Abstract

Background: Maternal mortality ratio, infant mortality rate, and underfive mortality rate are still high in Indonesia. Pasuruan is one of the districts in East Java with the highest infant mortality rate. One of the public health interventions that has been undertaken to address this issue is maternal and child health surveillance program with community health workers being the leading implementers. This study aimed to describe the role of community health workers in maternal and child health surveillance to optimize maternal and child health care services in Pasuruan District, East Java.Subjects and Method: This was a qualitative study using case study approach. It was conducted in Pasuruan, East Java. Key informants were selected by criterion sampling. The data were collected by in-depth interview, focus group discussion, observation, and document review. Data validity was checked by triangulation.Results: Community based maternal and child health surveillance program has been implemented for 4 years. However, only some of all community health workes have been trained in this program. The tasks of these community health workers included advocacy and monitoring of maternal and child health in the areas of posyandu. The implementation of these surveillance program had some obstacles, including low insentive of community health worker, inactive community health worker, lack of awareness among community members on maternal and child health issues, and medical factors of pregnant mothers that caused mortality.Conclusion: Community health workers help improve the quality of maternal and child health care by surveillance program, although their performances have yet to be enhanced.Keyword: maternal mortality rate, infant mortality rate, community health workersCorrespondence: Ninil Dwi Pangestu. Abstract

Background: Data from the Ministry of Social Services Republic of Indonesia Social Ministry in 2009 showed there were 1,541,942 people with disability in the Indonesian population. Problems often arise regarding access to public-transportation services for people with disability. Despite the existence of UU No 4 in 1997 and UU No 25 in 2009 on accessibility for people with disability, public transport seemed to be poor for them. This study aimed to determine the public transport accessibility for people with disabilities and to provide alternative solutions.Subjects and Methods: This was a qualitative explorative study. Data were obtained from people with physical and mental disability, and provider (Division Head, Surakarta Public Transport Service Bureau). Data were analyzed using triangulation.Results: The local government of Surakarta has provided some transport facilities. However, these transport facilities are in low standard and non-accessible to people with disability. Transportation accessible to people with disability is not of priority. According to the provider this was because of limited budget. Consultation and coordination amongst the providers were on going in order to provide better transportation system accessible to people with disability.Conclusion: Transportation system was not accessible to people with disability. Abstract

Background: Health worker performance is one of the key elements that determine the quality of hospital service. Health worker performance therefore is the target forimprovement. This study aimed to analyze the relationship between motivation, competence, workload of nurses, and nurse performance.Subjects and Method: This was an analytical observational study with cross sectional design. This study was conducted at Dr. Soediran Mangun Sumarso Hospital, Wonogiri, Indonesia. A total of 50 nurses was sampled for this study.The independent variables consisted of motivation, competence, and workload. The dependent variable was performance of nurse. The data were collected by a questionnaire and analyzed by multiple linear regression model.Results: This study did not find relationship between motivation and performance of nurse (b = 0.02; 95% CI=-0.19 to 0.24; p= 0.823). Similarly there was no relationship workload and perfor­mance of nurse (b=0.05; 95% CI= -0.15 to 0.26; p = 0.607). There was a positive and statististically significant relationship between competence and performance of nurse (b = 1.10; 95% CI= 1.75 to 0.46; p= 0.001).Conclusion: There is a positive and statistically significant relationship between competence and performance of nurse.Keywords: motivation, competence, workload, performance, nurseCorrespondence: Agus Sutarto. dr. Abstract

