Abstract

Abstract. Leptospirosis is still becoming a public health problem in Indonesia. Banyumas was one of the highest cases in Central Java by 2019 so it could be potentially endemic. GIS (Geographic Information System) is used to determine spatial patterns related to the environment. This research aimed to know the distribution and spatial grouping of leptospirosis in Banyumas 2019. The type of this research is an observational study with a cross-sectional spatial analysis design to observe the spreading and grouping pattern. The subjects of this study were 140 leptospirosis cases in Banyumas 2019. House coordinate was collected by using GPS (Global Positioning System). The data collection is done for a month. Data Analyzes was performed through ArcGIS 10.2, and SaTScan 9.7. The distribution of leptospirosis in Banyumas was spread over 14 districts, 45% cases in Cilongok, 25,71% cases were >56 years old, 62,1% cases were male, 40% cases were farmers. The results of the spatial analysis showed 77.14% cases in residential land use areas, 70% cases with moderate population density (5.00-1.249 people/km²), 62.85% cases in 0-199 altitude, 63.57% cases with low rainfall 500 meters, and significant grouping pattern with p-value = 0.009 primary which is located in Cilongok and Ajibarang. Leptospirosis spread over in residential land use areas, moderate population density, low altitude, low rainfall, no history of flooding, a radius of river 500 meters, and occurs clustering in Cilongok and Ajibarang. The location intervention of leptospirosis prevention and control can be prioritized in these areas. Abstrak. Leptospirosis masih menjadi masalah kesehatan masyarakat di Indonesia. Banyumas merupakan salah satu wilayah yang memiliki kasus tertinggi di Jawa Tengah pada tahun 2019 sehingga berpotensi terjadinya endemis. GIS (Geographic Information System) berguna untuk mengetahui pola spasial penyakit yang berkaitan dengan lingkungan. Penelitian ini bertujuan mengetahui distribusi dan pengelompokkan leptospirosis secara spasial di Banyumas Tahun 2019. Jenis penelitian ini adalah studi observasional dengan desain analisis spasial cross-sectional untuk mengamati pola penyebaran dan pengelompokkan kasus. Sampel yang dikumpulkan adalah 140 kasus leptospirosis di Banyumas tahun 2019. Pengumpulan data koordinat rumah menggunakan Global Positioning System (GPS). Pengambilan data dilakukan selama 1 bulan. Analisis data dilakukan melalui ArcGIS 10.2, dan SaTScan 9.7. Hasil penelitian menunjukkan distribusi leptospirosis di Banyumas tersebar di 14 kecamatan, 45% kasus berada di Cilongok, 25,71% mayoritas penderita berumur >56 tahun, 62,1% laki-laki, dan 40% bekerja sebagai petani. Hasil analisis spasial menunjukkan 77,14% mayoritas penderita berada pada lahan pemukiman, 70% kepadatan penduduk sedang (5.00-1.249 jiwa/km²), 62,85% ketinggian 0–199 mdpl, 63,57% curah hujan rendah 500 meter, dan pola cluster terindentifikasi signifikan secara statistik dengan nilai p-value = 0,009 cluster primer berlokasi di Cilongok dan Ajibarang. Kejadian leptospirosis cenderung menyebar di tata guna lahan pemukiman, kepadatan penduduk sedang, ketinggian tempat rendah, curah hujan rendah, tidak ada banjir, radius sungai 500 meter, dan terjadi kluster di Cilongok dan Ajibarang. Lokasi intervensi pencegahan dan pengendalian leptospirosis dapat diprioritaskan daerah tersebut.

