Abstract

Background: Patients with cardiovascular diseases (CVD) are vulnerable to experiencing drug-drug interactions (DDIs). DDIs are a concern among patients receiving multiple drug regimens but they are also an avoidable cause of adverse drug reactions. The study of potential DDIs (pDDIs) would make it easier for the healthcare provider to deliver better patient care and mitigate pDDIs incidence. Objective: This study aimed to assess the frequency, severity level and risk factors associated with pDDIs among medications used to treat hypertensive ambulatory patients in Universitas Airlangga hospital. Methods: A retrospective observational study was carried out from electronic prescriptions received by hypertensive patients in March 2021. Data collection includes demographic data, the profile of antihypertensive drug use, and pDDIs. pDDIs were identified by severity using Lexicomp Drug Interaction Checker (Application). Univariate logistic regression analysis was used to find associated factors of major pDDIs. A p-value less than 0.05 (≤ 0.05) was considered statistically significant. Results: From 704 patients, 53.98% women and 46.02% men, 89.06% (n = 627) patients had minor to major pDDIs; 1354 pDDIs were identified, 89.4% (n = 1,210) were moderate and 9.8% (n = 133) were major class. Multiple antihypertensive drug regimens had significance associated with the major pDDIs occurrence. Conclusion: We found a high prevalence of pDDIs among hypertensive patients. The majority of pDDIs were of moderate severity. Multiple antihypertensive drug regimens were associated factors in the presence of major pDDIs.

Abstract

ABSTRAK Penurunan jumlah pohon apel produktif mengakibatkan rendahnya daya saing apel di dalam negeri. Berkurangnya jumlah tanaman apel produktif salah satunya disebabkan oleh kapang patogen. Penelitian ini bertujuan untuk mengidentifikasi spesies kapang patogen yang menyerang batang tanaman apel berdasarkan sekuen ITS. Sampel diambil dari perkebunan Gabes, Dusun Junggo, Desa Tulungrejo Kecamatan Bumiaji, Kota Batu. Kapang diisolasi dari lima batang bergejala penyakit dan dimurnikan, diuji patogenisitas terhadap bibit tanaman apel, isolasi DNA, dan identifikasi molekular. Berdasarkan hasil penelitian gejala AN3 banyak ditemui pada perkebunan organik dengan prevalensi  59,6 , sedangkan gejala AN5 merupakan gejala penyakit yang paling sedikit dengan prevalensi  1,69. Isolat AN5.2 teridentifikasi sebagai Neofusicoccum parvum yang menyebabkan penyakit busuk kering dan berakibat kematian pada batang, sedangkan isolat AN3.2 teridentifikasi sebagai Aureobasidium pullulans.   Kata kunci: Aureobasidium pullulans, identifikasi molekular, kapang patogen, Malus sylvestris Mill., Neofusicoccum parvum

Abstract

Diabetes is one of the risk factors for cardiovascular disease (CVD). Diabetics patients have 2 to 4 times increased risk of cardiovascular disease compared with non-diabetics. TNF-alpha is a proinflammatory cytokine that can be used to determine the risk of atherosclerosis complications triggered by inflammation in diabetes. Statins are a class of HMG CoA reductase inhibitors that inhibit cholesterol biosynthesis and have pleioropic effects that inhibit the release of inflammatory cytokines likeTNF-alpha and stabilize atherosclerotic plaques. This study aims to determine the effect of atorvastatin 20 mg/day for 30 days in reducing the lipid profile and TNF-alpha inflammatory markers in patients with diabetes dyslipidemia. Diabetes patient with dyslipidemia who included the inclusion criteria in this observational prospective cohorts studies treated with atorvastatin for 30 days (n = 19). The efficacy of statin therapy was measured by lipid profiles (LDL, TG, HDL, and total cholesterol) and TNF-alpha. The results of the study showed that atorvastatin decreased 40.55% of LDL levels, 15.34% of TG levels, and 30.70% oftotal cholesterol levels which statistically significant (P <0.05). As for HDL, there is an increase of 6.06% but statistically non-significant (P >0.05). TNF-alpha levels increased by 11.30% which statistically non-significant (P >0.05). The use of atorvastatin 20 mg for 30 days gave reduction in LDL, TG, and total cholesterol and increased in HDL. Atorvastatin does not have a reducing effect on TNF-alpha. There was no correlation between lipid profile changes with TNF-alpha changes.

