Abstract

Tuberculous spondylitis accounts for around 2% of all cases of Tuberculosis (TB) and around 15% of extrapulmonary TB cases. It has been reported that a 17 years old boy with a complaint of a bump on the lumbar region and felt low back pain since two years before admission with a history of back trauma. There was a decrease of body weight. There was no paraesthesia nor paralysis. Defecation and micturition were normal. Basic immunization was incomplete. On physical examination found palpable lymph nodes 0,5x0,5x0,5 cm, multiple et regio colli. There was no BCG scar. Impression nutritional status was undernourished. There was fixed palpable mass at back size about 5x4x5 cm, hard, no fluctuations, no rebound tenderness. Lung examination was normal. Tuberculin test showed induration sized 20 mm. Gene Xpert result Micobacterium Tuberculosis (MTB) not detected. On chest X-ray examination found L1-2 corpus destruction. MRI Spine was suggestive of compressive fractures and suggestive of a bilateral psoas abscess. Decompression and lumbar stabilization surgery were performed. The histopathology examination results were consistent to spondylitis TB characteristics. The patient was discharged on 6th hospitalization and given anti-tuberculous drug.Keywords:  bump, extrapulmonary, fracture, spondylitis, tuberculosis

Abstract

Prevalence of asthma in children increase especially in the industrial city. Cement factory in Padang city is located near from community, cause air pollution, especially to children. We aim to know the prevalence and risk factor of asthma and allergic disease in this area. A cross sectional study to children age 0-15 years old has been done on May-June 2015, around the Cement factory, with distance from factory <5 km (exposed area) and >10 km (un-exposed area). We used International Study of Asthma and Allergy in Childhood (ISAAC) questionnaire to determine asthma and allergic disease. Data was taken from the parent about asthma, allergic rhinitis and atopic dermatitis symptoms and some risk factors. Data was analyzed with chi-square test. This study found 90 children, 43 (47.8%) from exposed area. Asthma prevalence and atopic dermatitis was found higher in un-exposed area than exposed area (19.5% vs 13.9% and 39.1% vs 37.2%), allergic rhinitis was higher in exposed area (60.4% vs 58.6%), though no significance difference. This study concluded that there were no significant differences in asthma and allergies disease in exposed and un-exposed area of semen factory emissions.

Abstract

The Severe Acute Respiratory Syndrome Corona Virus-2 (SARS-CoV-2) that cause COVID-19 global pandemic is a devastating disease causing thousands of victims every day. Therefore, there must be a certain effort to reduce the risk of the disease progress, including improvement of individual immune system like the probiotic supplementation. Meanwhile, dadih is a dairy product obtained from fermented buffalo milk contains various nutrients for human body, especially probiotics that beneficial for reducing the risk of COVID-19 infection in children. The aim of the study was to determine the nutritional and microbiological content of dadih pudding as additional supplementation in children confirmed positive for COVID-19. The study design was True Experiment using Completely Randomized Design (CRD). It consisted of four formulas. They were F0 as the standard formula, then F1, F2, and F3 as the treatment formula by adding dadih 50, 60, 70, and 80 grams, respectively. This study was initiated with the analysis of biochemical nutritional value  at a laboratory in Padang Research Center and Industry Standardization. Besides, it also determined the number of total Acid Lactic Bacterial quantification which was analyzed at microbiology laboratory, Agricultured Technology Faculty, Universitas  Andalas. Nutrients and lactic acid bacteria in dadih pudding are potential as food supplementation, especially for children with COVID-19 infection. It is strongly recommended to give probiotic dadih to children in order to counter oxidative stress and inflammation, and the setting of COVID-19.

Abstract

Food poisoning is a serious problem that can have an impact on public health. This incident occurs when the food consumed is contaminated by bacteria, viruses, parasites or hazardous chemicals. Medan City is an area where food poisoning incidents occur every year. The purpose of this study is to examine the importance of the role of health workers in preventing extraordinary incidents of food poisoning. This study is an exploratory qualitative study through a phenomenological approach. The location of the qualitative research was conducted at the Medan City Health Office, in the working area of the Medan Area, Medan Deli, and Tuntungan Health Centers which were designated as research areas. Data collection was carried out through several main techniques, namely in-depth interviews with interview guidelines, observation, and focus group discussions. While The data processing process involves several stages, namely starting with data transcription, data reduction, and data presentation. The final stage is drawing conclusions and verification. Data analysis uses a triangulation approach to increase the credibility of research results, including data triangulation, observer triangulation, theory triangulation, and triangulation Methods. This study was conducted in 2024 with informants including the Disease Prevention and Control Team and the Public Health Team, the head of the health center. The results of this study indicate that efforts to prevent food poisoning outbreaks have not been going well. Where the cause is the suboptimal role of health workers in monitoring and evaluation due to limited human resources, in addition to the lack of training related to prevention and handling and the absence of a special policy for food poisoning outbreaks. This prevention can only be implemented if there is collaboration between health workers and catering owners to add a shared commitment to food safety. and it is necessary to carry out comprehensive coaching and supervision efforts. Abstract