Abstract

Talasemia adalah penyakit kelainan darah akibat kekurangan atau penurunan produksi hemoglobin. Jumlah penderita talasemia di Yayasan Talasemia Indonesia cabang Banyumas terus meningkat. Tujuan penelitian ini untuk mendeskripsikan karakteristik penderita talasemia di Yayasan Talasemia Indonesia cabang Banyumas tahun 2012. Penelitian kuantitatif dengan desain penelitian cross sectional ini dilakukan terhadap 64 sampel yang diambil dengan teknik proportionate stratified random sampling . Data dikumpulkan meliputi melalui wawancara terstruktur menggunakan kuesioner. Metode analisis yang digunakan adalah analisis deskriptif univariat. Hasil penelitian menunjukkan bahwa penderita talasemia rata-rata berusia 12,28 tahun, berjenis kelamin laki-laki (51,6%), sedang sekolah SD (40,6%), bukan angkatan kerja (92,2%), talasemia β-mayor (90,6%), tidak splenektomi (84,4%), jaminan kesehatan masyarakat (Jamkesmas) (73,4%), kadar feritin ≥ 2000 ng/mL (90,4%), kelasi deferioprone, vitamin C, dan deferioksamin (81,2%), tinggal di Kabupaten Banyumas (79,7%), trans-fusi darah di Rumah Sakit Umum Daerah (RSUD) Banyumas (95,3%), rata-rata terdiagnosis usia 3,7 tahun, usia rata-rata mulai transfusi darah adalah 3,78 tahun. Frekuensi transfusi darah 1 bulan 1 kali (87,5%), dan frekuensi terapi kelasi zat besi mengonsumsi vitamin C 30 kali dalam 1 bulan (95,3%). Thalassaemia is blood disorder caused by deficiency or decrease in production/formation of hemoglobin. Number of thalassaemia patients in Yayasan Talasemia Indonesia (Indonesian Thalassemia Foundation) Banyumas branch countinue to increase. The research purpose is to describe the characteristics of thalassemia patients in YTI Banyumas. The research type is a quantitative approach with cross sectional research design. Sample of 64 people taken by proportionate stratified random sampling. Data collected includes characteristics of the respondents based on the variables of people, places, and time was obtained through interviews using a questionnaire. The univariate descriptive analysis were use to describe the characteristics of the respondents. The results showed that thala-ssaemia patients at YTI Banyumas have several characteristics: average age of 12,28 years, male (51,6%), elementary students (40,6%), the labor force (92,2%), b-thalassemia major (90,6%), no splenectomy (84,4%), Jamkesmas (73,4%), ferritin levels ≥ 2000 ng/mL (90,4%), sailor deferio-prone, vitamin C, and deferioksamin (81,2%), live in Banyumas (79,7%), blood transfusions in hospitals Banyumas (95,3%), the average age of diagnosis of 3,7 years, the average age of starting a blood transfusion was 3,78 years. Once a month blood transfusion (87,5%), and the frequency of iron chelation therapy by consuming vitamin C 30 times in one month (95,3%).

Abstract

Introduction: The problems that post-stroke patients still face after being hospitalized are weakness in one side of the body (hemiparesis), speech disorders (dysarthria/aphasia), difficulty swallowing (dysphagia) and psychological disorders such as post-stroke depression and the risk of recurrent attacks. The purpose of the service is to improve the knowledge and skills of partners about: early detection of stroke, post-stroke care, post-stroke physical exercise, post-stroke speech therapy, post-stroke swallowing therapy, post-stroke psychological management. The method carried out is training with lectures, discussions and demonstrations, the media used are modules and videos. The participants were elderly Posyandu cadres and families who took care of family members after a stroke totaling 19 people. The success of the training was evaluated by comparing pre and posttest scores and assessing the participants' ability to demonstrate the actions taught. The results obtained by the knowledge of early detection, psychological management and post-stroke care of participants were 100% improved. Skills about early detection of stroke, ROM exercises, swallowing exercises and speech therapy 89.47% of participants improved. Conclusion: The training is effective in improving the knowledge and skills of cadres and families in caring for post-stroke patients. Key word: post-stroke, training, cadres, family, care

