Abstract

Limboto Lake is a critically threatened lake experiencing declining environmental quality due to sedimentation and heavy metal contamination, including cadmium (Cd). This study aims to identify the Cd content in paddy soil and rice from agricultural fields surrounding the Limboto Lake watershed. This research employed a descriptive quantitative approach conducted at three stations (Bolihuangga, Hunggaluwa, and Dutulanaa). Sampling was carried out using a composite method to ensure field-representative data, followed by Cd analysis by Atomic Absorption Spectrophotometry (AAS). The results showed that Cd was detected in all paddy soil samples, with the highest to lowest concentrations found at Bolihuangga Station (0.29 ppm), Hunggaluwa Station (0.24 ppm), and Dutulanaa Station (0.23 ppm), respectively, all of which remained below the CCME quality standard of 0.6 ppm. In the rice samples, Cd was also detected, with the highest concentrations at Hunggaluwa Station (0.0024 mg/kg), followed by Bolihuangga (0.0020 mg/kg) and Dutulanaa (0.0019 mg/kg), all of which complied with the SNI 6128:2020 standard of 0.1 mg/kg. Although these levels are below the permissible thresholds, the presence of Cd indicates that heavy metals are entering the food chain. Given the persistent and bioaccumulative nature of Cd, this heavy metal will continue to accumulate in the environment and build up in body tissues. Regular environmental monitoring is required. It is recommended that communities and farmers reduce the excessive use of phosphatased chemical fertilizers and transition to organic fertilizers or to more sly friendly agricultural methods to prevent long-term accumulation of hebemetilsan paddy fields.

Abstract

The quality of healthcare services plays a crucial role in determining patient satisfaction, especially among BPJS Kesehatan users at primary healthcare facilities. This study aims to examine differences in service quality and healthcare workers’ workload and their relationship with patient satisfaction at the Kota Tengah Health Center and Medy’s Primary Clinic in Gorontalo City. This quantitative study employed an associative analytic design involving 324 respondents selected through accidental sampling. Data were collected using structured questionnaires that had undergone validity testing using item–total correlations (items accepted when r-count > r-table, p < 0.05) and reliability testing using Cronbach’s Alpha, with coefficients ≥ 0.70 indicating acceptable internal consistency. Data were analyzed using appropriate statistical tests, including bivariate correlation and t-tests. The findings revealed significant differences in service quality, workload, and patient satisfaction between the two healthcare facilities. The Kota Tengah Health Center demonstrated superior service quality, particularly in the dimensions of patient safety, effectiveness, and timeliness. At the Health Center, patient-centeredness was the strongest predictor of patient satisfaction (r = 0.891; R² = 79.4%). In contrast, at Medy’s Primary Clinic, healthcare workers’ workload had the greatest influence on patient satisfaction (r = 0.611; R² = 37.4%). A t-test further indicated that overall patient satisfaction was significantly higher at the Health Center (p = 0.000). Both service quality and healthcare workers’ workload significantly influence patient satisfaction.

Abstract

Patient satisfaction is a critical indicator in assessing the quality of healthcare services, as it influences patient loyalty and the sustainability of service utilization. This study aimed to evaluate the impact of the implementation of Integrated Primary Care (ILP) on the quality of healthcare services at Pilolodaa Primary Health Center. This study employed a qualitative research design with a case study approach. This qualitative study included nine purposively selected informants. Data were obtained through in-depth interviews and analyzed using thematic analysis with triangulation. Data were collected through in-depth interviews, direct observation, and document review. The data were then analyzed thematically using the Input–Process–Output–Impact (IPO-D) evaluation. The findings indicate that the implementation of ILP had a positive impact on service quality, as evidenced by improved cross-program coordination, enhanced service workflows, increased ease of access to healthcare services, and higher levels of patient satisfaction. However, several challenges remained, including limitations in human resources, increased digital administrative workload, and disparities in infrastructure and facilities. The implementation of ILP contributes positively to improving healthcare service quality and patient satisfaction at the primary care level. Nevertheless, addressing human resource constraints, administrative burdens, and infrastructural inequalities is essential to ensure the sustainability and optimal effectiveness of ILP implementation.

Abstract

Pharmaceutical services are a critical component of healthcare delivery in ensuring patient safety, particularly at Community Health Centers (Puskesmas). Suboptimal pharmaceutical service quality may increase the risk of medication errors and adverse drug events, thereby compromising patient safety. However, empirical evidence regarding the contribution of pharmaceutical service quality to patient safety in primary healthcare settings remains limited. A quantitative cross-sectional study was conducted at the Sipatana Community Health Center, Gorontalo City, Indonesia, in November 2025. The study population comprised patients receiving pharmaceutical services, with 295 respondents selected through purposive sampling. Data were collected using a structured questionnaire based on national pharmaceutical service standards. The instrument demonstrated acceptable validity and reliability, with item–total correlations above the critical value and Cronbach’s alpha coefficients exceeding 0.60. Data were analyzed using the Chi-square test and stepwise logistic regression at a significance level of p < 0.05. The results showed that all dimensions of pharmaceutical service quality were significantly associated with patient safety. Management of pharmaceutical supplies and consumable medical materials, prescription review, prescription services, drug information services, prevention of medication errors, prevention of adverse drug events, and patient medication adherence were significantly associated with patient safety (p = 0.000). Monitoring of adverse drug reactions also showed a significant association (p = 0.004). Simultaneously, pharmaceutical service quality had a significant effect on patient safety (F = 23.171; p = 0.000) with an R-square value of 0.326. Stepwise regression analysis indicated that management of pharmaceutical supplies and consumable medical materials was the strongest predictor of patient safety (B = 0.284; β = 0.278). The quality of pharmaceutical services plays a significant role in ensuring patient safety at the Sipatana Community Health Center. Management of pharmaceutical supplies and consumable medical materials emerged as the primary predictor of patient safety. Strengthening pharmaceutical supply management, improving the competence of pharmaceutical personnel, and optimizing strategies for preventing medication errors and adverse drug events are essential to enhance patient safety in primary healthcare settings.
