Abstract

Highlights: 1. The significance of Staphylococcus aureus and coagulase-negative Staphylococcus, which are more likely to infect immunocompromised patients, needed to be researched in greater depth. 2. Coagulase-negative Staphylococcus was found to form significantly more biofilm than Staphylococcus aureus. 3. Wound care and changing medical devices in immunocompromised patients on a regular basis may provide benefits to prevent biofilm formation by Staphylococcus spp. Abstract Staphylococcus spp. are typically commensal microorganisms that can exist in the human body without causing illness. However, these bacteria have virulence factors, e.g., biofilm formation, that are important to note. Because biofilms shield bacteria from opsonophagocytosis and antimicrobial agents, they can cause persistent or chronic infections. Once they form biofilms, both Staphylococcus aureus and coagulase-negative Staphylococcus (CoNS) can potentially cause incurable infections. This study aimed to compare biofilm formation in Staphylococcus aureus and coagulase-negative Staphylococcus as a guide for the prevention and management of infection, which will maintain and improve the good health of the general population. This was an analytic research with a cross-sectional design. The study began by collecting the samples, identifying the species, and testing the biofilm production with a microtiter plate, which was then analyzed with an enzyme-linked immunosorbent assay (ELISA). Data analysis was conducted using IBM SPSS Statistics for Windows, version 25.0 (IBM Corp., Armonk, N.Y., USA). Comparison tests were conducted using an independent t-test. A value of p<0.05 was used as the cut-off that indicated significance. The total samples were 36 clinical isolates, consisting of 18 Staphylococcus aureus and 18 coagulase-negative Staphylococcus. The specimens consisted of 20 blood samples (55.6%) and 7 wound swabs (19.4%). The biofilm test on the samples showed that 83.3% of the samples produced biofilms. The data revealed that the isolates formed biofilms, with 14 isolates (38.9%) in the strong category, 10 isolates (27.8%) in the moderate category, and each of 6 isolates (16.7%) in the weak and non-existent categories. Both Staphylococcus spp. appeared to have biofilm-forming activity, but coagulase-negative Staphylococcus appeared to be significantly more dominant (p=0.008). Strong biofilm was produced by 61.1% of coagulase-negative Staphylococcus isolates. In conclusion, coagulase-negative Staphylococcus formed a stronger biofilm than Staphylococcus aureus. Its presence as an infection-causing bacteria, particularly in immunocompromised patients, should not be underestimated.

Abstract

Stunting masih menjadi salah satu permasalahan kesehatan global yang perlu mendapat perhatian serius. Kondisi ini dapat bermula sejak masa kehamilan, berlanjut pada periode pertumbuhan anak, khususnya dalam 1000 hari pertama kehidupan. Penanggulangan stunting memerlukan pendekatan komprehensif yang menyasar berbagai faktor penyebabnya. Kegiatan pengabdian masyarakat ini bertujuan menekan prevalensi stunting di Desa Ban, Kecamatan Kubu, Kabupaten Karangasem, Bali. Metode yang diterapkan meliputi edukasi mengenai pencegahan dan penanganan stunting, praktik penyusunan menu bergizi seimbang melalui konsep “Isi Piringku”, serta penyaluran bantuan pangan berupa ayam buras, telur ayam, dan beras organik. Selain aspek kesehatan, kegiatan ini juga mencakup penyuluhan terkait perlindungan hukum bagi perempuan, pelatihan singkat Bahasa Inggris, serta pelatihan pembuatan pakan ternak ayam. Hasil kegiatan menunjukkan adanya peningkatan keterampilan ibu dalam menyiapkan makanan bergizi seimbang berbasis “Isi Piringku” sebesar 49,8%. Oleh karena itu, dukungan dan partisipasi masyarakat secara berkelanjutan sangat diperlukan untuk menurunkan angka kejadian stunting.

Abstract

Hypertension in Chronic Kidney Disease (CKD) remains a global health issue, often presenting as resistant hypertension even with multiple antihypertensive medications. This study aims to describe the characteristics of hypertension in CKD patients undergoing hemodialysis at Sanjiwani Regional Hospital. A descriptive observational study with a cross-sectional design was conducted using secondary data from medical records of CKD patients with hypertension who underwent hemodialysis from January 2022 to December 2023, involving 100 patients. The study found that the highest proportion of hypertensive CKD patients were aged 46-65 years (60%), predominantly male (63%), with grade 1 hypertension (69%). The majority had CKD for ≥5 years (60%) and were taking ≤3 antihypertensive medications (72%). The commonly used antihypertensive drug class was calcium channel blockers (68%), with diabetes mellitus identified as the primary etiology (45%). Many CKD patients experience resistant hypertension, primarily affecting males aged 46-65 years.