Abstract

Sampai dengan tahun 2013 dilaporkan ada 13 kasus leptospirosis di Kabupaten Banyumas.Terjadinya peningkatan kasus selama dua tahun terakhir perlu mendapat perhatian dari berbagai pihak agar kasus leptospirosis bisa segera ditangani. Tujuan penelitian ini adalah memetakan kasus leptospirosis dan menganalisis faktor risiko lingkungan dan perilaku yang memengaruhi leptospirosis. Penelitian ini menggunakan desain studi observasional kasus kontrol. Pengumpulan data dilakukan dengan wawancara, observasi, dan pengukuran. Analisis data menggunakan analisis spasial, analisis univariat, dan bivariat. Kasus adalah penderita leptospirosis berjumlah 13 orang dan kontrol adalah tetangga kasus yang tidak menderita leptospirosis berjumlah 52 orang. Hasil pemetaan menunjukkan kasus leptospirosis di Kabupaten Banyumas termasuk daerah aliran sungai dengan radius 600 meter ke sungai; seluruh kasus dekat dengan sawah (jarak < 1 km); sebagian besar memiliki vegetasi ³ 3 jenis dan berada di daerah dengan curah hujan tinggi. Faktor lingkungan yang terbukti berhubungan dengan leptospirosis adalah kondisi jalan yang buruk sekitar rumah (OR = 4,90; CI 95% = 1,35 - 17,10). Faktor perilaku yang berhubungan dengan leptospirosis adalah kebiasaan mandi/mencuci di sungai (OR = 4,35; 95% CI = 1,21 - 15,60), riwayat peran serta dalam kegiatan sosial yang beresiko (OR = 12,00; 95% CI = 1,45 - 99,09) dan penggunaan alat pelindung diri (OR = 7,50; 95% CI = 1,00 - 62,18). There were 13 Leptospirosis cases in Banyumas in 2013. The increasing incidents in last 2 years should get an attention from all institutions. The study conducted was to map incidence of leptospirosis and analyze the environmental and behavior risk factors which associated leptospirosis case. This research was observational by case-control approach. The data was collected through interviews, observation and test. For analyzing the data, the researcher applied spatial analysis, univariate, and bivariate analysis. It was 13 leptospirosis cases and 52 people as the controls; they are the neighbors who are free from leptospirosis.The mapping showed that leptospirosis cases in Banyumas were along the riverin 600 meters radius, all cases with the distance of 0 - 1 km into the rice field, had vegetation 3 species and those were in areas with high rainfall intensity. Environmental risk factor associated with leptospirosis was a bad road conditions around the house (OR = 4,90; CI 95% = 1,35 - 17,10). Behavior risk factors werethe bathing/washing habit in the river (OR = 4,35; 95% CI = 1,21 - 15,60, a history of participation in social activities (OR = 12,00; 95% CI = 1,45 - 99,09)and the use of personal protective equipment (OR = 7,50; 95% CI = 1,00 - 62,18).

Abstract

Pada dua tahun pertama kehidupan penderita talasemia mayor, umumnya menderita anemia dan membutuhkan tranfusi darah. Penelitian ini bertujuan membuat model prediksi kebutuhan darah bagi penderita talasemia mayor. Penelitian observasional dengan desain studi pendekatan potong lintang ini dilakukan pada sampel 79 penderita talasemia mayor yang melakukan transfusi rutin minimal satu bulan satu kali di Rumah Sakit Umum Banyumas, selama tahun 2012. Analisis regresi linier ganda digunakan untuk membuat model prediksi. Hasil penelitian menunjukkan bahwa 80,7% kebutuhan darah penderia talasemia mayor dijelaskan oleh variabel usia, berat badan, dan kadar hemoglobin sedangkan 19,3% dijelaskan oleh sebab-sebab yang lain. Rumus prediksi menyatakan setiap kenaikan usia 1 tahun maka kebutuhan darah akan bertambah sebanyak 0,816 mililiter dan setiap kenaikan 1 kilogram berat badan maka kebutuhan darah akan bertambah 13,4 mililiter serta apabila kadar hemoglobin mengalami penurunan 1 g/dL maka kebutuhan darah akan bertambah sebesar 81 mililiter. Patients with thalassemia major usually present within the first two years of life with severe anemia, need red blood cell transfusion. The objective of this study was to create a prediction model of blood transfusion need for patients with thalassemia mayor. This type of research was observational with cross sectional design. Samples are 79 patients with thalassemia major who perform routine transfusion at least once in a month at Banyumas hospital in 2012. Multiple linier regression analysis was used to create the model. The results showed that 80.7% blood requirements can be explained by variables of weight, haemoglobin level and age, while 19.3% is explained by other causes. Prediction formula states every increment of one year in age, the need for blood will increase by 0.816 millilitres and every increment of one kilogram of body weight, the need for blood will increase 13.4 millilitres, and when the haemoglobin level decreased 1 gr/dL the need for blood will increase by 81 millilitres.

