Hospital Information Systems, particularly in the outpatient registration process, are crucial for supporting service efficiency and data accuracy. However, RSUD Komodo has experienced several challenges in the implementation of its SIMRS module, including system slowdowns, sudden monitor failures, and unstable internet connectivity during service hours. These issues hinder operational effectiveness and risk compromising service quality. The objective of this research is to assess system performance comprehensively across human, organizational, and technological dimensions. A qualitative descriptive design was employed, involving in-depth interviews with five key informants: registration staff, IT personnel, coder, head of the medical records unit, and head of the casemix team. The findings show that in the human dimension, users lacked sufficient training and adaptation strategies. In the organizational aspect, weak coordination and the absence of standardized procedures were identified. In the technology dimension, hardware malfunctions and slow system performance significantly disrupted services. These interconnected issues reveal the need for capacity buildingan effective tool for evaluating hospital information systems, offering a structured approach to identifying and resolving performance gaps in outpatient service modules.

Abstract

The implementation of Electronic Medical Records (EMR) in healthcare institutions aims to improve the efficiency, accuracy, and accessibility of medical data. RSUD Weda, a regional hospital in Indonesia, has adopted an EMR system in its outpatient services to replace conventional manual processes. However, this transition has faced several challenges, including inadequate training, unstable network infrastructure, and inconsistent adherence to Standard Operating Procedures (SOPs). This study aims to evaluate the performance of the EMR implementation using a qualitative descriptive method based on the 5M framework—Man, Material, Machine, Method, and Money. Data were collected through interviews, direct observation, and document analysis involving stakeholders such as medical staff, administrators, and IT personnel. The findings indicate that while users acknowledge improvements in data accuracy and accessibility, key issues remain, particularly in training quality, infrastructure readiness, and system reliability. The study also observed gradual improvements in SOP compliance and reduced system failures over time. These results suggest that successful EMR adoption requires a balance of technical infrastructure, human resource readiness, financial support, and continuous system monitoring. The use of the 5M framework proved effective in identifying multidimensional challenges and offering holistic insights into the implementation process. This study contributes to the broader literature on EMR evaluation by providing contextualized evidence from a district-level hospital and offers practical recommendations for enhancing EMR system performance in similar healthcare settings.

Abstract

The increasing complexity of disease trends in the post-pandemic era necessitates more accessible and data-driven decision-making tools, particularly in primary healthcare services. This study aims to develop an interactive morbidity dashboard based on ICD-10 classifications using secondary data from BPJS Kesehatan. The research focuses on outpatient visit records in Karanganyar Regency from 2020 to 2024. A descriptive quantitative approach was applied, accompanied by system development using Python. Data preprocessing involved standardizing ICD-10 codes, handling missing values, and grouping by year, gender, and age category. The resulting dashboard allows users to filter morbidity trends based on demographic variables and disease categories. The ten most prevalent disease groups include respiratory, digestive, endocrine, and circulatory disorders. This dashboard facilitates data-based decision-making and enables targeted promotive and preventive interventions in primary healthcare facilities.

Abstract

Abstrak —Profesional Pemberi Asuhan meyakini atau mempersepsikan banyak kendala dalam penggunaan Rekam medis elektronik, termasuk tidak mudahnya penggunaannya. Hubungan antara variabel kemanfaatan dan kemudahan penggunaan terhadap sikap pengguna saat mulai implementasi rekam medis elektronik rawat jalan di RS Jiwa Grhasia Yogyakarta merupakan tujuan dalam penelitian ini. Metode yang digunakan adalah kuantitatif. Data dikumpulkan dengan membagikan kuisioner dan dilakukan uji statistik regresi linier berganda. Penelitian ini membuktikan adanya hubungan yang positif dan signifikan antara variabel kemanfataan dan sikap pengguna dan angka signifikansi yang dihasilkan yaitu 0.001<0.05. Hubungan yang diperoleh adalah positif dan signifikan antara persepsi kemudahan penggunaan dan sikap pengguna dengan nilai 0.003<0.05. Sedangkan kontribusi persepsi kemanfaatan dan kemudahan penggunaan rekam medis elektronik terhadap sikap pengguna adalah sebesar 61,9%, dan 39,1% sisanya dijelaskan oleh variabel yang tidak terdapat pada penelitian ini. Studi ini merekomendasikan bahwa ketika mengembangkan RME, perlu mempertimbangkan aspek kegunaan dan kemudahaan sehingga PPA terdorong untuk menggunakan RME untuk mendokumentasikan pelayanannya. Seperti harapan dari PPA yaitu RME dapat dengan cepat menyajikan data yang diperlukan.

