Abstract

Background: Development is one of the important things for the future of a child. The most crucial development occurs during the first five years of a child's life, this time is often called the golden period. At this time all the development of children must be by their age so that in the future there are no developmental delays. A child's development can be influenced by several factors, both internal and external factors. Based on previous research as many as 5-10% of Indonesian children experience general developmental delays, so the development of Indonesian children still requires serious attention. Therefore, this study aims to analyze factors that affect the development status of toddlers. Method: This study is an observational analytical study using a cross-sectional approach. With a sample number of 100 toddlers in the working area of Simomulyo Health Center that meets the criteria of inclusion and uses consecutive sampling techniques. Independent variables in the study were birth weight, nutritional status of toddlers, parents' last education, economic status, history of disease in toddlers, and history of exclusive breast milk. While the dependent variable in this study is the status of development. The chi-square statistical test is used to determine the significant level of data obtained at the meaningful level of ? =0.05. Results: The results of the study found 90% experienced appropriate developmental status, 86% had normal nutritional status based on BB / U, 45% had normal nutritional status based on TB / U, 64% had normal nutritional status based on BB / TB, 82% of toddler parents had the last high education, 67% of toddlers have economic status ?UMR, 50% of toddlers have a history of pain ?2 times sick and 56% of toddlers get exclusive breast milk ?6 months. From the results of statistical tests chi-square obtained birth weight ? = 0.444 (??0.05), nutritional status based on BB / U ? = 0.578 (??0.05), nutritional status based on TB / U ? = 0.375 (??0.05), nutritional status based on BB / TB ? = 0,706 (??0.05), parental education ?=0.488 (??0.05), economic status ?=0.103 (??0.05), history of childhood disease ?=0.046 (?<0.05), exclusive breast milk history ?=0.002 (?<0.05). It means that there is no relationship between developmental status with birth weight, nutritional status, parents' last education, economic status. However, there is a relationship between developmental status and a history of toddler disease, and a history of exclusive breast milk. Conclusion: A toddler's history of disease and a history of exclusive breast milk have relationships that can affect a toddler's development.

Abstract

Abstract: Heart failure patients with reduced ejection fraction (HFrEF) respond well to pharmacological therapy and show a better prognosis. Heart failure patients with reduced ejection fraction and type 2 diabetes who were given SGLT-2 inhibitor therapy showed a strong and consistent reduction in the risk of death and hospitalization. The therapy that has recently begun to be investigated for its benefits for heart failure from the SGLT-2 inhibitor class is Dapagliflozin. The systematic review aims to analyze the effect of Dapagliflozin on the prognosis of HFrEF patients with type 2 diabetes mellitus. Material and Methods: The literature was searched from e-database PubMed, ScienceDirect, and ClinicalTrial.gov. Quality assessment was done using the Critical Appraisal Skills Program (CASP) Randomized Controlled Trial Standard Checklist. Results: A total of 22,167 patients from 4 RCTs eligible studies were included. The analysis results of all of the included studies indicate that Dapagliflozin affected the patient's prognosis. Two studies discuss mortality and hospitalization, and two studies discuss symptoms, functional status, and Quality of Life (QoL). Conclusion: Dapagliflozin can improve the prognosis of HFrEF patients with type 2 DM. The improved prognosis includes reduced mortality, reduced hospitalizations by minimizing disease worsening, reducing symptoms, improving functional status and QoL. Abstract

