Abstract

Ventilator-associated pneumonia (VAP) can occur in critically ill patients undergoing mechanically ventilated 48 hours after intubation. The incidence of VAP can be prevented by performing a VAP bundle, one of which is by performing oral hygiene using antiseptic ingredients that are effective killing germs. The literature review aims to analyze materials that are effective in performing oral hygiene. A total of 25 articles from 1728 articles were selected in the review. Systematic reviews were carried out using the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) reporting technique. Article searches were carried out through the online databases of Pubmed, Scientdirect, and Google Scholar. Articles are limited by type and year (last 10 years), but not by language. The results of the review show that the ingredients used for oral hygiene and proven effective in preventing VAP are ingredients that contain antibacterial and antiseptic. The conclusion of this literature review is that oral hygiene using ingredients that can suppress growth and kill bacteria is proven to prevent the occurrence of VAP.

Abstract

Introduction : Health education about pregnancy care is important to increase the knowledge of pregnant women so that it will encourage positive attitudes and behavior towards the health of the mother and fetus. Audiovisual media and leaflets are often used to convey health information. Objective : This research aims to determine the differences in the effectiveness of the two media in increasing knowledge, attitudes and practices of pregnancy services for pregnant women at the Tambakromo Community Health Center. Method : This research uses an experimental design with a two group pretest posttest design. The research sample was 76 pregnant women at the Tambakromo Community Health Center who were divided into two groups randomly. The first group was given health education using audiovisual media, and the second group was given leaflet media. Data were analyzed using the Independent Sample T-test and Mann Whitney with p0.05. Result : Respondents' knowledge, attitudes and pregnancy care practices increased after being given health education. Audiovisual media is more effective than leaflet media in increasing knowledge (p-value = 0.004), practice (p-value = 0.007), and attitudes (p-value = 0.010) of pregnant women regarding pregnancy care. Conclusion : Increased knowledge, attitudes and practices show that both media are effective in increasing pregnant women's awareness about the importance of pregnancy checks. However, audiovisual media has proven to be more effective because it is multisensory, making it more interesting and easier to understand. Health education using audiovisual media is more effective than leaflet media in increasing knowledge, attitudes and practices of pregnancy care among pregnant women at the Tambakromo Community Health Center.

Abstract

Introduction: Stroke is a serious and life-threatening neurological disease worldwide that has long been a major concern in the world health sector. Stroke has become a global health challenge and is a major cause of disability and death in many countries. Many studies highlight the risk factors that contribute to the occurrence of a first stroke attact, but there is a limited study that focuses on the factors that influence someone to experience a recurrence of stroke. Objective: To identify the factors that contribute and are the most dominant in the incidence of recurrence stroke. Method: This study is a descriptive quantitative study that employs observation methods and a cross-sectional approach, with data analysis conducted using univariate, bivariate, and multivariate techniques. Results: There are 152 recurrent stroke patients partisipated in this study. The accidental sampling technique was used to obtain potential respondents. Researchers found that out of the 10 factors that affect the chance of having a recurrence stroke, three are the most important. These are self-rehabilitation (P value = 0.000; OR = 58.843, CI = 7.0497–491.165), family support (P value = 0.027; OR = 0.931, CI = 0.8734-0.992), and taking medications as prescribed (P value = 0.031; OR = 0.670, CI = 0.4666-0.963). Conclusion: There are 3 factors that most dominantly influence the incidence of recurrent stroke, namely early rehabilitation, family support, and patient compliance in taking medication. Furthermore, enhancing patients' adherence to self-management poststroke requires social support from family, peers, and healthcare providers.

