Abstract

Selama penerapan Diagnosis Related Group di Rumah Sakit Umum Pusat (RSUP) Dr. Sardjito, terjadi kesenjangan tarif biaya riil pelayanan kesehatan dengan tarif Indonesia Case Base Groups (INA-CBGs). Penyebab terbesar kesenjangan tarif tersebut adalah pelayanan obat dan penggunaan sumber daya laboratorium yang tidak efisien. Biaya pelayanan penunjang medis untuk pasien leukemia limfoblastik akut adalah sekitar 23,8% dari total biaya pelayanan kesehatan. Penelitian ini bertujuan untuk mengetahui tingkat efisiensi penggunan sumber daya laboratorium dan pengaruh terhadap kesenjangan tarif. Penelitian ini menganalisis semua rekam medis dan data biaya pelayanan laboratorium pasien leukemia limfoblastik akut tahun 2009-2010 di RSUP Dr. Sardjito Yogyakarta. Pemeriksaan kimia klinik menunjukkan pola pasien yang semakin parah, proporsi biaya pemeriksaan kimia klinik semakin rendah. Kondisi ini juga terjadi pada pemeriksaan radiologi, urine dan tinja rutin. Sementara pada pemeriksaan hematologi, mikrobiologi, dan imunologi/serologi menunjukkan pola semakin parah pasien maka semakin tinggi proporsi biaya pemeriksaan. Analisis regresi menemukan pemeriksaan kimia klinik meliputi mikrobiologi darah, ureum, magnesium, creatine kinase MB (blood) menyebabkan kesenjangan tarif semakin meningkat atau rumah sakit semakin dirugikan. Model regresi linier ini mempunyai nilai R2 sebesar 0,834 dengan nilai F = 84,475 (P < 0,05). Ketidakefisienan penggunaan sumber daya laboratorium pada pemeriksaan kimia klinik terdapat pada kelompok pasien tingkat keparahan ringan. Problems occurred during the implementation of Diagnosis Related Group in Sardjito Hospital Yogyakarta. There was gap price between the real cost of health care and Indonesia Case Base Groups (INA-CBGs) cost. The cause of the gap price was drug delivery and use of laboratory resources inefficiently. Cost of medical support services for acute lymphoblactic leukemia patients about 23.8% of the total cost of health care. This study aimed to analyze efficiency of the use of laboratory resources and their effect to discrepancy of price. This study analyzed all medical records and laboratory services cost data in 2009-2010 acute lymphoblastic leukemia patients at Sardjito Hospital Yogyakarta. Clinical chemistry test showed a pattern of more severe patients, the lower the percentage of clinical chemistry fees. This condition also occurs in radiological, urine test, and stool. While the test of hematology, microbiology and immunology/serology showed a pattern, the more severe the patient, the higher the percentage of the cost of the test. The results of the regression analysis showed that the more higher cost of clinical chemistry test (blood microbiology, urea, magnesium, creatine kinase MB (blood)) the higher discrepanct of price which causes the hospital getting harmed. Linear regression model has a value of determination coeficient 0.834 with a value of F = 84.475 (P < 0.05). Inefficiency of resource use in the test of clinical chemistry laboratory located on the mild severity of the patient group.

Abstract

The burden of ischemic heart disease (IHD), measured in disability-adjusted life years (DALYs), has significantly increased in Indonesia over the past two decades, indicating that current policies and programs have not been fully effective. This study aimed to analyze changes in the burden of IHD and its associated risk factors from 2002 to 2021 across 34 provinces in Indonesia, using secondary data from the Global Burden of Disease 2021. This cross-sectional analytical study used IHD-related DALYs as the dependent variable, and independent variables included non-optimal temperature, low physical activity, tobacco use, risky dietary patterns, air pollution, obesity, hypertension, high LDL cholesterol, and high fasting glucose. Descriptive statistics and linear regression were used, with Principal Component Analysis (PCA) applied to reduce multicollinearity. Separate regressions were conducted for 2002 and 2021 to assess changes over time. The results showed a 10.5% increase in IHD-related DALYs, from 2,753.87 per 100,000 population in 2002 to 3,043.08 in 2021. Hypertension emerged as the leading risk factor, followed by risky dietary patterns. Tobacco use and a principal component (low physical activity, risky dietary patterns, obesity, high LDL cholesterol, hypertension, and high fasting glucose) significantly contributed to the burden of IHD in both years. Environmental factors such as non-optimal temperature and air pollution did not show significant effects. Health programs must be strengthened with a focus on seven key risk factors contributing to IHD-related DALYs, especially modifiable ones like low physical activity, risky dietary patterns, and tobacco use to reduce IHD burden in Indonesia.