Introduction: Service quality of primary healthcare centre especially at HIV testing services must be improved. When the service quality is improved there will be a positive feedback such as patient’s satisfaction. Within the high patient satisfaction rate, People Living with HIV (PLWH) are expected to be more active, productive and also invited friends to check themselves, so the iceberg phenomenon will be revealed and prevention will be easier to control.Methods: This research used observational analytic design with purposive sampling method. The total research sample was 189  respondent, assess the questionnaire that consisted of five dimensions of service quality which were tangible, reliability, responsiveness, empathy and assurance, and also three indicators of patient satisfaction which were overall satisfaction, expectation and experience.Results: Among 189 respondents, mostly male with 129 responses or 68.3%, according to age group, mostly from 20-25 years old age group with 88 responses or 46.6% and according to time visit, mostly first visit patient with 162 responses or 85.8%. According to service quality, mostly categorized in a good group with 173 respondents or 91.5%. In patient satisfaction, mostly were categorized in a satisfy group with 157 respondents or 81.5%, There was a positive correlation between service quality and patient satisfaction with p value <0.01 and correlation coefficient 0.632.Conclusion: There is a correlation between every dimension of service quality which are tangible, reliability, responsiveness, empathy and assurance with patient satisfaction of HIV testing services patient in Kedungdoro Primary Health Care Surabaya

Abstract

Abstrak Latar Belakang: Pernikahan dini merupakan salah ssatu masalah kesehatan reproduksi di Indonesia. Berdasarkan laporan BPS 2017 angka pernikahan dini di Indonesia mencapai 25,71%, sedangkan di Kabupaten jember 28,66% perempuan menikah di usia dini. Antenatal care yang rutin dapat menurunkan angka kematian ibu.  Berdasarkan Kemenkes 2017 kehamilan pada ibu dibawah usia 20 tahun meningkatkan angka kematian ibu. Remaja perempuan yang telah menikah cenderung 11 kali memiliki pendidikan yang rendah. Penelitian ini bertujuan untuk menganalisis hubungan tingkat pendidikan dengan kepatuhan antenatal care pada ibu hamil primigravida dengan riwayat pernikahan dini. Metode: Metode penelitian ini adalah penelitian cross sectional. Instrument yang digunakan adalah kuesioner. Analisis data menggunakan uji chi square dengan α=0,05. Hasil: Hasil penelitian menunjukkan 81,35% ibu hamil dalam kategori usia remaja akhir, 54,2% Ibu hamil memiliki pendidikan terakhir SMP, Ibu hamil yang patuh dalam ANC sebesar 64,4%, ibu hamil dengan pendidikan SMA patuh dalam ANC (70,5%) dan didapatkan nilai p=0,005. Kesimpulan: Data tersebut menunjukkan bahwa terdapat hubungan antara tingkat pendidikan dengan kepatuhan ANC pada ibu hamil dengan riwayat pernikahan dini di Kecamatan Tanggul Kabupaten Jember.AbstractBackground: Early marriage is one of the reproductive health problems in Indonesia. Based on the BPS report 2017 the rate of early marriage in Indonesia reached 25.71%, while in Kabupaten Jember 28.66% of women married at an early age. Regular antenatal care can reduce maternal mortality. Based on the Ministry of Health 2017 under 20 years of gestation increases maternal mortality. Married girls tend to have 11 times lower education. This study aims to analyze the relationship of education with the agreement of antenatal care in primigravida pregnant women with a collection of early marriage.Method: The method of this study is a cross-sectional study. The instrument used was a questionnaire. Data analysis using chi square test with α = 0.05. Results: The results showed 81.35% of pregnant women in the final adolescent age category, 54.2% of pregnant women had the last junior high school education, 64.4% of obedient pregnant women in the ANC, pregnant women with high school education were obedient in the ANC (70 , 5%) and the value of p = 0.005 is obtained.Conclusion: The data shows that there is a relationship between education level and ANC compliance in pregnant women with a history of early marriage in the Tanggul District of Jember Regency.

