Abstract

Acute ischemic stroke is a leading cause of morbidity and mortality worldwide, significantly impacting patients' quality of life and healthcare systems. Understanding prognostic factors that predict clinical outcomes in acute ischemic stroke patients is crucial, especially with the rising prevalence of risk factors such as hypertension, diabetes, and dyslipidemia. Recent studies have focused on hematological biomarkers, particularly the platelet-to-white blood cell ratio (PWR), which reflects the interaction between immune system components and hemostasis. This literature review examines the prognostic value of PWR in acute ischemic stroke, correlating higher PWR with better clinical outcomes measured by the National Institute of Health Stroke Scale (NIHSS). Lower PWR values have been associated with increased mortality risk within 90 days post-stroke. The dynamics of PWR provide insights into the body's response to ischemia and inflammation, highlighting its potential as a valuable prognostic marker. While PWR shows promise as a prognostic tool, limitations such as individual variability in inflammatory response, timing of measurement, and lack of standardization must be considered. Further research is needed to explore PWR's potential across diverse populations and address existing limitations to enhance its clinical application in managing acute ischemic stroke.

Abstract

COVID-19 is an infectious disease caused by SARS-CoV-2. COVID-19 infect all groups, including pregnant women, resulting in a high positive rate pregnant women. COVID-19 diagnosis process can be supported by RT-PCR test. The RT-PCR test there is a CT-Value which is used to see the viral load which can affect the severity of COVID-19, including pregnant women who experience physiological changes in the body that can lead to maternal death. The purpose of this study was to analyze CT-Value RT-PCR on maternal mortality. This research is an observational analytic study with a cross-sectional approach. The sampling technique used was total sampling. The number of samples were 76 pregnant women with COVID-19 at Abdul Moeloek Regional General Hospital, Lampung Province period March 2020-October 2021. The number of pregnant women who died due to COVID-19 at Abdul Moeloek Hospital, Lampung Province period March 2020-October 2021, was 12 (15.8%) of all pregnant women who were confirmed COVID-19, namely 76 patients. In the statistical test, the p-value was 0.211 (p>0.05), it was concluded that the CT-Value of pregnant women with COVID-19 did not affect maternal mortality. Maternal mortality is not affected by CT-Value PCR pregnant women with confirmed COVID-19.

Abstract

Masalah kesehatan merupakan salah satu masalah di Indonesia yang sampai saat ini masih menjadi perhatian pemerintah. Salah satu masalah kesehatan adalah masalah akibat gangguan pada indra pendengaran yaitu telinga. Di antara sekian banyak gangguan pada telinga salah satunya adalah otitis media. Otitis media merupakan peradangan sebagian atau seluruh mukosa telinga tengah, tuba eustachius, antrum mastoid, dan sel-sel mastoid. Tujuan dari laporan kasus ini adalah menerapkan pelayanan dokter keluarga secara holistik dan komprehensif dengan mengidentifikasi faktor risiko, masalah klinis, serta penatalaksanaan pasien berbasis evidence-based medicine dengan pendekatan patient centered dan family approach. Studi ini merupakan studi kasus. Data primer diperoleh melalui autoanamnesis, pemeriksaan fisik dan kunjungan ke rumah. Data sekunder didapat dari rekam medis pasien. Evaluasi dilakukan dengan menggunakan kuesioner untuk mengukur tingkat pemahaman pasien terkait penyakitnya. Penilaian berdasarkan diagnosis holistik dari awal, proses, dan akhir studi secara kualitiatif dan kuantitatif. Hasil dari penelitian ini didapatkan Pasien Ny. S perempuan berusia 45 tahun, telah didiagnosis dengan Otitis Media Akut (OMA). Pada faktor internal pada kasus ketidaktahuan mengenai penyakit dan faktor risiko. Faktor eksternal kurangnya pengetahuan keluarga mengenai otitis media akut dan paparan asap rokok. Dilakukan intervensi farmakologis dan non farmakologis berupa edukasi dalam mencegah kambuhnya otitis media akut pada pasien.

