Abstract

Kerusakan ginjal akut merupakan penyakit yang berisiko tinggi menyebabkan pasien kritis hingga kematian, membutuhkan diagnosis yang cepat dalam penanganannya. Saat ini pendeteksian penyakit kerusakan ginjal akut masih terbatas dan belum dilakukan secara umum untuk dijadikan standar biomarker yang cepat dan tepat. Neutrophil gelatinase-associated lipocalin (NGAL) merupakan protein lipocalin 25 kDa yang terdapat pada sel epitel ginjal dan hati. NGAL dapat merespon langsung terjadinya kerusakan pada sel tubular epitel, ditandai dengan meningkatnya NGAL dalam urin dan plasma, sehingga NGAL dapat dijadikan penanda kerusakan ginjal akut dengan cepat dengan sensitivitas dan spesifisitas tinggi. Metode yang digunakan dalam review artikel ini menggunakan penelusuran melalui PubMed. Tujuan review artikel ini adalah untuk menunjukkan bahwa NGAL dapat dijadikan penanda gagal ginjal akut yang cepat dan sensitif.Kata Kunci: Kerusakan ginjal akut; biomarker NGAL; pengujian NGAL; sensitivitas; selektfitas.

Abstract

Cedera ginjal akut atau Acute Kidney Injury (AKI) adalah salah satu sindrom klinis yang menyebabkan risiko mortalitas dan morbiditas yang tinggi. Deteksi dini AKI sangat diperlukan untuk mencegah kerusakan ginjal permanen dan memperlambat perkembangan AKI. Kidney Injury Molecule-1 (KIM-1) adalah biomarker yang sensitif, spesifik dan cepat dalam deteksi dini AKI. KIM-1 akan diekspresikan berlebih pada sel epitel tubulus proksimal setelah adanya cedera pada ginjal. KIM-1 dapat menggambarkan disfungsi ginjal melalui cairan tubuh, seperti plasma dan urin. Kajian pustaka ini membahas karakteristik, peran dan metode pengukuran KIM-1 sebagai biomarker untuk deteksi dini AKI. Kajian dilakukan terhadap 51 artikel yang didapat dari basis data PubMed dengan kata kunci “Kidney Injury Molecule-1”, “KIM-1 Biomarker”, “KIM-1 assay”, “Acute Kidney Injury” dan “KIM-1 kidney injury biomarker”. KIM-1 adalah glikoprotein transmembran tipe I dengan berat molekul ~104 kDa yang mengalami peningkatan konsentrasi di membran apikal sel epitel tubulus proksimal dan berperan dalam patogenesis cedera sel tubulus ginjal. KIM-1 memiliki sensitivitas dan spesifisitas yang baik dengan nilai AUC-ROC >0,5. Metode pengukuran yang paling banyak digunakan adalah ELISA dan sampel yang paling banyak digunakan adalah urin karena peningkatannya lebih jelas dibanding dengan plasma. KIM-1 urin yang diukur menggunakan metode ELISA memiliki potensi untuk dikembangkan dan digunakan sebagai parameter diagnosis dini AKI di masa mendatang

Abstract

Inflamasi dermal, misalnya jerawat (acne vulgaris), secara tradisional, sering diobati menggunakan tanaman, sehingga beberapa riset mengeksplorasi ekstrak tanaman secara in vitro. Namun demikian, penggunaan ekstrak tanaman sebagai obat topikal tetap harus mengikuti Peraturan Kepala Badan Pengawas Obat dan Makanan No. 32/2019 tentang PERSYARATAN KEAMANAN DAN MUTU OBAT TRADISIONAL, yang meliputi tiga aspek yaitu keamanan (safety), mutu (quality), dan efektif (efficacy). Lebih lanjut lagi, keamanan obat topikal dipastikan melalui Uji Toksisitas Akut Dermal, seperti tercantum di dalam Peraturan Kepala Badan Pengawas Obat dan Makanan No. 7/2014 tentang PEDOMAN UJI TOKSISITAS NONKLINIK SECARA IN VIVO.  Kajian pustaka ini merangkum uji toksisitas akut dermal ekstrak tanaman yang telah dilakukan di berbagai negara. Kajian dilakukan menggunakan kata kunci “Acute”, “Dermal”, “Toxicity”, “Extract” pada basis data PubMed, dan menghasilkan 15 artikel. Hasil kajian pustaka menunjukkan bahwa semua ekstrak tanaman yang diuji di dalam kelimabelas artikel tersebut dinyatakan aman karena tidak menimbulkan gejala toksisitas ataupun kematian pada dosis di atas 2000 mg/kg BB. Dapat disimpulkan bahwa uji toksisitas akut dermal ekstrak tanaman merupakan faktor penting dalam keamanan pengembangan suatu obat herbal terstandar topikal