Abstract

Electronic Medical Records (EMR) are essential for improving the quality, efficiency, and continuity of healthcare services. Despite mandates requiring full implementation of EMRs in Indonesian healthcare facilities by the end of 2023, many hospitals continue to face challenges in achieving effective adoption. This study aims to analyze the implementation of EMRs in the outpatient services of Nur Hidayah Hospital, Yogyakarta, using the 5M framework: Man, Money, Machine, Materials, and Method. Employing a qualitative descriptive approach, data were collected through in-depth interviews, observations, and document analysis involving key informants from medical records staff, registration officers, and IT personnel. The findings revealed that while the availability of staff and supporting infrastructure such as computers and software was generally adequate, limitations persisted in human resource specialization, training access, and system integration. Financial planning for EMR-related needs was done through annual budgeting (RAB), but coordination between departments remained minimal. Key challenges included slow internet connectivity, limited IT staff, and lack of specific Standard Operating Procedures (SOPs) for EMR usage. Overall, the EMR system has been implemented in most outpatient units, yet not all units have fully transitioned due to technical and organizational barriers. This study concludes that optimizing EMR implementation requires not only technological readiness but also organizational alignment, continuous staff development, and robust inter-departmental coordination.

Abstract

Perkembangan teknologi informasi di bidang kesehatan mendorong rumah sakit untuk mengimplementasikan Rekam Medis Elektronik (RME) guna meningkatkan kualitas layanan dan efisiensi operasional. RSUD Merah Putih Kabupaten Magelang telah menerapkan RME secara penuh pada layanan rawat inap sejak Maret 2024. Penelitian ini bertujuan untuk menganalisis implementasi RME dengan menggunakan model HOT-FIT yang mencakup faktor manusia, organisasi, teknologi, dan kesesuaian (fit) antar faktor tersebut. Penelitian dilakukan dengan metode kualitatif deskriptif melalui wawancara mendalam terhadap 10 informan dari berbagai profesi, seperti dokter, perawat, farmasi, radiologi, laboratorium, dan IT. Hasil penelitian menunjukkan bahwa RME sangat membantu dalam mempercepat pencatatan medis, memudahkan koordinasi antardepartemen, dan mendukung pelayanan pasien. Manajemen mendukung melalui pelatihan dan penyediaan infrastruktur, namun masih terdapat kendala dalam penerapan SOP dan penanganan gangguan sistem. Pada aspek teknologi, sistem dinilai cukup baik tetapi perlu peningkatan pada kecepatan akses, integrasi data antarunit, serta stabilitas server. Secara umum, kesesuaian antara faktor human, organization, dan technology sudah baik, tetapi perlu perbaikan pada mekanisme feedback dan transparansi informasi saat terjadi error. Rumah sakit disarankan untuk mengoptimalkan infrastruktur teknologi, SOP teknis, dan pelatihan berkelanjutan guna mendukung keberhasilan implementasi RME. Abstract

Digitalisasi pelayanan kesehatan melalui rekam medis elektronik (RME) menjadi kewajiban bagi seluruh fasilitas pelayanan kesehatan di Indonesia. Namun, implementasi RME di tingkat puskesmas masih menghadapi beragam tantangan, mulai dari keterbatasan sumber daya manusia hingga infrastruktur yang belum memadai. Penelitian ini bertujuan menganalisis implementasi RME di Puskesmas Sukoharjo dengan pendekatan Fishbone Diagram untuk mengidentifikasi akar permasalahan dan memberikan rekomendasi perbaikan. Metode yang digunakan adalah deskriptif kualitatif dengan teknik wawancara semi-terstruktur, observasi, dan studi dokumentasi. Informan terdiri dari kepala unit dan petugas pelaksana, sementara analisis data dilakukan dengan teknik Miles dan Huberman serta triangulasi sumber dan metode. Hasil penelitian menunjukkan adanya kendala pada faktor Man (kompetensi dan pelatihan terbatas), Method (tidak adanya SOP yang tersosialisasi), dan Machine (perangkat dan jaringan tidak memadai). Selain itu, anggaran khusus untuk pemeliharaan dan pengembangan sistem tidak tersedia. Kesimpulan dari penelitian ini menekankan pentingnya peningkatan kapasitas SDM, penyediaan sarana prasarana yang memadai, penyusunan SOP yang terstruktur, serta advokasi anggaran berbasis output agar implementasi RME berjalan optimal. Abstract