Pregnant women should pay close attention to their health and the health of their fetuses. To prevent maternal and infant mortality, antenatal care visits are one of the actions that must be adhered to by pregnant women. This study aims to explore the perspectives of pregnant women in antenatal care visits in Padang Lawas Regency. The study was qualitative with a grounded theory approach. This study involved five mothers using purposive sampling. Data was collected through in-depth interviews and analyzed by content analysis. The results emerged namely: 1) support from husband and family, 2) accessibility, 3) health providers, and 4) mother’s behavior. Health providers play a very important role in making antenatal care visits a success routinely and completely.

Abstract

Food poisoning outbreaks are a serious problem that can have a significant impact on public health. Food poisoning occurs when food consumed is contaminated with bacteria, viruses, parasites, or hazardous chemicals. Medan City experiences fluctuating food poisoning incidents, so continuous monitoring is needed to identify and address the causes. The purpose of this study was to determine early warning by using a module to prevent food poisoning outbreaks. This study was conducted quantitatively using an experimental design with the one group pretest-posttest design (before and after) approach. The location of the study was carried out at the Medan City Health Office and a population of 20 respondents, while the sample, namely the total population, was the sample. The data obtained were analyzed univariately, bivariately. The results showed that knowledge in catering owners with n = 20, paired sample T-test there was a significant difference between the level of knowledge of respondents about food poisoning before and after the intervention using the module p value = 0.0001 (<0.05). The module that has been developed and validated by language experts with results of 94.3, material experts with 97.3 and media experts with 91.3 can effectively prevent poisoning. It is recommended to the local Health Service and Health Center to be able to use the module as an early warning model for food poisoning outbreaks, especially for catering business owners, so that they can increase knowledge, especially in carrying out prevention. Abstract

One of the nutritional problems of children in Indonesia is stunting. Fulfillment of nutrition in the first 1,000 days of life is related to stunting. WHO recommends breastfeeding and breastfeeding as a method of feeding babies to achieve optimal growth and development. This descriptive study aims to provide an overview of the feeding practices of stunted and non-stunting toddlers as seen in the feeding history included in the Toddler Feeding Area Chart from 0-6 months of age. This research found that giving breast milk alone to stunted toddlers (52.17%) was lower than non-stunting toddlers (66.67%), and the most common feeding pattern for stunted toddlers was formula milk.

Abstract

Tuberculosis (TB) is a leading public health concern in Indonesia. It ranks second on the list of high-burden TB countries. In West Sumatra, 47% of TB cases are undetected, late diagnosed, and received incomplete treatment because of low-level awareness and knowledge and stigma, especially among the hardest to reach populations. The study aims to identify the best communication channel to reach those who live in vulnerable and remote areas. This study was a qualitative study applying in-depth interviews to the informal leaders, health officers, cultural artists, and religious leaders across districts in Mentawai and Solok Districts, which are remote and had the lowest case detection rates compared with other districts. The questionnaire was prepared with the perception of the channel to identify TB cases. The data were analyzed using the content analysis technique. Involving religious and informal leaders and using traditional music as a communication channel improved the population's awareness of TB symptoms and access to TB testing and treatment, as well as reduced TB-related stigma. This study found that the cultural and religious contexts play a major role in health communication on TB control for hard-to-reach populations in West Sumatera, Indonesia.

Abstract

The low utilization of antenatal care services has an impact on the condition of the pregnancy not being monitored which will cause danger signs of pregnancy to go undetected and can lead to maternal death. Padang Lawas is the area with the lowest coverage of antenatal care visits in north Sumatra province. The majority of pregnant women in Padang Lawas district do not receive standard antenatal care services. Efforts to increase coverage rates for Antenatal Care visits have been carried out by the Health Service but have not produced any real impact. Objective: This study aims to find out more about the perspective of antenatal care visits for pregnant women. Method: This study was qualitative with a grounded theory approach. The informants in this study were 5 mothers, 5 husbands, 3 midwives, and 1 community figure who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 4 themes: (1) Culture about pregnancy, (2) Infrastructure in antenatal care, (3) Economic issues, and (4) Mother's behavior in carrying out antenatal care. Conclusions: The findings show that there are several barriers of antenatal care visits in rural maternal communities, so the government needs to pay attention to them in following up on the barriers found in these areas.