Abstract

The purpose of this study is to know the best of variation of retaninagent. Material used in this study are 30 pieces sheep skin wet blue leather while retanning agent added in three variables are Chrome-Blancorol, Chrome Lutan, Chrome-Tanesco. The results of the study were tested on their chemical, physical and visual properties. The data which are statistically analyzed using Completely Randomized Design (CRD) and Least Significant Difference (LSD) show that the best result is obtained by the sheep skins which are retannned using combination of chrome-Tanesco with stitch tear strength = 722.09 N/ cm, tear  strength = 140.45 N/ cm, chrome content = 4.4 %, dry rub fastness test show  no discolourisation, while wet rub fastness test show a little discolourisation and resistance of dry cleaning to perchloroetylene.    INTISARI Penelitian ini bertujuan untuk mengetahui kombinasi bahan penyamakan ulang terbaik pada pembuatan kulit nappa dari kulit domba untuk garmen. Bahan yang digunakan dalam penelitian ini sebanyak 30 lembar kulit domba belahan krom basah (wet blue). Bahan penyamak ulang yang digunakan adalah dengan 3 (tiga) macam variasi : Krom – Blancorol, Krom – Lutan dan Krom Tanesco. Kulit nappa hasil penelitian diuji secara kimiawi, fisis dan organleptis. Hasil analisa statistic dengan CRD dan LSD menunjukkan bahwa hasil terbaik dicapai oleh kulit nappa yang disamak ulang dengan kombinasi Krom Tanesco dengan nilai kekuatan jahit = 722,09 N /cm, Kekuatan sobek = 140, 45 N/cm, kadar krom = 4,40 %, ketahanan gosok cat tutup tidak luntur (secara kering) dan sedikit luntur (secara basah), serta tahan terhadap pencucian kering dengan perchloro ethylene.

Abstract

Drug information off label is not widely known and understood by health workers, both doctors and pharmacists. Prescribing drugs off label will also have an impact on ethical rules and drug costs, whether legal and can be covered by insurance. Therefore, there needs to be a continuing education activity for Pharmacists and health professionals to provide information and understanding of participants with the development of drugs off the label. This activity aims to improve the competencies of the participants, as well as understand the legal prescribing rules and as input for insurance and pharmaceutical regulations. The form of activities that will be carried out are workshops and training. The event was held for 2 days, 14-15 October 2018 at Graha Iptekdok, FKUA. There are 10 resource persons with 11 materials divided into 6 workshops, and 1 Micromedex training activity. This activity was attended by 383 participants and 5 invitations. Among the 383 participants, 55% were pharmacists working in hospitals, 30% were pharmacists working in pharmacies, and the rest were pharmacists working in industries, health centers, health services, and pharmacy students. Domicile participants come from various regions, with the largest percentage from Java. Based on the results of the questionnaire evaluation, the average material suitability assessment with participants’ needs was 8 out of 10. On the other hand, the material mastery by the speakers was also very good (8 out of 10). While in terms of the event, the average participant rated very well (7-9 of a scale of 10). A value of 7 is given to the parameters of room facilities. Seeing the enthusiasm of the participants of the Off Label Workshop andTraining in Community and Clinical Settings, the committee plans to follow up by holding a similar event to accommodate potential participants who cannot be accommodated in this event.AbstrakInformasi obat off label belum banyak diketahui dan dipahami oleh para tenaga kesehatan, baik dokter maupun apoteker. Peresepan obat off label juga akan berdampak pada aturan etik dan biaya obat, apakah legal dan bisakah ditanggung oleh asuransi. Oleh sebab itu, perlu adanya suatu kegiatan pendidikan berkelanjutan kepada para Apoteker dan profesi kesehatan untuk memberikan informasi dan pemahaman peserta dengan perkembangan obat off label. Kegiatan ini bertujuan untuk meningkatkan kompetensi para peserta, juga memahami aturan peresepan yang legal dan sebagai masukan untuk asuransi serta peraturan farmasi. Bentuk kegiatan yang akan dilakukan adalah workshop dan training. Acara dilakukan selama 2 hari, 14–15 Oktober 2018 di Graha Iptekdok, FKUA. Terdapat 10 narasumber dengan 11 materi yang dibagi dalam 6 workshop, dan 1 kegiatan pelatihan Micromedex. Kegiatan ini dihadiri oleh 383 peserta dan 5 undangan. Diantara 383 peserta, 55% merupakan apoteker yang bekerja di rumah sakit, 30% adalah apoteker yang bekerja di apotek, dan sisanya adalah apoteker yang bekerja di industri, puskesmas, dinas kesehatan, dan mahasiswa farmasi. Domisili peserta berasal dari berbagai daerah, dengan prosentase terbesar dari Jawa. Berdasarkan hasil evaluasi kuesioner, rata-rata penilaian kesesuaian materi dengan kebutuhan peserta adalah 8 dari skala 10. Selain itu, penguasaan materi oleh narasumber juga sangat baik (8 dari skala 10). Sedangkan dari segi acara, rata-rata peserta menilai sangat baik (7–9 dari skala 10). Nilai 7 diberikan pada parameter fasilitas ruangan. Melihat antusiasme dari peserta off Label Drugs Workshop and Training in Community and Clinical Settings, maka panitia berencana akan melakukan tindak lanjut dengan mengadakan acara yang serupa untuk mengakomodir calon peserta yang tidak dapat tertampung pada acara ini.