Abstract

Lead is harmful when its amounts in food are higher than the optimum. Plants with high levels of lead may trigger the generation of reactive oxygen species and eventually cell death. Our study aimed to assess lead distribution in rice plant leaves planted in Sukawening Village, Jatinangor District, West Bandung Regency, West Java. The rice plant leaves were collected each month until 4 months. The samples were dried and underwent wet destruction. Visible spectrophotometry with the addition of xylenol orange was employed to identify the lead metal in the samples, followed by a standard addition photometric titration method. Our results indicated that lead was identified in the samples since the plant was 1 month old as proven by the Pb-xylenol orange peak at 580 nm. The level of lead in the rice plant leaves were: 1st month (0.4118 mg/kg); 2nd month (0.5232 mg/kg); 3rd month (0.6206 mg/kg); and 4th month (0.5264 mg/kg). We concluded that the lead level in the rice plants is in the range of that required by the Verdict of the Director General of the National Agency of Drug and Food Control No. 03725/B/SK/89 about the Maximum Limit of Heavy Metal Contamination in Food.

Abstract

Roselle (Hibiscus sabdariffa L.) and stevia (Stevia rebaudiana Bertoni) are known to contain phenolic compounds and flavonoids that act as antioxidants and are beneficial in lowering blood sugar levels. This study aimed to determine the phytochemical content and evaluate the hypoglycemic effects of a combined aqueous extract of H. sabdariffa and S. rebaudiana in a 3:1 ratio on diabetic rats. Aqueous extracts were prepared and analyzed for total phenolic and flavonoid contents using the Folin-Ciocalteu and aluminum chloride methods. Diabetes was induced in male Wistar rats by intraperitoneal injection of alloxan (235 mg/kgBW). Rats in the test groups received 500 or 1000 mg/kgBW of the combination, with blood glucose levels measured on the 0th, 7th, and 14th days post-treatment using a glucometer. H. sabdariffa, S. rebaudiana, and their combination contained total phenolic contents of 102.75, 188.053, and 118.856 mgGAE/g, respectively, and flavonoid contents of 3.099, 6.987, and 4.791 mgQE/g, subsequently. Administration of the combination extract at 1000 mg/kgBW showed the highest hypoglycemic effect, reducing blood glucose levels by 35.98% on the 14th day compared to the negative control (p<0.05). The combination of H. sabdariffa and S. rebaudiana extracts demonstrates significant hypoglycemic activity, attributed to their phenolic and flavonoid content.

Abstract

BACKGROUND: Peronema canescens Jack is traditionally employed in the treatment of inflammation, malaria, and immune-related disorders. Despite its traditional use, scientific evidence on its immunomodulatory effects, interleukin (IL)-6 modulation, hematological impact, as well as its related acute toxicity data remains limited. Therefore, this study was conducted to investigate the immunostimulatory potential of P. canescens leaf extract through in vitro and in vivo assessments and evaluate its acute toxicity profile.METHODS: The in vitro immunomodulatory activity of P. canescens ethanolic extract, n-hexane fraction (NHF), ethyl acetate fraction (EAF), and water fraction (WF) were assessed by measuring IL-6 inhibition in lipopolysaccharide-stimulated RAW 264.7 macrophages. For in vivo analysis, Balb/C mice were divided into six groups: a normal control (Na-CMC), a positive control (50 mg/kg BW/day Stimuno Phyllanthus niruri extract), a negative control (80 mg/kg BW/day cyclophosphamide as immunosuppressant), and 3 treatment groups receiving P. canescens extract at doses of 100, 200, and 400 mg/kg BW/day. Hematological parameters, including white blood cell (WBC) counts, lymphocyte percentages, and neutrophil percentages, were analyzed. Acute toxicity studies were performed by administering P. canescens extract at doses of 300, 2000, and 5000 mg/kg BW over observation period.RESULTS: EAF exhibited the most pronounced IL-6 inhibition in vitro. In vivo, the administration of P. canescens extract at 200 and 400 mg/kg BW significantly elevated WBC and lymphocyte levels while concurrently reducing neutrophil counts. No mortality or neurotoxic manifestations were observed, confirming the P. canescens extract’s safety profile up to 5000 mg/kg BW.CONCLUSION: P. canescens leaf extract, particularly EAF, demonstrates robust immunomodulatory activity with a favorable safety margin. Abstract

