Abstract

Preeclampsia remains a leading cause of maternal and neonatal morbidity, with increased risk observed at both extremes of maternal age adolescents (<20 years) and women of advanced maternal age (>35 years). However, the comparative obstetric impacts of preeclampsia across these age groups remain underexplored in a structured synthesis. This study aims to provide an up-to-date, evidence-based understanding to support maternal health policy planning, clinical obstetrics, and reproductive education tailored to high-risk populations. This systematic review was conducted following PRISMA guidelines. Electronic databases including PubMed, Scopus, and Google Scholar were searched for peer-reviewed studies published between 2015 and 2025. Studies were eligible if they reported preeclampsia prevalence and obstetric outcomes among pregnant individuals aged <20 or >35 years. Seven observational and cohort studies met the inclusion criteria. Adolescent pregnancies complicated by preeclampsia showed high cesarean section rates (82.9% in those aged ?16 and 89.3% in ages 17–19), and increased risk of anemia and pregnancy complications despite comparable neonatal outcomes across age subgroups. In contrast, advanced maternal age was associated with a higher prevalence of severe preeclampsia (68.9%), eclampsia (12.3%), HELLP syndrome (8.7%), and emergency cesarean delivery (71.3%). Neonatal complications included low birth weight (34.4%) and lower five-minute Apgar scores. Use of assisted reproductive technologies in women ?40 further amplified the risk of early-onset preeclampsia and prematurity. Pregnancies at non-ideal maternal ages carry distinct pathophysiological risks and result in increased obstetric complexity and need for medical intervention. Age-specific clinical approaches such as early screening, targeted antenatal education, and enhanced maternal-fetal surveillance are essential. These findings support maternal health policies that incorporate maternal age as a key determinant for individualized risk assessment and care planning.

Abstract

Asma merupakan penyakit kronis yang ditandai dengan inflamasi dan penyempitan saluran napas, yang menyebabkan sesak napas, batuk, dan mengi. Prevalensi kejadian asma di perkotaan lebih tinggi dibandingkan pedesaan. Namun, beberapa penelitian menunjukkan bahwa penduduk di wilayah pedesaan juga menghadapi risiko signifikan terhadap penyakit asma. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang memengaruhi kejadian asma di wilayah pedesaan secara global. Penelitian ini menggunakan desain literature review dengan pendekatan PRISMA. Pencarian artikel melalui Google Scholar, PubMed, dan ScienceDirect, hingga diperoleh delapan artikel yang sudah sesuai kriteria penelitian. Berdasarkan hasil literature review ditemukan bahwa terdapat beberapa faktor yang memengaruhi kejadian asma di pedesaan, antara lain obesitas, polusi udara seperti paparan asap rokok, asap hasil pembakaran kayu bakar, dan asap dari penggunaan obat nyamuk bakar, udara dingin, riwayat genetik, serta alergi. Alergi merupakan penyebab yang paling umum ditemukan di lingkungan pedesaan.

Abstract

Indonesia is a tropical climate country with one of its characteristics having high rainfall and lasting relatively long. High rainfall levels cause waterlogging which has the potential to increase the number of mosquito populations. The purpose of this literature review is to determine the effect of weather conditions, namely rainfall and air temperature on the possibility of a malaria outbreak. The method used is to review selected articles with a search engine via Google Scholar, PubMed, and ScienceDirect by entering the keywords rainfall, climate and malaria. The search process to exclude articles used for this literature review uses the PRISMA method. And the results of the literature review show 11 articles discussing weather conditions that affect mosquito populations in Indonesia (Yogyakarta, Bengkulu and Ambon), India, Nigeria, Ethiopia, Mali, Uganda and South Africa. Research in Bengkulu City and Ambon City found a negative correlation between rainfall and temperature on malaria infections. In Limpopo Province, South Africa, data shows the seasonal dynamics of the role of rainfall identified where the December to February rainy season plays a major role in influencing malaria cases in the following year. In Oyam district, Uganda, a malaria outbreak was facilitated by aquatic habitats of Anopheles near houses created by human activities, following increased rainfall.