Abstract

Snakebites are a common occurrence in tropical regions, including Indonesia. This study aims to determine the demographic and clinical characteristics of patients bitten by snakes. It employs a descriptive, cross-sectional approach to analyze cases at Sanjiwani General Hospital. The sample includes all snakebite cases from January 2021 to December 2022, totaling 192 cases. The results indicate that the majority of snakebite incidents occurred in individuals aged 40-49 years (25.0%), predominantly among males (69.3%), and that most cases originated in Gianyar District (26.6%). The most frequently reported clinical symptom was pain (96.9%), with 91.1% of cases graded 2. Most patients did not experience complications (97.9%). Fang marks were the most common type of wound (96.9%), with the upper extremities being the most frequent bite site (51.6%). Laboratory examinations revealed that the average complete blood count was within the normal range, although prothrombin time (PT) and activated partial thromboplastin time (APTT) were prolonged. The treatments administered included antivenom (99.0%), Human Tetanus Immunoglobulin (94.3%), and antibiotics (99.5%). Notably, all patients in the study survived. The findings highlight the urgent need for targeted public health interventions to educate high-risk populations about snakebite prevention and the importance of seeking prompt medical treatment. Additionally, this study emphasizes the need for improved healthcare resources and training for medical personnel to manage snakebite cases effectively, thereby improving outcomes and reducing complications.

Abstract

Diabetes mellitus (DM) remains a major public health problem globally. Glycated hemoglobin (HbA1c) is widely used to assess long-term glycemic control in type 2 diabetes mellitus (T2DM), and persistent poor control may precipitate chronic microvascular and macrovascular complications. However, HbA1c profiles among hospitalized T2DM patients with established chronic complications in tertiary referral settings are still limited. This study compared HbA1c values between hospitalized T2DM patients with and without chronic complications at Bali Royal General Hospital. A cross-sectional analytic observational study was conducted using 130 inpatient T2DM medical records from 2022–2024 obtained through consecutive sampling. Descriptive variables included sex, age, antidiabetic regimen (oral vs injectable), and hypertension status. Chronic complications were categorized as macrovascular or microvascular based on clinical diagnoses documented in the medical records. Most patients were women (57.7%), with a mean age of 60.57 ± 10.41 years. Injectable therapy predominated (66.2%), and 58.5% had no documented hypertension. Patients with chronic complications had a higher median HbA1c than those without complications (8.60% [5.0–15.0] vs 7.50% [4.9–15.0]; p = 0.040). Overall, HbA1c levels were frequently above common treatment targets, indicating suboptimal long-term glycemic control in this inpatient cohort. Among hospitalized T2DM patients in a tertiary referral hospital, chronic complications were associated with significantly higher HbA1c levels, reflecting poorer long-term glycemic control. These findings highlight the need for sustained HbA1c optimization alongside comprehensive risk-factor management to mitigate long-term complication risk in referral settings.

Abstract

Inability to understand the pathogenesis of severe dengue, in particular the control mechanism of immune responses, has led to high mortality rate for patients with dengue shock syndrome (DSS). The aim of this study was to determine the control mechanism of cytokine production by mediator suppressor of cytokine signaling (SOCS), toll-like receptor 3 (TLR-3) and nuclear factor kappa B (NFκB) during DENV infection. Peripheral blood mononuclear blood cells (PBMC), isolated from healthy individuals, were infected with dengue virus (DENV)-2 strain SJN-006 Cosmopolitan genotype (isolated from Bali, Indonesia). The relative gene expression of SOCS-3, TLR-3, NFκB, and the cytokine genes (interleukin (IL)-6, IL-8, interferon inducible protein 10 (IP-10), and macrophage inflammatory protein-1 beta (MIP-1β)) were measured using qRT-PCR at 6, 12 and 24 hours post infection (hpi). Student t-test and Mann-Whitney test were used to compare the gene expressions while causal correlations were analyzed using regression test and path analyses. DENV-2 infection increased the gene expression of SOCS-3, TLR-3, and NFκB after 12 and 24 hpi. The expression of IL-6, IL-8, IP-10, and MIP-1β genes was increased and peaked at different times post-infection. NFκB and SOCS-3 genes likely have role in the upregulation of IL-8 and IL-6 gene expression, respectively. MIP-1β gene expression was significantly induced by both NFκB and SOCS-3. In conclusion, our study suggested that SOCS-3, TLR-3, and NFκB are important in regulating the production of IL-6, IL-8, IP-10, MIP-1β during early phase of DENV-2 infection. This enriches our understanding on pathogenesis pathway of DENV-associated cytokine storm.

Abstract

Dampak kesehatan global dari diabetes melitus telah diterima secara luas di berbagai tempat. Program pelayanan masyarakat yang menangani diabetes memainkan peran penting dalam meningkatkan kesehatan masyarakat dengan mempromosikan kesadaran, pencegahan, pengelolaan penyakit, dan pencegahan komplikasi. Tujuan dari pelayanan masyarakat ini adalah untuk meningkatkan pengetahuan para mitra dalam membimbing pasien. Metode program ini meliputi diskusi kelompok fokus untuk mengidentifikasi masalah mitra, penyuluhan pendidikan, permainan peran dengan tes pra dan pasca untuk mengevaluasi peningkatan, dan evaluasi program. Hasilnya menunjukkan peningkatan pengetahuan mitra tentang diabetes melitus sebesar 80%. Dengan melibatkan masyarakat dalam langkah-langkah kesehatan proaktif, program-program ini tidak hanya bertujuan untuk mengurangi prevalensi diabetes tetapi juga berupaya meningkatkan kualitas hidup bagi mereka yang terkena dampaknya, yang pada akhirnya berkontribusi pada komunitas yang lebih sehat dan pencapaian SDGs ke-3.