Abstract

Background: The effect of Proprioceptive Neuromuscular Facilitation (PNF) stretching, in addition to standard stretching, improves anticipatory balance in athletes with Cerebral Palsy (CP). We aimed to analyze the effects of PNF stretching to standard training over 4 weeks on anticipatory balance in Cerebral Palsy athletes at 6-month post-intervention evaluation. Methods: The subjects of this study are 24 athletes with Cerebral Palsy at the Indonesian National Paralympic Committee (NPC) training facility. Subjects divided into the treatment group (n = 12) received standard stretching exercises along with PNF stretching, and the control group (n = 12) performed only standard stretching exercises. Both groups received intervention 3 times per week for 4 weeks and evaluation 6 months post-intervention. Y balance tests (YBT) were measured in three directions: anterior (A), posterolateral (PL), and posteromedial (PM), pre- and post-intervention.Results: There was significant improvement of YBT in the treatment group after 4 weeks of intervention. For the right and left limbs, marked increases were observed in all directions (p < 0.05). No significant improvements in YBT were observed at 6 months post-intervention in all directions (p > 0.05).Conclusions: Significant improvements in the Y Balance Test were observed after 4 weeks, but no differences between the two groups at the 6-month evaluation. These findings suggest that while short-term gains in dynamic stability are achievable with PNF, sustained benefits require ongoing reinforcement to maintain neuromuscular adaptations over time.

Abstract

Hypertension has the potential to cause serious complications such as heart disease, stroke, and kidney failure if not treated properly. Apart from physical factors, hypertension sufferers also often face psychological problems such as stress, which can worsen their condition and affect their quality of life. This study aimed to explore the relationship between stress and quality of life in hypertension sufferers. This systematic review searched for relevant articles using MeSH-based keywords in databases such as Scopus, PubMed, Sage Journal, and Web of Science. The article inclusion criteria were: population: patients with hypertension, intervention: no intervention, comparison: no comparison, results: the article's results showed a relationship between stress and quality of life in people with hypertension. Article quality assessment was carried out using the Joanna Briggs Institute guidelines. Descriptive analysis was carried out using a narrative approach by considering articles published from 2012 to 2024. Based on a literature search, 36229 articles were found with appropriate keywords from Scopus (2578), PubMed (683), Sage Journal (31999), and Web of Science (969). After screening for inclusion and exclusion criteria, 11 articles were identified for inclusion in this review. These eleven articles show that there is a relationship between stress and quality of life in people with hypertension. The importance of a comprehensive approach that considers socioeconomic, demographic, and psychological factors to improve the quality of life of hypertension sufferers.