Abstract

Research on the influence of isoniazid on rifampicin metabolism using rat hepatocytes has been done. As the system metabolism, hepatocytes suspension was prepared by perfusion in the calcium binding media containing EDTA, citrate and glycine was used. In the experiment, combination of rifampicin and INH on various doses was incubated in those metabolism system at 30oC from 0 and up to 180 minutes duration. Then, analysis was done by thin layer chromathography and densitomeric method for each incubation intervals. Result calculated on the base of variation of rifampicin AUC-value beginning from 0 up to 180 minutes showed that isoniazid with concentration 5 µl, 10 µl, 15 µl, and 20 µl did not influence the metabolism of rifampicin (10 µl/ml) as showed by no significance changes of AUC value of rifampicin for α = 0.05.

Abstract

Resistensi antimikroba (AMR) merupakan masalah kesehatan yang mendunia. Resistensi tidak dapat dihilangkan namun dapat diperlambat dengan cara menggunakan antibiotika secara bijak. Salah satu cara yang dapat dilakukan untuk menanggulangi resistensi antimikroba dengan melakukan evaluasi penggunaan antibiotika baik secara kuantitatif maupun kualitatif. Tujuan penelitian ini adalah untuk mengetahui pola penggunaan antibiotika pada pasien ilmu  penyakit dalam RSUD Dr. H. Slamet Martodirdjo Pamekasan selama periode Januari-Maret 2019. Penelitian ini merupakan penelitian deskriptif-analitik (cross-sectional) dengan pengambilan data secara retrospektif serta jumlah sampel 482 pasien yang selanjutnya dianalisis menggunakan metode ATC dan DDD. Hasil penelitian menunjukkan nilai total DDD pemakaian antibiotika adalah 76,03/100 patient-days dengan total LOS pasien adalah 2270 hari. Penggunaan antibiotika tertinggi adalah ceftriaxone 37,56/100 patient-days, ciprofloxacin 25,78/100 patient-days, dan levofloxacin 6,05/100 patient-days. Golongan antibiotika yang masuk ke dalam segmen DU 90% yaitu ceftriaxone, ciprofloxacin, dan levofloxacin, sehingga harus dipantau penggunaannya karena berpotensi pada risiko resistensi. Kedepan perlu dilakukan penelitian lanjutan terkait evaluasi kualitas penggunaan antibiotika, serta dilakukan intervensi yang tepat untuk meningkatkan penggunaan antibiotika secara bijak dengan harapan dapat menekan angka resistensi antibiotika.

