Abstract

The Quality of Sleep in Brain Tumors Patients. Sleep quality is often compromised in patients with brain tumors. The underlying factors contributing to sleep quality disturbances in this patient population are multifaceted, and investigating these factors remains scarce. Sixty-eight patients with brain tumors, aged ≥ 18 years, were selected consecutively, and data was recorded in the form of sociodemographic characteristics (gender and age), clinical characteristics (clinical manifestations, tumor type, location, comorbid diseases) and therapy characteristics (management and types of drugs). Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Data were analyzed bivariate with chi-square and multivariate with logistic regression. Around 45.6% experienced poor sleep quality. Age, gender, systemic malignancies, pain, type of primary brain tumor, tumor location, operation, radiotherapy, and steroid have a significant relationship with sleep quality in patients with brain tumors (p-value <0.05). Multivariate analysis shows that age is the factor that most strongly influences the sleep quality of patients with brain tumors. Age, gender, systemic malignancies, pain, type of primary brain tumor, tumor location, operative treatment, radiotherapy, and steroids influence the quality of sleep in patients with brain tumors, but age has the most decisive influence.

Abstract

Vitamin D plays a role in the healing process of diabetic foot ulcers (DFU); it can improve the immune system, regulate inflammation function, and regulate growth factors in diabetes mellitus (DM). The immunomodulatory effects of vitamin D include reduced production of IL-2, IL-6, and TNF-alpha and increased production of IL-10. It also increases the activity of antimicrobial peptides (AMP) such as cathelicidin, and it promotes the differentiation of monocytes into macrophages, phagocytosis, and the production of lysosomal enzymes. Furthermore, vitamin D stimulates growth factors including vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-β). In this study, the incidence of DM and DFU in DM patients at RSUP Dr. Mohammad Hoesin Palembang will be compared to serum vitamin D levels. This research uses quantitative methods with an analytical observational design. The Mean vitamin D serum level was 11,727 ng/mL, and there was a significant association between serum vitamin D deficiency and DFU (p-value; 0.036 < 0.05). There was a significant association between serum vitamin D deficiency and DFU occurrence (p-value; 0.036 < 0.05) in Mohammad Hoesin General Hospital Palembang. Based on the results of this study, we proposed that patients with diabetic foot ulcers should have their serum vitamin D levels checked, and that vitamin D should be given to them as part of their therapy.

Abstract

Abstract

Preeclampsia (PE) is a pregnancy complication characterized by hypertension after 20 wk of gestation, often accompanied by proteinuria or organ dysfunction. This condition is linked to genetic, environmental factors, and exposure to secondhand smoke (SHS). This study explores the relationship between SHS exposure and PE risk through a narrative literature review. Epidemiological and molecular studies data demonstrate that SHS exposure increases PE risk via oxidative stress, inflammation, and angiogenic disruptions. The analysis reveals that SHS exposure enhances reactive oxygen species (ROS) production, triggers systemic inflammation through the NF-κB pathway, and impairs angiogenic function by lowering the PlGF/sFlt-1 ratio. The risk significantly increases with exposures ≥2 hr/d or weekly frequencies ≥4 d, particularly in poorly ventilated domestic and occupational settings. Biomarkers such as cotinine and carbon monoxide (CO) are employed to assess exposure levels, providing robust evidence that duration and intensity of exposure are pivotal in PE pathogenesis. These findings highlight the importance of preventive strategies, including smoking bans, public health education, and integration of biomarker-based assessments into prenatal care. Future research should aim to clarify the molecular and epigenetic mechanisms involved and explore therapeutic interventions to mitigate SHS-induced damage.

Abstract

Highlight: Insomnia is prevalent in ischemic stroke patients and can hinder the recovery process. Stroke patients have a higher prevalence of insomnia and related symptoms than the general population.  This study found a complex relationship between physical, psychological, and social risk factors for insomnia following ischemic stroke.   ABSTRACT Introduction: Insomnia is common in ischemic stroke patients and can negatively impact on the post-stroke recovery process by interfering with the body's natural healing process, reducing the effectiveness of rehabilitation therapy, and affecting the recovery of cognitive function. Effective management and early intervention in  insomnia are needed to enhance health services for stroke patients, promote optimal recovery, and improve their quality of life. Objective: This study aimed to identify and analyze the various factors associated with the occurrence of insomnia among patients with ischemic stroke. Methods: This is a cross-sectional study that used secondary data from medical records and primary data gathered through questionnaire-based interviews. The subjects were 105 ischemic stroke patients at the Neurology Clinic of Dr. Mohammad Hoesin Hospital, Palembang, between May to July 2024. The data acquired for further analysis included social and demographic details, comorbid conditions, depression, anxiety, pain, insomnia, medication use, sleep hygiene, and stroke clinical features. The data was then analyzed using IBM SPSS Statistics 24 and assessed through bivariate and multivariate analyses to evaluate the relationships between variables. Results: The prevalence of insomnia in this study was 42.9%, with mild insomnia observed in 26.7%, moderate insomnia in 13.3%, and severe insomnia in 2.9% of the subjects. Obstructive sleep apnea (OSA) (OR: 22.718), sleep hygiene index (OR: 6.490), and education level (OR: 3.453) were identified as determinants related to insomnia in ischemic stroke patients. Indirect factors associated with insomnia in ischemic stroke patients include depression, the number of comorbid diseases, pain, diabetes, a history of insomnia, anxiety, and stroke onset. Conclusion: There is a complex relationship between various physical, psychological, and social factors and the incidence of insomnia after ischemic stroke.

Abstract

Memory is related to the processes that allow us to record, store and then retrieve experiences and information. Memory as an information processor is analogized by psychologists and neurologists as a computer. The recording process, called encoding, is the process of obtaining information conveyed to the system by translating it into neural code which is processed in the brain. The goal of the research titled is to systematically evaluate and synthesize existing studies to determine the impact of memory training interventions on various aspects of cognitive function. This includes assessing how memory training may enhance skills such as attention, processing speed, working memory, and overall cognitive performance. By identifying patterns, strengths, and limitations in the current literature, the study aims to provide insights into the effectiveness of memory training programs and guide future research and practical applications in cognitive enhancement.Method: Articles used in systematic reviews are typically sourced from various academic databases such as PubMed, PsycINFO, Scopus, and Google Scholar. These include studies published in peer-reviewed journals. The keywords used for searching may include phrases like "memory training," "cognitive function," "cognitive enhancement," "working memory," and other related terms. This combination of keywords helps filter relevant articles. For instance, in this review, there might be 150 articles identified through the initial search. After a selection process based on inclusion and exclusion criteria, approximately 30 articles are chosen for further analysis. The articles included in the systematic review are examined systematically, following the PRISMA 2020 guidelines: Study Identification,Study Selection,Data Extraction,Analysis and Synthesis Analyzing the results from the selected studies and compiling conclusions based on the available evidence. Results: The findings from these five literature studies cumulatively imply that activities involving the Al-Quran, such as listening, reading, or memorizing, have a positive impact on various aspects of physical and mental health, cognitive function, and brain responses. The implications of these results may stimulate further research in the field of cognitive interventions, as well as provide a more in-depth look at the role of memorization in the development of cognitive abilities at critical developmental stages. Conclusion: From the five studies reviewed, it can be concluded that memory training interventions, especially in the context of memorizing the Qur'an, contribute positively to the improvement of cognitive function in various age groups, including children, adolescents, adults, and the elderly. The implications of these findings give impetus to further research in the field of cognitive intervention, focusing on memorization strategies, as well as providing deeper insight into the role of memorization in the development of cognitive abilities at critical stages of life.
