Abstract

Hypertension is one of the non-communicable diseases whose prevalence continues to increase and remains a major risk factor for cardiovascular disease. The management of hypertension does not rely solely on pharmacological therapy but also requires non-pharmacological interventions, such as adopting a healthy diet rich in fruits and vegetables and low in salt. However, public knowledge regarding types of foods that effectively help lower blood pressure remains limited, particularly in rural areas. This community service program (PKM) aimed to provide education on healthy foods that can reduce blood pressure as a form of non-pharmacological therapy for patients with hypertension in the community. The activity employed the Preparation, Action, Reflection, and Evaluation (PARE) approach and was conducted in Melinggih Village, Gianyar Regency, Bali Province, involving 29 participants with hypertension. The program began with a pre-test, followed by health education sessions supported by visual educational media, consultations, discussions, and question-and-answer sessions, and concluded with a post-test evaluation. The results showed that the proportion of participants with good knowledge increased from 34.5% before the intervention to 100% after the implementation of the program. The Wilcoxon test results indicated Z = -4.300; p = 0.001, demonstrating a statistically significant difference in participants’ knowledge levels before and after the activity. The success of the program was supported by lecture methods, consultations, and interactive discussions conducted bilingually (Indonesian and Balinese), as well as the use of colorful leaflets illustrating vegetables and fruits beneficial in lowering blood pressure. Thus, this community-based educational intervention proved effective in improving community knowledge and awareness regarding healthy food choices for hypertension management.

Abstract

Gagal ginjal kronik (GGK) stadium akhir merupakan kondisi kronis progresif yang memerlukan terapi pengganti ginjal jangka panjang dan menimbulkan beban multidimensional bagi pasien serta keluarga. Selain keluhan fisik, pasien sering mengalami tekanan psikologis, keterbatasan sosial, dan penurunan kualitas hidup. Namun, edukasi terstruktur mengenai perawatan paliatif dan perawatan jangka panjang masih terbatas, sementara pelayanan kesehatan cenderung berfokus pada aspek teknis dan kuratif hemodialisis. Kegiatan pengabdian kepada masyarakat ini bertujuan meningkatkan pengetahuan pasien dan keluarga mengenai GGK stadium akhir, perawatan paliatif, perawatan jangka panjang, serta strategi pengelolaan kecemasan melalui program edukasi terintegrasi. Kegiatan diikuti oleh 30 peserta yang terdiri dari pasien GGK stadium akhir dan anggota keluarga pendamping dengan latar belakang sosial dan pendidikan yang beragam. Sebagian besar peserta belum pernah memperoleh edukasi terstruktur mengenai asuhan holistik sebelumnya. Intervensi dilakukan melalui penyuluhan kesehatan, diskusi interaktif, serta pemberian media edukasi berupa leaflet dan materi presentasi. Pelaksanaan program menggunakan pendekatan manajemen POAC (Planning, Organizing, Actuating, dan Controlling). Evaluasi dilakukan menggunakan kuesioner pretest dan posttest yang mencakup empat aspek pengetahuan. Terjadi peningkatan pada seluruh aspek yang dinilai. Pemahaman tentang GGK stadium akhir meningkat dari 80% menjadi 92%. Pemahaman perawatan paliatif meningkat dari 82% menjadi 94%, perawatan jangka panjang dari 84% menjadi 93%, dan strategi pengelolaan kecemasan dari 85% menjadi 95%. Rata-rata skor keseluruhan meningkat dari 83% pada pretest menjadi 93% pada posttest. Program edukasi integrasi perawatan paliatif dan perawatan jangka panjang efektif dalam meningkatkan pengetahuan pasien dan keluarga mengenai asuhan holistik GGK stadium akhir. Program ini berpotensi memperkuat kesiapan pasien dan keluarga dalam menghadapi kondisi kronis secara berkelanjutan.

Abstract

Introduction: Maintaining the functional capacity of older adults is essential to support independence, particularly in rural areas. Understanding the factors contributing to the reduced ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) is essential for developing effective gerontological nursing strategies. This study identified the predictors of ADL and IADL impairment among older adults residing in rural communities. Methods: This cross-sectional study involved 318 older adults selected through simple random sampling. The Barthel Index and Lawton IADL Scale were used to measure ADL and IADL abilities. The Geriatric Depression Scale was used to assess depressive symptoms, whereas difficulties in vision, hearing, and communication were evaluated using the Washington Group Short Set. Chi-square tests and logistic regression were used to examine the associations and predictors. Results: ADL and IADL disabilities were identified in 4.4% and 10.1% of the respondents, respectively. Communication difficulties were a strong predictor of both ADL and IADL disabilities (AOR = 13.72; 95% CI = 3.26–57.76, p < 0.001 and AOR = 7.07; 95% CI = 2.34–21.34, p < 0.001). Depressive symptoms increased the risk of ADL disability (AOR = 4.17; 95% CI = 1.24–14.05, p = 0.021), and adults aged ≥ 75 years showed higher odds of IADL impairment (AOR = 4.39; 95% CI = 1.89–10.16, p < 0.001). Conclusions: Challenges in communication and depressive affect functional capacity among rural older adults. Maintaining independence and improving daily functioning by implementing early identification and intervention targeting communication and mental health are essential.

