Abstract

Chronic Kidney Disease (CKD) merupakan kondisi penurunan fungsi ginjal progresif yang dapat menyebabkan gangguan keseimbangan cairan dan elektrolit sehingga memicu terjadinya edema. Penelitian ini bertujuan mendeskripsikan penerapan ankle pump exercise dan elevasi kaki 30° untuk menurunkan derajat edema pada pasien CKD di RSUD Prof Dr Margono Soekarjo Purwokerto. Penelitian menggunakan desain studi kasus deskriptif pada satu pasien laki-laki usia 71 tahun dengan diagnosis CKD stadium V disertai anemia gravis. Intervensi diberikan berupa ankle pump exercise sebanyak 10 kali dan elevasi kaki 30° selama 15 menit, dilakukan satu kali per hari selama dua hari berturut-turut. Data dikumpulkan berdasarkan hasil pengkajian dan evaluasi yang diperoleh dari data subyektif, objektif, dan pemeriksaan penunjang. Hasil menunjukkan adanya perbaikan kondisi pasien dengan penurunan derajat edema dari derajat 1 dengan capillary refill time (CRT) 4 detik menjadi tidak terdapat edema dengan CRT <3 detik. Penerapan ankle pump exercise dan elevasi kaki 30° efektif membantu menurunkan derajat edema dan memperbaiki perfusi perifer pada pasien CKD.

Abstract

Congestive Heart Failure (CHF) is one of the fastest growing cardiovascular diagnoses. Heart failure patients will get therapy given at home. The role of the family is very important in the stages of health care, starting from the stages of health promotion, prevention, treatment, to rehabilitation. The purpose of this study was to find out how families are prepared to care for their family members with heart disease. This study was included in a descriptive study with a cross-sectional approach which was carried out at Priscilla Medical Center Cilacap Hospital. The population in this study were all families of CHF patients at Priscilla Medical Center Cilacap Hospital with a sample of 56 people using the consecutive sampling technique. The results showed that most of the 32 (57%) families were not ready to care for family members with heart disease, whereas 24 (43%) families were ready to care for family members with heart disease.

Abstract

Complications that many respondents complained about after surgery with general anesthesia, namely Post-Operative Sore Throat (POST). Post-surgery sore throat is a minor complication that occurs in ETT intubated patients and results in dysphonia, dysphagia and odynophagia. In Indonesia, 20%–60% of respondents complain of sore throat after surgery with GA-ETT. The purpose of this study was to see the description of post-general anesthesia sore throat with ETT at Dr. Soedirman Kebumen. The design of this study is a descriptive cross-sectional approach using the POST instrument. The research was conducted from March to April 2023 in the recovery room with 50 respondents. Data collection by accidental sampling and analyzed using univariate test. The results of the study showed that many respondents did not experience throat pain, totaling 20 respondents (40%) and complaining that the most complaints were mild pain, amounting to 17 respondents (34%). This study shows that the majority of patients only complain of mild pain and no sore throat. So after ETT intubation at Dr. Soedirman Kebumen can minimize the level of sore throat.

Abstract

Acute Myocardial Infarction (AMI) is one of the cardiac diseases that can arise when blood flow in the coronary artery is restricted, depriving the heart muscle of oxygen. Anxiety is a common mental health problem in Acute Myocardial Infarction (AMI) patients. One of the many treatments for anxiety that is accessible is using educational programs. Prevention, medical care, and lifestyle modifications like nutrition, stress reduction, exercise, and quitting smoking are all covered in the health education offered. This study aims to determine whether providing educational programs has an effect on individuals with acute myocardial infarction (AMI) in terms of anxiety. With a sample size of 44 participants, both intervention and control groups received education in the form of a 5-minute video as part of the study's Quasy Experimental Nonequivalent Control Group research design. The State Trait Anxiety Inventory (STAI), a questionnaire comprising 40 statement questions, was the instrument employed in this investigation. The findings of this study indicate that there is an impact of educational programs on anxiety in acute myocardial infarction (AMI) patients (P=0.042 0.05).

