Abstract

This paper aims to analyze the increase in victims of sexual violence in Pamekasan Regency from the perspective of the TPKS Law. This study focuses on the factors causing the increase in victims of sexual violence in Pamekasan Regency and efforts to protect and rehabilitate victims of sexual violence under Law No. 12 of 2022 concerning criminal acts of sexual violence. The research method used is a legal-normative study with a legislative approach. Legal materials were collected through literature review and analyzed using descriptive analysis. The results of the study show that, first, the factors contributing to the increase in sexual violence in Pamekasan Regency are the persistence of patriarchal attitudes, promiscuity, and the use of social media anonymity, which makes it easier for perpetrators to engage in grooming and sextortion. Infidelity occurs due to dissatisfaction in marriage, sexuality, and the need for freedom. Second, the forms of protection and recovery for victims of sexual violence regulated in the TPKS Law are stipulated in Article 66 paragraph (1), which provides legal certainty that every victim of sexual violence must receive protection from the beginning of the crime until the case is examined, tried, and decided by a panel of judges in court.

Abstract

Background: Hypertension is a global health problem that contributes significantly to deaths from cardiovascular disease. Unhealthy lifestyles, including excessive salt consumption, physical inactivity, and obesity, are the primary factors. Without management, hypertension can lead to serious complications such as heart disease, stroke, and kidney failure. Objective: This study aims to investigate, summarise, and identify various scientific evidence related to hypertension risk factors. Methods: This study employed a scoping review approach organised according to the PRISMA protocol. The Population– Concept–Context (PCC) framework was utilised as an initial guideline in formulating research questions and searching for relevant articles. The literature search process was conducted using several databases, including Google Scholar and Scopus. The included articles had to meet the inclusion criteria, which were limited to open-access journals published between 2020 and 2025. From a total of 512 articles found, only 14 met the selection criteria and were suitable for analysis. The data extraction process was conducted by eight people on the 14 selected articles, followed by qualitative data analysis. Results: The findings indicate that factors such as age, gender, length of employment, consumption of salt, sugary drinks, high-fat foods, elevated total cholesterol levels, low vegetable intake, lack of physical activity, stress, and abdominal circumference that exceeds the recommended play a role in increasing the risk of hypertension. Conclusion: These factors trigger fluid retention, insulin resistance, dyslipidemia, and central obesity that together raise blood pressure and accelerate vascular damage.

Abstract

Background: Chronic Kidney Disease (CKD) is a common kidney disorder among elderly individuals and is a leading cause of global mortality. Patients undergoing Continuous Ambulatory Peritoneal Dialysis (CAPD) are at high risk of complications and mortality. Therefore, analyzing risk factors for mortality in CAPD patients is crucial. Aim: To identify mortality risk factors in CAPD patients. Methods: This study was an observational retrospective study design. The sample consisted of CAPD patients at Saiful Anwar Hospital in Malang, with data collection from August 1st, 2019 to July 31st, 2023. Information collected included demographic data, medical history, and laboratory parameters. Data were analyzed using chi-square tests, t-tests, Kolmogorov-Smirnov tests, and multiple logistic regression to evaluate relationships, normality, risk factor effects, and the mortality rate of CAPD patients. Results: There were 377 subjects who met the inclusion criteria, consist of deceased (n=89) and alive (n=288) subjects. Older age significantly increased mortality risk with an odds ratio (OR) of 1.277 (95% CI: 1.260-1.294; p = 0.007). Diabetes mellitus increased mortality risk in CAPD patients, with an OR of 3.185 (95% CI: 1.427-7.105; p = 0.005). Heart failure increased mortality risk in CAPD patients, with an OR of 4.201 (95% CI: 2.170-8.133; p = 0.000). Decreased potassium levels increased mortality risk in CAPD patients, with an OR of 1.352 (95% CI: 1.020-1.792; p = 0.036). Conclusion: Older age, diabetes mellitus, heart failure, and decreased potassium levels increase mortality risk in CAPD patients.

Abstract

Asthma and chronic kidney disease (CKD) are chronic disorders with substantial morbidity and increasingly recognized overlap. This narrative review summarizes the current evidence on the relationship between asthma and CKD, with emphasis on patients undergoing dialysis. Available data indicate that asthma, particularly when severe or poorly controlled, is associated with a higher risk of CKD, whereas CKD more consistently modifies pulmonary physiology and worsens control of pre-existing asthma than clearly causing new-onset asthma. The interaction is supported by several mechanisms, including systemic inflammation, immune dysregulation, endothelial injury, oxidative stress, disordered acid-base balance, and fluid overload. In advanced CKD, anemia, uremia, metabolic disturbances, and volume excess may mimic or aggravate respiratory symptoms, while dialysis adds further complexity through rapid intravascular volume shifts, complement activation, membrane bioincompatibility, and electrolyte changes that can influence lung mechanics and airway reactivity. Management generally follows standard asthma therapy based on inhaled corticosteroids and bronchodilators, but treatment must be individualized according to kidney function, dialysis status, polypharmacy, and the risk of medication-related adverse effects. Careful integration of respiratory assessment, renal monitoring, and volume management is therefore essential in this population.

Abstract

Introduction: Exit-site infection (ESI) is one of the most frequent complications in CAPD patients and can progress to peritonitis if inadequately managed. Granulomatous ESI caused by MRSA remains sparsely reported in the literature. Case Presentation: A 20-year-old woman with end-stage kidney disease secondary to lupus nephritis, on CAPD for two years, presented with a five-day history of a moist, erythematous nodule (±1 cm) at the Tenckhoff catheter exit site with purulent discharge. The modified Schaefer exit-site score was ≥7, confirming active infection. There was no peritoneal involvement. Management and Outcome: Gram staining revealed Gram-positive cocci (3+), and culture confirmed MRSA susceptible to levofloxacin and TMP-SMX. Empirical oral ciprofloxacin 500 mg twice daily and topical gentamicin were initiated. Clinical improvement was observed at day seven. Therapy was then transitioned to oral TMP-SMX (cotrimoxazole 480 mg once daily) for an additional seven days. MRSA carrier screening via nasal and throat swabs was planned. Discussion: Empirical ciprofloxacin deviated from ISPD guidelines recommending anti-staphylococcal penicillin or first-generation cephalosporin. Transition to TMP-SMX post-culture was appropriate and guideline-concordant. The favorable outcome highlights the importance of microbiological confirmation and targeted therapy. Conclusion: MRSA-associated infectious granuloma at the CAPD exit site can present in young patients without systemic features. Timely microbiological workup, guideline-directed antibiotic therapy, and MRSA decolonization are essential to prevent progression to peritonitis.
