Abstract

Keselamatan pasien rumah sakit adalah suatu sistem dimana rumah sakit membuat asuhan pasien lebih aman. Upaya peningkatan mutu pelayanan dan keselamatan pasien di rumah sakit merupakan sebuah gerakan universal. Perbaikan pada kualitas pelayanan seharusnya sejalan dengan meningkatnya keselamatan pasien dengan penerapan sasaran keselamatan pasien dan meminimalkan terjadinya insiden. Penelitian ini bertujuan untuk menganalisis pengaruh pengetahuan, sikap, motivasi, supervisi dan fasilitas rumah sakit terhadap penerapan sasaran keselamatan pasien di Ruang Rawat Inap di Rumah Sakit Tentara TK IV 01.07.01 Pematangsiantar. Desain penelitian ini adalah observasional analitik dengan rancangan cross sectional. Sampel dalam penelitian ini sebanyak 72 perawat yang bertugas di Ruang Rawat Inap dengan tehnik pengambilan sampel dengan purposive sampling. Tehnik pengumpulan data dengan menggunakan instrumen berupa kuesioner pengetahuan, sikap, motivasi, supervisi dan fasilitas rumah sakit. Analisa data pada penelitian ini menggunakan uji chi-square dan analisa data multivariat dengan menggunakan uji Regresi Linear Berganda. Berdasarkan hasil analisis bivariat didapatkan bahwa terdapat pengaruh pengetahuan (p- value=0,000), sikap (p-value=0,001, motivasi (p-value=0,003), supervisi (p-value=0,000) dan fasilitas rumah sakit (p-value=0,000) terhadap penerapan sasaran keselamatan pasien. Berdasarkan hasil analisis multivariat didapatkan faktor paling dominan berpengaruh terhadap penerapan sasaran keselamatan pasien adalah supervisi dengan nilai standardized coefficient Beta=0,316. Efforts to improve the quality of service and patient safety in hospitals are a universal movement. Improvements in service quality should be in line with increasing patient safety by implementing patient safety goals and minimizing incidents. This study aims to analyze the effect of knowledge, attitudes, motivation, supervision and hospital facilities on the implementation of patient safety targets in the Inpatient Room at the Army Hospital TK IV 01.07.01 Pematangsiantar. The design of this research is observational analytic with a cross sectional design. The sample in this study was 72 nurses who served in the Inpatient Room using a purposive sampling technique. Data collection techniques using instruments in the form of questionnaires on knowledge, attitudes, motivation, supervision and hospital facilities. Data analysis in this study used the chi- square test and multivariate data analysis using the Multiple Linear Regression test. Based on the results of bivariate analysis, it was found that there was an effect of knowledge (p-value=0.000), attitude (p-value=0.001, motivation (p-value=0.003), supervision (p- value=0.000) and hospital facilities (p-value = 0.000) on the implementation of patient safety goals. Based on the results of multivariate analysis, it was found that the most dominant factor influencing the implementation of patient safety targets was supervision with a standardized coefficient value of Beta = 0.316. Abstract

Cesarean delivery (C-section) rates continue to rise worldwide, with Thailand showing a significant spike in the past two decades, surpassing WHO recommended rates. Hospital characteristics, such as ownership, financial incentives, and medical staffing patterns, can influence cesarean delivery rates. Private hospitals may be more susceptible to non-medical motivations, while public hospitals often face high patient volumes and resource constraints. This study aimed to investigate the use of cesarean delivery in relation to hospital characteristics, with a comparative focus between public and private hospitals in Thailand. This study used a cross-sectional quantitative approach with secondary data from the Thailand Multi Indicator Survey (MICS) 2022. The study sample consisted of 1,046 women who delivered by cesarean section and 1,720 women who did not, with the dependent variable being hospital utilization by caesarean section delivery. Data analysis was performed using univariate, bivariate (Chi-square test), and multivariate (binary logistic regression) tests to analyze the effect of independent variables on the choice of hospital type. The study findings were that in women undergoing cesarean section, wealth and region of residence were significantly associated with private hospital utilization, with women from the fourth wealth quintile (OR = 14.61; 95% CI: 1.88–113.85) and the richest (OR = 23.67; 95% CI: 3.03–185.09) more likely to use private hospitals than the poorest, while those living outside Bangkok were less likely. In women without cesarean section, living in rural areas (OR = 0.36; 95% CI: 0.16–0.81) and outside Bangkok also significantly decreased the odds of private hospital utilization.