Abstract

Faktor usia, jenis kelamin, riwayat keluarga, gaya hidup tidak sehat, dan kurangnya aktivitas fisik berkontribusi pada peningkatan penyakit seperti hiperkolesterolemia, hiperurecemia, dan hipertensi. WHO melaporkan bahwa 15-20% penduduk dunia mengalami hipertensi. Data Riskesdas menunjukkan prevalensi hiperurecemia pada perempuan 8,46% dan laki-laki 6,13%. Studi ini menerapkan prinsip pelayanan kedokteran keluarga dan melakukan penatalaksanaan secara holistik berdasarkan kerangka penyelesaian masalah pasien dengan pendekatan patient-centered, family focused, dan community oriented berbasis Evidence Based Medicine. Studi ini merupakan laporan kasus. Data primer diperoleh melalui anamnesis, pemeriksaan fisik dan kunjungan ke rumah untuk melengkapi data keluarga, data psikososial dan lingkungan. Data sekunder didapat dari rekam medis pasien. Penilaian berdasarkan diagnosis holistik dari awal, proses, dan akhir studi secara kualitiatif dan kuantitatif. Pada kasus ini telah dilakukan diagnosis dan tata laksana holistik sesuai dengan teori dan jurnal terkait serta dilakukan intervensi dengan menggunakan media poster. Pada evaluasi didapatkan pemahaman yang lebih baik terkait penyakit yang diderita serta perubahan perilaku yang berdampak pada keberhasilan terapi. Penilaian pemahaman dilakukan dengan menggunakan Post-test dan didapatkan hasil peningkatan pengetahuan sebesar 40 poin. Penatalaksanaan secara holistic dapat meningkatkan pengetahuan, sikap, perilaku pasien dan keluarga dalam mencegah komplikasi dan meningkatkan kualitas hidup.

Abstract

Water is an essential necessity for humans on Earth. Its role is crucial for daily activities, including household needs, industry, offices, and food production. In Indonesia, each person is estimated to require approximately 30–60 liters of water per day, whereas in developed countries, the demand per person ranges between 60–120 liters per day. The feasibility of water usage is regulated based on quality characteristics, referred to as water quality. One of the key indicators related to household water quality is biological parameters, particularly concerning the presence of coliform bacteria and Escherichia coli. These bacteria have a fecal-oral entry route, meaning that if consumed in excessive amounts, they can cause gastrointestinal diseases, primarily diarrhea. This study aims to examine the impact of coliform and Escherichia coli contamination in household water sources, leading to diarrhea cases. The research method used is a literature review and analysis. The findings indicate that the presence of Escherichia coli in household water sources, especially in areas with poor sanitation, poses a high health risk, particularly increasing the likelihood of diarrhea. Several factors contribute to this contamination, including the proximity of wells to pollution sources, inadequate waste management, and improper well construction that does not meet safety standards. These conditions suggest that environmental sanitation and the distance between water sources and potential contaminants play a significant role in water quality and public health. Through this study, it is expected that the community will be more aware of the importance of maintaining clean water used for household needs to prevent diarrhea-related diseases.

Abstract

Dysmenorrhea is pain or cramps felt in the lower abdomen before or during menstruation. Dysmenorrhea is one of the most common problems experienced by adolescents and adult women. Dysmenorrhea is often accompanied by other symptoms such as dizziness, fatigue, sweating, back pain, headache, nausea, vomiting, and diarrhea. There are 2 types of dysmenorrhea, namely primary dysmenorrhea and secondary dysmenorrhea. Dysmenorrhea has a negative impact on women's quality of life. Pharmacological management has side effects of treatment, so there is a need for safer and more effective non-pharmacological management to reduce the intensity of dysmenorrhea pain. Non-pharmacological therapy consists of various non-invasive or minimally invasive treatments. This study uses a literature observation method or literature review, where the main sources of scientific articles come from national and international journals. The articles used in this study were articles published within a period of 5 years, namely 2020-2025 through the electronic databases PubMed and Google Scholar which were selected based on exclusion criteria and inclusion criteria. The purpose of this literature review is to determine non-pharmacological management of dysmenorrhea. The results of the literature review found 21 articles showing that there are several non-pharmacological therapies that are safe and effective in reducing the intensity of pain felt by dysmenorrhea sufferers such as Transcutaneous electrical Nerve Stimulation (TENS), music therapy, acupuncture, acupressure, warm compresses, exercise or aerobic exercise, and aromatherapy.

Abstract

Allergic contact dermatitis is a skin inflammation caused by exposure to antigens that causes a type IV hypersensitivity reaction and can be experienced by everyone from all ages, races, and genders. The definitive treatment of the disease is to find and avoid the allergen that causes dermatitis. This is a case report study. Data were obtained from anamnesis, physical examination, and patient’s medical records.  Mrs. P, 65 years old, came to the Kebon Jahe Community Health Center with complaints of red spots appearing with itching between the fingers of the patient’s right hand since 7 days ago since patient was wearing a metal ring that the patient had used for the first time. The patient was diagnosed with allergic dermatitis based on the history and physical examination. Non-medical and medical interventions were carried out on patients and their families with education regarding the patient’s illness during three home visits. Holistic management with a family doctor approach has been carried out for Mrs. P, 65 years old with allergic contact dermatitis, which is adjusted based on initial holistic diagnostics.

