Abstract

Emergency healthcare services play a crucial role in patient survival, particularly in emergency departments (ED) where timely and accurate decision-making is vital. This study aims to analyze the personal, behavioral, and environmental factors that influence the accuracy of patient management in emergency departments, using the Social Cognitive Theory framework. An observational, cross-sectional study was conducted among 107 emergency nurses from all hospitals in Toraja, selected using total sampling. Independent variables included personal, behavioral, and environmental aspects, while the dependent variable was patient management accuracy. Data were collected using a validated questionnaire and analyzed through univariate, bivariate, and multivariate linear regression using statistical application. The study was approved by the Health Research Ethics Committee (Approval No: 0723419/EC/KEPK/I/05/2025). Results indicated an R-squared value of 0.502, suggesting that 50.2% of the variance in patient management accuracy could be explained by the three independent variables. Multivariate analysis revealed that the behavioral factor had the most significant influence (p=0.000; B=0.567). Behavioral factors play the most significant role in determining the accuracy of patient management in emergency departments, followed by environmental and personal factors. Strengthening clinical skills, teamwork, and adherence to protocols is essential to improving emergency care outcomes. Hospitals should prioritize continuous training, competency-based evaluations, and supportive work environments to enhance behavioral performance among emergency nurses. Future research is recommended to employ longitudinal designs to understand causal pathways better and to include broader healthcare roles beyond nursing.

Abstract

One option for community health services is the community health center (Puskesmas). Puskesmas can adaptably enhance health services by using the Regional Public Service Agency Financial Management Pattern (PPK-BLUD). However, it will affect health care by increasing the desire of health workers to leave if it is not backed by effective human resource management. The purpose of this study is to ascertain how health workers' intentions to leave their jobs relate to their workload, job satisfaction, and wage satisfaction. Using all 418 BLUD contract workers from 37 Puskesmas in Kediri Regency as the population, a cross-sectional design was employed. 204 participants who satisfied the inclusion criteria namely, having been employed as a contract employee at the Kediri Regency Health Center for more than six months were selected through the use of a straightforward random sample procedure. Data were gathered by a questionnaire in December 2023. Multiple linear regression was employed in the data analysis. The results showed that there was a significant relationship between workload (p-value = 0,001) and job satisfaction (p-value = 0,043) with turnover intention; however, salary satisfaction was not associated with turnover intention (p-value = 0,169). Long-term organizational success depends on effective planning as a method to enhance service quality. Puskesmas can hire BLUD contract workers based on employee payroll needs and strengths by implementing PPK-BLUD.

Abstract

Background: Chronic Energy Deficiency (CED) is one of the health problems that often occurs in pregnant women in various countries, especially in regions with low economic levels and vulnerable communities. Purpose: The purpose of this study is to examine the influence of socioeconomic and cultural contextual factors on the occurrence of CED in pregnant women. Methods: The research design is explanatory research with a cross-sectional approach. The research sample consists of 200 pregnant women with CED and 200 pregnant women without CED in Madura. The dependent variable in this study is the incidence of CED, while the independent variables are regional conditions, access to health services, parity, food availability, and CED preventive behavior. Data analysis in the study is univariate and linear regression with a multilevel approach using SPSS 23. The research ethics certificate 001867/EC/KEPK/I/II/2024. Results: Level 1 is rural and urban areas, access to health services, parity, food availability, and preventive behavior for CED. Level 2 is the subdistrict level. Subdistrict conditions have a contextual influence on the variation in the occurrence of CED in pregnant women, namely significant values in rural and urban areas (p=0.009), significant values in access to health services (p=0.013), significant values for parity (p=0.022), significant values for food availability (p=0.049), and significant values for CED preventive behavior (p=0.029). Conclusion: The highest prevalence of CED was found in coastal areas, followed by villages, and the lowest in cities. Women with low parity (primiparas) are not entirely free from the risk of CED. Conversely, pregnant women in urban areas tend to have better access to health facilities, nutritional information, and a variety of foods, resulting in a relatively lower risk of CED.