Abstract

ABSTRACT Heart failure is a chronic disease that has a widespread impact on the physical and psychological condition of patients and their families, including causing anxiety during hospitalization. This study aims to determine the level of anxiety experienced by families and patients with heart failure at Surakarta Regional Hospital. The research design is a quantitative descriptive study with 290 heart failure patients and their families as the population, yielding a total sample of 580 respondents selected using purposive sampling. The data collection instrument used the Hamilton Anxiety Rating Scale (HARS) questionnaire. The results showed that most heart failure patients experienced moderate anxiety (32.8%), with the majority experiencing mild anxiety (27.2%). These results indicate that anxiety is a common psychological problem experienced by patients and families during heart failure treatment. In conclusion, continuous psychological support and a holistic nursing approach are needed to help patients and families cope with anxiety and improve their quality of life during the treatment process. Keywords: Anxiety, Patient and Family Care, Heart Failure. ABSTRAK Gagal jantung merupakan penyakit kronis yang berdampak luas terhadap kondisi fisik dan psikologis pasien serta keluarga, termasuk menimbulkan kecemasan selama proses rawat inap. Penelitian ini bertujuan mengetahui gambaran tingkat kecemasan keluarga dan pasien gagal jantung di RS Daerah Surakarta. Jenis penelitian yang digunakan adalah deskriptif kuantitatif. Populasi penelitian mencakup pasien gagal jantung dan keluarganya, masing-masing sebanyak 290 orang, sehingga total sampel berjumlah 580 responden yang dipilih menggunakan teknik purposive sampling. Instrumen pengumpulan data menggunakan kuesioner Hamilton Anxiety Rating Scale (HARS). Hasil penelitian menunjukkan bahwa sebagian besar pasien gagal jantung mengalami tingkat kecemasan sedang (32,8%), mayoritas mengalami kecemasan sedang (27,2%). Hasil ini menunjukkan bahwa kecemasan merupakan masalah psikologis umum yang dialami pasien dan keluarga selama perawatan gagal jantung. Kesimpulannya, diperlukan dukungan psikologis berkelanjutan dan pendekatan keperawatan holistik untuk membantu pasien dan keluarga mengatasi kecemasan serta meningkatkan kualitas hidup selama proses perawatan. Keywords: Kecemasan, Perawatan Pasien dan Keluarga, Gagal Jantung.

Abstract

Heart failure is a condition of being unable to pump enough blood throughout the body, which can cause symptoms of fatigue and shortness of breath. These symptoms affect the physical activity of heart failure patients. Family support is one factor that can help influence the ability to engage in physical activity, especially in the daily activities of heart failure patients. This study aimed to determine the relationship between family support and physical activity in heart failure patients. This type of research uses quantitative research with a descriptive correlational design through a cross-sectional approach. It assesses family support using the PSS-Fa questionnaire and physical activity ability using the RAPA questionnaire. Sampling used a purposive sampling technique and determined the number of samples using the Slovin formula with 154 respondents. The results showed that 93 respondents (60.4%) received good family support. Optimal physical activity was 131 respondents (85.1%). The test results showed a significant p-value of 0.000 (p <0.05), which indicated a relationship between family support and physical activity ability.

Abstract

Pasien yang mengalami stroke sering menunjukkan gangguan fungsi motorik berupa kelemahan pada anggota gerak, khususnya bagian atas, yang biasanya disertai berkurangnya kekuatan otot. Studi kasus ini disusun untuk menggambarkan pelaksanaan latihan menggenggam bola karet pada pasien dengan stroke non-hemoragik. Menggunakan pendekatan studi kasus terhadap satu pasien stroke non-hemoragik yang mengalami kelemahan otot pada ekstremitas bagian atas. Latihan genggam bola karet dilakukan sebagai intervensi selama tiga hari berturut-turut dengan frekuensi dua kali sehari, dan durai selama 15 menit. Evaluasi kekuatan otot dilakukan menggunakan metode Manual Muscle Testing (MMT). Hasil studi kasus menunjukkan terdapat perubahan minimal pada kekuatan otot setelah intervensi, dengan skor kekuatan otot ekstremitas kiri atas dari skor awal 7 meningkat menjadi 8. Kesimpulan studi kasus ini menunjukkan adanya peningkatan kekuatan otot setelah pemberian latihan genggam bola karet. Meskipun demikian, temuan tersebut hanya menggambarkan respons pada satu pasien sehingga belum dapat digunakan untuk menyimpulkan efektivitas intervensi secara umum maupun dalam jangka pendek.