Abstract

Tonsilitis merupakan peradangan tonsil palatina yang merupakan bagian dari cincin Waldeyer. Penyebaran infeksi ini dapat terjadi melalui udara dan tangan. Tonsilitis telah menjadi salah satu penyebab penurunan prestasi belajar dan ketidakhadiran anak di sekolah. Penderita tonsilitis juga merupakan pasien yang sering datang pada praktek dokter ahli bagian telinga hidung tenggorok-bedah kepala dan leher (THT-KL), dokter anak, maupun tempat pelayanan kesehatan lainnya.. Studi ini merupakan laporan kasus. Data primer diperoleh melalui anamnesis (autoanamnesis dan alloanamnesis  dari  keluarga  dan  pasien),  pemeriksaan  fisik,  dan  kunjungan  ke  rumah. Penilaian dilakukan  secara kualitiatif dan kuantitatif. Pasien An. C, usia 12 tahun dengan diagnosis tonsilitis kronik. Aspek resiko internal berupa usia anak, pengetahuan pasien, pola hidup bersih dan sehat, higiene gigi dan mulut, serta pola makan yang kurang baik. Aspek resiko eksternal yang mempengaruhi kondisi pasien berupa kurangnya pengawasan keluarga tentang konsumsi makanan pasien, kurang nya pengetahuan mengenai penyakit yang diderita serta lambatnya pengambilan keputusan untuk tindakan lanjutan. Selanjutnya penatalaksanaan secara holistik terhadap pasien dan keluarga melalui media intervensi poster. Penatalaksaan pasien tonsilitis kronik secara holistik dan komprehensif agar dapat ditatalaksana dengan tepat dan merubah perilaku.

Abstract

The world's population has increased many times over the last 200 years, so that the number is now recorded at more than seven billion people. The size of the population is not evenly distributed, therefore it is necessary to have something that can prevent population spikes. Family Planning Policy aims to control population growth through efforts to reduce the birth rate. Many women have difficulty in choosing the type of contraception. This is not only due to the limited availability of methods, but also due to ignorance about the requirements and safety of these contraceptive methods. The purpose of this study was to analyze the factors that influence women to choose the long-term contraceptive method. The design of this study used an analytic observational type with a cross sectional approach. The sampling technique is Consecutive sampling. The number of samples in this study were 60 women of childbearing age who used the long-term contraceptive method and 60 women of childbearing age who used the long-term contraceptive method.Results: The results showed that Age had a P-Value of 0.714, Education had a P-Value of 0.004, Employment had a P-Value of 0.711, Knowledge had a P-Value of 0.015, Number of children had a P-Value of 0.030, Attitudes had a P-Value of 0.037, Role Health Workers have a P-Value of 0.027, Husband's Support has a P-Value of 0.031, Social Culture has a P-Value of 0.032. The conclusion in this study is that there is an influence of Education, Knowledge, Number of Children, Attitudes, Role of Health Workers, Husband's Support and Socio-cultural Affairs on the selection of the long-term contraceptive method, while Age and Occupation have no influence in choosing the long-term contraceptive method.

Abstract

Musculoskeletal disorders (MSDs) are among the most common occupational health problems faced by workers in the agricultural sector, including sugarcane farmers in Indonesia. Excessive physical activity and performed without adequate ergonomic support make this group particularly vulnerable to musculoskeletal injuries. This study aims to synthesize determinant of MSDs among sugarcane farmers and to recommend targeted ergonomic strategies suitable for tropical agricultural environments through a literature review of scientific articles from relevant database. The findings indicate that MSDs are primarily influenced by repetitive work patterns, poor working posture, the use of non-ergonomic traditional tools, and adverse environmental conditions such as high temperatures and humidity. Additionally, psychosocial stress and limited access to ergonomic education and health facilities further exacerbate the condition. The review highlights the urgent need for comprehensive preventive strategies, including ergonomic interventions, occupational health education, improvements in working conditions, and psychosocial support. These efforts are essential to reduce the incidence of MSDs and enhance the occupational well-being of sugarcane farmers. The findings can guide policymakers in designing ergonomic-based agricultural health programs, especially among sugarcane farmers.
