Abstract

The COVID-19 virus that attacks the respiratory system, ranging from mild, and moderate, to death can affect anyone, including pregnant women. Vaccination has proven effective in dealing with the spread of COVID-19 cases. However, the majority of pregnant women are reluctant to receive vaccinations because of their fear. The research aims to determine the determinants of receiving COVID-19 vaccination in pregnant women to reduce their anxiety and be encouraged to be vaccinated. This study uses a literature review method from journals published between 2017-2022 in Science Direct, PubMed, and Google Scholar with the keywords acceptance, vaccine COVID-19, and pregnant. The total results from the literature study obtained 13 articles that met the inclusion criteria. The result of this literature review is that the low acceptance of COVID-19 vaccination is seen in young pregnant women, but in China itself, younger pregnant women receive the vaccine higher; exposure to negative information about vaccines; non-white race; religious beliefs in Ethiopia; lack of knowledge about the COVID-19 vaccine and evidence of vaccine safety in pregnant women, causing fear of the vaccine's effect on the fetus; perceived low susceptibility to COVID-19; low-income mothers; residence in rural areas; and a less motivating environment for COVID-19 vaccination.

Abstract

Background: Mortality in COVID-19 patients is related to several factors, including comorbidities, age, gender, increased D-dimer levels, elevated CRP levels, lymphopenia, leukocytosis, and hypoalbuminemia. Nevertheless, there is a scarcity of research on the impact of hypoalbuminemia on COVID-19 mortality, particularly in Indonesia. Objective: This study aimed to determine the effect of hypoalbuminemia, age, gender, and comorbid status on the mortality rate of COVID-19 patients at Prof. Dr. Margono Soekarjo Hospital. Methods: This research employed an observational analytic method, utilising a cohort design and survival analysis approach. The study was conducted at Prof. Dr. Margono Soekarjo Purwokerto Hospital in December 2023. The study utilised September 2021 medical records as secondary data. The data was analysed using Cox regression to determine the hazard ratio value. The results were deemed statistically significant if the p-value was < 0,05.Results: In September 2021, 217 individuals were hospitalised due to COVID-19. Out of all the patients, only 119 satisfied the specific criteria to be considered as study samples. A total of 49,6% of patients suffered from hypoalbuminemia. The mortality rate was 27,73%, with a higher incidence observed in patients with hypoalbuminemia (25 cases) compared to those without hypoalbuminemia (8 cases). The multivariate analysis revealed that patients with hypoalbuminemia had a 1,8-fold increased risk of mortality compared to patients without hypoalbuminemia (p = 0,007). The age (p = 0,08), gender (p = 0,38), and comorbid status (p = 0,22) of COVID-19 patients did not significantly affect mortality. Conclusion: COVID-19 patients with hypoalbuminemia had a 1,8-fold increased risk of mortality compared to those without hypoalbuminemia. Therefore, it can be inferred that hypoalbuminemia may serve as a prognostic indicator of mortality in COVID-19 patients. KeywordsAlbumin, COVID-19, death, hypoalbuminemia

Abstract

Background: Stroke is one of the non-communicable diseases with the highest mortality rate in the world. Hemorrhagic stroke is a type of stroke caused by a rupture of blood vessels in the brain that can threaten the lives of patients. Mortality and morbidity caused by hemorrhagic stroke are higher than ischemic stroke. This study aims to determine what risk factors cause hemorrhagic stroke. Methods: This study is a quantitative analytical study with a case-control study design conducted at Banyumas Regional Hospital. The study sample consisted of 100 case samples of hemorrhagic stroke patients and 100 control samples of ischemic stroke patients using random sampling techniques. This study was conducted in September-October 2024. The variables studied were age, gender, ancestry, history of hypertension, history of DM, dyslipidemia, smoking habits, diet, physical activity, obesity, and control compliance. The research data consisted of secondary data obtained from the medical records section of Banyumas Regional Hospital and primary data obtained through interviews using questionnaires. Data analysis used SPSS 23 software with univariate tests, bivariate with chi-square, and multivariate with logistic regression to calculate the odds ratio (OR). Results: The average age of the study respondents was 57.4 years and female. Variables that influenced the incidence of hemorrhagic stroke were non-compliance with control (p-value 0.000 and OR of 4.869), history of hypertension (p-value 0.004 and OR of 3.110), and poor diet (p-value 0.015 and OR of 2.204). Variables that did not influence were age, gender, heredity, history of DM, dyslipidemia, smoking habits, physical activity, and obesity. Conclusions: Patients who have a history of hypertension, poor diet, and are not compliant with medication controls are at greater risk of suffering from hemorrhagic stroke. Abstract

Introduction: The problems that post-stroke patients still face after being hospitalized are weakness in one side of the body (hemiparesis), speech disorders (dysarthria/aphasia), difficulty swallowing (dysphagia) and psychological disorders such as post-stroke depression and the risk of recurrent attacks. The purpose of the service is to improve the knowledge and skills of partners about: early detection of stroke, post-stroke care, post-stroke physical exercise, post-stroke speech therapy, post-stroke swallowing therapy, post-stroke psychological management. The method carried out is training with lectures, discussions and demonstrations, the media used are modules and videos. The participants were elderly Posyandu cadres and families who took care of family members after a stroke totaling 19 people. The success of the training was evaluated by comparing pre and posttest scores and assessing the participants' ability to demonstrate the actions taught. The results obtained by the knowledge of early detection, psychological management and post-stroke care of participants were 100% improved. Skills about early detection of stroke, ROM exercises, swallowing exercises and speech therapy 89.47% of participants improved. Conclusion: The training is effective in improving the knowledge and skills of cadres and families in caring for post-stroke patients. Key word: post-stroke, training, cadres, family, care

Abstract

Lymphatic filariasis is spread almost throughout Indonesia, some areas are categorized as filariasis endemic areas. The endemicity of filariasis in an area is determined by the microfilariae number (MF rate) above 1% by means of a finger blood survey (SDJ). Simbang Kulon Village in Buaran District, Pekalongan Regency is one of the filariasis endemic areas with an MF rate of 2.65%. This study aims to measure the entomological parameters and molecular examination of the transmission of lymphatic filariasis in Simbang Kulon Village. This study used a survey with a cross-sectional study design. Mosquito catching was carried out using the Human Landing Collection and Resting method both inside and outside the house. The results of the mosquito catching were brought to the laboratory for rearing process for 14 days. Data were analyzed descriptively. The results of the identification of 50 mosquitoes found 94% Culex quinquefasciatus mosquitoes and 6% Culex vishnui mosquitoes. Parous rate, infection rate and infective rate of Culex quinquefasciatus and Culex vishnui were zero per cent. Culex quinquefasciatus had the highest relative abundance (23.5%). Based on the results of the analysis of research data, filaria larvae were not found in the mosquito body because the two Culex mosquito species had never laid eggs (nulliparous).
