Abstract

Cakupan pemberian air susu ibu (ASI) eksklusif di Kota Pagar Alam, tahun 2011 sekitar 43% tergolong rendah. Sebaliknya, pemberian susu formula meningkat tiga kali lipat dari 10,3% menjadi 32,5%. Iklan susu formula telah menyentuh bidan swasta dan puskesmas melalui pendekatan produsen susu formula dan pemberian susu formula secara gratis kepada ibu menyusui. Penelitian yang bertujuan mengetahui determinan kegagalan praktik pemberian ASI eksklusif di Kota Pagar Alam Provinsi Sumatera Selatan ini menggunakan desain studi unmatching kasus kontrol. Populasi adalah seluruh ibu yang mempunyai bayi berusia 7-12 bulan. Penarikan sampel dilakukan dengan metode proportional random sampling. Variabel terikat praktik adalah pemberian ASI eksklusif, variabel bebas adalah pemberian sampel susu formula. Ibu yang mendapat sampel susu formula dan yang tidak mendapat dukungan tenaga kesehatan berisiko 3,67 dan 4,2 kali lebih besar untuk tidak memberikan ASI eksklusif. The coverage of exclusive breastfeeding in the City of Pagar Alam in 2011 was by 43%. Advertising of infant formula has reached privately practicing midwives or health centers. The approach from infant formula manufacturers to midwives in health centers is by providing free milk formula to nursing mothers to be distributed under the pretext of promotion. The objective of this study is to analyze the determinants of exclusive breastfeeding practice failures in the City of Pagar Alam of South Sumatra Province. The population study with an unmatched case-control design was conducted in the City of Pagar Alam. The population was all breastfeeding mothers who had babies in the city of Pagar Alam of South Sumatra Province. The research subjects are breastfeeding mothers who had babies aged 7 - 12 months who selected with proportional random sampling method. The variables of the study included the dependent variable, i.e, the practice of exclusive breastfeeding, the independent variable, i.e, promotion of free milk formula samples. The risk of not exclusively breast feeding amang mothers who had accepted formula milk samples and who absence of support posed by health workers is 3.67 and 4.20 times higher the mothers who had not accepted the formula milk sample and who absence of support posed by heath worker.

Abstract

Perilaku seksual pranikah yang tinggi pada remaja disebabkan oleh faktor personal seperti pengetahuan kesehatan seksual, Infeksi Menular Seksual (IMS) dan HIV / AIDS, sikap terhadap seksualitas, harga diri dan efikasi diri. Penelitian ini bertujuan untuk mengetahui pengaruh faktor personal seperti pengetahuan tentang kesehatan seksual, IMS dan HIV / AIDS, sikap, harga diri dan efikasi diri terhadap perilaku seksual pranikah pada remaja di Kota Kendari yang diukur melalui kuesioner dan wawancara mendalam. Desain penelitian adalah studi potong lintang dengan jumlah sampel 200 remaja yang berasal dari empat sekolah negeri di Kota Kendari mulai dari Agustus sampai dengan Oktober 2014. Analisis regresi logistik menunjukan bahwa remaja yang memiliki pengetahuan yang rendah tentang kesehatan seksual, IMS dan HIV / AIDS berisiko sebesar 4,28 kali, sikap permisif terhadap seksualitas berisiko 5 kali, harga diri rendah berisiko sebesar 3,3 kali dan efikasi diri rendah sebesar 2,5 kali untuk melakukan perilaku seksual pranikah berisiko. Analisis kualitatif menunjukan variabel sikap sebagai faktor yang memberikan risiko terbesar di dalam berperilaku seksual pranikah yang berisiko pada remaja. High premarital sexual behavior among teenagers are caused by personal factors, such as health sexual knowledge, Sexually Transmitted Infections (STIs) and HIV / AIDS, attitudes towards sexuality, self-esteem and self-efficacy. This study aimed to find out the influence of personal factor to premarital sexual behavior among teenagers in the Kendari City as assessed through questionnaires and in-depth interviews.The study design was cross-sectional study with a sample of 200 adolescents from four public schools in Kendari City from August to October 2015. Logistic regression analysis showed teenagers having lack of knowledge of sexual health, STIs and HIV / AIDS had 4.28 times risk having permissive attitude toward sexuality had 5 times risk, having low self-esteem had 3.3 times risk and having low self-efficacy had 2.5 times to perform premarital sexual behavior. Qualitative analysis showed that attitude variable was the factor giving the biggest risk in risky premarital sexual behavior among teenagers.