Abstract

Kabupaten Banyumas merupakan kabupaten endemis malaria di Provinsi Jawa Tengah. Data Dinas Kesehatan Kabupaten Banyumas tahun 2008 - 2013 menunjukkan angka annual paracite incidence (API) yang selalu mengalami fluktuasi. Dari 27 kecamatan, 22 di antaranya termasuk dalam kategori medium case incidence (MCI) dan low case incidence (LCI). Faktor lingkungan, perilaku, sosial ekonomi, dan iklim berdampak pada tinggi rendahnya kejadian malaria. Tujuan penelitian adalah mengetahui faktor yang berhubungan dengan API di Kabupaten Banyumas. Data dikumpulkan dari Januari 2011 - Desember 2013 menggunakan penelitian analitik dengan rancangan penelitian potong lintang dan menggunakan data sekunder dari Dinas Kesehatan, Dinas Pertanian, dan Badan Pusat Statistik Kabupaten Banyumas. Populasi adalah seluruh kecamatan di Kabupaten Banyumas dan sampel diambil menggunakan teknik total sampling, sebanyak 27 kecamatan dengan pengamatan selama 3 tahun menjadi 81 sampel. Hasil penelitian menunjukkan 44,4% pengamatan termasuk kategori LCI dan MCI, 48,1% termasuk kategori curah hujan tinggi, 49,4% termasuk kategori wilayah yang luas, 49,4% termasuk kategori jumlah pendatang tinggi, 48,1% termasuk kategori kepadatan penduduk sedang. Sementara itu, dari 27 kecamatan, yang termasuk ketinggian rendah adalah 63,0%. Faktor yang terbukti berhubungan dengan API adalah luas wilayah, jumlah pendatang, kepadatan penduduk, sedangkan yang tidak berhubungan adalah curah hujan dan ketinggian. Banyumas is malaria endemic district in Central Java. Banyumas Health Office data of 2008 - 2013 showed that, the Annual Parasite Incidence (API) always fluctuated. From 27 subdistricts in Banyumas, there are 22 subdistricts which fall into the category of middle case incidence (MCI) and low case incidence (LCI). Malaria is a disease that closely associated with the enviroment, behaviour, social economy, and climate. The purpose of this study was to determine factors associated with API in Banyumas. Data were collected from Januari 2011 - Desember 2011 using an analytic crosssectional design using secondary data from Banyumas Health Office, Agriculture Office and Statistic Center. The population in this study were all subdistricts in Banyumas and samples were taken using total sampling technique. The sample of this study was 27 districts. The results showed that from 81 samples we obtained 44.4% of observations included in LCI and MCI category, 48.1% with high rainfall, 49.4% with large areas, 49.4% with high number of entrants, 48.1% with medium population density. Meanwhile, from 27 districts, 63.0% included in low altitude category. Factors associated with API in Banyumas were the extensive areas, the number of entrants, and population density. The factors that are not associated to the API were the rainfall and altitude regions.

Abstract

The transmission of Covid-19 to health workers is an important issue in the pandemic era. Health workers are the front line in handling Covid-19 so they are very vulnerable to being exposed to and even contracting Covid-19 disease. It is necessary to conduct an assessment of the factors that influence the transmission of Covid-19 to health workers, especially in the Southeast Asian Region. The aim of the systematic review is to examine the factors that influence the transmission of Covid-19 cases to health workers in Southeast Asia. The research journal database was retrieved by accessing PubMed and Google Scholar, and using the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) method using the keywords “risk factors” AND “transmission” AND “Covid-19” AND “health workers”. After going through a series of selections, 11 research journals of 241 journals found that were published in 2020-2023 and were considered relevant for study. Based on the results and discussion of the systematic review, it is known that the factors that influence the transmission of Covid-19 to health workers in Southeast Asia are: average age, type of profession, comorbid history, travel history, community contact at work, contact with Covid-19 patients, adherence to the use of PPE and implementing health protocols, levels of workload, levels of stress and immunization status of health workers.