Abstract

The performance of inpatient nurses at AmaliaMedika Riau Hospital has not been implemented optimally, this is due to ineffective two-way communication both to superiors and fellow nurses. Teamwork that is not well established, so that there is work that is not carried out optimally. The purpose of this study is to determine the influence of communication and teamwork on the performance of nurses at Amalia Medika Riau Hospital. The research design used in this study is quantitative research. The population in this study is all nurses in the inpatient room at Amalia Medika Riau Hospital as many as 45 people. The sampling technique uses the saturated sample technique so that the sample totals 45 people. The data collection technique uses a questionnaire. Data analysis was carried out by univariate and bivariate analysis with chi square and multivariate tests. The results of the study showed that there was an influence of communication on the performance of nurses at Amalia Medika Riau Hospital and there was an influence of teamwork on the performance of nurses at Amalia Medika Riau Hospital. A multivariate analysis was obtained that the communication variable had the most influence on the performance of nurses at Amalia Medika Riau Hospital. The conclusion in this study is that there is an influence of communication and teamwork on the performance of nurses at Amalia Medika Riau Hospital and the variable that has a higher chance is communication. The implication of this study is that the performance of inpatients at Amalia Media Riau Hospital can be improved by improving two-way communication by creating openness, empathy and mutual support between fellow nurses and superiors. Improving the teamwork of inpatient nurses by contributing both energy and mind to improve performance.

Abstract

The corona virus has become an epidemic in several countries. At Kualanamu International Airport, several cases of corona virus were found in several employees at the airport terminal who were in direct contact with several passengers every day in carrying out checks. The reason for the large number of employees being exposed is because there are still workers at the airport and passengers and visitors who do not comply with health protocols, namely not using masks properly and correctly and there are still passengers who do not restrict distance. The COVID-19 case at Kualanamu International Airport was found in employees in June and 9 employees were found to be exposed to COVID-19. The purpose of this study was to determine the implementation of the new normal health protocol at Kualanamu International Airport. This research is descriptive using qualitative research methods conducted at Kualanamu International Airport by using in-depth interviews with 4 AVSEC officers, 3 airlines, 5 passengers, 2 health workers, and 2 restaurant managers. From the results of the research that has been done, it can be seen that the implementation of the new normal COVID-19 health protocol at Kualanamu International Airport is in accordance with the regulations set by the government. The application of the health protocol, several obstacles and causes of non-compliance with the health protocol were found, because there were some passengers and visitors who were negligent in implementing the health protocol not wearing masks properly, such as lowering the mask on the mouth or chin area and passengers who did not restrict distance, especially on during check-in, validate the results of the rapid / swab test as well as when going to or from the plane. For this reason, Kualanamu International Airport Officer is expected to continue to convey the importance of implementing the new normal COVID-19 health protocol. Passengers are also expected to increase discipline in implementing the COVID-19 health protocol. Abstrak Virus corona telah menjadi wabah di beberapa negara. Di Bandara International Kualanamu ditemukan beberapa kasus corona virus pada beberapa karyawan yang berada di terminal bandara yang setiap hari kontak langsung dengan beberapa penumpang dalam melakukan pemeriksaan. Penyebab banyaknya karyawan terpapar dikarenakan masih ada pekerja di bandara dan penumpang serta pengunjung yang tidak mematuhi protokol kesehatan yaitu tidak menggunakan masker dengan baik dan benar serta masih adanya penumpang yang tidak melakukan pembatasan jarak. Kasus COVID-19 di Bandara International Kualanamu ditemukan pada pegawai di bulan Juni dan didapati 9 orang karyawan terpapar COVID-19. Sampai bulan Desember saat ini didapati ada 35 orang karyawan yang terpapar COVID-19. Tujuan dari penelitian ini adalah untuk mengetahui penerapan protokol kesehatan masa new normal di Bandara Internasional Kualanamu. Penelitian ini bersifat deskriptif dengan menggunakan metode penelitian kualitatif yang dilakukan di Bandara Internasional Kualanamu dengan menggunakan metode wawancara mendalam terhadap 4 orang Petugas AVSEC, 3 orang Pihak Maskapai, 5 orang Penumpang, 2 orang Petugas Kesehatan, dan 2 orang Pengelola Restaurant. Dari hasil penelitian yang telah dilakukan dapat diketahui bahwa penerapan protokol kesehatan COVID-19 masa new normal di Bandara Internasional Kualanamu sesuai dengan aturan yang telah ditetapkan oleh pemerintah. Penerapan protokol kesehatan ditemukan beberapa hambatan dan penyebab tidak dipatuhinya protokol kesehatan, dikarenakan ada beberapa penumpang dan pengunjung yang lalai dalam penerapan protokol kesehatan tidak memakai masker dengan baik dan benar seperti menurunkan masker pada area mulut atau dagu serta penumpang yang tidak melakukan pembatasan jarak terutama pada saat check-in, memvalidasi hasil rapid/swab test serta pada saat akan naik atau turun dari pesawat. Untuk itu Petugas Bandara Internasional Kualanamu diharapkan dapat terus menyampaikan pentingnya penerapan protokol kesehatan COVID-19 masa new normal. Penumpang juga diharapkan untuk lebih meningkatkan disiplin dalam menerapkan protokol kesehatan COVID-19.

