Abstract

Hipertensi merupakan kondisi tekanan darah melebihi 140/90 mmHg, oleh karena itu pemeriksaan rutin diperlukan untuk memantau kestabilan nilai sistolik dan diastolic. Penelitian ini bertujuan menelaah pengaruh konseling terhadap kepatuhan minum obat serta tekanan darah pasien hipertensi terapi kombinasi di poliklinik Tasikmalaya. Metode yang digunakan yaitu observasional prospektif menggunakan analisis deskriptif rancangan Non-Equivalent Control Group, pada Desember 2018-Januari 2019 terhadap pasien hipertensi rawat jalan yang diberi terapi kombinasi. Pada penelitian ini responden diminta untuk mengisi kuesioner sebelum intervensi dan setelah intervensi dengan menggunakan Medication Adherence Rating Scale. Jumlah responden adalah 60 pasien hipertensi yang dibagi menjadi kelompok kontrol dan intervensi. Karakteristik demografi responden dari 60 pasien menunjukkan: responden perempuan = 39 orang (65,0%); berusia 50-59 tahun = 30 orang (50,0%); berpendidikan SD = 19 orang (31,67%); tidak bekerja = 28 orang (46,7%). Responden tidak mengetahui dan tidak melaksanakan diet rendah garam (48,3%); mengetahui tetapi tidak melaksanakan olahraga 51,7%; tidak merokok (96,7%); dan tidak mengkonsumsi alkohol (96,7%). Rata-rata skor MARS pre-test 4,53 dan post-test 4,70, sedangkan rata-rata sistolik responden sebelum intervensi adalah 157,27 mm Hg dan diastolik 86,45 mm Hg. Setelah intervensi, rata-rata sistolik 148,56 mmHg dan diastolik 83,45 mmHg. Pemberian konseling dapat meningkatkan kepatuhan dan menurunkan nilai tekanan darah T2 hitung 44,54 lebih besar dari T2 tabel 6,92. Dapat disimpulkan bahwa konseling berpengaruh dalam meningkatkan kepatuhan minum obat pasien hipertensi terapi kombinasi sebesar 6,18%, serta menurunkan nilai sistolik sebesar 15,93% dan diastolik sebesar 5,5% (p = 0,0001).Kata kunci: hipertensi, kepatuhan minum obat, konseling, diastolik, sistolik, MARS (Medication Adherence Report Scale).

Abstract

The restriction of social mobility and activity during the COVID-19 pandemic has been implemented to stop the deadly transmission of the SARS-CoV2 virus. People are forced to stay at home and strictly perform the COVID-19 health protocol in their daily activities. Currently, a continuous self-maintenance of the health, including oral health, is considered the best strategy worldwide. This community service activity aimed to assess the knowledge of the urban and rural adult community about oral health, comorbidity, and the quality of life (QoL) during this pandemic situation by using a pre-and post-test quasi-experimental design with an intervention of health-knowledge sharing using leaflets and videos, and a WHOQOL 2012 questionnaire to study the QoL. Paired t-test was used as statistical analysis. Total respondents were 131 (n = 76 for urban and n = 55 for rural), selected using the purposive sampling method. There was a significant difference between the results of the pre-test and post-test in both urban and rural groups (t count ranged from 1.69 to 5.98; p 0.05). Based on the WHOQOL 2012 questionnaire, both urban (90.79%) and rural (87.27%) respondents indicated a good QoL, while the remaining was scored as medium. Physical conditions/pain was the main domain that directly affects the QoL in both communities. It could be concluded that the knowledge-sharing intervention to the community gave a good impact in enhancing the knowledge of the respondents, however, a continuous program should be further carried out for better results.

Abstract

Lead is harmful when its amounts in food are higher than the optimum. Plants with high levels of lead may trigger the generation of reactive oxygen species and eventually cell death. Our study aimed to assess lead distribution in rice plant leaves planted in Sukawening Village, Jatinangor District, West Bandung Regency, West Java. The rice plant leaves were collected each month until 4 months. The samples were dried and underwent wet destruction. Visible spectrophotometry with the addition of xylenol orange was employed to identify the lead metal in the samples, followed by a standard addition photometric titration method. Our results indicated that lead was identified in the samples since the plant was 1 month old as proven by the Pb-xylenol orange peak at 580 nm. The level of lead in the rice plant leaves were: 1st month (0.4118 mg/kg); 2nd month (0.5232 mg/kg); 3rd month (0.6206 mg/kg); and 4th month (0.5264 mg/kg). We concluded that the lead level in the rice plants is in the range of that required by the Verdict of the Director General of the National Agency of Drug and Food Control No. 03725/B/SK/89 about the Maximum Limit of Heavy Metal Contamination in Food.

