Abstract

Peningkatan jumlah lansia di Indonesia diikuti oleh meningkatnya prevalensi demensia yang belum diimbangi dengan perilaku pencarian layanan kesehatan yang optimal. Banyak lansia dengan demensia tidak terdeteksi dan terlambat mendapatkan penanganan akibat rendahnya health literacy dan keyakinan kesehatan yang keliru, seperti anggapan bahwa demensia merupakan proses penuaan normal. Kondisi ini memperlebar treatment gap dan meningkatkan beban keluarga serta sistem kesehatan. Penelitian ini menjadi urgen untuk mengidentifikasi peran health literacy dan health belief terhadap health seeking behavior sebagai dasar pengembangan intervensi keperawatan komunitas yang efektif dan berbasis bukti. Tujuan dari penelitian ini yaitu menganalisis hubungan health literacy dan health belief dengan health seeking behaviour pada lansia dengan demensia. Metode penelitian menggunakan desain penelitian yaitu deskriptif analisis korelasi dengan pendekatan cross sectional. Variabel independen yaitu health literacy dan health belief serta variabel dependen yaitu health seeking behaviour. Penelitian dilakukan pada Bulan Januari 2025 di UPTD Puskesmas Dawe Kabupaten Kudus. Sampel penelitian sebanyak 106 lansia dengan Teknik purposive sampling. Kriteria Inklusi penelitian ini lansia yang berusia lebih dari 60 tahun, bisa membaca dan menulis, mampu berkomunikasi, dan tinggal di wilayah UPT Puskesmas Dawe. Kriteria Eksklusi, tidak mempunyai komplikasi penyakit berat, tidak disabilitas dan memiliki gangguan kognitif. Instrumen penelitian menggunakan kuesioner health literacy 12, keyakinan Kesehatan HBM, dan perilaku pencarian bantuan kesehatan HCSBQ. Analisis data menggunakan uji chi square. Hasil Penelitian ini menunjukkan ada hubungan antara health literacy dan health belief dengan health seeking behavior dengan nilai p value p=0,01 (p0,05).

Abstract

Penyakit neurologis merupakan gangguan pada sistem saraf yang dapat menimbulkan dampak signifikan terhadap kondisi fisik dan status fungsional pasien. Kondisi tersebut tidak hanya memengaruhi kemampuan pasien dalam melakukan aktivitas sehari-hari, tetapi juga berdampak pada keluarga sebagai caregiver, khususnya pada kesehatan fisik dan kesiapan caregiver dalam merawat pasien di rumah setelah masa hospitalisasi. Penelitian ini bertujuan untuk menganalisis hubungan antara status fungsional pasien dan kesehatan fisik caregiver sebagai variabel independen dengan kesiapan merawat pasien di rumah sebagai variabel dependen pada pasien dengan penyakit neurologis. Desain penelitian yang digunakan adalah penelitian kuantitatif dengan metode deskriptif korelatif. Populasi dalam penelitian ini adalah pasien saraf di RS Tingkat II dr. Soedjono Magelang dengan rata-rata kunjungan 1.237 pasien. Teknik pengambilan sampel menggunakan purposive sampling dengan jumlah responden sebanyak 100 orang, dengan kriteria caregiver yang merawat anggota keluarga yang terdiagnosis medis penyakit saraf oleh dokter di rawat jalan dengan minimal tingkat pendidikan SD dan mampu membaca dan menulis. Pada kriteria ekslusi adalah caregiver yang tidak mengikuti proses penelitian sampai akhir. Penelitian dilakukan pada bulan November hingga Desember 2025. Instrumen penelitian meliputi Barthel Indeks, kuesioner Short Form-12 Health Survey (SF-12), serta Kuesioner Kesiapan Merawat Pasien di Rumah. Analisis data menggunakan uji korelasi Spearman Rank. Hasil penelitian menunjukkan terdapat hubungan antara status fungsional pasien dengan kesiapan merawat pasien di rumah (p-value = 0,000; r = 0,469) dan terdapat hubungan antara kesehatan fisik caregiver dengan kesiapan merawat pasien di rumah (p-value = 0,000; r = 0,448), dengan kekuatan korelasi sedang. Penelitian ini merekomendasikan agar keluarga meningkatkan kesehatan fisik caregiver serta melakukan pemantauan status fungsional pasien secara berkelanjutan guna mendukung kesiapan perawatan pasien di rumah secara optimal.