Abstract

In the implementation of stunting convergence in Pringsewu District in 2023, there is no direct coordination between the Human Development Cadres and the Family Assistance Team and no way to facilitate collaboration between the human development cadres and the family assistance team to work together to carry out data collection tasks as indicators of stunting achievement and stunting prevention in Pringsewu District. Objective to find out more about the phenomenon of human development cadres and family human assistenace teams collaboration and the perspectives of each agency representative in Pringsewu District, Lampung Province that allows human development cadres and family assitance team collaboration to be carried out in stunting prevention efforts in Pringsewu District, Lampung Province. The type of research used was qualitative using a phenomenological approach and data collection techniques with in-depth interviews and FGDs which were analysed using Atlas t.i version 8. The informants in this study were from the relevant Dinas and OPD in the Pringsewu Regional Government work area. Data collection was carried out using in-depth interviews and FGD techniques which were analyzed using Atlas t.i version 8. From the results of the analysis of the synthesis of interviews and Focus Group Discussions with informants from the relevant Departments and OPDs in the Pringsewu Regional Government work area, it was found that the collaboration between human development cadres and family assistance teams was very needed for the effectiveness and efficiency of human development cadres and family assistance team work as stunting cadres. Collaboration between human development cadres and family assitance team should be facilitated by the Pringsewu Regency Team for Accelerating Stunting Reduction and advocated to the Pringsewu Regency Regional Government for inclusion in the PRINCES (Pringsewu Prevent Stunting) Program. Collaboration between Human Development Cadres and Family Assistance Workers is very crucial in efforts to reduce stunting. This collaboration needs to be facilitated by the Stunting Reduction Acceleration Team and integrated into the PRINCES (Pringsewu Prevent Stunting) program.

Abstract

ABSTRACTBackground: The Phenomenon of unwanted pregnancy among adolescents is still rife in Indonesia. Results of interviews with the head of the National Population and Family Planning Agency (BKKBN), Hasto Wardoyo, explained in 2020 that there are not enough unwanted pregnancies in Indonesia which reach 17.5%.Objectives: Provide an overview of the determinants of unwanted pregnancy in adolescents in Indonesia.Methods: An article search was conducted using Google Scholar and Garuda Portal with the keywords "unwanted pregnancy", "unwanted pregnancy in adolescents aged 12-24 years", "unwanted” pregnancy factor in adolescents". Discussion: The total articles obtained were 157 articles, found 5 articles that met the inclusion criteria, namely articles with research conducted in Indonesia with publication years from 2015 to 2020, in the form of original articles, articles available in full text and free access, as well as factors causing pregnancy in adolescents. Lack of knowledge, permissive attitude of adolescents, easy access to pornographic media and parenting patterns are factors that cause pregnancy in adolescents.Conclusions: Lack of knowledge of adolescents about reproductive health and sexual health is the most dominant factor as a cause of unwanted pregnancy among adolescents, because knowledge can affect the attitudes and behavior. It is necessary to provide guidance and counseling related to knowledge andunderstanding to adolescents and parents regarding adolescent reproductive health.Keywords: Unwanted pregnancy, adolescents, unwanted pregnancy factors in Indonesia

Abstract

ABSTRAKLatar Belakang: Konsumsi air yang layak merupakan salah satu faktor untuk mencegah terjadinya masalah kesehatan seperti diare, kolera, disentri, tipes, cacingan, penyakit kulit, dan keracunan.. Diare menjadi penyebab kedua kematian tertinggi pada balita di seluruh dunia. masih ada beberapa daerah di Indonesia yang mengalami kekurangan air bersih dan air yang tercemar berat.Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan kepemilikan sumber air minum dan pengolahan air minum sebelum diminum dengan jenis wilayah tempat tinggal di Provinsi Jawa Timur berdasarkan data SDKI tahun 2017.Metode: Penelitian ini menggunakan data dari Survei Demografi dan Kesehatan Indonesia tahun 2017 di wilayah Jawa Timur dengan desain cross sectional. Jumlah sampel yang digunakan adalah 4113 rumah tangga. Data yang diperoleh berasal dari situs resmi DHS. Analisis yang digunakan pada penelitian ini adalah analisis Chi Square.Hasil: Hasil penelitian didapatkan adanya hubungan yang signifikan sumber air minum dan pengolahan air minum sebelum diminum dengan jenis wilayah tempat tinggal (p < 0,05). Sumber air minum dan pengolahan air minum memiliki korelasi dengan jenis wilayah tempat tinggal. Prevalensi rasio sumber air minum adalah 0,355 dan pengolahan air minum adalah 3,321.Kesimpulan: Rumah tangga di perkotaan mayoritas memiliki sumber air minum yang layak serta rumah tangga di perkotaan cenderung mengonsumsi air kemasan untuk minum. Rumah tangga di pedesaan melakukan pengolahan air minum sebelum dikonsumsi.   ABSTRACTBackground: Adequate water consumption is one of the factors to prevent health problems such as diarrhea, cholera, dysentery, typhoid, intestinal worms, skin diseases, and poisoning. Diarrhea is the second highest cause of death in children under five in the world. There are still some areas in Indonesia that experience a shortage of clean water and heavily polluted water.Objectives: This study aims to determine the relationship between ownership of drinking water sources and treatment of drinking water before drinking with the type of area of residence in East Java Province based on the 2017 IDHS data.Methods: This study uses data from the 2017 Indonesian Demographic and Health Survey in the Java region. East withdesign cross sectional. The number of samples used is 4113 households. The data obtained comes from the official website of DHS. The analysis used in this study is Chi Square analysis.Results: The results of the study found a significant relationship between drinking water sources and drinking water treatment before drinking with the type of area of residence (p < 0,05). Drinking water sources and drinking water treatment have a correlation with the type of area of residence. The prevalence ratio of drinking water sources is 0,355 and drinking water treatment is 3,321.Conclusions:. The majority of households in urban areas have a decent source of drinking water and households in urban areas tend to consume bottled water for drinking. Households in rural areas treat drinking water before consumption.