Abstract

Infant mortality has a close relationship with the quality of maternal care, so the process of childbirth and baby care must be in an integrated system at the national and regional levels. The study was conducted at Dr. Pirngadi Hospital Medan 2019. The research aims at describing the administration and management of PONEK, describing the adequacy of PONEK HR, and describing the fulfillment of PONEK facilities. This research approach is descriptive qualitative with data collection techniques through interviews. The informants of this study were the PONEK Implementation Team and the PONEK Management Team with a total of 8 people. The research instrument was the researcher himself and the collection of data through interviews and documentation. Processing and analysis of data used in the Content Analysis method. The results of the study were that the PONEK funding source comes from the Regional Public Service Agency. Incorrect PONEK Team Placement. Many PONEK Team members have not yet received PONEK training and certificates. 24-hour PONEK facilities. Blood supply of RSUD Dr. Pirngadi Medan is a Blood Transfusion Unit. The conclusion of the research is that management and administration are less than optimal. The PONEK executive officer does not get special PONEK incentives. The placement of Human Resources in implementing PONEK officers is not in accordance with their main tasks and functions, many PONEK Team members have not received PONEK certificate training but have provided emergency obstetric services. PONEK facilities serve 24 hours. The suggestion is that the budget for the procurement of facilities be proposed in the Regional Revenue and Expenditure Budget / Medan Government so that the Regional Public Service Board budget obtained by the hospital can be used optimally for the need to improve PONEK training and provide special PONEK incentives for PONEK implementing officers. Abstrak Kematian bayi mempunyai hubungan erat dengan mutu penanganan ibu, maka proses persalinan dan perawatan bayi harus dalam sistem terpadu di tingkat nasional dan regional. Penelitian dilakukan di RSUD Dr. Pirngadi Medan Tahun 2019. Penelitian bertujuan mendeskripsikan administrasi dan pengelolaan PONEK, mendeskripsikan kecukupan SDM PONEK, dan mendeskripsikan pemenuhan fasilitas PONEK. Pendekatan penelitian ini deskriptif kualitatif dengan teknik pengumpulan data melalui wawancara. Informan penelitian ini Tim Pelaksana PONEK dan Tim Manajemen PONEK dengan jumlah seluruhnya 8 orang. Instrument penelitian adalah peneliti sendiri dan pengumpulan data melalui wawancara dan dokumentasi. Pengolahan dan analisis data menggunakan metode Content Analysis. Hasil penelitian adalah Sumber dana PONEK berasal dari Badan Layanan Umum Daerah. Penempatan Tim PONEK yang tidak sesuai. Banyak anggota Tim PONEK yang belum mendapatkan pelatihan dan sertifikat PONEK. Fasilitas PONEK 24 jam. Persediaan darah RSUD Dr. Pirngadi Medan bersifat Unit Transfusi Darah. Kesimpulan penelitian yaitu Pengelolaan dan administrasi kurang maksimal. Petugas pelaksana PONEK tidak mendapatkan insentif khusus PONEK. Penempatan Sumber Daya Manusia petugas pelaksana PONEK tidak sesuai tupoksinya, banyak anggota Tim PONEK yang belum mendapatkan pelatihan sertifikat PONEK tetapi sudah memberikan pelayanan obstetri emergensi. Fasilitas PONEK melayani 24 jam. Saran yaitu anggaran pengadaan fasilitas diusulkan dalam Anggaran Pendapatan dan Belanja Daerah/Pemerintah Kota Medan, sehingga anggaran Badan Layanan Umum Daerah yang didapat rumah sakit bisa digunakan secara maksimal untuk kebutuhan meningkatkan pelatihan PONEK dan pemberian insentif khusus PONEK bagi petugas pelaksana PONEK.