Abstract

A diabetic ulcer is a complication that requires serious treatment because it is the highest cause of amputation. This condition has an impact on the morbidity, mortality, and socioeconomics of people with diabetes. Therefore, accuracy is needed in managing diabetic ulcers, one of which is natural ingredients that have potential as topical ulcer medicines, namely the Lumbricus rubellus earthworm. This study aims to find out the most effective formula for the treatment of diabetic ulcer wounds. The formula for this drug candidate is made using the hydrogel preparation formulation using an HPMC 940 base with variations in the active ingredient of lumbricus worm extract of 20%, 50%, and 80% to obtain the desired hydrogel preparation. The candidate drug will be tested, namely physical tests including organoleptic tests, homogeneity tests, pH tests, viscosity tests, spreadability tests, and adhesive tests. After that in vivo trial using Wistar rat experiments. The results of the organoleptic test showed that the three formulas were in gel form with a distinctive extract odor, but the three preparations showed different colors, namely brownish yellow, dark brown, and blackish brown. The three formulations appear homogeneous, having a pH between 4.36 and 4.44 and a viscosity between 5,376 cp and 5,894 cp. The spreading power in formulas 2 and 3 is good, and formula 2 has good adhesion. The results of this study demonstrate that the group treated with 50% L. rubellus topical extract experienced the most significant and rapid wound healing among all intervention groups. L. rubellus extract can be used as a topical gel preparation for wound care, with the best formulation at a concentration of 50%.

Abstract

Background: Integrated primary care (IPC) is recognized as essential for strengthening health systems. However, Indonesia currently lacks a validated instrument to assess stakeholder support, which is a critical determinant of successful IPC implementation. This study evaluated the content validity, internal structure, and reliability of the newly developed Stakeholder Support Questionnaire for Integrated Primary Care (SSQ-IPC), grounded in Freeman’s Stakeholder Theory. Methods: A cross-sectional psychometric validation was conducted with 570 primary healthcare nurses recruited through stratified random sampling across Indonesia. The initial 18-item instrument, encompassing four constructs, underwent content validation by a panel of seven experts. Data analysis included calculating the Content Validity Index (CVI), conducting exploratory factor analysis (EFA), confirmatory factor analysis (CFA), and second-order CFA. Model fit was assessed using multiple fit indices, while construct validity and reliability were evaluated through factor loadings, Average Variance Extracted (AVE), Cronbach’s alpha, and McDonald’s omega. Results: EFA supported a four-factor structure. First-order CFA indicated acceptable model fit (CFI = 0.933, TLI = 0.921, SRMR = 0.037), with all factor loadings statistically significant (p<.001) and strong indicator reliability. Convergent validity was established with AVE values ranging from 0.650 to 0.869, and internal consistency was high across constructs (α= 0.877–0.978; ω = 0.884–0.973). Second-order CFA supported a hierarchical model, with all four constructs significantly loading onto a higher-order factor. Conclusion: The SSQ-IPC demonstrates strong evidence of validity and reliability, supporting its multidimensional and hierarchical structure. Abstract

