Lilly and NovoNordisk, but their use is more restricted given the problems encountered by the use of human insulin, see Dolinar et al for a recent review of the field. Several companies have developed or are developing depot preparations that can be administered one a week instead of once a day. Recombinant DNA technology is an important development in science that has made the human life much easier. The clinical effect of Growth hormone therapy in children can be best measured on an annual basis and compared with predicted adult height whereas failure to lower blood sugar can be seen immediately in insulin or insulin analogue therapy. Abstrak. Pertumbuhan Manusia dan insulin manusia adalah protein pertama yang diproduksi dalam skala industri. Metode penelitian yang digunakan dalam penelitian ini adalah tinjauan literatur klinis mengenai teknik DNA rekombinan dari insulin pada bakteri dari tinjauan literatur yang diambil dari Pubmed dan SCopus, untuk selanjutnya dicari dan dianalisis dengan penelitian lain yang sudah ada. Teknologi DNA rekombinan terdiri dari pengubahan materi genetik di luar suatu organisme untuk memperoleh karakteristik yang ditingkatkan dan diinginkan pada organisme hidup atau sebagai produknya. Dua analog insulin biosintetik mempunyai durasi kerja yang cukup lama untuk digunakan sebagai insulin basal sekali sehari. Beberapa biosimilar telah dikembangkan untuk insulin manusia seperti yang diproduksi oleh Lilly dan NovoNordisk, namun penggunaannya lebih dibatasi mengingat permasalahan yang dihadapi dalam penggunaan insulin manusia, lihat Dolinar dkk untuk tinjauan terbaru di lapangan. Beberapa perusahaan telah mengembangkan atau sedang mengembangkan persiapan depot yang dapat diberikan satu kali dalam seminggu, bukan sekali sehari. Teknologi DNA rekombinan merupakan perkembangan penting dalam ilmu pengetahuan yang telah membuat hidup manusia lebih mudah. Efek klinis terapi hormon pertumbuhan pada anak-anak paling baik diukur setiap tahun dan dibandingkan dengan prediksi tinggi badan orang dewasa, sedangkan kegagalan menurunkan gula darah dapat langsung terlihat pada terapi insulin atau analog insulin.

Abstract

The COVID-19 pandemic has changed many things in the medical education system. This change requires adaptation and modification from education providers, teachers, and students. A virtual learning system has been implemented instead of a face-to-face learning system. However, this makes several challenges and issues that must be considered. Virtual learning is considered less attractive and interactive and not effective in teaching clinical skills. For student clerks, the reduced time to undergo education in the hospital leads to fewer case exposures. Another problem is the facilities and infrastructure. Several ways to solve this problem have been implemented while minimizing risk and taking strict precautions. Some institutions modify and innovate to maximize the effectiveness of learning activities. In addition to technical and effectiveness issues, changes in the education system also have an impact on the mental health of medical students, therefore it is important for institutions to be sensitive to changes and risks to students' mental health during the pandemic.

Abstract

Introduction. The need for Vitamin D can be fulfilled through direct synthesis from Vitamin D stores in the skin with the help of ultraviolet B rays. Therefore, sufficient Vitamin D intake and sun exposure are needed. Most of the activities of medical students on campus are indoors from morning to evening, so it is suspected that there is a risk of Vitamin D deficiency. The purpose of this study was to identify the risk of Vitamin D deficiency based on the characteristics of Vitamin D from food intake and sun exposure in Faculty of Medicine students, Universitas Sriwijaya. Methods. This research was a descriptive study using a questionnaire and observation. Age, sex, BMI and use of related drugs were recorded. The food survey method used was the repeated food recall which was then analyzed using Nutrisurvey 2007. Subjects filled out a questionnaire to obtain data on exposure scores and sun protection scores. The research subjects were students of the Faculty of Medicine, Universitas Sriwijaya who were willing to sign informed consent and were present at the time of sampling. Results. The results from the data of 259 subjects showed that 98.8% of the subjects did not meet their vitamin D intake needs, with an average consumption of 123 ± 142 IU. Types of food that contribute the most Vitamin D are catfish (40%) and eggs (20%). None of the subjects took supplements containing Vitamin D. Sun exposure was sufficient for most subjects (62.9%), but most subjects were at risk of deficiency due to the use of sun protection (62.2%) Conclusion. There is a risk of deficiency in most medical students due to a lack of Vitamin D intake and the use of sun protection.