Monascus purpureus red yeast rice (MpRYR), popularly known as angkak, is a Chinese fermented rice health product. The red pigments and monacolin K contained in MpRYR are sensitive to various factors, including acidity, temperature, oxidation, and light; thus, MpRYR is unstable and susceptible to damage during storage and processing. It was reported that monacolin K in red yeast rice solution was easily degraded under thermal treatment at 85–121°C, and the absorbance of MpRYR extract increased at an alkaline pH. This work aimed to study the effects of pH on the stability of pigment extract  MpRYR. MPRYR was purchased from Raja Godhong herbal supplier and extracted with 70% ethanol for 3 × 24 h. The resulting MpRYR extracts were subjected to various pH conditions, and the pH and absorbance were measured, and the degradation rate of MpRYR extracts was calculated. The initial pH of MpRYR extract is 5.75, which increased to 5.97 (+3.82%) at 3 h. The MpRYR extract was more stable in an acidic medium with a degradation rate of 3.80% at 3 h. Research results suggest that acidic conditions are preferable for storing and formulating MpRYR-based products to preserve their functional efficacy.

Abstract

Herbal medicine (HM) is a part of future alternative health care. According to the World Health Organization (WHO), almost 70-80% of the population of developing countries rely on HM as an immediate need for health services, one of which is the Asian region. With the high trend of a healthy lifestyle using natural ingredients, drug safety must be a significant concern. This review article aims to provide information on studying the toxicity of Asian herbal plants to hepatotoxic and nephrotoxic activities. The research method was conducted by searching using the keywords “Herbal Medicine(HM),” “Efficacy,” “Nephrotoxicity,” “Hepatotoxicity,” “Asia,” “extract” on the Google site and Google Scholar. The primary data sources used consisted of national journals, international journals, and the WHO website. Articles were screened using the inclusion criteria of Indonesian and English journals published in the last ten years. Of the toxicity study articles discussing 10 herbs-induced liver injury (HILI) and 10 drug-induced liver injury (DILI) that we reviewed, it is known that the dose consumed has a more significant effect on the incidence of hepatotoxicity and nephrotoxicity than the duration of administration with low doses slot deposit 5000. However, the period of administration with high doses has a significant relationship with liver and kidney damage. Therefore, disseminating safety studies to the public is very important to maximize drug efficacy and avoid hepatotoxicity and nephrotoxicity. Abstract

Nephrolithiasis (kidney stone) cases have a prevalence ranging from 1% to 13%. The disease not only hampers kidney function but also elevates the risk of chronic kidney diseases. The pathophysiology of nephrolithiasis is closely linked to elevated concentrations of calcium, oxalate, and/or uric acid in the urine. Pharmacotherapy to manage nephrolithiasis includes thiazide diuretics, allopurinol, citrate supplements, and alpha-blockers that have been prescribed to relieve symptoms. In addition, there is growing interest in exploring the potential of phytotherapy. This review aims to identify suitable phytotherapy approaches by examining relevant articles on nephrolithiasis. The research methodology involved searching PubMed articles using the keywords of (("Plant Extracts") AND ("Nephrolithiasis")) OR ("Ureterolithiasis"). The articles obtained from the initial search were n =123. 2 authors screened articles for their eligibility. The inclusion criteria were limited to articles written in English, topics related to nephrolithiasis and not other kidney diseases, the botanical names of the plants and the method of extraction are clearly described, complete studies with clear descriptions regarding the methods and results, not synthetic drugs, and not involving medical instruments or laser to remove the stones. Articles included in the review were n = 17. Water and hydro-alcohol were the solvents used to extract the plants. Twenty-two plants have confirmed their anti-nephrolithiasis activity, and nine articles described the assay by in vivo study, two by in vitro study, three by both in vitro and in vivo study, and only one article mentioned a study in humans by a randomized-control trial on a mixture of herbs. It is concluded that Rhizoma alismatis, Poria cocos, Polyporus umbellatus, Atractylodis macrocephalae, and Cinnamomi Cassiae prepared as a mixture in the Wu-Ling-San formula have a potential therapeutic effect on nephrolithiasis, as this formula has been studied in humans.
