Abstract

Stigma against people living with human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) (PLWHA) poses significant health threats and contributes to the uncontrolled transmission of HIV. Although tools for measuring stigma against PLWHA have been validated in Indonesia, a previous study was geographically limited and conducted with small sample sizes. The aim of this study was to perform cross-cultural adaptation and validate the psychometric properties of the 9-item Indonesian HIV/AIDS Stigma Scale (InHASS-9) in the Indonesian general population. Utilizing a cross-sectional design, the study recruited 1,302 participants from six major islands in Indonesia. The validity of the instrument was determined using recognized group validity and construct validity, while internal consistency was evaluated using Cronbach's alpha. During the adaptation phase, nine simple items were finalized for inclusion in the scale. The findings revealed that the InHASS-9 instrument exhibited strong internal consistency, with Cronbach's alpha coefficient of 0.83, indicating high reliability. In conclusion, the InHASS-9 instrument is a valid and reliable tool for assessing the extent of social stigma associated with PLWHA in Indonesia.

Abstract

Background: Community-acquired pneumonia (CAP) is a leading cause of morbidity and mortality worldwide, affecting the lower respiratory tract. It is caused by diverse pathogens, including bacteria, viruses, fungi, and chemical exposures. Antibiotics, such as ceftriaxone, are widely used for treatment. However, the high economic burden associated with CAP management necessitates a cost-effectiveness analysis to optimize treatment strategies. Objectives: This study aimed to evaluate the cost-effectiveness of three antibiotic regimens—co-amoxiclav, meropenem + levofloxacin, and ceftriaxone—for CAP inpatients at Mimika Hospital, Indonesia, to identify the most efficient therapeutic option. Methods: A retrospective, cross-sectional observational study was conducted using medical records of CAP patients admitted to Mimika Hospital between January and December 2021. Inclusion criteria were patients aged ≥18 years who received a single antibiotic regimen. Treatment effectiveness was assessed based on hospitalization duration (≤3 days) and physician-reported recovery. Cost-effectiveness was evaluated using the Average Cost-Effectiveness Ratio (ACER).Results: A total of 120 pneumonia inpatients were analyzed, predominantly male (60%) and aged 26–45 years (34%). The most frequently administered antibiotic was ceftriaxone (51%), followed by co-amoxiclav (29%) and meropenem + levofloxacin (20%). Co-amoxiclav demonstrated the highest clinical effectiveness (88.57%) and the lowest median total cost (IDR 2,696,114), resulting in the lowest ACER value (IDR 2,696,114/effectiveness unit) and a dominant ICER status. In contrast, meropenem + levofloxacin showed moderate effectiveness (75%) at the highest cost (IDR 3,088,961), with an ICER of IDR 18,538.64. Ceftriaxone had the lowest effectiveness (65.57%) and the highest ACER (IDR 44.443,22), indicating poor cost-efficiency. These findings position co-amoxiclav as the most cost-effective regimen across both clinical and economic parameters. Conclusion: Co-amoxiclav is the most cost-effective antibiotic regimen for CAP inpatients at Mimika Hospital, offering optimal therapeutic outcomes at a lower cost. These findings support its recommendation as the first-line treatment for CAP in similar healthcare settings

Abstract

This study examines the impacts of partnerships for sustainability certifications in coffee value chains on farm income in Lampung Province, Sumatra-Indonesia. We conducted farm-household surveys, interviewing 171 samples in West Lampung and Tanggamus Districts, consisting of 98 farmers joining partnership and 73 farmers not joining. A probit model is used to determine decisions to join the partnership; ordinary least square (OLS) is used to analyze the determinants of farm income; and Heckman selection-correction model is used to reduce the selection bias in partnership participation. We find some selection in partnerships for sustainability certifications, driven by the age and education of household head, land holding size of coffee farm and the proximity to rural cooperatives-KUBE. The partnership farmers earn higher farm income than those not joining, due to a higher proportion of productive family members. After employing the treatment-effect model, we find that the partnership for sustainability certifications raises farm income, mostly due to higher coffee yield and farm-gate prices. These results reveal the need for policies to support the establishment and encouragement of partnerships for sustainability certifications. The internal control system (ICS) initiated by coffee corporations in implementing the sustainability certifications have improved the trust level between smallholders and global corporations.

Abstract

This study examines the impacts of coffee agroforestry system and sustainability certification on farmers’ livelihood in Sumatra-Indonesia. The study applies farm-income analysis and quasi-experimental impact evaluations by interviewing 408 coffee farm-households in Lampung Province. Propensity score matching (PSM) adopted here compares some matching characteristics of adopters of coffee agroforestry system and sustainability certification and the control groups. The results show that both coffee agroforestry systems and sustainability certification have positive significant impacts in improving economic benefits and environmental benefits, although the magnitudes slightly differ. Future policy shall provide better alternatives for farmers’ land-use systems that could ensure a better livelihood.