Abstract

Abstrak Latar Belakang: HIV merupakan sejenis virus yang dapat menyebabkan turunnya kekebalan tubuh manusia. Pada wanita hamil dengan HIV mempunyai kemungkinan melahirkan anak dengan HIV. Dalam tugasnya, bidan mempunyai peran untuk melakukan pencegahan penularan HIV/AIDS dari ibu ke anak, termasuk melakukan perencanaan kehamilan terhadap wanita dengan HIV/AIDS. Penelitian bertujua untuk mengetahui kesiapan bidan dalam perencanaan kehamilan pada wanita dengan HIV/AIDS. Metode: Penelitian deskriptif observasional dengan pendekatan cross sectional. Besar sampel sebanyak 88 bidan yang bekerja di 12 puskesmas wilayah Surabaya. Teknik sampling dilakukan sesuai dengan daftar puskesmas yang telah diberikan oleh Dinas Kesehatan Kota Surabaya. Variabel dalam penelitian yaitu pengetahuan, sikap, dan kesiapan bidan dalam perencanaan kehamilan pada wanita dengan HIV/AIDS. Hasil: Didapatkan mayoritas bidan berusia 23-32 tahun (62,5%) dan lama bekerja 1-10 tahun (71,6%). Puskesmas yang memiliki layanan PMTCT sebanyak 11 puskesmas (91,7%) dan 100% puskesmas mempunyai lembar balik HIV/AIDS. Sebagian besar bidan mempunyai pengetahuan baik (87,5%). Sebanyak (48,9%) bidan memiliki sikap negative dan (53,4%) bidan tidak siap dalam perencanaan kehamilan terhadap wanita dengan HIV/AIDS. Kesimpulan: Sebagian besar bidan tidak siap dan masih memiliki stigma negatif terhadap ODHA. Mayoritas bidan belum mengetahui keuntungan dan kerugian dari jenis persalinan pada ibu dengan HIV/AIDS serta belum mengetahui pemberian nutrisi pada bayi bagi ibu dengan HIV/AIDS.Abstract Background: HIV is a type of virus that can cause a decrease in human immunity. In pregnant women with HIV it is possible to give birth to children with HIV. In their duties, midwives have a role to prevent HIV / AIDS transmission from mother to child, including pregnancy planning for women with HIV / AIDS. The study aimed to determine the readiness of midwives in pregnancy planning in women with HIV / AIDS. Method: An observational descriptive study with cross sectional approach. The sample size was 88 midwives who worked in 12 Puskesmas in Surabaya. The sampling technique was carried out according to the list of health centers that had been given by the Surabaya City Health Office. The variables in the study were the knowledge, attitudes, and readiness of midwives in pregnancy planning in women with HIV / AIDS. Results: The majority of midwives aged 23-32 years (62.5%) and working duration 1-10 years (71.6%) were obtained. Puskesmas that have PMTCT services are 11 health centers (91.7%) and 100% puskesmas have a back sheet of HIV / AIDS. Most midwives have good knowledge (87.5%). As many as (48.9%) midwives have a negative attitude and (53.4%) midwives are not ready for pregnancy planning for women with HIV / AIDS. Conclusion: Most midwives are not ready and still have a negative stigma towards PLWHA. The majority of midwives do not yet know the advantages and disadvantages of the type of childbirth to mothers with HIV / AIDS and do not yet know the provision of nutrition to infants for mothers with HIV / AIDS.

