Abstract

Leukemia is a blood cancer that occurs due to abnormal blood cell growth, especially white blood cells in the bone marrow. Research shows that curcumin is proven to be effective in inhibiting the activity of the NF-κB transcription factor which is often involved in the proliferation of cancer cells. This study aims to determine the potential of natural curcuminoid compounds against specific blood cancers, namely chronic lymphocytic leukemia. Through in-silico testing of curcuminoid derivative compounds against the Bruton Tyrosine Kinase protein target which plays a role in the pathophysiological process of chronic lymphocytic leukemia. The results of docking the test ligand molecule on the Tyrosine-protein kinase BTK enzyme, the native ligand has the lowest binding energy value (ΔG) of -12.04 kcal/mol with an inhibition constant (KI) of 1.49 nM. As a comparison drug, imatinib showed ΔG -11.05 kcal/mol with a KI of 7.88 nM. Test ligands s02, s05, and s08. Ligand s02 showed a ΔG of -9.43 kcal/mol with a KI of 123.19 nM, while s05 had a ΔG of -9.09 kcal/mol with a KI of 218.10 nM. Meanwhile, s08 recorded a ΔG of -9.33 kcal/mol and a KI of 143.83 nM. Although the affinity values of the three test ligands were lower than those of the natural ligands, they had the potential to bind to the target enzyme. Based on the interaction analysis, it was seen that s05 had the potential for competitive inhibition, while s02 and s08 showed lower interaction stability compared to imatinib. This indicates that s05 has the greatest potential as a potential inhibitor of the BTK enzyme compared to other test ligands.

Abstract

Abstract

Corn (Zea mays L.) is an important commodity in Indonesia, but in Citaman Village, Nagreg District, its utilization is still limited to fresh sales with low added value. To address this issue, a community service program was implemented that focused on empowering village women through training in processing corn into corn nuggets as an innovative product with economic value. The activity was designed with a community-based participatory approach that included coordination with village officials, extension sessions, processing demonstrations, hands-on practice, and evaluation through pre- and post-training questionnaires. The results of the activity showed that more than 85% of participants were very satisfied and more than 80% expressed a high interest in developing corn nuggets as a small-scale business opportunity. These findings indicate a significant increase in participants' knowledge, skills, and entrepreneurial motivation. The corn nugget innovation is not only a means of optimizing the use of local commodities, but is also relevant to the global trend of plant-based foods and supports the achievement of the Sustainable Development Goals (SDGs), specifically Zero Hunger, Decent Work and Economic Growth, and Responsible Consumption and Production. Overall, this program proves that local food-based innovation can be used as a model for replicating community empowerment that contributes to strengthening economic resilience while encouraging sustainable development in rural areas.

Abstract

Stunting is strongly associated with impaired immune function, as chronically malnourished children tend to have weaker immunity and are more vulnerable to infection. This study aimed to examine the effect of temulawak–black cumin herbal honey on the adaptive immune system in stunted children. A quasi-experimental one-group pre-test and post-test design was used. The findings showed that the intervention significantly increased height-for-age (TB/U) in the treatment group compared to the control group (p = 0.014). Weight-for-age (BB/U) also rose significantly within the treatment group (p = 0.013), but no significant difference was found relative to the control group (p = 0.192). The supplement significantly elevated IgM levels in the treatment group (p = 0.001), though again without a significant difference from the control (p = 0.546). CD4 concentration decreased in the treatment group (p = 0.003), but the change was not significantly different from the control (p = 0.682). In conclusion, temulawak–black cumin herbal honey can improve nutritional status and enhance adaptive immunity by increasing IgM levels, although it does not raise CD4 levels and instead slows CD4 decline. However, its effectiveness in increasing IgM and CD4 concentrations compared to controls is not significant.

Abstract

Insulin therapy is essential for managing type 2 diabetes mellitus (T2DM), particularly in patients who fail to achieve glycemic targets with oral antidiabetic agents. Long-acting insulin is primarily used to control basal glucose levels, while rapid-acting insulin targets postprandial hyperglycemia. However, comparative real-world evidence regarding their effectiveness on glycated hemoglobin (HbA1c) and fasting blood glucose (FBG) remains limited. This study aimed to evaluate and compare the effectiveness of long-acting and rapid-acting insulin in improving HbA1c and FBG levels among patients with T2DM. A retrospective before–and–after observational study was conducted involving 122 T2DM patients treated at the outpatient unit of Majalaya Regional General Hospital between January and December 2024. Patients received either long-acting insulin (e.g., insulin glargine) or rapid-acting insulin (e.g., insulin lispro and insulin aspart) as monotherapy. Changes in HbA1c and FBG before and after therapy were analyzed using paired t-tests or Wilcoxon signed-rank tests. Clinical effectiveness was defined according to American Diabetes Association criteria as a reduction of ≥1% in HbA1c or ≥30 mg/dL in FBG. Insulin therapy significantly reduced HbA1c (−7.77 ± 3.09, p < 0.001) and FBG levels (Z = −5.53, p < 0.001). Based on ADA criteria, 90.3% of patients achieved an effective reduction in HbA1c, while 43.5% achieved an effective reduction in FBG. Insulin lispro and insulin glargine showed the highest HbA1c-based effectiveness (100%), whereas FBG-based effectiveness varied across formulations. Insulin therapy significantly improves long-term and short-term glycemic control in T2DM patients, with insulin lispro and insulin glargine demonstrating the most consistent effectiveness.