Stunting is a health problem that threatens human quality and the competitiveness of the nation. This condition has an impact on the physical, mental, and cognitive development of toddlers and increases the risk of disease. There are still many toddlers who are diagnosed with stunting but do not receive treatment. Treatment-seeking behavior is influenced by knowledge, attitudes, stigma, and community beliefs. This research aims to analyze the relationship between stigma and health literacy with treatment-seeking behavior. The research design uses a correlational analytical approach that is cross-sectional. The independent variables are stigma and health literacy, while the dependent variable is treatment-seeking behavior. This research was conducted in the Keling II Community Health Center Working Area, Jepara Regency, in January 2025. The research sample was 100 caregivers of stunted toddlers and was taken using techniques of purposive sampling. The research instrument includes a stigma questionnaire using (SSCI-8), a questionnaire on health literacy using (HLS-EU-Q16), and a questionnaire on treatment-seeking behavior using (GHSQ). Data analysis using chi-square tests. The results of this study show that there is a significant relationship between stigma (p=0.002; r=0.394) and health literacy (p=0.000; r=0.448) with treatment-seeking behavior in caregivers of stunted toddlers. Stigma and health literacy play a role in shaping behavior in seeking health assistance, especially among caregivers of toddlers with stunting in rural areas. There needs to be intervention to reduce negative stigma and increase health literacy so that caregivers of stunted toddlers have the awareness to carry out screening and treat stunting in health service facilities.

Abstract

Background: Chronic Kidney Disease (CKD) is a life-threatening problem. CKD patients who require hemodialysis therapy experience a significant increase; one of the important indicators in hemodialysis therapy is interdialytic weight gain (IDWG). In practice, there are still cases where patients experience high IDWG, which is suspected to be due to non-compliance with the schedule or length of hemodialysis sessions. Objectives: This study aimed to determine the relationship between frequency and compliance with the value of IDWG in CKD patients undergoing hemodialysis. Methods: This quantitative study uses a correlative analytical research method with a cross-sectional approach. The population was 47 chronic kidney failure patients undergoing hemodialysis at the Muhammadiyah Mayong Hospital, Jepara. The instruments used were weight scales, patient medical records, and End-Stage Renal Disease Adherence Questionnaire (ESRD-AQ) questionnaires. Bivariate analysis in this study used the Spearman Rho test. Results: The results of this study are that most respondents have undergone hemodialysis for more than 12 months, as many as 20 respondents (42.6%); respondents who are compliant with hemodialysis are 25 respondents (53.2%); and most respondents experienced a mild weight gain category of 16 respondents (34%). There is a relationship between the length of time (p-value = 0.001, r= 0,762) and hemodialysis compliance (p-value = 0.003, r= 0,578) with the interdialytic weight gain value in chronic kidney disease patients undergoing hemodialysis at the hospital. Conclusion: Frequency and compliance have a relationship with the value of IDWG in CKD patients undergoing hemodialysis. The correlation between two variables are direct and positive means that the more compliant with HD, the value of IDWG is lower.

Abstract

Hypertension is a major risk factor for increasing mortality and morbidity rates. This is supported by the increasing prevalence of hypertension cases every year worldwide. If blood pressure cannot be controlled, it will lead to complications such as stroke, retinopathy, heart failure, heart attack, kidney failure, and disability. Therefore, adherence to treatment is crucial to prevent these complications. However, in the treatment of long-term diseases such as hypertension, which require lifelong treatment, barriers to medication adherence often occur. The purpose of this study was to analyze the relationship between knowledge and stress with medication adherence in hypertension patients in rural areas. The study design used correlation analysis with a cross-sectional approach. The independent variables were knowledge and stress, while the dependent variable was medication adherence. This study was conducted in two villages in Kudus Regency, which is the locus of hypertension, in April 2025. The study sample consisted of 97 respondents selected through a cross-sectional approach through purposive sampling based on inclusion and exclusion criteria. The instruments used include the Hill-Bone Compliance to High Blood Pressure Therapy Scale, the Hypertension Knowledge-Level Scale, and the Perceived Stress Scale (PSS-10). Data analysis using the chi-square test. The results of the study showed a significant relationship between knowledge (p=0.000) and stress (p=0.000) with medication adherence. Knowledge and stress are associated with medication adherence in hypertensive clients. Community health centers need to implement innovative programs to improve medication adherence through structured health education and stress management.