Abstract

Heart failure is a chronic disease characterized by a decline in heart function, causing physical symptoms such as shortness of breath, edema, and fatigue, as well as psychological symptoms such as anxiety and depression. It is more common in low-income countries and is closely related to psychosocial factors such as anxiety, depression, social support, and economic status, which affect the severity of symptoms and treatment adherence. Although mental health and social support can improve patients' quality of life, previous studies have not provided significant evidence regarding psychosocial determinants that exacerbate heart failure symptoms. Therefore, this study aims to identify psychosocial factors contributing to worsening symptoms in heart failure patients. This study used a descriptive quantitative design with purposive sampling of 317 respondents from a total population of 1,528 heart failure patients at the Sebelas Maret University Hospital Heart Clinic from February to April 2025, using the HADS (α=0,85 for anxiety; α=0,80 for depression), MS-PSS (α=0,85), and ESAS (α=0,78) instruments with logistic regression analysis. The results showed that the majority of respondents were aged 40–59 years, male, and had hypertension as a comorbidity, with 51.4% experiencing worsening symptoms, and anxiety was found to be the most significant determinant (p = 0.0001; OR = 3.043). These findings confirm that psychosocial factors play an essential role in the worsening of heart failure symptoms, with anxiety being the main factor that significantly increases risk compared to depression or low social support.

Abstract

Heart failure causes functional impairment and impacts patients' quality of life. A family-centered approach involving families as partners in care is an effective strategy for improving patients' quality of life. Objective: This study aims to identify the role of families in implementing family-centred care for heart failure patients. A quantitative descriptive design involving 317 families of heart failure patients. Purposive sampling based on predetermined criteria was applied in this study.  The Family Centered Care Assessment of Family (FCCA-F)  instrument was used to identify the role of families in decision-making, providing support, and family-to-family and peer support. Data analysis was performed using computer software. Analysis to present data used descriptive statistics. The results showed that the domain of the role of families in decision-making and the support provided by the immediate family were "relatively strong" in the majority of cases (57.1% and 63.1%). This indicates the active involvement of the immediate family in the care and support process for heart failure patients. However, most (70.3%) of the family-to-family and peer support domain  results were "weak." This finding indicates that initiatives to connect patients' families have not been fully realised. These findings show that the family's role in decision-making and supporting heart failure patients is decisive, improving quality of life and therapy compliance. However, the family-to-family and peer support domains are still weak, requiring the development of support group initiatives to optimise patient care and quality of life.

Abstract

Heart failure is a chronic condition that affects patients' quality of life and requires long-term management. Physical activity plays a crucial role in controlling the progression of heart failure. This study aimed to analyze the relationship between physical activity levels and disease duration in heart failure patients. This study used an observational study method with a cross-sectional design in 158 heart failure patients treated at a regional general hospital in Surakarta during August 2023–August 2024. Physical activity levels were measured using the International Physical Activity Questionnaire (IPAQ), and disease duration was obtained from patient medical records. The IPAQ instrument is a standardized instrument that underwent validity and reliability tests by Craig in 2003. The results of the validity test ranged from 0.14 to 0.53, with a median value of 0.30. The reliability test results ranged from 0.34 to 0.89, with a median value of 0.80. Spearman's correlation analysis showed a significant negative relationship between physical activity levels and disease duration (r = -0.951, p < 0.05), indicating that patients with higher levels of physical activity tended to have shorter disease duration. These results demonstrate the importance of increasing physical activity in the management of heart failure patients to improve prognosis.

Abstract

Asthma is a chronic health problem that significantly impacts physical and psychological conditions, especially during emergencies in the emergency department (ER). Anxiety in asthma patients can worsen clinical conditions and prolong recovery time if not promptly addressed. This study aims to assess the effectiveness of Spiritual Emotional Freedom Technique (SEFT) as a non-pharmacological technique to reduce anxiety in asthma patients in the emergency department of Soeradji Tirtonegoro Regional Hospital, Klaten. This study was conducted descriptively through a case study of one asthma patient with anxiety disorders, using the Hamilton Anxiety Rating Scale (HARS) instrument before and after the SEFT intervention. The SEFT implementation was structured for 15 minutes, which included the stages of setup techniques, tune-in, and tapping on the body's energy points. The results of the measurements and assessments were analyzed using a nursing care approach. The measurement results showed a decrease in the patient's anxiety score from 17 (moderate) to 14 (mild) after SEFT, accompanied by clinical improvements such as respiratory rate stability and increased psychological comfort. Family support during therapy plays an important role in accelerating the reduction of anxiety and patient recovery. Based on the results and discussion, it can be concluded that SEFT is effective as a psychological intervention to reduce anxiety in asthma patients in emergencies, and can be integrated with breathing education and oxygen saturation monitoring for optimal results.
