Abstract

Background: The provision of diagnosis codes on medical record documents needs to be carried out accurately based on the ICD-10 book. The role of medical personnel and coding officers affects the accuracy of the diagnosis code. The coding officer must ensure that the medical record data is clear and complete such as severe or critical conditions, spesific anatomical positions, the course of health disorders, complications or the patient’s history to support the primary diagnosis. Objectives: The research objevtives to be achieved in this study evaluate and examine several causes that affect the accuracy of the selection of patient diagnosis codes at Lawang Hospital. Research Metodes: The type of research implemented is descriptive with a quantitative approach. Data collection is taken by observation, interviews, and code accuracy checklist. The population used in this study is all medical record files of inpatients in May with a total of 645 medical record documents. The sample was taken using a simple random sampling technique with as many as 25 medical record files of inpatients. Data analysis was carried out by univariate analysis by looking at the percentage of each research variable. Results: This study shows that the level of code accuracy is still said to be low because it still reaches 84% with as many as 21 documents and the code inaccuracy reaches 16% with as many as 4 documents. . Conclusion: The cause of inaccuracy will be due to the completeness of the primary and secondary diagnoses that are less specific, lack of communications between officers regarding clarity in writing in medical record documents, and some diagnoses do not include the 4th character in the case of fracture, namely the close or open description. For the development of evaluation, officers can take part in codification training and make help books such as pocket books that contain a collection of diagnosis codes. Abstract

Pindad Turen General Hospital has implemented a hospital management information system (SIMRS) since 2018. However, some users still complain of dissatisfaction with the system. One important factor affecting the successful implementation and use of information technology is user readiness. The level of user readiness in accepting information technology greatly affects the success or failure of the application of the technology. This study used a quantitative descriptive approach with a sample of 36 SIMRS users at RSU Pindad Turen. Data collection techniques were carried out through questionnaires to collect information about SIMRS user perceptions, as well as observations of the system. The variables were measured using dimensions of the Technology Acceptance Model (Perceived Ease of Use, Perceived Usefulness, attitude toward using, behavioral intention, and actual usage). The data obtained was analyzed quantitatively and the results are presented in table form. The study results show the average values for the dimensions of Perceived Ease of Use (3.08), Perceived Usefulness (2.87), attitude toward using (2.67), behavioral intention (3.01), and actual usage (2.89). User perceptions of SIMRS indicate that the system is running well, but still needs improvement due to several shortcomings. Therefore, it is necessary to evaluate the existing information system and further socialize it to officers who use SIMRS. This evaluation aims to measure the benefits of SIMRS implementation and identify potential problems faced by the hospital. Keywords: Evaluation, Technology, Hospital, SIMRS, TAM.

Abstract

Online health consultations via telemedicine, conducted through websites or digital platforms, have become a trend with the advancement of communication technology. However, data privacy is a major concern, especially when it involves patients' personal health information. This study analyzes the factors influencing public trust in telemedicine services. The aim is to examine the impact of economic, educational, environmental, usefulness, ease of use, and information quality factors on public trust in telemedicine. The research method involves path analysis using a quantitative approach, followed by qualitative analysis. The study involved 90 participants, with a 2:1 ratio between the user group and the control group. The results indicate a significant influence of economic status, education level, environment, system ease of use, and information quality on public trust in data privacy within telemedicine, with a total trust score reaching +0.545. These findings underscore the need for the public, as telemedicine users, to be more cautious about the security and confidentiality of their personal data. Additionally, service providers must make efforts to ensure patient data security and privacy, while establishing clear terms in every privacy policy and providing legal guarantees regarding data protection.

Abstract

Pending claims are a problem that still often occurs in Indonesia, where many hospitals face obstacles in the BPJS Kesehatan claim process. This problem can have a significant impact on the quality of health services. This study aims to analyze the factors that cause BPJS Kesehatan pending claims in hospitals. The method used is literature review by searching for articles through the Google Scholar database in the last five years. The study follows PRISMA guidelines and uses the PICO framework to determine inclusion and exclusion criteria. The keywords used in the search involve the operator's boolean: "Factor" AND "Pending Claim" OR "Return" AND "BPJS" OR "JKN". From the search results, 20 articles were found that were worthy of systematic review. The results of the review showed that 17 articles identified internal factors as the main cause of pending claims, especially the incompleteness of supporting documents such as laboratory results, ECG, and chronological letters. In addition, 19 articles revealed external factors as the main cause, namely lack of thoroughness and responsibility in carrying out tasks, such as data input errors, incompleteness in filling out medical resumes, and inaccuracies in diagnostic or action codes. Internal and external factors influence each other, so the right solutions are needed for every aspect, including improving the quality of documentation, improving the e-claim information system, and developing human resource competencies through training.

Abstract

This study aims to identify the extent to which the Health Center is ready for the implementation of Electronic Medical Records. The methodology applied in this study is Literature Review. The data sources used are secondary data collected from various literature sources available on the internet. The journal used is a reputable publication and focuses on the theme of readiness to implement electronic medical records at the Health Center. The collection of journals was carried out with inclusion criteria that included Population, Intervention, Comparator, Results, Time, Study Design, and Language, so that the total journals analyzed in this study reached 10 Journals. The readiness in the implementation of electronic medical records seen from the aspect of human resources in the Puskesmas shows that they are not fully prepared, because there are still several challenges such as the shortage of medical personnel responsible for medical records and the mismatch between the educational background of employees and the tasks carried out. In terms of cost, the Puskesmas is quite ready for the implementation of electronic medical records and no obstacles were found in terms of funding. In terms of materials, the Puskesmas is not fully prepared, because there are still several problems faced, such as the lack of computers, network problems that are often unstable, and system disruptions that cause errors. In the mechanical aspect, the health center showed quite good readiness, with the results of the study showing that the health center developed more electronic medical record systems using the waterfall approach. However, there are no health centers that implement an electronic system for RME developed by the Ministry of Health in this study. In terms of methodology, the Puskesmas is quite ready, which can be evidenced by the existence of Standard Operating Procedures (SOP) for the implementation of Electronic Medical Records in several Puskesmas.

Abstract

Hospitals as health service providers are responsible for ensuring the storage of medical records in accordance with regulations, both in conventional and electronic form. At Mitra Sehat Situbondo Hospital, the process of transitioning from conventional to electronic medical records is ongoing, but has not yet been fully implemented. Some of the obstacles faced include delays in digitalization and no clear operational procedures (SOP). This research aims to evaluate the management of medical records in support of digitalization, using a qualitative descriptive approach involving observation and interviews. The research results show that the use of RME at Mitra Sehat Hospital has only been implemented in outpatient settings, while scanning and digitizing medical records is not yet optimal. Digitalization of conventional medical record files is carried out through a scanning and retention process to reduce the accumulation of files in the filing unit. Storage efficiency and security of physical documents that indicate damage, loss and natural factors can be maintained with digital files. Through this research, it is hoped that Mitra Sehat Hospital can design features that support media transfer to ensure consistent medical records and ensure an efficient data migration process.