Abstract

Kebijakan pemberian bantuan dana bantuan operasional kesehatan ( BOK) dari pemerintah tujuannya untuk meningkatkan akses dan mutu pelayanan kesehatan masyarakat dalam menyelenggarakan upaya pelayanan kesehatan promotif dan preventif. Hanya saja dengan anggaran BOK yang sudah diberikan oleh Pemerintah untuk Puskesmas, realisasinya belum bisa terserap dengan optimal. Penelitian ini menggunakan jenis penelitian kualitatif dengan pendekatan studi kasus. Penelitian ini bertujuan untuk menggali lebih dalam terkait informasi implementasi kebijakan penggunaan dana BOK di Puskesmas Kabupaten Cilacap. Informan dalam penelitian ini didapatkan dengan menggunakan purposive sampling. Informan penelitian ini terdapat informan kunci dan pendukung. Sebagai informan kunci yaitu Kepala Puskesmas, Ka Sub Bag TU, Bendahara BOK, petugas pengelola perencanaan BOK di Dinas Kesehatan Kabupaten Cilacap. Implementasi kebijakan penggunaan dana BOK di Kabupaten Cilacap dari segi komunikasi dilakukan melalui koordinasi dengan pengelola BOK melalui Wa Grup, pertemuan offline dan onlain, dan supervisi langsung oleh Dinas Kesehatan. Hanya saja masih ditemukan adanya misskomunikasi terkait pemahaman penggunaan dana BOK. Terkait sumber daya di Kabupaten Cilacap, masih terjadi penumpukan tugas dikarenakan belum ada kebijakan khusus untuk tenaga pengelola BOK yang latar pendidikannya seperti ekonomi maupun akuntan. Disposisi terkait kebijakan BOK dirasa tepat hanya saja kurang fleksibel terkait petunjuk teknis penggunaan dana BOK itu sendiri. Dari struktur birokrasi di Kabupaten Cilacap tidak ada pembentukan birokrasi ataupun organisasi khusus terkait BOK. Namun, masih terdapat tata urutan dalam melaporkan hasil kegiatan terkait penggunaan dana BOK di Kabupaten Cilacap. Penggunaan dana bantuan operasional dalam pelaksanaannya tidak bisa fleksibel karena sudah ada petunjuk teknis yang ditetapkan.

Abstract

Stroke tidak hanya menjadi penyebab utama kematian dan kecacatan pada usia lanjut, tetapi kini menunjukkan tren peningkatan pada kelompok usia produktif. Penulisan artikel ini dilakukan dengan tujuan untuk mereview faktor risiko dominan yang berkontribusi terhadap kejadian stroke pada usia produktif di masyarakat Indonesia berdasarkan kajian literatur ilmiah terkini. disusun dengan pendekatan Literature review, menggunakan referensi dari publikasi ilmiah yang diterbitkan dalam rentang tahun 2016 hingga 2025. Seluruh sumber diperoleh melalui pencarian pada basis data Google Scholar, ScienceDirect, dan PubMed. Artikel dipilih melalui proses skrining, kriteria inklusi dan eksklusi. Hasil terhadap sepuluh artikel menunjukkan bahwa faktor risiko yang paling dominan mencakup faktor hipertensi, kurangnya aktivitas fisik, diabetes melitus, dan perilaku merokok. Faktor tambahan seperti stres, obesitas, konsumsi makanan berlemak, serta riwayat keluarga juga memiliki peran dalam kejadian stroke usia produktif. Kesimpulan dari kajian ini adalah bahwa kejadian stroke pada usia produktif merupakan hasil dari kombinasi kompleks berbagai faktor risiko klasik dan khusus. Oleh karena itu, pendekatan promotif dan preventif yang personal dan berbasis karakter individu sangat diperlukan untuk menurunkan angka kejadian stroke sejak usia muda.

Abstract

Background: Sumber Waras Hospital is committed to enhancing the quality of healthcare services through the implementation of an Electronic Medical Record (EMR) system. EMR is an essential tool for managing patient health data to improve service efficiency and effectiveness. However, its implementation still encounters various challenges, such as limited user training, interoperability issues, and inconsistent data quality. Therefore, this study aims to evaluate the success of the inpatient EMR system implementation at Sumber Waras Hospital using the Delone and McLean model approach. Research Methods: This research applied a descriptive quantitative method with a cross-sectional design. The study population consisted of 344 healthcare personnel, with a total of 77 respondents selected using the Slovin formula at a 10% margin of error. Result: The evaluation showed that system quality was categorized as moderate with an average score of 2.86. Information quality was also in the moderate category with a score of 3.15, followed by service quality at 3.14. System use was categorized as moderate with a score of 3.47, user satisfaction was categorized as good with a score of 3.50, and net benefits were categorized as moderate with a score of 3.30. Conclusion: The implementation of the inpatient EMR system at Sumber Waras Hospital is generally running quite well, although improvements are still required, particularly in system quality, information quality, and service quality aspects. The evaluation results are expected to serve as a reference for system improvements and user training efforts to enhance the overall quality of healthcare services. Abstract