Abstract

Cancer is the cause of death and loss of hope for cancer patients around the world, especially breast cancer. Breast cancer sufferers have psychological, social, and physical effects. One of the physical effects is decreased sexual function. Sexual function in cancer patients is an important element that can affect their quality of life. One of the causes of decreased quality of life in cancer is influenced by sexual function. The purpose of this study was to find out the description of sexual satisfaction in breast cancer patients at RSUD, Prof. Dr. Margono Soekarjo Purwokerto. This type of research is quantitative, using descriptive research methods with a cross-sectional approach. The sampling technique using non-probability sampling with purposive sampling using the Slovin formula obtained the results of 79 respondents. The research instrument used was the Sexual Satisfaction Scale for Women (SSS-W) questionnaire, which consisted of 30 questions. Data analysis using a univariate test. Data collection was carried out on April 5–18, 2023. The results showed sexual satisfaction in breast cancer patients at RSUD, Prof. Dr. Margono Soekarjo Purwokerto, namely in the satisfied category, 56 respondents (70,9%)

Abstract

Introduction: Anxiety is a psychological problem that often arises in patients before undergoing surgery, especially in the pre-anesthesia phase. This condition can affect physiological responses such as increased heart rate, irregular breathing patterns, and high blood pressure. At Ajibarang Regional General Hospital, most pre-anesthesia patients are known to experience moderate to severe anxiety, while standardized non-pharmacological interventions to address this problem are not yet available. Therefore, an effective and easy-to-implement approach is needed to help reduce patient anxiety before surgery. Objective: Assessing the effectiveness of deep breathing relaxation techniques as a non-pharmacological intervention in reducing anxiety levels in pre-anesthesia patients at Ajibarang Regional General Hospital. Method: The intervention was carried out using deep breathing relaxation techniques through controlled breathing exercises designed to stimulate the parasympathetic nervous system, thereby creating a state of relaxation. The patients' anxiety levels were measured using the APAIS (Amsterdam Preoperative Anxiety and Information Scale) instrument before and after the intervention. A total of 30 pre-anesthesia patients underwent this structured therapy process. Result: The results show a significant decrease in anxiety after administering deep breathing relaxation techniques. A total of 30 participants took part in the activity, predominantly adults (96.7%) and women (66.7%). The majority underwent major surgery (83.3%). Before the intervention, anxiety levels were in the moderate (93.3%) and severe (6.7%) categories. After being given deep breathing relaxation techniques, there was a decrease in anxiety, with 93.3% of respondents in the mild anxiety category and no one experiencing severe anxiety. Conclusion: Deep breathing relaxation techniques have been proven effective in reducing anxiety levels in pre-anesthesia patients at Ajibarang Regional General Hospital. This intervention can be recommended as a simple, safe, and easy-to-implement non-pharmacological procedure as part of preoperative preparation in the pre-anesthesia room.

Abstract

Introduction: Patients undergoing regional anesthesia are at increased risk of falls during the early postoperative period due to temporary motor and sensory impairment, as well as orthostatic hypotension. Falls may result in serious complications, including spinal cord injury. Therefore, health education on fall risk prevention and safe mobilization is essential to improve patient safety during recovery. Objective: To improve patients' knowledge regarding fall prevention through health education and safe mobilization demonstrations among postoperative patients receiving regional anesthesia. Method: This community service program was conducted in the Recovery Room of RSI Purwokerto involving 30 postoperative patients receiving regional anesthesia. Activities included fall risk screening using the Morse Fall Scale, motor function assessment using the Bromage Score, sensory block assessment, pre-test, health education using leaflets, safe mobilization demonstrations, discussion, and post-test evaluation. The results were analyzed descriptively using frequency and percentage distributions. Result: Most participants were classified as having a moderate fall risk (53.3%). Before mobilization, all participants had a Bromage Score of 3 (100%). After safe mobilization, 66.7% improved to Bromage Score 2. Participants with good knowledge increased from 83.3% before education to 100% after the intervention. Conclusion: Health education combined with fall risk screening, motor function assessment, and safe mobilization demonstrations improved patients' knowledge and supported safer postoperative mobilization. This program can be implemented as part of patient safety improvement efforts in the recovery room.