Background: Indonesia ranks third in the world in the number of people with limited access to sanitation. Surakarta municipality government in collaboration with local drinking water supplier (PDAM) operate Indonesian Urban Water, Sanitation and Hygiene (IUWASH) to overcome sanitation problems in the form of Sanitation Kampung Program in Semanggi Village, Surakarta. This study aimed to review community participation in Sanitation Kampung Program, Semanggi Village.Subjects and Method: This was a qualitative study conducted at RW 23, Semanggi Village, Pasar Kliwon subdistrict, Surakarta, Central Java. Informants were selected purposively for this study comprising 12 community members as key informants, 2 community leaders, head of Sanitation Kampung Program, and 1 environmental health expert as supporting informants, The data were collected by in-depth interview, focus group discussion, and document review. Data credibility was checked by triangulation. The data were analyzed by content analysis.  Results: At the beginning of Sanitation Kampung Program planning there were some pros and cons from the community. Some of the community accepted the program but some others refused it due to negative perception that Sanitation Kampung Program would cause bad smell and pollute well water. In order to overcome community refusal toward Sanitation Kampung Program, IUWASH, Surakarta municipality government, and community leaders, carried out socialization, community approach, and study tour to other places. In the end, the community accepted Sanitation Kampung Program. Community members participated the program by providing support for water and sanitation facility development. Community members made use of the water and sanitation facility for daily activities. They also maintained the water and sanitation facility.Conclusion: By developing good and trustable collaboration, community participate in Sanitation Kampung Program from planning, developing, using,  and maintenance of the water and sanitation facility.Keywords: water, sanitation, community, participation, programCorrespondence: Anak Agung Alit Kirti Estuti Narendra Putri. Abstract

Background: Maternal and infant mortality has the third rank in East Java. The effort to reduce the number of maternal and infant mortality is through Mentoring Program Pregnancy and High Risk Infants Mortality or Gerakan Pendampingan Ibu Hamil Beresiko Tinggi (Gerdaristi). This study aimed to investigate the success rate of implementation of Gerdaristi program in reducing Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in Nganjuk.Subjects and Method: This was a case study with qualitative method. This was conducted in Nganjuk, East Java.  A sample of 10 pregnant women, 4 cadres, 2 coordinator midwives, 2 community leaders, 1 of the IBI informant, 2 key informant from department of health were selected by using purposive sampling. The data collection used interview, Focus Group Discussion (FGD), observation and documentation. The data were analyzed by chain of evidence, triangulation, member check, checks the extension of participation and colleagues.Results: Implementation Gerdaristi program every month hosted in the health center.  Cadres assisted health centers and conducted home visits every week. Gerdaristi program success is as high as the number of maternal and infant mortality from year to year decline. Nevertheless they found several things including not maximal assistance from the volunteer and mothers level of knowledge about high risk pregnancies are still lacking, and still have a maternal and infant mortality in some districts. This is due to several factors that inhibit awareness among pregnant women to give birth at the hospital as well as their lower mortality factor that cannot be predicted from the beginning.Conclusion: Gerdaristi program has been implemented in accordance with the initial plan involving several related sectors, although there is a few things that implementation is not optimal, but rated this program has been quite successful.Keywords: gerdaristi program, maternal mortality rate, infant mortality rateCorrespondence: Titin Ratnaningsih. Abstract

Background: Patient safety is a crucial issue and a focus of policy standard in internationally accredited organizations. The specific committee responsible for quality improvement in patient safety is the committee for quality improvement and patient safety (PMKP). Incidence of malpractice often occurs but are not reported. It indicates that the committee has not worked according to required accreditation standard. This study aimed to analyze patient safety management with the committee for quality improvement and patient safety at Sumbawa Hospital, West Nusa Tenggara. Subjects and Method: This was a qualitative study with phenomenology approach. Nine study participants were selected purposively, consisting of 3 committee members (chairperson, secretary, and patient safety sub-committee), 3 chiefs of ward (pediatrics, internal medicine, and surgery), and 4 nurses or midwives (pediatrics, surgery, ICU/ICCU, and obstetrics).Results: The committee for quality improvement and patient safety has been established at Sumbawa Hospital, West Nusa Tenggara to meet the requirement of hospital accreditation standard. It aimed to improve the quality of services and to assure patient safety. However, its work has not meet the required standard. The lack of knowledge among involving parties, including hospital management, PMKP member, medical professionals was identified as one important obstacle for the implementation of patient safety management. This has led to the incidence of malpractice at the hospital, sub-standard quality of services and patient safety.Conclusion: PMKP has been established at Sumbawa Hospital, West Nusa Tenggara. However, their work have not meet the required standard. Abstract

