Abstract

Stunting merupakan kondisi kurang gizi kronis disertai dengan komplikasi penyakit. Prevalensi stunting anak balita di Indonesia 29,9% dan provinsi Lampung 27,4% serta terbanyak di pedesaan. Kondisi prevalensi stunting di Lampung pada posisi di bawah angka nasional tetapi masih di atas 20% (target WHO kurang dari 20%). Kabupaten Tanggamus angka sunting turun menjadi 3,3%, namun demikian terdapat 25 desa yang mempunyai angka prevalensi stunting di atas 10%. Desa dengan lahan pekarangan yang rata rata cukup luas dapat dimanfaatkan secara maksimal sebagai lahan ekonomis dan perlu penanganan, sehingga dapat menjadi program percepatan pencegahan stunting. Tujuan penelitian ini menjelaskan pengaruh pemanfaatan lahan pekarangan rumah terhadap kejadian stunting di Kabupaten Tanggamus.Motode menggunakan disain case control dengan lokasi di 16 desa wilayah kerja kabupaten Tanggamus sebanyak 200 responden, menggunakan analisa regresi logistik ganda.Terdapat 2 variabel dari 12 variabel secara simultan berpengaruh terhadap kejadian stunting yaitu motivasi kepala keluarga dalam memanfaatkan lahan pekarangan rumah, pengalaman kepala keluarga dalam memanfaatkan lahan pekarangan rumah.  Motivasi pengalaman memanfaatkan lahan pekarangan yang kurang baik secara tidak langsung berpengaruh terhadap kejadian stunting di Kabupaten Tanggamus sebesar 27,6% dan 73,4% dipengaruhi oleh faktor lain.

Abstract

Indonesia memiliki permasalahan kesehatan berupa anemia yang paling banyak diderita oleh wanita usia produktif. Kejadian anemia pada wanita usia produktif setiap tahunya mengalami peningkatan. Salah satu penyebab anemia adalah kekurangan zat besi pada tubuh yang berpengaruh terhadap produksi sel darah merah terganggu. Kegiatan pengabdian ini berguna untuk meningkatkan pengetahuan dan keterampilan dalam pengelolaan anemia pada wanita usia produktif di tingkat masyarakat. Kegiatan ini terdiri dari penyuluhan dan pemberian informasi kepada wanita usia subur terkait pentingnya tablet tambah darah sebagai pencegahan anemia

Abstract

Noise Induced Hearing Loss (NIHL) is a form of hearing loss caused by noise caused by a work environment that is exposed for a long time. Noise is sound that is produced with an uncertain intensity and volume. Noise sources or sound sources that exceed 85 dB have an impact on the damage to the inner ear meaning receptors. Workers who work in an environment with a noise source above 85dB and with an exposure time of more than 8 hours a day without any rotation (shift) will greatly impact NIHL permanently for a long period of time if they do not use adequate ear protection equipment (APT). The research that has been carried out by Fitriyani et al in 2016, involved 10 respondents who work in the weaving department of a textile company located in Salatiga. The results obtained from 10 respondents who are workers in the weaving department, 70% of workers do not use ear protection in the form of ear plugs while working, 30% of respondents wear ear protection in the form of ear plugs when working, some workers reported buzzing ears due to noise generated in work environments with noise levels above 99 dB. The high prevalence of hearing loss in workers is due to the low wearing of PPE while working in a noisy work environment. Based on the studies that have been conducted, the low use of ear protection is a factor in the high number of NIHL among workers.

Abstract

The elderly group is included in the vulnerable population because of the process of decreasing the ability to avoid disease and exposure to risk factors for chronic diseases such as Diabetes Mellitus (DM). DM can increase the risk of tuberculosis (TB) infection three times greater than the normal population, and can also increase the risk of reactivation of TB and latent TB. The study conducted was a case report. This case report aims to implement evidence-based medicine-based family doctor services to patients by identifying risk factors, clinical problems, and management with a patient-centered approach and a family approach. Primary data were obtained through anamnesis and physical examination by making home visits, filling in family folders, and filling in patient files.

