Abstract

Background: Diabetes mellitus (DM) is a metabolic disease that is developing into a serious global problem. Diabetes mellitus is characterized by an increase in blood levels that are more than the normal reference. In type 2 DM generally occurs due to reduced insulin secretion and sensitivity over time. Control of blood sugar levels can improve the patient's quality of life. This study aimed to analyze the effect of curcuma longa rhizome extract on fast­ing blood sugar levels and HbA1C in patients with type 2 diabetes.Subjects and Method: This was a meta-analysis of a number of randomized con­trolled trials. The articles were obtained from PubMed, Google Scholar, Springerlink, BMJ, and Sci­encedirect data­bases, published from 2010-2020. The article search was carried out by con­sidering the eligibility criteria defined using the PICO model. P: Type 2 diabetes patients, I: Cur­cuma longa rhizome extract, C: Placebo, and O: Fasting blood sugar levels and HbA1C. The keywords to find articles are as follows: "Curcuma longa", OR "Curcu­min" OR "Tume­ric" OR "Curcuma" AND "Fast­ing blood glu­cose" AND "HbA1C" OR "Glice­mic" OR "Dia­betes Mellitus" AND "Rando­mized Controll Trials". Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 14 articles were reviewed in this study. Meta-analysis of 12 articles showed that the curcuma longa rhizome extract reduced fasting blood sugar levels (Stan­dar­dized Mean Diffe­rence= -0.48; 95% CI= -0.61 to -0.34; p <0.001). The meta-analysis of 11 articles showed that admi­nis­tration of curcuma longa rhizome extract decreased HbA1C levels (Standardized Mean Difference= -0.40; 95% CI= -0.59 to -0.20; p <0.001). Abstract

Latar Belakang: Rumah sakit sebagai fasilitas rujukan tingkat lanjut memiliki peran penting dalam implementasi program Jaminan Kesehatan Nasional (JKN). Untuk pembayaran pelayanan kesehatan bagi peserta JKN yang dirawat di RS, BPJS kesehatan melakukan pembayaran berdasarkan tarif Indonesian case based groups (INA CBGs). Namun hal ini belum efektif karena dengan tarif INA CBGs berpotensi menimbulkan kerugian bagi RS. Penelitian ini bertujuan untuk menganalisis apakah tarif RS lebih tinggi dari tarif INA CBGs dan apakah terdapat hubungan antara tarif RS dan faktor-faktor : jenis RS, kelas perawatan, tingkat keparahan, penggunaan ICU dan lama perawatan.Subjek dan Metode : Desain penelitian ini adalah analitik observasional dengan pendekatan cross sectional. Penelitian dilaksanakan pada bulan Oktober-Desember 2016 di dua RS pemerintah dan 2 RS Swasta. Pengambilan sampel dengan teknik consecutive sampling.Jumlah sampel 100 sampel. Variabel terikat : tarif RS dan tarif INA CBGs, variabel bebas : jenis RS, kelas perawatan, tingkat keparahan, penggunaan ICU dan lama perawatanHasil: Tarif RS lebih rendah dari tarif INA CBGs. Mean tarif RS Rp. 2.280.000 ,- dan mean tarif INA CBGs Rp. 3.060.000,-. Korelasi positif dan signifikan (r= 0.51 p = <0.001). Terdapat hubungan negatif antara tarif RS dan Jenis RS. Tarif RS swasta lebih rendah daripada RS pemerintah (b=-5.66xRp. 1.000.000, CI=95%, p= 0.078).Terdapat hubungan negatif antara tarif RS dan kelas perawatan (kelas 2 : b= -0.34 XRp.1.000.000,- , CI= 95 %, p =0.371 dan kelas 3 : b =-0.50 XRP. 1.000.000,-, CI =95%, p= 0.177). Terdapat hubungan positif antara tarif RS dan penggunaan ICU (b= 1.58 X Rp. 1.000.000, CI= 95 %, p= <0.001).Terdapat hubungan positif antara tarif RS dan tingkat keparahan sedang (b= 0.55 XRp, 1.000.000,-, CI =95 %, p= 0.150) dan terdapat hubungan negatif antara RS dan tingkat keparahan berat ( b= -0.12 XRp. 1.000.000,- , CI= 95 %, p= 0.894.7. Terdapat hubungan positif antara tarif RS dan lama perawatan (b= 0.27 X Rp. 1.000.000,-, CI= 95%, p= 0.005 )Kesimpulan : tarif RS lebih rendah dari tarif INA CBGs. Abstract