This study investigates the adoption of Electronic Medical Records (EMR) by registration staff at Tangkiling Health Center, Palangka Raya, using the Technology Acceptance Model (TAM) framework. Out of 61 total healthcare workers, five medical record staff with educational backgrounds in midwifery, nursing, dental nursing, and non-medical high school were purposively selected as research participants. The study employed a qualitative descriptive method, combining in-depth interviews and direct field observations. Results indicate that while the staff acknowledged the usefulness of EMR in improving service efficiency and data accessibility, several barriers hindered its effective use—primarily a lack of training, limited digital literacy, unstable internet connectivity, and inadequate hardware such as computers and printers. Observations showed that only the registration unit had a single shared computer, while other units like outpatient clinics, pharmacy, and cashier still relied on manual documentation. This fragmentation has led to duplicate workloads and disrupted patient data continuity. Despite these challenges, the staff demonstrated a positive attitude and strong behavioral intention to adopt EMR if provided with sufficient infrastructure and regular training. The study concludes that successful EMR implementation in primary healthcare requires not only user acceptance but also institutional readiness, adequate resources, and integrated systems across all service units.

Abstract

Transformasi digital dalam pelayanan kesehatan mendorong fasilitas pelayanan primer, seperti puskesmas, untuk mengadopsi sistem Rekam Medis Elektronik (RME). Namun, keberhasilan implementasi RME tidak hanya bergantung pada teknologi, melainkan juga kesiapan sumber daya manusia, organisasi, serta dukungan kebijakan internal. Penelitian ini bertujuan untuk mengevaluasi kesiapan implementasi RME di UPTD Puskesmas Jimbaran, Kabupaten Semarang menggunakan pendekatan DOQ-IT. Metode yang digunakan adalah deskriptif kualitatif dengan desain studi kasus, melibatkan delapan informan kunci yang dipilih secara purposif. Data dikumpulkan melalui wawancara, observasi, dan studi dokumentasi, lalu dianalisis secara tematik. Hasil penelitian menunjukkan bahwa kesiapan infrastruktur dan sumber daya manusia berada pada kategori cukup siap, sementara kesiapan organisasi serta hambatan teknis dan operasional masih tergolong kurang siap. Evaluasi menggunakan metode DOQ-IT efektif dalam mengidentifikasi aspek yang perlu ditingkatkan sebelum penerapan RME secara menyeluruh. Kesimpulan dari penelitian ini menegaskan pentingnya dukungan kebijakan internal, pelatihan berkelanjutan, serta penguatan peran manajemen untuk mewujudkan transformasi digital layanan kesehatan primer yang berkelanjutan.

Abstract

The adoption of Electronic Medical Records (EMR) in Indonesia has been accelerated by national regulations; however, many hospitals continue to face significant challenges such as limited infrastructure, varying levels of digital literacy among healthcare workers, and resistance to shifting from paper-based systems. These issues highlight the urgency of evaluating user acceptance in specific hospital contexts. This study evaluates the acceptance of EMR implementation at Panti Waluyo Hospital Surakarta using the Technology Acceptance Model (TAM). A descriptive qualitative design was applied through in-depth interviews, observations, and document reviews. Fifteen participants, including physicians, nurses, medical record officers, and administrative staff, were purposively selected to represent core EMR user groups. The findings indicated that 73% of respondents perceived EMR as useful in improving documentation efficiency and accuracy, while 67% agreed that the system was easy to navigate. Moreover, 80% expressed willingness to continue using EMR. Barriers identified included technical disruptions (40%) and the need for continuous training (53%). Overall, the results suggest that EMR adoption is positively accepted within the hospital, although organizational support and infrastructure improvements remain necessary. The study concludes that perceived usefulness and perceived ease of use play significant roles in shaping user attitudes and behavioral intentions, which in turn influence actual system use. Strengthening user competence, addressing digital literacy gaps, and ensuring technical reliability are essential to sustain EMR implementation in healthcare settings.