Background: Stunting in Indonesia still has a high prevalence at 21.5% in 2023. Stunting is also caused by the mother's condition before marriage. Prospective brides still have low awareness about the importance of preventing stunting at a young age. Premarital health conditions can be influenced by various factors, including the type of ​​residence which may influence physical, social and economic environmental conditions. Purpose: To determine whether the regional category affects the status of prospective brides who run the risk of having children who are stunted. Method: A quantitative analytical study used a cross-sectional method and was conducted in June-July 2024 in Banyumas Regency, Central Java. The sampling technique used total sampling with a sample size of 3.399 respondents. The independent variables in this study were rural and urban areas of residence, while the dependent variable was stunting. Data analysis used univariate in the form of frequency distribution and bivariate using chi-square. Results: The chi-square test results showed a p-value of 0.216 (>0.050). This means that there is no significant relationship between rural and urban areas and the risk of stunting in prospective brides. Conclusion: The status of prospective brides by category puts them at risk of giving birth to stunted children, and this risk does not differ between rural and urban areas. Pre-marital and pregnancy education should be provided evenly across all regions, and assistance should be provided to prospective brides at risk to delay pregnancy until conditions are more ideal.   Keywords: Premarital Health; Stunting; Town; Village.   Pendahuluan: Stunting di Indonesia masih memiliki prevalensi yang tinggi, yaitu 21,5% pada tahun 2023. Stunting ikut dipengaruhi oleh kondisi ibu sejak sebelum menikah. Namun, calon pengantin wanita cenderung memiliki kesadaran yang rendah akan pentingnya pencegahan stunting sejak dini. Kondisi kesehatan pranikah dapat dipengaruhi oleh berbagai faktor, termasuk wilayah tempat tinggal yang memungkinkan adanya kondisi lingkungan fisik, sosial, dan ekonomi yang mempengaruhi. Tujuan: Untuk mengetahui apakah kategori wilayah tempat tinggal berhubungan dengan status calon pengantin wanita berisiko melahirkan anak stunting. Metode: Penelitian kuantitatif analitik menggunakan metode cross sectional, dilaksanakan pada bulan Juni-Juli 2024 di Kabupaten Banyumas, Jawa Tengah. Teknik pengambilan sampel menggunakan total sampling dengan jumlah sampel yang digunakan sebanyak 3399 responden. Variabel independen dalam penelitian ini adalah wilayah tempat tinggal perdesaan dan perkotaan, sedangkan variable dependen adalah stunting. Analisis data yang digunakan univariate dalam bentuk distribusi frekuensi dan bivariate menggunakan chi square. Hasil: Uji chi square menunjukkan p-value 0.216 (>0.050). Artinya, tidak terdapat hubungan yang signifikan antara wilayah perdesaan dan perkotaan dengan risiko stunting pada calon pengantin wanita Simpulan: Status calon pengantin wanita oleh kategori berisiko melahirkan anak stunting, dan risiko tersebut tidak berbeda antara wilayah perdesaan maupun perkotaan. Edukasi pranikah dan kehamilan secara merata di seluruh wilayah, serta pendampingan bagi calon pengantin berisiko untuk menunda kehamilan hingga kondisinya lebih ideal.   Kata Kunci: Kesehatan Pranikah; Perdesaan; Perkotaan; Stunting.

Abstract

Tuberculosis which shows resistance to various types of drugs poses a significant burden in efforts to prevent and control tuberculosis globally. Since 2020, guidelines from the WHO have recommended a shorter treatment with an all-oral regimen for Drug Resistant Tuberculosis (DR-TB), with the inclusion of bedaquiline instead of an injectable agent. However, the treatment success rate for DR-TB in Indonesia is still low. This study aimed to determine factors contributing to unsuccessful DR-TB treatment using STR in Central Java. This was a nested case-control study of 412 DR-TB patients enrolled with a Short-Term Regimen, registered in 2021 to 2023 who had treatment results in a subset of the Tuberculosis Information System cohort data. Independent variables analyzed included age, gender, patient employment status, history of previous TB treatment, DM status, HIV status, resistance pattern, initial sputum examination results, drug side effects, treatment initiation interval, body mass index, and BPJS ownership status. Statistical analysis was done using SPSS version 22 software, with logistic regression analysis to identify the determinants. The determinant of unsuccessful treatment outcome in Central Java Province which is 51.04% was the positive results of initial sputum examination of treatment (aOR=10.501; 95%CI=5.056-21.807), drug side effects (aOR=1.853; 95%CI=1,000-3.436), obesity (aOR=3.115; 95%CI=1.188-8.166) and BPJS non-possession status (aOR=2.213; 95%CI=0.932-5.255). More strategies are needed to improve the success of treatment with STR.