Abstract

Abstrak Latar Belakang: Kanker serviks merupakan salah satu penyakit kanker yang menyerang wanita dan menduduki urutan kedua setelah kanker payudara di Indonesia. Salah satu pencegahan yang dapat dilakukan adalah dengan melakukan pemeriksaan deteksi dini kanker serviks metode IVA. Keinginan melakukan pemeriksaan IVA dipengaruhi beberapa faktor, diantaranya pengetahuan, kesadaran, keternagkauan jarak, keterjangkauan biaya, serta persepsi Ibu. Penelitian ini bertujuan untuk menganalisis faktor aksesibilitas pelayanan kesehatan dan persepsi ibu terhadap pemeriksan IVA di Puskesmas Benculuk Kabupaten Banyuwangi. Metode: Penelitian ini merupakan penelitian observasional deskriptif dengan desain cross sectional. Pengambilan sampel menggunakan teknik random sampling sejumlah 60 responden dengan instrument penelitian kuisioner. Variabel penelitian terdiri dari pengetahuan, kesadaran, keterjangkauan jarak, keterjangkauan biaya, dan persepsi Ibu. Hasil: Hasil penelitian didapatkan bahwa 93,3% belum pernah melakukan pemeriksaan IVA, 90% WUS memiliki pengetahuan baik, 70% memiliki keterjangkauan jarak yang jauh dengan tempat pelayanan kesehatan, 100% mampu dalam pembiayaan pemeriksaan IVA, sebagian besar WUS memiliki persepsi yang baik terkait kanker serviks dan pemeriksaan IVA. Kesimpulan: Faktor aksesibilitas pelayanan kesehatan dan persepsi Ibu terhadap pemeriksaan IVA termasuk dalam golongan baik, namun dari 60 responden hanya 4 responden yang sudah pernah melakukan pemeriksaan IVA. Beberapa alasan mengapa WUS belum melakukan pemeriksaan  diantaranya karena rasa malu dan takut kepada petugas kesehatan, rasa takut apabila terdeteksi adanya penyakit kanker serviks, serta merasa belum membutuhkan pemeriksaan IVA karena belum merasakan adanya gejala yang mengarah pada penyakit kanker serviks.Abstract Background: Cervical is a cancer that involves women. Cervical cancer rank’s is the second after breast cancer in Indonesia. Prevention that can be done is by conducting an examination of the early detection of cervical cancer VIA method. The desire to conduct VIA test is influenced by several factors, including knowledge, awareness, distance awareness, cost affordability, and mother's perception. This study aims to analyze the accessibility factors of health services and maternal perceptions of VIA test at the Puskesmas Benculuk Kabupaten Banyuwangi. Methods: This study was a descriptive observational study with a cross sectional design. Sampling used a random sampling technique of 60 respondents with questionnaire research instruments. The research variables consisted of knowledge, awareness, affordability, affordability, and women's perception. Results: The results showed that 93.3% had never done VIA examination, 90% of women of childbearing age had good knowledge, 70% had far-reaching affordability with health care facilities, 100% were able to finance IVA examinations, most women of childbearing age had a good perception of cancer cervix and VIA test. Conclusion: Accessibility factors for health services and maternal perceptions of IVA examination are included in the good category, but from 60 respondents only 4 respondents have already conducted IVA examinations. Some of the reasons why women of childbearing age has not conducted an examination include the feeling of shame and fear of health workers, fear of cervical cancer being detected, and feeling that they have not needed an IVA examination because they have not felt any symptoms that lead to cervical cancer.

Abstract

Background: Exclusive Breastfeeding (ASI) according to WHO is the best method of feeding babies, in the period from newborns to 6 months of age without being given other additional foods. The coverage of exclusive breastfeeding in Indonesia is still around 37.3% of the 50% target. . Literature review and previous research show that the practice of exclusive breastfeeding is influenced by several factors, including maternal age, mother's occupation, mother's knowledge, family support, support from health workers and the IMD process. The purpose of this study was to analyze the factors of maternal age, occupation, mother's knowledge, family support, support from health workers and the IMD process for the process of exclusive breastfeeding to infants. Methods: An observational quantitative analytical study with a cross sectional approach. The study was conducted in the Ilebura Health Center area, from August to September 2020 with a sample of breastfeeding mothers who had babies aged 6-12 months, with total sampling. The variables in this study consisted of from the independent variables, namely maternal age, mother's occupation, mother's knowledge, family support, support from health workers, and the IMD process. The dependent variable is exclusive breastfeeding. The instrument used is a questionnaire. Data analysis using chi square test and multivariate test using logistic regression. Results: This study obtained 71 samples of breastfeeding mothers. Most of the respondents (78.9%) who gave exclusive breastfeeding were aged 25-35 years, had a good level of knowledge and did not work. All respondents in this study received support from their families and health workers, and most of the respondents (85.9) who gave exclusive breastfeeding experienced the IMD process. Bivariate test results showed the relationship between age factor (p = 0.163), occupation factor (p = 0.592), knowledge factor (p = 0.452), IMD process (p = 0.010) on exclusive breastfeeding. The results of the multivariate test showed that the most influential factor on exclusive breastfeeding was the IMD factor. Conclusion: Research shows that there is no relationship between maternal age, mother's occupation, mother's knowledge with exclusive breastfeeding, but there is a relationship between the IMD process and exclusive breastfeeding. The most dominant factor influencing exclusive breastfeeding to infants is the implementation of early initiation of breastfeeding (IMD). ). Keywords: Factors, Success, Exclusive Breastfeeding