Abstract

Metabolit sekunder yang paling melimpah dan berperan penting dalam pertumbuhan, pigmentasi dan reproduksi pada tumbuhan yaitu senyawa fenolik. Senyawa ini telah menjadi fokus utama dalam beberapa penelitian karena efek biologis yang menguntungkan. Lokasi tumbuh tanaman merupakan salah satu yang dapat mempengaruhi kandungan senyawa aktif pada tumbuhan. Lembang, Bandung, Jawa Barat berada di daratan tinggi, memiliki suhu rendah, kondisi tanah yang subur dan sehingga cocok untuk budidaya tanaman herbal, termasuk temukunci. Polifenol dalam ekstrak tumbuhan bereaksi dengan pereaksi redoks spesifik, dalam hal ini yaitu pereaksi F-C dapat membentuk kompleks biru yang dapat diukur dengan spektrofotometri. Dalam penelitian ini dilaporkan bahwa kandungan fenolik total EEBR adalah 18.32± 0.615 atau setara dengan 183,83 mgGAE/100 g.

Abstract

Nephrolithiasis (kidney stone) cases have a prevalence ranging from 1% to 13%. The disease not only hampers kidney function but also elevates the risk of chronic kidney diseases. The pathophysiology of nephrolithiasis is closely linked to elevated concentrations of calcium, oxalate, and/or uric acid in the urine. Pharmacotherapy to manage nephrolithiasis includes thiazide diuretics, allopurinol, citrate supplements, and alpha-blockers that have been prescribed to relieve symptoms. In addition, there is growing interest in exploring the potential of phytotherapy. This review aims to identify suitable phytotherapy approaches by examining relevant articles on nephrolithiasis. The research methodology involved searching PubMed articles using the keywords of (("Plant Extracts") AND ("Nephrolithiasis")) OR ("Ureterolithiasis"). The articles obtained from the initial search were n =123. 2 authors screened articles for their eligibility. The inclusion criteria were limited to articles written in English, topics related to nephrolithiasis and not other kidney diseases, the botanical names of the plants and the method of extraction are clearly described, complete studies with clear descriptions regarding the methods and results, not synthetic drugs, and not involving medical instruments or laser to remove the stones. Articles included in the review were n = 17. Water and hydro-alcohol were the solvents used to extract the plants. Twenty-two plants have confirmed their anti-nephrolithiasis activity, and nine articles described the assay by in vivo study, two by in vitro study, three by both in vitro and in vivo study, and only one article mentioned a study in humans by a randomized-control trial on a mixture of herbs. It is concluded that Rhizoma alismatis, Poria cocos, Polyporus umbellatus, Atractylodis macrocephalae, and Cinnamomi Cassiae prepared as a mixture in the Wu-Ling-San formula have a potential therapeutic effect on nephrolithiasis, as this formula has been studied in humans.

Abstract

Dyspepsia, commonly known as indigestion, is delineated as suffering the manifestations of epigastric pain, nausea, burning, or after-meal fullness. The common therapy for dyspepsia, e.g., proton pump inhibitors or histamine-H2 receptor antagonists, has shown some adverse effects, thus, phytotherapy is currently becoming the drug of interest. Phytotherapy, additionally known as herbal medicine or botanical medicinal drug, includes the use of plant-derived compounds to prevent, alleviate, or therapy for diverse ailments. Most medications commonly used in Chinese traditional medicine or Asian folklore drugs contain mixtures of several plant extracts. Twelve clinical trials reported the effect and safety of phytotherapy on gastrointestinal disorders. Taken together, phytotherapy may be considered safe for the management of dyspepsia with close monitoring of the dose and duration of therapy.

Abstract

The geriatric population in Indonesia is increasing rapidly every year. The large number of drugs consumed by geriatric patients results in potentially inappropriate prescriptions (PIP) in the form of mis-prescribing drugs (potentially inappropriate medicines or PIM) and under-prescribing drugs (potentially prescription omission or PPO). In Indonesia, especially in Bandung, there is limited research on PIP in geriatric patients with cardiovascular disorders (CVDs). This study aims to identify PIP and factors that influence PIM and PPO based on STOPP/START criteria and the relationship between the incidence of PIM and kidney function in geriatric inpatients with CVDs at Hasan Sadikin Hospital, Bandung, West Java, Indonesia. The present retrospective study was cross-sectionally conducted from January to December 2022. The Chi-square tests were carried out to determine factors that were significantly related to PIM and PPO. Of the 192 patients, 33.4% patients experienced PIM with the most being NSAIDs with eGFR <30 mL/min/1.73 m2 (36%). As many as 35% of patients experienced PPO with the most frequent PPO not being given angiotensin-converting enzyme inhibitor (ACEi) in patients with systolic heart failure (41.8%). Factors that were significantly associated with PIM were comorbid kidney dysfunctions (OR 5; 95% CI: 2.367-10.342), and no factors were found that were significantly associated with PPO. Taking everything into consideration, it was found that PIM and PPO were common in hospitalized geriatric patients with CVDs. Kidney dysfunctions are a risk factor for PIM. These findings suggest that it is important to evaluate prescribing for geriatric patients during hospitalization using STOPP/START criteria to reduce the prevalence of PIP