Background: Coverage of exclusive breastfeeding in Kalibawang, Kulon Progo, Yogyakartawas 688 in mothers (69.8%)in 2015, which mean did not reach intended target set by Health Ministry of 80%. One of the factors that can affect the failure of exclusive breast feeding is the flurry of mother andstart working. This study was aimed to investigate the exclusive breast feeding lactation management of qualitative study on working mother.Subject and Methods: This was a qualitative study, conducted in Yogyakarta, in July-September 2016. A total of 12 mothers with breast feeding and start working and 4 midwives who came from four Posyandu in Kalibawang. The data collected using the in-depth interviews, observation, document, and the data analyzed was using interactive analysis Miles and Huberman.Results: The result showed that knowledge management of lactation nursing mothers on mother and started working pretty well. Lactation management socialization barriers still prevailing lack of consciousness of mothers against the practice of lactation management because of lack of support from family, while the barriers experienced by mothers and started working in the lactation management do was the lack of opportunity is flushed and the unavailability of facilities in the workplace, as well as the existence of the syndrome of exclusive breastfeeding for less.Conclusion: the implementation of lactation management on mom and started the work influenced factor, there is cake and family support. Abstract

Background: Adolescents are susceptible to positive and negative influences from the social environment. One of the negative influences is smoking behavior. Smoking in adolescents can have deleterous effect on health and academic achievement. This study aimed to determine the association between cigarette advertisement, peer group, parental education, family income, and pocket money on smoking behavior among adolescents.Subjects and Method: This was an analytic observational study using cross-sectional design. This study was carried out in Colomadu Sub district, Karanganyar District, Central Java.  A sample of 100 teenagers was selected for this study. The dependent variable was smoking behavior. The independent variables were exposure to cigarette advertisement, peer group, parental education, family income, and pocket money. The data were collected by a set of questionnaire. The data were analyzed by logistic regression.Results: Smoking behavior in adolescents was associated with exposure to cigarette advertisement (OR=22.58; 95% CI =2.42 to 210.69; p=0.006), peer group (OR=44.00; 95% CI =3.99 to 485.33; p=0.002), parental education (OR= 36.92; 95% CI =3.12 to 427.81; p=0.004), family income (OR=0.09; 95% CI= 0.01 to 0.97; p=0.047), and pocket money (OR=10.56; 95% CI=1.22 to 91.56; p=0.032).Conclusion: Smoking behavior in adolescents was associated with exposure to cigerrete advertisement, peer group, parental education, family income, and pocket money.Keywords: cigarette advertisement, peer group, parental education, family income, pocket moneyCorrespondence: Windiarti Dwi Purnaningrum. Abstract

This article answers the question of how the form of revitalization of local wisdom in Aceh culture as the capital to resolve conflicts in the community. The important thing that will support this article is how Islamic ideas and Acehnese culture combine into one formulated from within, at least strengthening traditional institutions as the basis for managing Aceh culture is an important discourse to measure indicators of the ongoing settlement of Islamic conflicts. The results indicate that the re-actualization of Aceh culture which had been submerged by the conflict and tsunami, has now been revived especially on the social bases that were lost and even blurred. When there was a conflict in the Acehnese community, the mechanism for resolving conflicts was carried out using the Acehnese cultural approach known as Di’et, Sayam, Suloh, Peusijuek and Peumat Jaroe, involving Aceh traditional institutions at the village level, such as the geuchik (village headman), teungku imum and traditional leaders. This local wisdom in the discourse of the Aceh Government has reached the implementation of policies by making regional regulations through qanun to revitalize Aceh’s local wisdom by reviving some traditional institutions that have not been functioning.

Abstract

Serat Jangka Jayabaya is a literary work that can be used as a source of historical learning. The local genius of Javanese worldview is reflected in humanism values in the manuscript. Which is much misunderstood by most people even historians. This study uses qualitative research methods that reveal the value of Jayabaya humanism based on content, authorship and axiological manuscripts through interviews and literature studies. The results show that the concept of "jangka" is the guidance as well as the control of Java society in viewing various social phenomena. The guidelines are summarized in the concept of humanism values reflected as knowledge, equity, equality, dignity, and moral ethics.
