Abstract

Background: esophageal stricture is loss of esophageal lumen patency and may involve injuries after ingesting caustic substances. This systematic review aims to review clinical presentations, diagnostic modalities involved, treatments and outcomes, and complications of esophageal strictures following caustic injuries. Methods: this systematic review involves original researches conducted between 2002-2022 indexed in PubMed and EuropePMC searched using following MeSH terms: “((caustic injury) OR (caustic burn)) AND (ingestion) AND (inflammation)” following PRISMA algorithm. Biases were assessed using STROBE checklist and presented as narratives. Results: searches yielded 326 results with 16 studies included in this review. Majority of caustic ingestion and strictures involved pediatric population due to accidental ingestion. Alkali ingestion has more severe presentation. Radiological imaging, including CT and MRI, may help in determining damages involved. Degrees of damages are related with mortalities or morbidities, and dilatation may improve quality-of-life in some cases. Conclusion: pediatric patients are at risk for accidental ingestions and alkali ingestion creates more severe damage. Radiological imaging may assist in determining involvement of underlying tissues.

Abstract

Investigation into the mechanism underlying excessive trophoblast invasion yields further strategies and insights for the diagnosis and treatment of placenta accreta spectrum disorder (PASD). We conducted a comprehensive literature review to analyze the relationship between CXCR2 expression and PASD, as well as the possibility of CXCR2 being used as a therapeutic and diagnostic biomarker for PASD. Chemokines are well-known mediators in the immune system, particularly for cell recruitment, angiogenesis, and tumor infiltration. CXCR2 is an important component of the immune system, particularly in neutrophils. One of the CXCR2 ligands, IL-8, has also been found to be expressed in the decidua and trophoblasts of humans and to promote autocrine or paracrine trophoblast migration and invasion. The potential role of CXCR2 in trophoblast invasion in PASD provides researchers with a glimpse into the molecular pathogenesis of PASD.CXC Motif Chemokine Receptor 2: Sekilas tentang Patogenesis Molekuler Gangguan Spektrum Plasenta AkretaAbstrakInvestigasi mekanisme invasi berlebihan trofoblas pada gangguan Spektrum Plasenta Akreta (SPA) akan memberikan lebih banyak strategi dan ide untuk diagnosis dan pengobatan.

Abstract

Scabies is an infectious skin disease caused by the Sarcoptes scabiei. This mite infestations not only cause skin damage, but can develop into local, even systemic skin infections. Therefore, instruments are needed that help early detection of scabies, especially through diagnosis in communities such as Islamic boarding schools. This study aimed to analyze the reliability of the scabies detection form (DESKAB) between Madrasah Aliyah female student rater and dermatovenerologist rater. Based on the results of the examination and Cohen's kappa test, a coefficient of 0.84 was obtained, which means that the level of reliability of the DESKAB form is strong, and a p value of 0.019 means that there is agreement in the perception of the diagnosis of scabies by the two raters. The DESKAB form can be recommended as an early detection instrument for non-medical personnel before being confirmed by medical personnel Keywords: DESKAB; diagnosis; non-medic; scabies

Abstract

Background: Limb salvage surgery (LSS) is a crucial treatment for malignancies, which mostly affects the distal femur and proximal tibia. LSS encompasses biological and non-biological reconstructions, including megaprosthesis. However, limited access to megaprosthesis in developing countries has led to alternative methods like arthrodesis such as modified arthrodesis with metal and cement (MAMC). LSS may lead to complications, requiring careful technique and implant selection but none of them conduct research about mechanical test. This research was comparing the effectiveness of techniques like MAMC with Juvara, in mechanical axial loading perspective. Methods: In this true experimental study, conducted at the Metallurgy Laboratory of the Faculty of Engineering, Universitas Sriwijaya, femur and tibia bones from cattle were used. Five samples of each variable were used based on the purposive sampling method. The research focused on variables like deformity angle and yield point which assessed using X ray. Bone constructs underwent various fixation techniques, including MAMC and Juvara. Testing involved axial pressure on the femur head using a mechanical press machine. Statistical analysis compared implant angulation after compression between Juvara and MAMC techniques under 30 kg and 60 kg loads, ultimately measuring the yield point. Results: All five samples from each group, subjected to both 30kg and 60kg pressure, exhibited no discernible changes. The average yield point for the MAMC group was notably 160KgF higher than Juvara, with respective means of 296 and 126kgF (p>0.05). Further analysis employing an Independent T-test confirmed a significant disparity between the MAMC and Juvara techniques concerning the yield point with a p-value of <0.001. Conclusion: Both the MAMC and Juvara techniques in this study effectively supported 30 and 60-kilogram loads, demonstrating their suitability for full weight-bearing compression, with MAMC exhibiting significantly greater compression resistance compared to Juvara (p = 0.001).

Abstract

Vitamin D plays a role in the healing process of diabetic foot ulcers (DFU); it can improve the immune system, regulate inflammation function, and regulate growth factors in diabetes mellitus (DM). The immunomodulatory effects of vitamin D include reduced production of IL-2, IL-6, and TNF-alpha and increased production of IL-10. It also increases the activity of antimicrobial peptides (AMP) such as cathelicidin, and it promotes the differentiation of monocytes into macrophages, phagocytosis, and the production of lysosomal enzymes. Furthermore, vitamin D stimulates growth factors including vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-β). In this study, the incidence of DM and DFU in DM patients at RSUP Dr. Mohammad Hoesin Palembang will be compared to serum vitamin D levels. This research uses quantitative methods with an analytical observational design. The Mean vitamin D serum level was 11,727 ng/mL, and there was a significant association between serum vitamin D deficiency and DFU (p-value; 0.036 < 0.05). There was a significant association between serum vitamin D deficiency and DFU occurrence (p-value; 0.036 < 0.05) in Mohammad Hoesin General Hospital Palembang. Based on the results of this study, we proposed that patients with diabetic foot ulcers should have their serum vitamin D levels checked, and that vitamin D should be given to them as part of their therapy.
