Abstract

Chronic Suppurative Otitis Media (CSOM) is a persistent middle ear infection often associated with Pseudomonas aeruginosa, a multidrug-resistant bacterium. Nigella sativa, commonly known as black seed, is known to have antimicrobial properties and is traditionally used in alternative medicine. This study aims to evaluate the inhibitory effect of Nigella sativa extract on the growth of Pseudomonas aeruginosa isolated from ear secretions of CSOM patients. A laboratory experimental study was conducted using ear discharge specimens from patients diagnosed with CSOM. The bacteria were cultured and identified as P. aeruginosa. Ethanol extract of Nigella sativa was prepared in various concentrations (25%, 50%, 75%, and 100%). The antibacterial activity was measured using the disc diffusion method by assessing the inhibition zone diameters. The Nigella sativa extract demonstrated a concentration-dependent antibacterial effect. The largest inhibition zone was observed at 100% concentration, with a mean diameter of 16.1 mm, indicating vigorous antibacterial activity against P. aeruginosa. Statistical analysis showed significant differences in inhibition zone diameters across concentrations (p < 0.05). Nigella sativa extract exhibits promising antibacterial activity against Pseudomonas aeruginosa isolated from CSOM patients. It has potential as a natural therapeutic alternative for treating infections associated with CSOM.

Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease with a high prevalence in primary healthcare settings; however, real-world evidence regarding the effectiveness of oral antidiabetic therapies remains limited. This study aimed to compare the effectiveness of oral antidiabetic monotherapy and combination therapy in patients with T2DM at the Teladan Community Health Center (Puskesmas) in Medan. A non-experimental observational study with a cross-sectional design was conducted on 78 patients selected via systematic random sampling, utilizing medical records and data from the "referral-back" program covering the period from September to November 2025. Effectiveness was assessed using random blood glucose (RBG) levels and analyzed with the Kruskal–Wallis test. The results indicated that the combination of glimepiride and metformin was the most frequently used regimen (64.10%), followed by metformin monotherapy (26.92%) and glimepiride monotherapy (8.97%). The lowest mean RBG level was observed in the metformin monotherapy group (147.80±47.27 mg/dl), followed by the metformin-glimepiride combination group (168.72 ± 60.56 mg/dl) and the glimepiride monotherapy group (178±61.31 mg/dL); however, no statistically significant difference was found among the treatment groups (p=0.322). The study concludes that there is no significant difference in effectiveness between monotherapy and combination therapy regarding glycemic control, although metformin showed a trend toward better outcomes. These findings underscore the importance of an individualized approach to therapy for T2DM patients in primary healthcare facilities.

Abstract

Dispepsia merupakan salah satu gangguan saluran pencernaan yang umum terjadi di masyarakat dan berkaitan erat dengan pola hidup tidak sehat serta penggunaan obat yang tidak rasional. Rendahnya literasi kesehatan masyarakat mengenai faktor risiko dispepsia dan penggunaan obat lambung yang aman menjadi permasalahan yang perlu mendapat perhatian. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan masyarakat mengenai dispepsia, mendorong penerapan gaya hidup sehat, serta meningkatkan penggunaan obat lambung yang rasional. Kegiatan dilaksanakan di Puskesmas Teladan Kota Medan dengan melibatkan 87 peserta yang terdiri dari pasien rawat jalan, keluarga pasien, dan kader kesehatan. Metode yang digunakan meliputi sosialisasi, edukasi interaktif, distribusi media edukasi (leaflet, booklet, dan video), pendampingan, serta evaluasi menggunakan pre-test dan post-test. Analisis data dilakukan secara deskriptif. Hasil kegiatan menunjukkan adanya peningkatan rata-rata skor pengetahuan peserta dari 55,6 menjadi 83,6 dengan selisih sebesar 28,0 poin. Selain itu, terjadi perubahan perilaku kesehatan, dimana 71% peserta mulai menerapkan pola makan teratur, 64% mengurangi konsumsi makanan pemicu, dan 58% mulai berkonsultasi dengan tenaga kesehatan sebelum menggunakan obat lambung. Kegiatan ini terbukti efektif dalam meningkatkan literasi kesehatan masyarakat serta mendorong perubahan perilaku terkait pencegahan dan pengelolaan dispepsia. Program ini berpotensi untuk dikembangkan secara berkelanjutan dan direplikasi di fasilitas kesehatan lainnya.

Abstract

Consumption of sugar-sweetened packaged beverages among elementary school children continues to increase and may contribute to various health problems, including kidney damage. Limited knowledge regarding the health risks of sugary beverages is one of the factors contributing to high consumption among children. This community service program aimed to improve students’ knowledge about the dangers of packaged sugary drinks on kidney health through health education activities conducted at SD Ar-Ridha, Biru-Biru District, Deli Serdang Regency. The activity was carried out on November 23, 2024, involving 30 elementary school students. The methods used included interactive lectures, discussions, question-and-answer sessions, and evaluation through pre-test and post-test questionnaires. Educational materials covered types of sugary packaged beverages, added sugar content, kidney function, the effects of excessive sugar intake, and kidney disease prevention efforts. The results demonstrated an increase in participants’ knowledge levels after the educational intervention. The percentage of students categorized as having good knowledge increased from 20.0% during the pre-test to 80.0% during the post-test, while the poor knowledge category decreased from 46.7% to 3.3%. These findings indicate that interactive health education methods are effective in improving students’ understanding of the importance of limiting sugary packaged beverage consumption to maintain kidney health from an early age.

