Abstract

Keberlanjutan perawatan ibu nifas dan bayi BBLR di rumah sangat penting dilakukan melalui pendampingan perawat. Tujuan pengabdian masyarakat ini untuk meningkatkan pengetahuan dan keterampilan ibu dalam perawatan nifas dan bayi prematur di rumah. Kegiatan ini dilakukan pada Januari-Maret 2020 dengan metode konseling secara homevisit. Ibu yang berpartisipasi dalam kegiatan ini adalah yang melahirkan bayi BBLR hidup dan bertempat tinggal di wilayah Malang & Batu. Pengumpulan data melalui kuesioner. Dua puluh lima responden ikut berpartisipasi memiliki tingkat pengetahuan baik (88%), parenting self-efficacy dalam kategori tinggi (84%), dan tingkat stress ringan (84 %). Hasil uji korelasi antara tingkat pengetahuan dengan parenting self-efficacy signifikan dan berkorelasi sedang (p-value =0.009; r =0.510). Sedangkan antara pengetahuan dan stress ibu tidak terdapat hubungan yang signifikan dalam merawat bayi BBLR di rumah (p-value =0,404; r =0,175). Kesimpulannya, pentingnya peningkatan pendampingan oleh tenaga kesehatan pada ibu nifas yang memiliki bayi BBLR di rumah.

Abstract

Human trafficking is being global crime worldwide. The practice of human trafficking in the form of human exploitation implicated in human rights violations. Survivors of human trafficking will experience health problems both physical and mental problems. The purpose of this systematic review was to identify and summarize the mental health problems of human trafficking victims. Systematic search conducted on March 12, 2019 in data of Proquest, Springer Link, and Scopus by keyword *human health of human trafficking* between the years of 2013-2018. The result of this search found 14 journals that match with inclusion criteria. The inclusion criteria were peer-reviewed journal articles, written in English, all forms of exploitation, the samples are aged >15 years old. The results shown the incidence of human trafficking greatly affected the mental health of victim. Victims experienced anxiety, depression, isolation, disorientation, aggression, suicidal ideation, attention deficit, psychotic disorders, behavioral and post-traumatic stress disorder (PTSD). For further research required in efforts to overcome the mental health impact of human trafficking victims. The focus on preventing efforts was through government campaigns, IOM and law enforcement and also the fulfillment of the rights of human trafficking victims

Abstract

The family has an important role in elderly care at home, where the majority of assistance and long-term care for the elderly is provided by the family. The nurse is responsible for assisting the family in caring for the elderly, so that in the end the family is able to provide elderly care independently at home. This study aims to explore family experiences in caring for the elderly at home. Researchers used qualitative research methods with a phenomenological approach. Data obtained through semi-structured interviews with 6 (six) participants. The research produced 5 themes: care for the elderly is a form of devotion to parents, care for the elderly requires physical and mental readiness, care for the elderly requires knowledge related to the elderly and how to care for the elderly, care for the elderly requires support from extended families, and feel the importance of supporting cadres and health workers. For conclusion, elderly families have a responsibility to meet the needs of the elderly, so that efforts are needed to improve the knowledge and skills of the family in order to properly care for and meet the needs of the elderly.

Abstract

Introduction: Indonesian nurses living and working in Japan with a culture likely to be very different from their own may be additional sources of culture shock, feeling confusion and discomfort when living in a new country. They also experience difficulties in maladaptive adjustments to the new environment to overcome culture shock, then they get stressed in a working environment which can   decrease   work motivation and quality of performance.Methods: This study used a cross-sectional approach with a non-probability sampling method. The sample is 90   Indonesian nurses having been working in Japanese hospitals, both those passing the National Board Examination (NBE) and those not. Working environment (job satisfaction, colleague, rule of employment) and job experience were independent variables and culture shock was dependent variable. Work environment research instruments were from the revised questionnaire modification Scale of Socio-Cultural Adaptation (SCAS-R), the substance of work performance, job experience questionnaire and the Culture Shock Questionnaire (CSQ). Data analysis used Spearman-rho and multiple logistic regression statistical tests.Results: The average of respondents who did not experience culture shock or normal  was 44 (49%) and respondents who experienced culture shock were a poor level of 36 (40%) and moderate level of 10 (11%). Job satisfaction (ρ value = 0.001) and colleague (ρ value = 0.000) have a significant correlation with culture shock. Rule of employment (ρ value = 0.174) and job experience (ρ value = 0.209) were not significant with culture shock. Multiple logistic regression analysis was conducted to figure out the predictors that influence the change in culture shock status while a colleague (β = 2.445, p value < 0.000).Conclusion: The results of the research showed that colleagues in the work environment had a significant influence effect on cultural shock. Therefore, it is necessary to provide social support to adaptive adjustment and stress management to Indonesian nurses in Japan so that they can impact increased quality performance.