Latar belakang. Anemia merupakan tahap akhir defisiensi zat besi. Kekurangan zat besi pada siklus 270 hari pertama kehidupan dapat membawa dampak buruk pada periode kehidupan selanjutnya. WHO merekomendasikan seluruh wanita hamil agar mendapatkan suplementasi zat besi. Namun, faktanya prevalensi anemia masih tinggi pada kelompok ibu hamil. Tujuan. Penelitian ini bertujuan untuk menganalisis hubungan kejadian anemia pada 270 hari pertama kehidupan dan usia ibu terhadap berat bayi lahir. Metode. Penelitian dilaksanakan di tiga Puskesmas di Sukoharjo, Indonesia pada Febuari-April 2018. Pemilihan tiga Puskesmas dilakukan secara purposive sampling dengan jumlah populasi 670 ibu. Sebanyak 129 ibu terpilih, subjek merupakan ibu yang memiliki balita (12-24 bulan). Subjek diwawancara menggunakan kuesioner sosio demografi, sedangkan data anemia didapat dari buku KIA. Analisis multivariat menggunakan regresi logistik untuk mengetahui faktor yang paling berhubungan dengan berat bayi lahir. Penentuan rasio menggunakan Confidence Interval (CI) 95% untuk menentukan kekuatan hubungan. Nilai p <0.05 digunakan untuk menyatakan signifikansi statistik. Hasil. Dari 129 ibu, sebanyak 100 (77,5%) mengalami anemia pada siklus 0-90 hari; 83 (64,3%) pada siklus 91-180 hari; 123 (95,3%) pada siklus 181-270 hari; dan 73 (56,6%) memiliki usia risiko kehamilan. Hasil uji regresi logistik, anemia pada siklus 0-90 hari dan 91-180 hari tidak ada hubungan yang bermakna dengan berat bayi lahir (p>0.05), sedangkan anemia pada siklus 181-270 hari (p=0.01;OR=6.14) dan usia risiko kehamilan (p=0.03;OR=2.81) berhubungan dengan berat bayi lahir. Kekurangan zat besi pada siklus 181-270 hari pertama kehidupan dan usia ibu hamil yang rentan berkontribusi pada berat lahir bayi. Kesimpulan. Probabilitas ibu anemia pada siklus 181-270 hari pertama kehidupan dan memiliki usia risiko kehamilan dalam melahirkan bayi dengan Berat Bayi Lahir Rendah (BBLR) sebesar 7,63%.

Abstract

Background: Injectable contraception is the most popular contraceptive method in Indonesia with a user percentage of 63.71% of the coverage of modern family planning users. In addition to the benefits and practicality of its use, this method also has various negative impacts that can endanger the health of the acceptor. This study aims to explore and describe the acceptor's response to the negative impacts experienced as a form of responsibility for the health of the acceptor. Subjects and Method: This study was conducted using descriptive qualitative method with triagulation analysis technique that connects empirical facts. The dependent variable is injectable family planning. The independent variables are responsibility, patriarchal culture, and decision making. The study was conducted on 18 informants who are injection family planning acceptors in Timor Tengah Selatan Regency. Results: The informants do not mind experiencing the negative impacts of using injectable contraceptives and prioritize the benefits and practicality of their use. The low level of education of informants is a problem in receiving and digging information. Limited access to information is also hindered by the lack of infrastructure to reach health facilities. The high pressure of patriarchal culture is one of the decision-making factors and the high number of injecting family planning users. Conclusion: The acceptor's lack of information regarding the contraceptive method used is a barrier in responding to the negative impacts experienced.