Abstract

AbstractBackground: Breastmilk  is the best food that a mother can give to her baby from birth until the first 6 months of life to support growth and development during the first 1000 days of life. The success of exclusive breastfeeding is influenced by several factors. The purpose of this study is to analyze the support of family and mother to support exclusive breastfeeding. Method: This research method is cross sectional with a retrospective study design. The number of samples was 48. The independent variables were family and psychological assistance from the mother. The successful variable is exclusive breastfeeding. The research instrument used was a questionnaire. Data analysis using Fisher Exact statistical tests. Results: The results showed that there is no influence between family support and psychological of mother on the success of exclusive breastfeeding. Fisher Exact trial results related to family support for exclusive breastfeeding showed p = 0.339 (p> 0.05), psychological motivation of mothers towards exclusive breastfeeding showed p = 0.167 (p> 0.05). Conclusion: This data shows inappropriate between family support and psychological mother to exclusive breastfeeding.

Abstract

Abstract Background: Low Birth Weight Babies (LBW) accounts for 60 - 80% of all neonatal deaths in the world. Not the only major predictor of mortality and morbidity in neonates, individuals with a history of LBW have a greater risk of experiencing various diseases such as hypertension, heart disease, and diabetes later in life. This systematic review study aims to identify maternal risk factors that contributing to LBW Method: Articles published between 2015 - 2020 in the Science Direct, Scopus and Cochrane library databases are systematically reviewed. Eleven articles that met the inclusion and exclusion criteria were included as samples for further review. Results regarding maternal risk factors for LBW events are synthesized systematically .Results: From the search results, there were 818 articles, after the screening process, the final results were 11 articles. Risk factors that are significantly related to LBW incidence in the article, are grouped in the subtheme, include: maternal age (<20 years, and each increase in age), parity (primipara and grandemultipara), low maternal upper arm circumference, hemoglobin levels less than 11 gr / dl, gestational age below 37 weeks, and complications during pregnancy. Conclusion: maternal age, parity, arm circumference, hemoglobin level, gestational age, and complications during pregnancy were significaly found to be maternal risk factors for LBW.

Abstract

AbstractBackground: In Indonesia the Neonatal mortality rate was 15 infants per 1000 births and in Surabaya in 2018 LBW deaths were 50 per 1000 live births, while in Haji General Hospital the LBW deaths were 12 babies in 1 year. Kangaroo care was the most effective way to meet the basic needs of LBW. Babies with 1500-2500 grams body weight either due to prematurity or small pregnancy period will lose weight ranging from 10-15% in 7 days of life. To overcome the problem Kangaroo care method4 component aims to promote weight loss, maintain body temperature remains stable and facilitate breastfeeding Methods: The method of this study was observational analytic with the design of the study cohort. Total sample of 40 LBW with sampling techniques using Sequential Sampling in the period August-October period 2019. Variabel was free kangaroo care method  given 1-2 times a day with a duration of more than 120 minutes. The dependent variable was increased in body weight carried out 3x the measurement. Results: The results of the study using paired T test showed an increase in body weight, namely 1) the first measurement of 31.82 grams for 3 days with p = 0.037, 2) the second measurement increased by 227.15 for 7 days, with p = 0,000, 3) the third measurement was 258.97 with p = 0,000. The conclusion: of the studied, that there were a significant difference between the kangaroo method treatments for weight gain in LBW in the NICU Room Haji Hospital Surabaya