Abstract

This study examines the optimal partnership structure for impacting the well-being of coffee farmers. It is crucial to elucidate the variances in each business model within the context of collaborations involving coffee farmers, traders, and exporters along the coffee supply chain, and their subsequent effect on the welfare of coffee farmers within each specific partnership configuration. We employed a questionnaire to assess the influence of partnership models on the well-being of farmers. The survey was carried out in Central Aceh, known for Arabica coffee production, and Lampung, recognized for Robusta coffee cultivation. we find that farmers participating in globally certified partnership constellations score better on economic performance than farmers in local partnership constellations (both certified and conventional).

Abstract

This research intended to look at the clearing mechanism in the viewpoint of classical and contemporary Islamic thought that in Islamic law the clearing is identical to term “wakalah” (granting power of attorney) because in wakalah, the customer authorized the bank to represent himself to perform certain service work with various mechanisms stated in the Regulation of Bank Indonesia. An important issue in this paper contained philosophical aspects of the clearing application in sharia banking. This research was reviewed by qualitative method with library research with philosophical approach. The results obtained as follows: The thoughts of Al-Ghozali and Muhammad Najetullah As-Shiddiqy were said to be relevant to the clearing mechanisms in sharia banking today. With the maslahah (welfare) concept of Al-Ghozali, which was the core of some of the five basic objectives (religion, life, response/reason, descendants and property). The concept of maslahah (welfare) for the community welfare, the clearing mechanism used in conventional banks can be used in Sharia banking by having wakalah agreement. While the neoclassical approach (a combination of the new model and the classic model), Muhammad Najetulloh As-Shiddiqy, in which although there was a new method, he still used the old method without erasing all the results of the analysis of the previous figures. Therefore, this approach can also be said to be relevant to the clearing mechanism in sharia banking.

Abstract

Abstract

Morals are the implementation of one's faith when meeting other people, other creatures or even meeting their Lord. So the discussion will not run out of time. Then in modern times, people begin to appear who heresy someone else just because they think that person does not practice morals in accordance with the guidance of the Prophet. The accusation is often leveled at the Sufis, especially the Sufis who often 'uzlah and do things outside the habits of other humans. Even though this is not true, the true Sufis are people who do worship both socially and individually in a way that is based on the Prophet Muhammad Saw and they will do it with sincerity. So we aim to combine the thoughts of a hadith expert with a Sufism expert to see how true the irregularities they throw at the Sufis are. This study uses a literature review method or literature study with analytical techniques comparing the opinions of two figures related to noble character. This study aims to find common ground between the opinions of Sheikh Abdul Wahhab al-Sya'rani and Sheikh Muhammad Abdul Aziz al-Khawli regarding morality.

Abstract

Schizophrenia is a prevalent mental health disorder often marked by relapses, significantly affecting the health-related quality of life (HRQoL) of both patients and their families. The aim of this study was to compare the EuroQol 5-Dimension 5-level (EQ-5D-5L) responses of schizophrenia patients and their caregivers. Using an observational cross-sectional design, HRQoL was assessed among schizophrenia patients and their family caregivers recruited from a provincial referral hospital. Sociodemographic (age, sex, education, marital status, income) and clinical variables (diagnosis, treatment duration, comorbidities) were analyzed alongside HRQoL using structured interviews, medical record reviews, and the EQ-5D-5L instrument (self-report by patients and proxy-reported by family caregivers). Statistical analyses included chi-square tests for associations, Wilcoxon tests for patient-family caregiver comparisons, and multivariate modeling of HRQoL determinants. A total of 526 participants (263 patients and 263 accompanying family caregivers) were included. Significant differences were observed between patients and family caregivers in two domains: pain/discomfort and anxiety/depression. Also, the agreement between patients' and family caregivers’ reports showed good results. A substantial agreement was observed between patient-reported and family caregiver-assessed HRQoL, as indicated by a Cohen’s Kappa value of 0.8. This result suggests a strong level of consistency between the two assessments, supporting the potential use of family caregivers as reliable proxies for evaluating patient HRQoL when self-reports are unavailable or unreliable. In the self-care domain, mobility, and daily activities, patient and caregiver assessments show high agreement. In conclusion, the closeness between patients and caregivers significantly influences patients' HRQoL, providing critical insights for evaluating treatment effectiveness in schizophrenia cases. While discrepancies exist between patient and caregiver assessments, these interactions are particularly impactful in subjective domains like pain/discomfort and anxiety/depression, but not for other domains that are visible.