Abstract

This study aims to analyze the long-term impact of stunting in childhood on cognitive development and educational attainment through a meta-analysis approach. Stunting, which is indicated by stunted linear growth due to chronic malnutrition, has long been associated with various negative consequences on health and social aspects. By systematically examining the results of quantitative studies from various countries and socio-economic contexts, this study identified a consistent pattern of association between stunting in early life and decline in cognitive function and academic achievement in adolescence and adulthood. Data were obtained from 25 studies that met the inclusion criteria published in the 2015 to 2024 range, and were analyzed using fixed-effect and randomized effects models with the help of the JASP application. The results of the meta-analysis showed that children who experienced stunting had an effect on cognitive and education with a pooled effect size value (Hedges' g = 1.10 t= 9.103; p < 0.001) with a high effect size category. These findings confirm that stunting not only has an impact on short-term physical conditions, but also has significant consequences for human resource development in the long term. Therefore, comprehensive nutrition and education interventions in early life are essential to break the chain of loss due to stunting.

Abstract

Rumah sakit menyelenggarakan kegiatan pelayanan kesehatan berperan mendukung pencapaian derajat kesehatan masyarakat yang optimal. Upaya tersebut dapat dinilai dengan Turn Over Interval (TOI) sebagai Indikator untuk mengetahui tingkat pemanfaatan tempat tidur, mutu pelayanan, dan efisiensi pelayanan rawat inap suatu rumah sakit. Tujuan penelitian ini adalah untuk menilai Turn Over Interval (TOI) pelayanan rawat inap di rumah sakit berdasarkan standar Barber-Johnson. Penelitian ini menggunakan jenis penelitian kualitatif dengan pendekatan retrospektif menggunakan metode fenomenologi. Penelitian dilakukan dengan cara wawancara semi terstruktur yang sudah termasuk dalam kategori Indepth Interview. Dalam penelitian ini terdapat 7 orang informan. Informan yang dipilih merupakan orang yang sangat mengetahui permasalahan yang akan diteliti mengenai rendahnya Nilai Turn Over Interval (TOI) yang menyebabkan tidak efesiennya pelayanan rawat inap di Rumah Sakit. Berdasarkan hasil penelitian ditemukan input, proses dan output lama tempat tidur tidak terisi (TOI) pelayanan rawat inap di Rumah Sakit didapatkan rata-rata nilai indikator TOI yaitu diruang mawar 0,43-0,78 hari, ruang kenanga 0,73-0,90 hari dan ruang dahlia 0,90-1,26 hari. Kesimpulan dalam penelitian ini adalah data yang di kumpulkan oleh petugas sensus harian rawat inap masih menggunakan media kertas sehingga sangat memungkinkan terjadinya ancaman pada akurasi dan ketelitian data dan dihitung berdasarkan standar Depkes RI, hal ini menjadi penyebab sulitnya untuk diinterpretasikan dibandingkan dengan Grafik Baber-Johnson. Rata-rata nilai TOI di ruang mawar, kenanga dan dahlia yaitu 0-1 hari.