Abstract

Salted anchovies (Stolephorus sp.) are a long-known and popular delicacy among Indonesians. Salting is the most common method of preserving fish. Table salt (NaCl) is the most common and widely used method. The salting process takes a long time, leading traders who want to make large profits to add formalin to the fish to prevent it from rotting quickly and to preserve it for a long time. This study aims to determine whether anchovies sold at the Simpang Limun market in Medan City contain formalin and the levels of formalin identified in salted anchovies using the VIS spectrophotometry method. The research was carried out employing a purposive approach, and the samples analyzed were anchovies sourced from both the Simpang Limun market and the Modern Market. The independent variables in this research included anchovies, Schryver reagent, KMnO4 reagent, Schiff reagent, as well as VIS Spectrophotometry. To perform qualitative analysis of formalin, KMnO4 reagent, Schryver reagent, and Schiff reagent were utilized, while the quantitative analysis was conducted using the VIS spectrophotometry method with Schiff reagent. The findings from the validation of the method indicated a detection threshold of 10.39 µg/mL and a quantitative limit of 34.65 µg/mL, with formalin recovery rates in anchovy samples between 98.8 and 101.16%. Qualitative examinations of six anchovy samples returned negative outcomes, and the quantitative analysis across all samples supported the findings, revealing that only one sample was free of formalin.

Abstract

Amoxicillin is widely used to treat gastric infections; however, conventional oral dosage forms often exhibit rapid gastric emptying and short gastric residence time, which may reduce therapeutic effectiveness. Gastroretentive floating drug delivery systems can prolong gastric retention and provide sustained drug release. This study aimed to develop gastroretentive floating beads of amoxicillin using pectin extracted from breadfruit peel (Artocarpus altilis) and compare their characteristics with those prepared using commercial pectin. Floating beads were prepared by ionotropic gelation using calcium chloride as a crosslinking agent and sodium bicarbonate as a gas-forming agent. The beads were characterized for morphology, floating behavior, entrapment efficiency, and drug release. All evaluations were performed in triplicate (n = 3). The prepared beads exhibited spherical morphology with diameters of 0.24–0.42 mm and weights of 9.26–11.38 mg. Floating lag time ranged from 10–22 seconds, and all formulations remained buoyant for up to 8 hours. Entrapment efficiency ranged from 31–47%, with formulations prepared using commercial pectin showing values of 34–47% and breadfruit peel-derived pectin showing values of 31–43%, with formulation F3 containing 3.5% commercial pectin showing the highest value. In vitro drug release studies demonstrated sustained release of amoxicillin for up to 8 hours, whereas conventional tablets released 64.77% of the drug within 2 hours. Drug release followed the Higuchi model (R² = 0.989–0.996), indicating diffusion-controlled release. These findings demonstrate that breadfruit peel-derived pectin is a promising natural polymer for gastroretentive floating bead formulations and exhibits performance comparable to commercial pectin.

Abstract

Acute respiratory infections (ARIs) remain one of the leading causes of morbidity worldwide and contribute substantially to outpatient visits in primary healthcare facilities. Inappropriate antibiotic use in ARI management may accelerate antimicrobial resistance, a major global public health concern. Therefore, monitoring antibiotic utilization through Rational Drug Use (RDU) indicators is essential to promote appropriate prescribing practices. This study aimed to evaluate antibiotic prescribing patterns and assess compliance with Rational Drug Use indicators among ARI patients at Teluk Merbau Primary Health Care Center. A retrospective descriptive cross-sectional study was conducted using prescription records of patients diagnosed with ARIs from January to December 2022. Total sampling was applied, and all eligible prescriptions were included in the analysis. Data were analyzed descriptively using frequencies and percentages based on the Rational Drug Use indicators established by the Indonesian Ministry of Health. A total of 261 ARI prescriptions were analyzed. Female patients accounted for 55.94% of cases, while adults represented the largest age group (62.07%). Among all prescriptions, 90 contained antibiotics. Amoxicillin was the most frequently prescribed antibiotic (56.13%), followed by cefixime (18.33%), ciprofloxacin (11.83%), cefadroxil (10.10%), cotrimoxazole (2.02%), and metronidazole (1.59%). The proportion of antibiotic prescribing was 26.97%, exceeding the national RDU target of less than 20%. Antibiotic prescribing among ARI patients at Teluk Merbau Primary Health Care Center exceeded the recommended RDU indicator. Continuous monitoring, adherence to clinical guidelines, and implementation of antimicrobial stewardship programs are necessary to promote rational antibiotic use.