Abstract

Cisplatin is a platinum-based anticancer drug that, in long-term use, causes nephrotoxicity due to oxidative stress and increases total cholesterol and triglycerides in animal models. Angelica keiskei (Miq.) Koidz., (A. keiskei) or Japanese celery ashitaba, has been reported for its antioxidant and nephroprotective activity. This study aims to determine the activity of A. keiskei sap on total cholesterol levels of cisplatin-induced Wistar rats. The sap of A. keiskeiwas freeze-dried until a yield of 3.62% w/v was obtained. The fat content in A. keiskei sap powder was obtained at 7.36%. A total of 60 g of A. keiskei sap powder was macerated with 96% ethanol solvent (1:10) for 5 x 24 h until the ethanol extract of A. keiskei sap (ASEE) of 82.08% w/w was obtained. The pharmacology activity was conducted on male Wistar rats, which were divided into 5 groups, namely normal (treated with CMC Na 0.3%), negative (nephrotoxicity induced with cisplatin 5 mg/kg BW), positive (nephrotoxicity induced with cisplatin 5 mg/kg BW and treated with quercetin 20 mg/kg BW), and two test groups which were nephrotoxicity induced with cisplatin 5 mg/kg body weight and treated with ASEE 1000 mg/kg BW, and ASEE 1500 mg/ kg BW. It was found that neither dose of ASEE altered the total cholesterol levels in cisplatin-induced male Wistar rats and could maintain the cholesterol levels in the normal range.

Abstract

Superoxide dismutases (SODs) are metalloenzymes that defend the body against reactive oxygen species and contribute to combating inflammation. Human cells have three distinct SODs, i.e., manganese SOD (MnSOD), extracellular SOD(ECSOD), and copper-zinc SOD (Cu-ZnSOD) or SOD1. The crystal structure of human SOD1 in a complex with a naphthalene-catechol-linked compound revealed hydrogen bonds and hydrophobic interaction. Catechins arepolyhydroxylated polyphenols contained in the leaves of Camellia sinensis L. This work aims to study the binding mode and molecular dynamics of two major catechins, epigallocatechin (EGC) and epigallocatechin gallate (EGCG) with human SOD1 (in complex with SBL1, a naphthalene-catechol linked compound). Both catechins demonstrated a binding mode with the enzyme, in terms of hydrogen bonds and hydrophobic interaction, similar to the native ligand (SBL1). Of the two catechins, EGC possesses a better binding affinity (docking score of -4.15 kcal/mol) for human SOD1 compared to EGCG (docking score of -4.02 kcal/mol), thus the EGC-SOD1 complex was continued in MD simulation to investigate the conformational stability and time-dependent ligand binding ability in the binding pocket. The molecular dynamics simulation confirmed that EGC is more stable than the native ligand, SBL1, with the RMSD average value of SBL1 and EGC being 1.1669 Å and 0.5607 Å, respectively. Taken together, this study confirms the antioxidant activity of catechins in C. sinensis L.

Abstract

Asupan tinggi fruktosa dapat menyebabkan gangguan metabolisme terhadap timbulnya sindrom metabolik dan diabetes melitus tipe 2. Dalam penelitian ini akan disampaikan beberapa dampak yang di dari konsumsi fruktosa. Penelitian ini bertujuan untuk membuat pemodelan hewan uji dengan pemberian fruktosa 60% terhadap resistensi insulin. Metode penelitian menggunakan 8 ekor tikus putih jantan galur Wistar yang dikelompokkan menjadi 2 kelompok yaitu kelompok normal dan kelompok fruktosa 60%. Parameter yang diamati terhadap bobot badan, kadar glukosa darah, Konstanta Tes Toleransi Insulin (KTTI), kadar adiponektin dan kadar TNF-α serta histologi jaringan adiposa. Hasil penelitian menunjukkan bahwa pemberian fruktosa 60% selama 60 hari menunjukan adanya perbedaan untuk kadar glukosa darah yang dumulai pada hari 30-60, Nilai KTTI kelompok fruktosa lebih kecil dibandingkan kelompok normal, kadar adiponektin kelompok fruktosa lebih kecil dibandingkan kelompok normal , sedangkan  kadar TNF-α kelompok fruktosa lebih besar dibandingkan kelompok normal  dan terjadi pembesaran diameter jaringan adiposa putih dan pengecilan adiposa cokelat  pada kelompok fruktosa, namun belum terjadi perbedaan pada bobot badan. Dapat disimpulkan bahwa pemberian fruktosa 60% selama 60 hari dapat digunakan sebagai pemodelan hewan uji resistensi insulin.