Abstract

Sick Building Syndrome (SBS) is a collection of symptoms that can be experienced by someone who works in a building. A person is declared to have Sick Building Syndrome (SBS) if they have complaints of at least two or three symptoms of the total symptoms (eye irritation such as sore, red or watery eyes, throat irritation, pain when swallowing, itchy throat, sneezing, dry cough, shortness of breath, heaviness in chest, dry, red and itchy skin, nausea, fatigue, difficulty concentrating, drowsiness, dizziness). SBS can be enforced if the various complaints that exist are felt by around 20% -50% of users of a building, and these complaints disappear when workers leave the building. This study was an observational analytic study with a cross sectional design. The sampling technique used was total sampling with a minimum sample size of 63 people. Data were collected using the The Miljo-Medicin 040 Questionnaires (MM040EA) questionnaire for individual characteristics and symptoms of SBS and direct measurements for physical factors. Data analysis used chi square test and logistic regression test. Based on the data obtained the p value of each variable which states a relationship, namely gender (0.102), age (0.028), length of work (0.002), smoking habits (0.006), psychosocial conditions (0.007), air flow rate (0.029 ), lighting (0.042). The results of the multivariate analysis test showed that the length of work variable was the most dominant factor on SBS with a p value (0.015). The variables that affect the incidence of SBS are age, length of work, smoking habits, psychosocial conditions, air flow rate, lighting and the most dominant variable is length of work.

Abstract

Abstrak: Penatalaksanaan Pasien Wanita Usia 51 Tahun Dengan Ulkus Diabetikum Melalui Pendekatan Dokter Keluarga Di Wilayah Puskesmas Sukamaju. Diabetes Melitus adalah penyakit kelainan metabolik yang dikarakteristikkan dengan hiperglikemia kronis. Ulkus diabetikum adalah keadaan adanya ulkus, infeksi, dan atau kerusakan dari jaringan, yang berhubungan dengan kelainan neurologi dan penyakit pembuluh darah perifer pada ekstremitas bawah. Pendekatan keluarga dalam penatalaksanaan ulkus diabetikum dan Diabetes Melitus membantu mengidentifikasi faktor-faktor yang berpengaruh baik secara klinis, personal, dan psikososial keluarga sehingga penatalaksanaan akan lebih komprehensif. Tujuan penelitian ini untuk menerapkan pelayanan dokter keluarga berbasis evidence-based medicine pada pasien dengan mengidentifikasi faktor risiko, masalah klinis, serta penatalaksanaan pasien berdasarkan kerangka penyelesaian masalah pasien dengan pendekatan patient centered dan family approach. Studi ini merupakan laporan kasus. Data primer diperoleh melalui anamnesis (autoanamnesis), pemeriksaan fisik, pemeriksaan penujang dan lingkungan fisik. Penilaian berdasarkan diagnosis holistik dari awal, proses, dan akhir studi secara kualitiatif dan kuantitatif. Hasil penelitian didapatkan P Diagnosis dan penatalaksaan pada pasien ini dilakukan secara holistik dan komprehensif, patient center, family appropried dengan pengobatan DM secara teratur sesuai EBM diperlukan agar dapat mengontrol gula darah dengan merubah perilaku pasien sehingga dapat mencegah timbulnya komplikasi lain.