Abstract

Objectives: This study aimed to examine the relationship between family structure and parenting style and their association with adolescent pregnancy in Surabaya. Materials and Methods: A mixed-methods approach was employed, combining a case–control design with a case study. Data were collected through questionnaires and in-depth interviews. The study included 46 adolescents (23 cases and 23 controls) selected using purposive sampling, along with six adolescents as key qualitative participants. Quantitative data were analyzed using univariate, bivariate (Chi-square), and multivariate (multiple logistic regression) analyses with a significance level of p = 0.05 and a 95% confidence interval (CI). Qualitative data were analyzed thematically based on in-depth interviews. Results: Family structure was not statistically significant (OR = 10.53; 95% CI = 0.657–168.93), though it showed social relevance. Parenting style was also not significantly associated with adolescent pregnancy (OR = 1.191; 95% CI = 0.373–3.806). However, respondent education was significantly associated with adolescent pregnancy (OR = 559.76; 95% CI = 3.608–23,026.4). No significant associations were found for parental education, employment status, or residential setting. Parenting style appeared to be influenced by the quality of communication between parents and adolescents. Conclusions: Low educational attainment was identified as a major risk factor for adolescent pregnancy. Additionally, adolescents from extended families or single-parent households appeared more likely to experience teenage pregnancy.

Abstract

Purpose: Efforts to reduce low birth weight (LBW) cases can be made by regularly conducting antenatal care (ANC) during pregnancy. The aim is to improve the quality of ANCs and the quality of mothers, fetuses, and newborns associated with ANC. The World Health Organization (WHO) recommends reducing perinatal mortality and increasing the ability of care of pregnant women to make at least eight visits during pregnancy. This study aims to prove WHO's recommendation of at least 8 times the effect of ANC on LBW events in Indonesia. Methods: This study used a retrospective cohort and observational study design. The sample in this study were all women of childbearing age who were respondents to the 2017 IDHS. Data testing was performed using univariable, bivariable, and multivariable analysis. The analysis was performed by logistic regression. Results: ANC frequency was 8 times proven to reduce LBW risk; the lower the ANC contact, the greater the LBW risk. The incidence of LBW is lower in mothers who get quality ANC services. Exposure to cigarette smoke, the area of ​​residence in Sulawesi, NTT, and NTB, and the distance of pregnancy affect the incidence of LBW without changing the relationship of the ANC frequency with LBW. Conclusions: Mothers who regularly perform pregnancy checks at least 8 times by the standards become protection for LBW.

Abstract

Purpose: The purpose of this study is to determine the relationship between coping strategies and the quality of life of breast cancer patients undergoing chemotherapy at RSUD Prof. Dr. W.Z. Johannes Kupang, East Nusa Tenggara. Methods: This study used a cross-sectional design involving 59 breast cancer patients undergoing chemotherapy as respondents. The chi-square test was used for bivariate analysis, and logistic regression was used for multivariate analysis. Results: There is a relationship between emotion-focused coping strategies (p=0.000; PR=4.7; 95% CI: 1.87-11.9) and problem-focused coping strategies (p=0.000; PR=5.0; 95% CI: 2.0-12.7) with the quality of life of breast cancer patients undergoing chemotherapy at RSUD Prof. Dr. W.Z. Johannes Kupang, East Nusa Tenggara. After considering the variables of education, occupation, family income, and cancer stage, there is a relationship with quality of life, whereas age and marital status do not have a relationship with the quality of life of breast cancer patients undergoing chemotherapy. Conclusion: The quality of life of breast cancer patients undergoing chemotherapy at RSUD Prof. Dr. W.Z. Johannes Kupang, East Nusa Tenggara can be improved by maximizing coping strategies.

Abstract

Purpose: This study aims to determine the association of perceptions with participation behavior in cervical cancer screening using VIA in Gunungkidul District. Methods: This research used a 1:1 unmatched case-control study design. The research sample consisted of 132 respondents (66 cases and 66 controls) selected by simple random sampling. Data collection was conducted through structured questionnaires administered during interviews from February to May 2019. Data analysis was done by chi-square test and multiple logistic regression. Results: Most respondents had an average age of 31-40 years, 47% had senior high school, 72% were housewives, and 78% had ≤ 2 children. Bivariate analysis results showed that perceptions (OR=19.74), knowledge (OR=3.09), and accessibility of health care (OR=7.87) were associated with participation in cervical cancer screening. Multivariable analysis results showed that the husband’s support was a significant confounder in the association between perceptions and cervical cancer screening participation. Conclusion: A good perception of cervical cancer and VIA screening will increase the chances of encouraging VIA screening participation behavior in women. Effective health education and community engagement by health workers, supported by local cadres, are essential to building accurate perceptions of cervical cancer and VIA screening. These efforts are expected to increase VIA screening coverage in the Gunungkidul District.

Abstract

Purpose: This study aims to analyze cigarette expenditures among poor households and examine whether economic status, chronic diseases, health insurance ownership, and demographic factors influence these expenditures. Methods: This study employs a quantitative research design with a cross-sectional approach, utilizing secondary data from the Indonesia Family Life Survey (IFLS) conducted in 2007 and 2014. The criteria for the respondents are poor families, smokers, and age > 15 years old, with a sample of 1,243 respondents (2007) and 1,549 respondents (2014). Results: In 2007, cigarette expenditure was influenced by low economic status (OR = 3.34), interest in health insurance (OR = 1.10), age, sex, and place of residence, while a history of chronic disease and education level had no effect. In 2014, economic status and health-related factors no longer had a significant impact; however, age (OR = 2.45) and gender (OR = 0.54) remained influential, with no effect observed from education or residence. Conclusion: Economic status and demographic factors are key determinants of high cigarette expenditures among poor families in Indonesia. Monthly spending on cigarettes remains relatively high, and many poor households still lack health insurance. The government is expected to strengthen awareness and policies related to smoking, particularly those concerning health and health insurance for the poor.