Abstract

Soygurt is lower in fat and has more active compounds than yogurt. Pectin acts as a prebiotic, increasing the number and activity of probiotic bacteria and preventing oxidative stress, one of the triggers for insulin resistance. Substitution of pumpkin in the formulation of soygurt to increase the organoleptic properties and lactic acid bacteria in the product to optimize its benefits as an anti-diabetes mellitus functional food. The research design was an experiment with a completely randomized design with one control formula and three treatment formulas with comparisons of soybeans and pumpkin as follows F0 100:0, F1 80:20, F2 70:30, and F3 60:40. Organolpetic tests with a scale of 1–6 from dislike to like. The best formula was continued for physicochemistry and proximate tests. The results of the organoleptic test showed that the best treatment was a 20 percent substitution of pumpkin (F1) with a preference scale of 4, which means that this formula was sensory acceptable to the panelists. The nutritional quality of F1 was 3.15 percent protein, 1.35 percent fat, 0.26 percent carbohydrates, 1.03 percent fiber, LAB 9.5x107, and pH 4.13. Substitution of pumpkin in yogurt has the potential to control blood glucose levels because it is low in carbohydrates and fat. Further research is needed to analyze the content of anti-nutritional substances such as phytate, tannins, and trypsin inhibitors, which are generally found in soybeans, the raw material for this product. Keywords: diabetes mellitus, pumpkin, soybean, soygurt ABSTRAK  Soygurt lebih rendah lemak dan memiliki senyawa aktif daripada yogurt konvensional. Pektin berperan sebagai prebiotik yang dapat meningkatkan jumlah maupun aktivitas dari bakteri probiotik serta mampu mencegah terjadinya stres oksidatif yang merupakan salah satu faktor pencetus resistensi insulin. Subsitusi labu kuning dalam pengembangan soygurt diharapkan mampu meningkatan sifat organoleptik dan jumlah bakteri asam laktat pada produk sehingga dapat mengoptimalkan manfaatnya sebagai pangan fungsional anti diabetes melitus. Desain penelitian adalah eksperimen dengan rancangan acak lengkap dengan satu formula kontrol dan tiga formula perlakuan dengan perbadingan kedelai dan labu kuning sebagai berikut F0 100:0, F1 80:20, F2 70:30 dan F3 60:40. Uji mutu organolpetik dilakukan skala 1–6 dari tidak suka sampai dengan sangat suka. Formula terbaik dilanjutkan untuk uji fisikokimia dan proksimat. Hasil uji organoleptik menunjukkan perlakuan terbaik adalah substitusi labu kuning sebanyak 20 persen (F1) dengan skala kesukaan 4 yang artinya formula ini secara sensori dapat diterima oleh panelis. Mutu gizi dari produk terpilih yaitu protein 3,15 persen, lemak 1,35 persen, karbohidrat 0,26 persen, serat 1,03 persen, kadar BAL 9,5x107 dan pH 4,13. Subtitusi labu kuning pada soygurt berpotensi mengendalikan kadar glukosa darah karena rendah karbohidrat dan lemak serta mengandung BAL. Penelitian lebih lanjut diperlukan untuk menganalisis kandungan zat anti gizi seperti fitat, tanin dan tripsin inhibitor yang umumnya terdapat pada kedelai yang menjadi bahan baku dari produk ini. Kata kunci: diabetes melitus, kedelai, labu kuning, soygurt

Abstract

Several factors that can affect primary dysmenorrhoea are poor dietary habits and low intake of macro and micronutrients. One of the micronutrients that affect dysmenorrhoea is calcium intake. Calcium plays a role in regulating the work of hormones in growth. Calcium deficiency can encourage overproduction of prostaglandins which can lead to uncontrolled uterine contractions that can trigger pain. Based on data from the USDA Nationwide Food Consumption Survey in 48 countries, the average calcium intake at the age of 15-18 years is less than 70% of the RDA. A preliminary study conducted by the author at a public high school in Padang found that 80.2% had primary dysmenorrhoea. Individual eating habits can have an impact on behavior related to healthy food choices and can affect nutritional intake including calcium intake. This study aims to determine the relationship between healthy food selection and calcium intake in adolescent girls who experience primary dysmenorrhoea. 100 respondents were determined using the Slovin formula with the criteria of female students aged 15-18 years who experienced primary dysmenorrhoea. Data collection on healthy food choices was measured using a questionnaire of healthy food choices which consists of 30 statements with a score for selecting important healthy foods > 120. Calcium intake data uses the SQ-FFQ form with the adequate calcium intake category ? 1200 mg/day. Data analysis using Spearman rank correlation test. The results of statistical tests show that there is a relationship between healthy food choices and calcium intake (r = 0.23, p = 0.021). The results of this research can be used as a basis for providing therapeutic options for dysmenorrhoea. The implications for young women are knowing comprehensively the importance of choosing healthy foods and calcium intake to reduce the pain of primary dysmenorrhoea.