Abstract

Gout arthritis is a progressive chronic inflammatory disease due to deposition of monosodium urate (MSU) crystals in the joints due to long-lasting hyperuricemia with several associated risk factors including excessive consumption of purine compounds and lack of physical activity. Gout arthritis needs to be managed comprehensively and holistically through the role of the family doctor in order to improve the patient's quality of life and prevent complications that can occur. Apply the principles of family medicine services to patients based on the problems found and carry out holistic and comprehensive management using the principles of patient-centered and family approach based on Evidence Based Medicine. This study is a case report. Primary data were obtained from anamnesis, physical examination, and home visits to assess the physical environment. Secondary data was obtained from the patient's medical record. Assessment based on holistic diagnostics at the beginning, process, and end of the study. Mrs. M, 67 years old, came with complaints of joint pain in both knees since 1 week ago. The patient was diagnosed with gout arthritis. In this case, diagnosis and treatment have been carried out according to the theory and related journals. Management has been carried out with the approach of family doctor Ny. M 67 years old with Gout arthritis adjusted based on initial holistic diagnostics. The interventions carried out have increased patient knowledge and improved the healthy lifestyle of patients and their families as indicated by improvements in the final holistic diagnostic.

Abstract

Neurodermatitis Circumscripta is a chronic inflammatory skin condition that often occurs in individuals aged 30 - 50 years, characterized by itching and thickening of the skin due to repeated friction. Applying the principles of family medicine and implementing holistic management based on the patient-centered, family-focused, and community oriented approach grounded in Evidence-Based Medicine. This study is a case report utilizing primary data obtained through anamnesis, physical examination, and home visits to get family, psychosocial, and enviromental data. Secondary data were obtained from the patient’s medical records. Patient Mrs. I, 50 years old, came with a complaint of itching on the back of the right leg since the last 3 months, the itching increased when the patient was resting or at night. The patient often scratched it, causing thickening and darkening of the skin. The patient was diagnosed with neurodermatitis circumscripta. Holistic management was performed with a family physician-designed approach for Mrs I, 50 years old, with neurodermatitis based on the initial holistic diagnosis. The intervention has improved the patient’s knowledge and changed some behaviours of the patient and her family.

Abstract

Nurses are often interacted with patients for a long time, thus nurses have a high enough risk factor. Therefore, Nurses need Personal Protective Equipment (PPE) to protect themselves from various potential hazards in the workplace and addition to prevent work accidents. This study aims to determine the relationship between knowledge, attitudes, and behavior of using PPE in inpatient nurses with the incidence of work accidents in RSUDAM Lampung Province. This study used an observational method with a cross sectional approach with a proportional random sampling technique. The research subjects were 158 RSUDAM inpatient installation nurses and the measuring instrument was a questionnaire. Data analysis was performed using Fisher's exact test. The results of this study showed a good level of knowledge as much as 81.6%, 50.6% positive attitude, 70.3% good behavior in the use of PPE. Meanwhile, the category of low incidence of work accidents was 93.7%. The results of statistical tests showed that there was a significant relationship between knowledge (p = 0.001), attitude (p = 0.001), and behavior (p = 0.001) using PPE with the incidence of work accidents. There is a relationship between knowledge, attitudes, and behavior of using PPE on inpatient installation nurses with work accident at RSUDAM Lampung Province.

Abstract

Migraine is a neurovascular disease which is one of the main factors of disability in adults under 50 years. Migraines are generally characterized by headache attacks that are unilateral, recurrent, hereditary and multifactorial. The prevalence of migraine in the world reaches 10-14% with the highest incidence in North America, Central America and South America. In Indonesia alone, the prevalence of migraine has reached 3.5 million people with the highest coming from individuals aged 15-24 years. Until now, the exact cause of migraines is not known. However, there are many risk factors that can influence the occurrence of migraines, including work-related factors. Occupational factors related to migraines include work stress, environmental factors, and excessive use of electronic screens. Work-related stress can be caused by workload, time pressure, and relationships with coworkers. Work environmental factors can include excessive lighting, noise, changes in temperature and air pressure, and strong odors. Meanwhile, the use of electronic screens can influence migraines because they are related to the use of blue light, computer flashes, light reflections, and prolonged exposure to light. The diagnosis of migraine can be made through the results of the history and physical examination, and if necessary, supporting examinations can also be carried out to rule out other secondary causes that may influence migraine.