Abstract

Background: Diarrhea is one of the leading causes of child morbidity and mortality in low and middle-income countries. 23.8% prevalence of diarrhea was found among children under five years of age. Area-based management is required to address and provide specific interventions. Purpose: The study aims to analyze the effect of unsafe toddler feces management and incomplete primary immunization on the incidence of diarrhea using a spatial analysis in East Java, Indonesia. Methods: The data used is the 2018 Basic Health Research data. The measurement of the relationship between region and diarrhea was carried out by studying Moran's I index. Lisa Cluster Map and the Lisa Significance Map were also studied to understand distribution and significance. The regression used is OLS regression, spatial lag, and spatial error. The best model is assessed by comparing various parameters. Data were analyzed using GeoDA. Results: Moran's I result shows an index value of 0.489 with a pseudo-value of 0.001 (p <0.05) and a z-value of 3.7515. Lisa Cluster Map shows seven high-high category areas, five low-low category areas, and one high-low category area. Lisa's Significance Map shows six areas p=0.05, three areas p=0.01, and four areas p=0.001. The unsafe management of toddler feces and incomplete primary immunization related to diarrhea shows a significance of p<0.05. By comparing the R square, log-likelihood, p(sign), and sigma square values, the better model is the spatial lag model. Conclusion: The unsafe toddler feces management and incomplete primary immunization related to diarrhea.

Abstract

Background: Insulin resistance (IR) is considered the common underlying cause of metabolic syndrome (MetS). Predicting the incidence of MetS and IR at the individual level is essential, especially in obese adolescents who are at high risk of cardiovascular disease (CVD). InsuTAG has shown reliable results in predicting IR and MetS in healthy elderly populations. This study aimed to investigate the use of InsuTAG as an assessment tool to predict IR and MetS in obese adolescents, using the homeostasis model assessment (HOMA) to quantify insulin resistance. Methods:  A cross-sectional analytic study was conducted from January to May 2020 in healthy obese adolescents aged 13-18 years-old. Blood analyses included lipid profile (LDL-c, HDL-c, total cholesterol, and triglycerides), fasting blood glucose, and fasting insulin. IR was defined as HOMA-IR > 5.22 for boys and > 3.82 for girls during the pubertal period. Results: The receiver operating characteristic (ROC) curve for InsuTAG demonstrated a greater area under the curve (AUC) for identifying IR than MetS (0.877 vs. 0.743, p = 0.000; 95% CI). The optimal InsuTAG cut-off to determine IR was > 23.48, with 78.43% sensitivity and 87.38% specificity. The cut-off to identify MetS was > 23.36, yielding 67.97% sensitivity and 72.82% specificity. Conclusion: InsuTAG is a useful tool for identifying IR and MetS. The optimal cut-off values are > 23.48 for IR and > 23.36 for MetS in obese adolescents. These findings support early prevention and health promotion efforts, ultimately strengthening self-care practices among obese adolescents.
