Abstract

Introduction: Nurses play a vital role in healthcare services, but various challenges make them consider leaving the profession, leading to nursing shortages both nationally and globally. However, studies exploring the factors influencing this intention remain very limited. This study aimed to investigate factors associated with nurses' turnover intentions, focusing on work environment and burnout. Methods: This study employed a cross-sectional design involving 252 nurses in Indonesia who had been working for at least six months, selected using non-probability sampling. Data were collected using a sociodemographic form, the Practice Environment Scale of the Nursing Work Index (PES-NWI), the Copenhagen Burnout Inventory (CBI), and the Turnover Intention Scale (TIS-6). Statistical analysis was conducted using the Chi-square test and logistic regression analysis, with α = 0.05. Results: 41.3% of participants intended to leave the nursing profession. Factors significantly associated with the turnover intentions were age: <30 (AOR=6.851; 95% CI=1.908-24.601; p=0.003) and 30-40 (AOR=3.017; 95% CI=1.214-7.499; p=0.017); region of origin: Java (AOR=4.276; 95% CI=1.296-14.107; p=0.017); highest education: bachelor’s (AOR=3.391; 95% CI=1.413-8.139; p=0.006); work experience: 2-4 years (AOR=8.828; 95% CI=1.792-43.477; p=0.007) and ≥5 years (AOR=6.806; 95% CI=1.047-44.224; p=0.045); work environment: poor (AOR=5.032; 95% CI=1.133-22.344; p=0.034); and burnout: moderate (AOR=3.064; 95% CI=1.304-7.198; p=0.01) and severe (AOR=12.741; 95% CI=3.940-41.198; p<0.001). Conclusion: This study underscores the importance of targeted retention strategies that address nurses’ diverse needs throughout their careers. Initiatives should focus on enhancing supportive work environments and addressing the various challenges nurses face at different career stages.

Abstract

Background: Research worldwide has highlighted the physical, psychological, social, and spiritual challenges faced by patients living with a permanent stoma following colorectal cancer. However, limited research has examined the spiritual dimensions of living with a stoma among Indonesian patients.Purpose: This study aimed to explore the spiritual meanings and experiences of patients living with a permanent stoma due to colorectal cancer in Indonesia. Methods: A qualitative phenomenological design using Heidegger’s interpretive phenomenological approach was employed. Fifteen participants who had been living with a permanent stoma following colorectal cancer for more than three months were purposively recruited, 12 of whom were Muslim. Data were collected at the National Cancer Hospital in Indonesia over nine months through in-depth, semi-structured interviews and analyzed using QSR NVivo 14 software. Results: Three overarching themes emerged: (1) experiencing spiritual distress following cancer diagnosis and stoma formation (including rejection of life diversion, anger toward God, and social withdrawal); (2) embracing life with a stoma (including making peace with oneself and God, receiving social and professional support, and accepting the stoma); and (3) reconstructing self-existence after stoma formation (including developing meaning, hope, life goals, and pride in living with a stoma). Conclusion: This study shows that spirituality helps Indonesian patients with colorectal cancer and a permanent stoma find meaning, achieve self-actualization, and make peace with themselves and God. Healthcare professionals should anticipate these patients’ spiritual needs through routine spiritual assessment and health education and, when necessary, facilitate their participation in support groups to help meet those needs.

Abstract

Background: Both cancer and its treatments have profound effects on sexuality. Sex, sexuality, and intimacy hold equal importance for cancer patients as they do for individuals without cancer. Discussions surrounding sexuality are often not viewed as urgent or essential and may even be considered taboo in certain cultural contexts. The quality-of-life issues have become secondary to the primary goal of curing cancer, with little emphasis on addressing intimacy-related problems. This study aims to identify the effective interventions for addressing the sexual needs of patients with cancer. Method: This is a systematic literature review. A thorough literature review in three data bases including PubMed, Science Direct and Sage was conducted.. The article analysis process used the PRISMA method. Results: 10 articles were reviewed that met the criteria, and the articles used randomized controlled trials, quasi-experimental studies, and cross-sectional methods. Conclusion: Cancer and its treatment can cause various physical and psychological changes that can significantly impact the patient's sexual function. The interventions that address the sexual needs of cancer patients, such as psychosexual support, sexual psychoeducation, sexual communication, and social support, are an important need for cancer patients to overcome their sexual problems, to adapt, to maintain intimacy, and to improve their quality of life. Sexual needs are an important aspect that must be addressed and managed comprehensively in the management of cancer patient care to improve their quality of life.