Abstract

WHO (World Health Organization) menunjukkan bahwa Indonesia masuk dalam lima Negara dengan kinerja perawat paling rendah. Rendahnya kinerja perawat berpotensi mengancam kepuasan pengguna pelayanan kesehatan dan reputasi rumah sakit sebagai produsen pelayanan kesehatan. Tujuan penelitian mengetahui faktor yang mempengaruhi kinerja perawat rawat inap di Rumah Sakit Umum Bunda Thamrin Medan. Metode penelitian yang digunakan adalah survey analitik dengan rancangan cross sectional. Populasi adalah seluruh perawat, Tehnik pengambilan sampel adalah purposive sampling sebanyak 75 orang. Instrumen yang digunakan berupa kuisioner. Tehnik pengumpulan data dengan mengumpulkan data primer dan skunder. Uji statistik yang digunakan adalah uji chi square dan regresi logistik. Hasil uji chi-square motivasi (P-value 0,000), kedisiplinan (P-value 0,017), insentif (P=Value 0,002), lingkungan kerja (P-value 0,000), kemampuan kerja (P-value 0,000) yang artinya ada pengaruh variabel yang diamati terhadap kinerja perawat. Hasil uji regresi logistik variabel dominan berpengaruh terhadap kinerja perawat adalah variabel kemampuan kerja p=0,003 < 0,025 (OR: 277.759; 95%CI=6.862-11243.720) artinya kemampuan kerja berpengaruh 277.759 kali terhadap kinerja perawat.

Abstract

Pelayanan kesehatan administrasi di rumah sakit sangat erat kaitannya dengan penggunaan Sistem Informasi Manajemen Rumah Sakit (SIMRS). SIMRS merupakan suatu platform teknologi informasi yang dirancang khusus untuk mengelola dan mengintegrasikan berbagai aspek administrasi dan operasional di rumah sakit. Penelitian ini bertujuan untuk menganalisis dampak implementasi SIMRS terhadap pelayanan administrasi menggunakan metode PIECES. Metode penelitian yang digunakan adalah metode kualitatif dengan melibatkan 11 informan. Pengumpulan data dilakukan melalui wawancara mendalam untuk memperoleh informasi yang komprehensif tentang pengalaman dan pandangan para informan terhadap implementasi SIMRS. Analisis data dilakukan dengan pendekatan deskriptif kualitatif untuk memahami secara mendalam fenomena yang terjadi. Hasil penelitian menunjukkan bahwa implementasi SIMRS di Rumah Sakit Umum Haji Syaiful Anwar telah membawa peningkatan signifikan dalam efisiensi waktu dan kemudahan pengisian data pasien, serta mengurangi penggunaan kertas. Namun, terdapat beberapa kendala yang masih perlu diatasi, seperti kurangnya pemahaman dan keterampilan SDM, keterbatasan komputer, dan modul yang belum terintegrasi. Kesimpulan dalam penelitian ini adalah implementasi SIMRS di RSU Haji Syaiful Anwar yang ditinjau dari Performance (Kinerja), Information/Data (Informasi/Data), Economic (Ekonomi), Control/Security (Kontrol/Keamanan), Efficiency (Efisiensi), Service (Pelayanan) sudah cukup baik namun masih perlu dilakukan perbaikan agar SIMRS dapat berfungsi secara optimal dan memberikan manfaat maksimal bagi rumah sakit. Disarankan evaluasi berkala, pelatihan tambahan, Strategi untuk mengatasi turnover pegawai yaitu perekrutan yang baik, pengembangan karier dan kenaikan gaji, serta peningkatan infrastruktur teknologi untuk mengoptimalkan integrasi modul dan efisiensi pelayanan.