Abstract

Dispepsia Fungsional merupakan salah satu gangguan fungsi pada gastrointestinal dan dapat berpengaruh terhadap kualitas hidup pasien. Fucoidan memiliki banyak efek farmakologi, salah satunya sebagai mukoprotektor dan bersifat sebagai anti ulcer pada gastrointestinal yang diduga dapat memperbaiki gejala dispepsia. Tujuan dari penelitian ini yaitu menganalisis efek penambahan fucoidan pada omeprazol terhadap perbaikan gejala dispepsia dan kualitas hidup pasien pada pasien dispepsia fungsional. Hasil yang diperoleh adanya perbaikan gejala dan kualitas hidup sebelum dan setelah menerima penambahan terapi fucoidan dengan nilai p < 0,05. Pada perbandingan perbaikan yang dilakukan pada kedua kelompok tidak terjadi perbaikan yang signifikan secara statistik dengan nilai p > 0,05 untuk beberapa domain kuesioner. Akan tetapi dalam perbaikan gejala klinik yang diobservasi terdapat perbaikan gejala dispepsia dan kualitas hidup yang lebih baik pada penambahan fucoidan.

Abstract

The COVID-19 pandemic is still an unresolved global health concern, although half of the world's population has been vaccinated. The pharmaceutical industries are still struggling to develop effective antivirals against SARS-CoV-2. Molnupiravir is a new oral antiviral with antiviral properties by targeting coronavirus RNA. This literature review aims to describe the mechanism of action, efficacy, and safety of molnupiravir based on published preclinical and clinical studies for COVID-19 treatment. Relevant studies were collected by electronic databases, including Google Scholar, PubMed, and Science Direct. The inclusion criteria were preclinical and clinical trials related to molnupiravir as an antiviral for the COVID-19 treatment published in December 2019 to January 2022. Preclinical trials demonstrated therapeutic and prophylactic properties against SARS-CoV-2 in cell culture and animal models. Molnupiravir is currently under the emergency use authorization from the FDA to treat COVID-19. Its potent and broad antiviral activity is demonstrated through a mechanism of error catastrophe that causes coronavirus RNA mutagenesis. The published clinical trials have shown that molnupiravir is well-absorbed, well-tolerated, and has relatively mild side effects such as headache, nausea, and diarrhea with a minimal incidence at a dose of 800 mg twice daily. Time to viral RNA clearance was significantly decreased in patients administered molnupiravir 800 mg compared to those who administered placebo (14 days vs 15 days, P value=0,013). Molnupiravir is a promising oral antiviral that can reduce the incidence of COVID-19 hospitalization or death. Further clinical trials regarding its efficacy for severe symptoms and other clinical aspects such as drug interactions and contraindications are still needed.

Abstract

Diabetes mellitus is a syndrome characterized by a condition of hyperglycemia, and changes in the metabolism of lipids, carbohydrates, and proteins. Diabetes mellitus is characterized by an increase in glucose levels caused by insulin deficiency either in absolute or relative terms. Diabetes mellitus therapy is a long-term therapy and is relatively expensive.   Therefore, it is necessary to look for effective alternative drugs with low prices such as herbal medicines derived from plants. Indonesia is one of the countries that has a diversity of plants, so many plants that grow in Indonesia can be used as an alternative therapy to overcome diabetes mellitus. This paper aims to provide a relevant review of medicinal plants used for diabetes mellitus that has been carried out in research on tested animals. This research was conducted by searching for national and international articles from the internet, which were searched from 2011 to 2019, including original articles, and experimental articles, about Traditional medicine for diabetics. The following keywords are used : [[ “studi in vivo tumbuhan obat untuk diabetes” OR “Uji efektifitas ekstrak tumbuhan sebagai antidiabetik” OR “tumbuhan sebagai antidiabetik” ]] OR [["Test extract activity against diabetic mice" OR "antidiabetic plant" OR "antidiabetic herbal medicine" OR "antidiabetic in rat”, “Traditional medicine of diabetisin Indonesia” ]]. From the results of the literature study, it was found that 21 articles with 24 medicinal plants were tested in vivo on test animals to reduce blood sugar in diabetes mellitus. These medicinal plants contain compounds of the type of flavonoids, alkaloids, tannins, saponins, polyphenols, terpenoids, and phenolics that can have a therapeutic effect on decreasing blood sugar levels in test animals.