Abstract

Keterbatasan dalam segi kognitif, fisik dan motorik pada Anak Berkebutuhan Khusus dapat menimbulkan masalah pada gigi dan mulut sehingga mempengaruhi status kesehatan gigi dan mulut. Kelainan erupsi gigi, kerusakan lingkungan rongga mulut, karies gigi dan penyakit periodontal adalah gangguan kesehatan gigi dan mulut yang sering dijumpai. Tujuan penelitian ini adalah untuk mengetahui gambaran terkait permasalahan kesehatan gigi dan mulut pada anak berkebutuhan khusus. Metode yang digunakan merupakan studi literatur tahun 2014 sampai 2023 dalam bahasa Indonesia dan bahasa Inggris. Data yang diolah diperoleh dari database science direct,  pubmed dan, google scholar, dengan keywords gangguan kesehatan gigi dan mulut, karies, gingivitis, kelainan periodontal, anak berkebutuhan khusus. Hasil tinjauan Studi literatur ini didapatkan 17 artikel dengan kriteria inklusi dan eksklusi, diketahui adanya gangguan kesehatan gigi dan mulut berupa karies dan penyakit periodontal yaitu gingivitis pada anak berkebutuhan khusus. Kejadian karies pada anak downsyndrome lebih rendah karena erupsi gigi yang terlambat. Pada anak tunagrahita angka kejadian karies lebih tinggi karena keterbatasan dalam membersihkan area gigi dan mulut. Kondisi gingiva pada anak-anak tunagrahita (disabilitas intelektual) menunjukkan hasil yang tidak lebih baik daripada kondisi anak dengan keterbatasan fisik.

Abstract

Background: Studies on anemia have so far focused more on the incidence of anemia in pregnant women and adolescent girls even though farmers are one of the vulnerable groups suffering from anemia. The incidence of anemia in farmers has not been well documented, and there is still very limited research related to the determinants of anemia incidence in farmers. Objective: The study aim to know risk factor anemia in the farmers around the world Method: This study used the literature review method by collecting journals from google scholar, PubMed, and cienceDirect. Data collection using Anaemi A keywords Anemia AND farmers AND risk faktor. The results of the livelihood obtained a total of 4,959 articles which were then carefully selected so that 8 of the most relevant articles were obtained for article review. Result: The results showed that there are three main faktors that cause the incidence of anemia in farmers, namely individual factors, environmental factors, and sociodemographic factors. Conclusion: Risk factor of anemia in the farmers is individual factors, environmental factors, and sociodemographic factors Keyword: Anemia, Farmers, Risk Factor

Abstract

Background: The incidence of pulmonary tuberculosis (pulmonary TB) increases every year in Indonesia. Various supporting risk factors influence the increase in the number of the diseases. Risk factor analysis needs to do to find out the kind of risk factors that can increase the incidence of pulmonary tuberculosis in Indonesia Objectives: This research is qualified to analyze various factors that risk increasing the incidence of pulmonary TB in Indonesia. Research Metodes: 2015-2023 Literature Study in English and Indonesian. Data were obtained from databases including ScienceDirect, PubMed, ResearchGate, and Google Scholar with keywords Risk factors for pulmonary tuberculosis, tuberculosis in Indonesia, and tuberculosis. Results: Obtained 15 articles according to the topic of the risk factor of pulmonary TB in Indonesia caused by several dominating factors such as the environment including ventilation, temperature, and type of floor, male is riskier than female, distance and accessibility of health services and latest education. Conclusion: Physical environment, gender, accessibility in health service facilities, and the latest education is the main risk factor that causes high morbidity due to pulmonary TB in Indonesia.