Abstract

Syok septik merupakan manifestasi berat dari sepsis yang timbul akibat respons tubuh yang tidak teratur terhadap infeksi, yang menyebabkan disfungsi organ, gangguan perfusi jaringan, dan potensi berkembang menjadi kegagalan multiorgan. Pada kondisi ini, perfusi perifer tidak efektif menjadi masalah keperawatan prioritas yang memerlukan pengkajian dan penanganan berkelanjutan. Studi kasus ini bertujuan untuk mendeskripsikan asuhan keperawatan yang diberikan kepada pasien dengan syok septik dan diagnosis keperawatan perfusi perifer tidak efektif di High Care Unit (HCU) RSUD Prof. Dr. Margono Soekarjo. Studi ini menggunakan metode studi kasus dengan pendekatan proses keperawatan, yang meliputi pengkajian, perumusan diagnosis, perencanaan intervensi, implementasi, dan evaluasi. Subjek studi adalah Tn. J, seorang laki-laki berusia 69 tahun yang didiagnosis mengalami syok septik serta memiliki riwayat medis hipertensi, congestive heart failure (CHF), dan penyakit ginjal kronis (CKD). Pengumpulan data dilakukan selama dua hari masa perawatan melalui wawancara, observasi, pemeriksaan fisik, telaah dokumentasi, dan analisis rekam medis. Asuhan keperawatan difokuskan pada penanganan perfusi perifer tidak efektif melalui intervensi seperti pemantauan status hemodinamik dan perfusi perifer, pemberian terapi oksigen, menjaga kehangatan tubuh, pembatasan aktivitas, pemberian edukasi pasien, serta kolaborasi dalam terapi cairan dan antibiotik. Hasil evaluasi menunjukkan adanya perbaikan klinis, yang dibuktikan dengan peningkatan tekanan darah dari 90/60 mmHg menjadi 110/70 mmHg, penurunan denyut nadi dari 110 menjadi 92 denyut per menit, peningkatan saturasi oksigen dari 90% menjadi 96%, perbaikan waktu pengisian kembali kapiler capillary refill time (CRT) dari lebih dari tiga detik menjadi kurang dari tiga detik, serta perubahan suhu ekstremitas menjadi hangat. Perbaikan ini bertahan hingga hari ketiga, sehingga pasien dinilai cukup stabil untuk dipindahkan ke ruang rawat inap umum. Temuan studi ini menunjukkan bahwa penerapan intervensi keperawatan yang berfokus pada perawatan sirkulasi, didukung oleh terapi medis, berkontribusi terhadap perbaikan perfusi perifer dan status hemodinamik pada pasien dengan syok septik

Abstract

Palliative care is aimed at the patient's recovery process in accepting the illness he is suffering from, and palliative care is to reduce the suffering experienced by the patient. This study aimed to determine nurses' level of knowledge and caring behavior in palliative care. This type of research is quantitative with a cross-sectional approach. The sampling technique uses convenience sampling. The sample of this study is nurses finding 63. The measuring instruments used in this study are Palliative Care Quiz For Nursing (PCQN) and Caring Behaviors Inventory (CBI). Statistical analysis using Spearman rank correlation test. The study results obtained a 2-tailed Sig value of 0.000 (p-value> 0.005), thus indicating a relationship between knowledge and nurse behavior. This research is expected to illustrate Nurse's nursing behavior in providing care to palliative patients.

Abstract

Postoperative pain is one of the most common complaints among patients undergoing orthopedic surgery and may interfere with comfort, mobilization, and recovery. Music therapy is a non-pharmacological intervention that can provide relaxation and distraction effects to reduce pain perception. This study aimed to analyze the effect of music therapy on postoperative pain among orthopedic surgery patients at dr. R. Goeteng Taroenadibrata Regional General Hospital, Purbalingga. This study used a quantitative quasi-experimental design with a two-group pretest-posttest approach. A total of 36 respondents were recruited using purposive sampling and divided equally into intervention and control groups. Pain intensity was measured using the Numeric Rating Scale. Data were analyzed using the Wilcoxon Signed Rank Test and Mann-Whitney Test. The mean age of respondents was 39.75±9.28 years, with a predominance of female respondents. There was a significant reduction in pain intensity in the intervention group after music therapy (p<0.001). The control group also showed a significant reduction in pain intensity after receiving standard care (p<0.001). However, the decrease in pain intensity differed significantly between groups, indicating that music therapy provided an additional benefit in postoperative pain management. Music therapy can be considered a complementary non-pharmacological intervention for postoperative orthopedic patients.