Abstract

WHO reported 1.9 million death cases of diabetes patients in 2019. Diabetes is caused by damage in the pancreas which resulted in a lack of insulin or insulin resistance. Medication for T2DM mainly focuses on lowering blood glucose and treating affected organs. Current medications are still lacking, thus research is needed in finding novel medications to accommodate T2DM. This paper aims to present the current research on potential plant extract in increasing GLUT4 translocation in diabetes conditions. insulin resistance state affecting GLUT4 translocation which is important in affecting glucose uptake. Some research shows that plant extract proved to be potential in increasing the translocation of GLUT4 and helping lowering blood glucose levels.

Abstract

Introduction: Rhesus (Rh) incompatibility problem arises exclusively when an Rh-positive male impregnates an Rh-negative female, resulting in maternal Rh sensitization to produce anti-D antibodies that can bind and destroy Rh-positive erythrocytes of the fetus. Hemolytic disease of the neonate due to Rh incompatibility ranges from self-limited hemolytic anemia to severe hydrops fetalis. Rh incompatibility can be prevented by administering anti-D injection therapy containing Rh Intravenous Immunoglobulin (RhIVIG). We report a rare case of good perinatal outcome of rhesus incompatibility in multigravida without anti-D therapy injection due to weak D phenotype.Case Illustration: A gravida 3, para 2 woman at 27 weeks gestation with Rh-negative blood type, who has not experienced any previous compatibility problems, came to our facility for routine antenatal care. The husband has an Rh-positive blood type with a Dd genotype (heterozygous), suggesting a 50% probability that the offspring will have an Rh-positive blood type. Laboratory results showed a negative Coombs test and weak D phenotype. The patient had never received an anti-D therapy injection in this pregnancy and her previous two pregnancies. None of her children developed hemolytic disease in the neonate. Ultrasonography showed a well-developed 27-week gestational age fetus with no major congenital disorders. The good perinatal outcomes of her children might be due to weak D phenotype. Pregnant women with weak D phenotype have fewer D antigens that can still result in Rh sensitization but not enough to cause serious complications to the fetus. Conclusion: Rhesus incompatibility with weak D phenotype can have good perinatal outcomes without anti-D injection therapy. Administration of Anti-D injection remains a viable option to prevent subsequent Rh alloimmunization.Inkompatibilitas Rhesus pada Multigravida dengan Luaran Perinatal Baik tanpa Terapi Injeksi Anti-D: Laporan KasusAbstrakPendahuluan: Masalah inkompatibilitas Rhesus (Rh) muncul secara eksklusif pada ayah Rh-positif dan ibu Rh-negatif, sehingga terjadi sensitisasi Rh ibu untuk menghasilkan antibodi anti-D yang dapat mengikat dan menghancurkan eritrosit janin yang Rh-positif. Penyakit hemolitik pada neonatus akibat ketidakcocokan Rh dapat berupa anemia hemolitik yang bisa sembuh sendiri hingga hidrops fetalis berat. Inkompatibilitas Rh dapat dicegah dengan pemberian terapi injeksi anti-D yang mengandung Rh Intravenous Immunoglobulin (RhIVIG). Kami melaporkan kasus langka dengan hasil perinatal yang baik inkompatibilitas rhesus pada multigravida tanpa injeksi terapi anti-D akibat fenotip D yang lemah.Ilustrasi Kasus: Seorang wanita gravida 3, para 2 pada usia kehamilan 27 minggu dengan golongan darah Rh-negatif, yang sebelumnya tidak mengalami masalah kompatibilitas, datang ke fasilitas kami untuk pemeriksaan antenatal rutin. Suami mempunyai golongan darah Rh-positif dengan genotipe Dd (heterozigot), sehingga kemungkinan keturunannya mempunyai golongan darah Rh-positif sebesar 50%. Hasil laboratorium menunjukkan tes Coombs negatif dan fenotipe D lemah. Pasien belum pernah menerima suntikan terapi anti-D pada kehamilan ini dan dua kehamilan sebelumnya. Semua anaknya tidak menderita penyakit hemolitik pada neonatus. Hasil USG menunjukkan janin usia kehamilan 27 minggu berkembang baik tanpa cacat bawaan mayor. Hasil perinatal yang baik mungkin disebabkan oleh lemahnya fenotip D. Ibu hamil dengan fenotip D yang lemah memiliki antigen D yang lebih sedikit sehingga masih dapat menyebabkan sensitisasi Rh, namun tidak cukup menyebabkan komplikasi serius pada janin.Kesimpulan: Inkompatibilitas rhesus dengan fenotip D lemah dapat memberikan outcome perinatal yang baik tanpa terapi injeksi anti-D. Injeksi Anti-D tetap dapat diberikan untuk mencegah aloimunisasi Rh di kemudian hari.Kata kunci: Fenotip D lemah, Imunoglobulin Rh, Inkompatibilitas rhesus, Multigravida, Terapi injeksi anti-D