Abstract

Background: Low back pain is common in healthcare workers including doctors, nurses, midwives and physiotherapists, due to the nature of their work. These healthcare workers are often involved in tasks that involve heavy lifting. Manual patient handling, and awkward postures that are an integral part of care may have an effect on musculoskeletal injuries. This study aims to analyze the effect of heavy lifting on low back pain among healthcare workers. Subjects and Method: This study is a systematic review and meta-analysis study. The search for articles was carried out in accordance with the PICO Model eligibility criteria which include: P = Health workers, I = Heavy lifting, C = No heavy lifting, O = Low back pain. The articles used came from 2 databases, namely: PubMed and Science Direct. The keywords used were "heavy lifting" AND "low back pain" OR "musculoskeletal of low back pain” AND "healthcare professional" OR "healthcare workers". The inclusion criteria in this study included full-text articles with a cross-sectional study design, the relationship measure used was the adjusted Odds Ratio (aOR), articles published in the range 2014-2022, and the outcome was Low back pain. Articles were analyzed using the PRISMA diagram and the Review Manager 5.4.1 application. Results: 9 articles from Ethiopia, Japan, Malaysia, Nigeria, Pakistan, Saudi Arabia and Uganda. Healthcare workers who performed heavy lifting increased lower back pain by 4.36 times compared to those who did not, and the results were statistically significant (aOR=4.36; 95% CI=3.07 to 6.20; p<0.001). Conclusion: A meta-analysis of 9 cross-sectional studies concluded that heavy lifting increases low back pain in healthcare workers.

Abstract

Background: The management of diabetes mellitus is often faced with the challenge of adherence to antidiabetic herbs. Poor adherence to diabetes medication increases the risk of complications and death. This study aims to analyze the determinants related to treatment adherence in Type 2 DM patients. Subject and Method: The cross-sectional study was carried out at UPF Yankestrad Dr. Sardjito Tawangmangu Hospital. A total of 201 adult patients with Type 2 DM were selected in this study. The dependent variable is treatment adherence in Type 2 DM patients. The independent variables were education, knowledge, access to health services, family support, income, and side effects of treatment. Data was collected using questionnaires. Data were analyzed by double linearregression. Result: There was a statistically significant positive association between knowledge and medication adherence (b = 0.19; 95% CI = 0.12 to 0.26; p&lt;0.001); access to healthcare (b = 0.37; 95% CI = 0.26 to 0.47 p&lt;0.001); family support (b = 0.29; 95% CI = 0.20 to 0.39; p&lt;0.001); income (b = 0.15; 95% CI = 0.01 to 0.29; p= 0.030); and there was a negative and statistically significant association between adverse events and medication adherence (b = -0.33; 95% CI = -0.49 up to -0.18; p&lt;0.001). The model fit between the independent variables included in the linear multiple regression analysis model together was able to explain the variation in treatment adherence by 67.12% (Adj R-squared= 67.12%). Conclusion: Treatment adherence in Type 2 DM patients is positively related to knowledge, access to services, family support, and income. The independent variables included in the linear multiple regression analysis model together were able to explain the variation of treatment adherence by 67.12% (Adj R-squared = 67.12%).

Abstract

The problem of stunting occurs because children are malnourished during 1000 days of life, characterized by shorter height compared to children of the same age. Various factors cause stunting, and lack of maternal nutritional knowledge can affect maternal nutritional behavior in improper feeding. Nutrition counseling interventions were carried out for mothers so that changes in maternal nutritional behavior occur for the better. This study aimed to analyze the effect of nutrition counseling on maternal nutrition knowledge and attitudes, nutrient intake, and nutritional status of stunted toddlers. This study used a randomized control trial, which was located in 5 health centers including Puskesmas Kedungkandang, Arjuno, Ciptomulyo, Dinoyo, and Polowijen Malang City, in June-August 2023. Respondents were mothers of stunted children aged 6-24 months. The number of respondents in the treatment and control groups was 44 people each group. Statistical analysis used was the Paired Sample T-test, Wilcoxon Test, Independent Sample T-test, and Mann-Whitney U test. There was a significant difference in the mean between the two groups on nutritional knowledge (p=0.000), respondents' attitudes (p=0.000), nutrient intake of energy (p=0.000), carbohydrate (p=0.021), and protein (p=0.000), and nutrition status of stunted toddlers (p=0.000). Nutrition counseling can improve nutrition knowledge, maternal attitudes, nutrient intake energy, carbohydrates, and protein, and nutrition status of stunted toddlers.