Abstract

Schizophrenia is a psychiatric illness that causes the burden and maladaptive coping mechanism on the family. The burden and maladaptive coping mechanisms have an impact on the quality of family life and the ability of families to care for schizophrenic patients. The purpose of this study was to prove the effect of supportive group therapy on the burden and coping mechanisms in caring for family members who suffer schizophrenia. The design of this research was a pretest-posttest quasi-experimental study with control group design. The number of samples was 14 in the treatment group and 15 in the control group with purposive sampling. Variable of burden was measured with the Zarit Burden Interview (ZBI) and the coping mechanism was assessed by using Brief COPE Inventory (BCI). This supportive therapy was performed in 4 sessions with a total of 6 meetings. The results showed that there were significant differences on burden values and coping mechanisms before and after supportive therapy in the treatment group (p-value= 0.000). In the control group, there was a significant difference on the value of burden before and after health counseling (p-value =0.010), but there was no difference of coping mechanism value before and after health counseling (p-value=0.225). There were significant differences on the burden value (p-value=0.002) and coping mechanisms (p-value= 0.001) between treatment and control group. dr Rehatta Hospital is expected to facilitate the formation of peer support group in the psychiatry clinic so mental health in the family can be achieved.

Abstract

Birth rate through Caesarean surgery (C-section) keeps increasing globally. Research states that expecting mothers suffer from anxiety prior to C-section. Impacts of pre-Caesarean anxiety are increasing post-Caesarean pain, increasing analgesic need, longer hospitalization, and postpartum depression. One of the methods to overcome anxiety before C-section is conducting Progressive Muscle Relaxation (PMR) therapy. This study described the influence of PMR therapy on anxiety level of Pre-Caesarean mothers in the Surgery Room of RSUD dr Haryoto, Lumajang. The study used quasi-experimental pre-post test with control group design. The sampling technique was consecutive sampling and the number of the sample was 26 respondents. The instrument for PMR therapy was PMR therapy tool. Self-Rating Anxiety Scale (SAS) was used to measure anxiety level. Progressive Muscle Relaxation (PMR) had a significant influence towards decreasing level of anxiety for the pre-Caesarean mothers (p= 0.000). Anxiety will cause physiological tension, one of which is characterized by muscle tension. Muscle relaxation will lower the physiological tension that will ultimately decrease anxiety. Therefore, PMR may become an alternative therapy to overcome anxiety, especially anxiety before sectio caesarean surgery.