Abstract

In life, women go through many significant events in their growth and body development, one of which is experiencing their first menstruation that commonly occurs during adolescence. In sexual development, the first episode of the first bloodstream is called menarche. The age of menarche is different for each individual, and on average happens in the age range of 11-15 years old. Like many other complex traits, menarcheal age is influenced by many factors, such as genetic and environmental. This study is done to know more about the age distribution of mothers and their daughters to observe the inheriting trend of menarcheal age. This is a descriptive study using 154 samples collected from Faculty of Medicine Universitas Sriwijaya University students batch 2017 that meet the inclusion criteria. Based on the results of research conducted on female students of the Doctor Program Faculty of Medicine Universitas Sriwijaya batch 2017, there is a similarity of distribution in the age of menarche experienced by mothers and their children. Among respondents, the highest age of distribution for female students are between 12 to 14 years old which is 66,9%. For mothers, the age of menarche is the highest between the age of 12 to 14 years old, namely 77,3%. Female students' BMI has the highest distribution of data in normal scores, which is 70,8%. The distribution of menarcheal age on female students Doctor Program Faculty of Medicine Sriwijaya University and mothers have the biggest number in between the age of 12 and 14.

Abstract

Depression, one of the common mental disorders; generally identified by feelings of sadness or disappointment when experiencing a change, loss, or failure and becomes pathological when unable to adapt. According to WHO, Indonesians have a suicide ratio of 3,7 per 100.000 population. People with depression have some dysfunction in the hypothalamic-pituitary-adrenal (HPA) axis, which takes a part in the organism's reaction to depression. As a hormonal response to stress signals, thus the cortisol hormone will be released. The cortisol hormone mainly modulates emotions, especially fear and anxiety. Hence, patients with depression tend to have higher cortisol hormone levels. The most commonly used psychopharmacological medication is antidepressants. However, only about 40-60 out of 100 individuals are helped by taking antidepressants. Thereupon, as support for antidepressants, an adjuvant, i.e., omega-3 fatty acids in mackerel (Rastrelliger kanagurta) is needed. Therefore, the author uses Google Scholar, WHO, PubMed, and HealthLine as references for this literature review. After reviewing the literature, omega-3 fatty acids in mackerel can act as an adjuvant to antidepressants in lowering cortisol hormone levels. The dosage of omega-3 fatty acids needs to be analyzed to optimize the potential of adjuvant therapy for fluoxetine.