Abstract

Malaria is a communicable disease that poses a serious problem in Papua New Guinea, with the country recording the highest incidence of malaria in the Asia-Pacific region each year. However, research on the risk factors for malaria in Papua New Guinea is still minimal, especially among pregnant women. This study aims to investigate the correlation between malaria incidence in pregnant women in Papua New Guinea. This study is an observational study with a cross-sectional approach conducted on 200 pregnant women from January to March 2024 in Abou District, Papua New Guinea. Data collection used questionnaires and Rapid Diagnostic Tests (RDT) to diagnose malaria infection in pregnant women. The independent variables in this study are healthcare-seeking behavior, prevention of malaria, knowledge of malaria, maternal age, marital status, occupation, education level, and residential location. The dependent variable in this study is the incidence of malaria in pregnant women. In this study, the data analysis used included univariate analysis, bivariate analysis with chi-square, and multivariate analysis with logistic regression. The results showed that the risk factors for malaria incidence in pregnant women in Papua New Guinea are malaria prevention (p-value 0.014; OR= 4.426) and healthcare-seeking behavior (p-value 0.033; OR= 4.033), meaning that pregnant women with poor malaria prevention behavior and poor healthcare-seeking behavior are four times more likely to suffer from malaria. It is hoped that pregnant women will increase their awareness of malaria prevention during pregnancy and make full use of healthcare facilities during pregnancy.

Abstract

Abstract. German cockroaches in Indonesia have developed resistance to various insecticides. Although cases of german cockroach resistance have been reported in several areas in Indonesia, there have been no reports of such resistance cases in Purwokerto. The specific purpose of this study was to determine the resistance status of german cockroaches from three traditional markets (pasar) in Purwokerto to fipronil. The method used was the contact and bait method, using ten male cockroaches and repeated five times for each strain. The data obtained were analyzed using probit analysis to calculate the time of  death (Lethal Time), and then the resistance level was calculated. The results showed that the time of death (LT50) for field cockroaches using the contact method, the highest resistance level was obtained from the Pasar-1 strain with an LT50 value of 3.05 hours, while the lowest was in the Pasar-3 strain with an LT50 value of 1.83 hours. In testing with the bait method using gel bait containing 0.03% fipronil, the LT50 value of cockroaches from Pasar-1 was 14.16 hours, while cockroaches from Pasar-3 had an LT50 value of 8.02 hours. The resistance ratio value (RR50) calculation showed that all cockroaches from three traditional markets in the city of Purwokerto did not show resistance to fipronil which was tested with the resistance ratio value of all field strains below 1. Susceptible to insecticides with the active ingredient fipronil. The active ingredient fipronil in bait formulations may be used for monitoring and control of German cockroaches. Abstrak. Lipas jerman telah resisten terhadap berbagai macam insektisida, dibuktikan dengan adanya kasus resistensi lipas jerman yang dilaporkan terjadi di beberapa wilayah di Indonesia. Meskipun demikian, di wilayah Kota Purwokerto sampai saat ini belum ada laporan mengenai kasus resistensi tersebut. Tujuan khusus penelitian ini adalah mengetahui status resistensi lipas jerman dari tiga pasar tradisional yang ada di Kota Purwokerto terhadap fipronil. Metode yang digunakan adalah metode kontak dan umpan dengan menggunakan sepuluh ekor lipas jantan dan diulang lima kali untuk masing-masing strain. Data yang diperoleh dianalisis menggunakan analisis probit untuk menghitung waktu kematian (Lethal Time), untuk kemudian dihitung tingkat resistensinya. Hasil penelitian menunjukkan bahwa waktu kematian (LT50) lipas lapangan menggunakan metode kontak pada tingkat resistensitertinggi adalah dari strain Pasar-1 dengan nilai LT50 sebesar 3,05 jam, sedangkan terendah adalah strain Pasar-3 dengan nilai LT50 sebesar 1,83 jam. Hasil pengujian dengan metode umpan menggunakan gel bait mengandung fipronil 0.03% diperoleh nilai LT50 lipas asal Pasar-1 sebesar 14,16 jam, sedangkan lipas dari Pasar-3 mempunyai nilai LT50 sebesar 8,02 jam. Hasil penghitungan nilai rasio resistensi (RR50) menunjukkan bahwa semua lipas asal tiga pasar tradisional di Kota Purwokerto tidak menunjukkan resistensi terhadap fipronil yang diujikan dengan nilai rasio resistensi semua strain lapangan di bawah 1. Simpulan hasil penelitian ini adalah lipas strain pasar tradisional di Kota Purwokerto masih rentan terhadap insektisida berbahan aktif fipronil. Bahan aktif fipronil dalam formulasi umpan dimungkinkan untuk digunakan dalam monitoring dan pengendalian lipas jerman.