Abstract

Latar Belakang: Asesmen praanestesi adalah prosedur penting yang bertujuan untuk memastikan keamanan tindakan bedah yang akan dilakukan. Prosedur ini sebenarnya adalah tanggung jawab dokter anestesiologi. Namun, dalam prosesnya melibatkan dokter, perawat serta pasien dan keluarganya. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan asesmen praanestesi serta menggali praktik pendelegasian tugas (task shifting) dalam pelaksanaan asesmen praanestesi di ruangan rawat inap. Metode: Penelitian ini dilakukan di sebuah rumah sakit (RS) tipe C (140 tempat tidur) di kota Malang Jawa Timur. Peneliti melakukan observasi selama 11 hari terhadap pelaksanaan asesmen praanestesi pada kasus operasi elektif. Untuk melengkapi data, dilakukan juga pengamatan terhadap dokumen rekam medis pasien yang sama. Untuk menilai praktik pendelegasian tugas dari dokter ke perawat, peneliti menggunakan kuesioner yang berisi 28 aspek aktivitas asesmen praanestesi. Kuesioner ini didistribusikan kepada 80 orang responden yang terdiri dari perawat dan dokter spesialis yang terlibat dalam operasi elektif. Kuesioner ini menilai persepsi responden mengenai harapan dan kenyataan tentang siapa yang seharusnya melaksanakan aktivitas tersebut. Data dianalisis secara deskriptif untuk menggambarkan pola pelaksanaan pendelegasian tugas dan asesmen praanestesi. Hasil: Asesmen praanestesi hanya dilakukan pada 19,5% dari total 43 kasus operasi elektif, namun tidak satupun yang menuliskan hasil asesmen pada dokumen yang semestinya. Dokumen rekam medis tentang asesmen praanestesi akan dilengkapi saat setelah pelaksanaan operasi (di kamar operasi). Sebagian besar responden beranggapan bahwa asesmen praanestesi dilaksanakan secara kolaborasi antara dokter dan perawat. Namun, dalam praktiknya, kebanyakan aktivitas dalam asesmen praanestesi dilakukan oleh perawat. Pendelegasian tertinggi dari dokter ke perawat terjadi pada aspek meminta tanda tangan persetujuan tindakan medis (informed consent) serta aspek menjelaskan tentang puasa kepada pasien dan keluarganya. Kesimpulan: Asesmen praanestesi belum terdokumentasi dengan lengkap dan tepat sebelum pelaksanaan operasi, serta terjadi pendelegasian tugas ke perawat terutama pada proses persetujuan tindakan medis.

Abstract

Caring for LBW babies at home is must be intensively. One of the methods used is Kangaroo Mother Care (KMC) by providing warmth to LBW babies. Family involvement in the care of LBW babies is a very important. This community service aims to optimize the support of family in the care of LBW babies. Team members conducted this activity in Februari-Mei 2020 with the home visit & counseling method. Participants in this activity were 25 mothers who gave birth to LBW babies and who live in Malang Raya. Data Collection used questionnaires. The result of this activity, the majority of family support in the care of LBW babies was in good category (76%). In conclusion, family support plays an important role in the sustainability of the implementation of KMC at home.

Abstract

Clinical decision making is important for nurses to do. Clinical decision making is a complex process consisting of careful assessment of pathological conditions and patient history and the use of nursing knowledge and learning based on critical thinking experiences. Improper clinical decision making will have fatal consequences for patients This review system was compiled using Proquest. Science Direct and Pubmed databases. Article selection is carried out to identify relevant articles then screening is conducted to adjust the inclusion criteria. 13 articles were obtained in accordance with inclusion criteria and reviewed by using JBI (Joanna Briggs Institute Levels). Data shows that nurses have a very important role in clinical decision making. Nurses' clinical decision making is considered due to the risks faced by patients, thus the nurses should think critically. Clinical decision making for nurses is influenced by several factors including experience, knowledge, environment, team, supervisor support, and professionalism.

Abstract

A woman's transition into motherhood brings about physical and psychological changes. The changes require adjustments to her daily roles and activities. If the postpartum mother is not able to adapt well, then there will be a mood disorder. Depression is a part of mood disorder. The impact of postpartum depression will disrupt the mother-infant bonding, breastfeeding self-efficacy and the changing of marriage partnership. The purpose of this research was to analyze the impacts most affected by the incidence of postpartum depression in the working area of public health Malang City. The research used descriptive correlational method with cross sectional, with purposive sampling of 70 respondents in the period April 26th, 2017 - May 30th, 2017. The results are 93% of post-partum mothers experience moderate depression affecting low breast-feeding ability 54.3%, poor marital relationship situation 52.9% and moderate emotional bonding disturbances 47.1%. The result of bivariate analysis is known to the effect of postpartum depression, namely breastfeeding ability (p value = 0,011), marital relationship situation (p value = 0.014), emotional bond (p value = 0,021). The results of logistic regression test showed that the impact most influenced by postpartum depression was the ability of breastfeeding (p value = 0,011; ExpB 6,394). Postpartum mothers can predict the incidence of depression by looking at three aspects directly including the breastfeeding self-efficacy, marriage partnership and mother-infant bonding. Of the three aspects of the most influential is the breastfeeding self-efficacy. In the next research should be done at a wider population level in order to be able to cover the area of Malang generally.