Abstract

Ibu yang melahirkan dengan seksio sesarea memiliki hambatandalam proses menyusui lebih besar dibandingkan dengan lahirnormal karena ibu pasca bedah sesar masih membutuhkanpengawasan, kondisi tubuh masih lemah, rasa nyeri, kecemasan danhambatan mobilisasi. Pijat punggung merupakan salah satu intervensiyang diharapkan dapat membantu ibu post partum meningkatkanproduksi ASI sehingga mampu mendukung pemberian ASI bagi bayibaru lahir. Penelitian ini bertujuan untuk Mengetahui efek pijatpunggung sebagai salah satu cara dalam meningkatkan produksi ASIpada ibu pasca bedah sesar. Metode Penelitian yang digunakanadalah Randomized Controlled Trial. Teknik Pengambilan sampeldengan simple random sampling dengan jumlah sampel sebanyak 27responden pada masing-masing kelompok (intervensi dan kontrol).Analisis bivariat menggunakan chi square dengan tingkat kemaknaanp<0,05, analisis multivariat menggunakan regresi logistik untukmengidentifikasi variabel perancu terhadap produksi ASI pasca bedahsesar. Hasil Penelitian menunjukan adanya perbedaan produksi ASIantara kelompok intervensi dan kelompok kontrol dengan RR sebesar1,9 (95%CI: 1,097-3,291). Paritas dan frekuensi menyusui mempunyaihubungan yang bermakna dengan produksi ASI pada ibu pasca bedahsesar. Usia ibu, obesitas, waktu rawat gabung tidak memilikihubungan yang bermakna dengan produksi ASI pada ibu pasca bedahsesar.

Abstract

The direct causes of stunting include inadequate nutritional intake and infectious diseases. Lack of awareness and knowledge of women of childbearing age, especially pregnant women, regarding the importance of nutritious food and types of nutritious food during pregnancy, can contribute to stunting. The solution offered is the provision of health education, this activity will be aligned with the government. Activities were carried out synergistically in one of the villages, namely Bange Village with the participation of the Community Health Center, District Head, Village Head, and nutrition practitioners. The objective of this study is to increase the knowledge and understanding of pregnant women about the importance of consuming nutritious food during pregnancy to prevent stunting in their babies. Beside it, to conduct the health promotion at the community base by providing health education about the importance of nutritious food and nutritious foods for pregnant women and their processing. Before the activity was carried out, a pre-test was carried out and after that a post-test was given to evaluate the results of the health education carried out. Evaluation of activities by assessing the comparison of pre-test scores and post-test scores. After the health education activities are completed, pregnant women are given additional food packages. Health education about the importance of nutritious food and nutritious foods for pregnant women and their processing is proven to increase knowledge.

Abstract

Deteksi dini resiko terjadinya kanker serviks pada wanita usia subur merupakan salah satu cara dalam menurunkan angka kejadian penderita kanker serviks. Inspeksi Visual Asam Asetat (IVA) merupakan salah satu jenis pemeriksaan skrining dalam deteksi dini kanker servik namun menurut laporan dari Dinas Kesehatan Kabupaten Ketapang cakupan program IVA tahun 2022 menunjukkan tidak terpenuhinya angka cakupan seperti yang telah direncanakan yaitu hanya 1,7 %. Penelitian ini bertujuan untuk mengeksplorasi dan menganalisis dampak pemberdayaan wanita usia subur dalam deteksi kanker serviks melalui pemeriksaan IVA. Penelitian ini menggunakan desain penelitian kualitatif dengan pendekatan partisipatif atau (Participatory Action Research) melalui empat tahapan, yaitu: Perencanaan, tindakan, pengamatan, refleksi. Partisipan dalam penelitian ini yaitu wanita usia subur, kader, Stakeholders yang relevan, pemegang program kesehatan reproduksi dan petugas kesehatan di wilayah kerja Puskesmas Tuan-Tuan Kabupaten Ketapang dengan menggunakan tehnik purposive sampling. Pengumpulan data dilakukan dengan wawancara tatap muka dan focus group discussion. Masalah utama yang perlu diselesaikan yaitu rendahnya partisipasi wanita usia subur dalam pemeriksaan IVA selanjutnya disepakati kegiatan sosialisasi dan edukasi tentang kanker serviks dan pemeriksaan IVA sebagai alternatif penyelesaian masalah. Hasil evaluasi terjadi peningkatan kunjungan 4 bulan terakhir dan perlu dilakukan sosialisasi kembali terkait pemeriksaan IVA dengan berbagai macam metode.