Abstract

Background: Diabetes mellitus is a metabolic disease that is a global problem, with the number of sufferers increasing annually. Adherence to the five pillars of diabetes management is the cornerstone of preventing and reducing morbidity and mortality from diabetes. Purpose: To analyze how to improve adherence in type 2 diabetes patients in implementing the five pillars of diabetes management. Method: The research method used a literature review approach to map and identify interventions for implementing the 5 pillars of diabetes management. Results: The study found 20 articles describing interventions for people with diabetes mellitus through educational and informational delivery mechanisms. The types of interventions included: mobile phone application interventions, physical exercise interventions, audiovisual interventions, flipchart interventions, and peer leader interventions. Conclusion: Five types of interventions improve adherence in patients with type II diabetes mellitus to implementing the 5 pillars of diabetes management.

Abstract

Electrocardiography (ECG) interpretation is an essential competency for healthcare providers, particularly nurses, in managing cardiac emergencies during Basic Trauma and Cardiac Life Support (BT&CLS). However, ECG learning in BT&CLS training is still predominantly delivered through conventional, paper-based methods, which may limit learners’ ability to rapidly and accurately interpret cardiac rhythms. Advances in web-based learning offer opportunities to enhance flexibility, interactivity, and learner engagement in emergency care education. This study aimed to develop and evaluate a web-based ECG learning application for BT&CLS training, focusing on its usability and technology acceptance. This study employed a mixed-method Research and Development (R&D) design combined with a quantitative approach. The web-based application was designed to include ECG theory modules, rhythm interpretation exercises, interactive quizzes with immediate feedback, and progress tracking features. Usability was assessed using an adapted System Usability Scale (SUS), while technology acceptance was measured using a modified Technology Acceptance Model (TAM) questionnaire. A total of 31 participants involved in BT&CLS training were recruited. Instrument reliability testing demonstrated excellent internal consistency. The results indicated high levels of user agreement across all usability dimensions, including functionality, reliability, ease of use, and portability of the application. Furthermore, all TAM components perceived usefulness, perceived ease of use, user attitude, behavioral intention, and user trust were rated in the high category. Participants reported that the application was intuitive, supportive of self-paced learning, and relevant to the competencies required in BT&CLS training. Overall usability and technology acceptance findings indicate that the web-based application effectively addresses key limitations of traditional paper-based ECG learning by enhancing flexibility, interactivity, and learner engagement in BT&CLS training. These findings suggest that digital learning tools can support self-paced practice and reinforce ECG interpretation competencies required in emergency care contexts. The developed web-based ECG learning application demonstrated strong usability and high technology acceptance among BT&CLS trainees. The application shows substantial potential to be integrated into BT&CLS training programs as a complementary learning tool, supporting flexible, interactive, and learner-centered acquisition of ECG interpretation skills. Further studies are recommended to evaluate its effectiveness in improving learning outcomes and clinical performance in emergency settings.

Abstract

Stroke is a chronic condition that significantly reduces the patient's quality of life and increases the need for support from family caregivers. This situation places the family as a crucial element in patient care, yet often presents significant challenges for family caregivers. This study aims to identify and describe the various challenges faced by families caring for stroke patients in the community. A literature review design was employed. Relevant articles published between 2020 and 2025 were retrieved from international databases including PubMed and ScienceDirect, using the keywords "family challenges" OR "family burden" AND "stroke" OR "stroke survivor." The inclusion criteria were determined using the PEO framework. A total of 346 articles were identified, and fifteen articles met the inclusion criteria for the review. The findings indicate that family caregivers of stroke patients face various challenges due to the complex demands of care, ranging from emotional stress at the time of diagnosis to physical and social challenges during home care. Challenges are further exacerbated when the patient experiences dysphagia, severe disability, or psychological problems such as depression and anxiety. Factors contributing to these challenges include the family caregiver's employment status, educational level, kinship relationship, social support, knowledge of stroke rehabilitation, the transition process from hospital to home, limited access to healthcare, and limited social activities. Caring for stroke patients presents significant challenges for families, who serve as the primary caregiver system and play a central role in community care. Interventions that support family caregivers through education, psychosocial support, and access to adequate health and rehabilitation services are crucial for community nurses to reduce caregiver burden and improve the quality of life for both family caregivers and stroke patients in the community.

Abstract

Latar Belakang: Kesiapsiagaan masyarakat dalam mengikuti jalur evakuasi menjadi faktor penting dalam upaya pengurangan risiko bencana. Tingkat pengetahuan masyarakat diduga berperan dalam membentuk sikap masyarakat saat menghadapi kondisi darurat, khususnya dalam mengikuti jalur evakuasi. Tujuan: Untuk mengetahui hubungan antara tingkat pengetahuan dengan sikap masyarakat dalam mengikuti jalur evakuasi saat terjadi bencana erupsi Gunung Agung di Desa Tianyar. Metode: Penelitian ini menggunakan metode kuantitatif analitik korelatif dengan pendekatan cross-sectional. Penelitian dilaksanakan di Dunus Tunas Sari Kabupaten Karangasem dengan jumlah sampel yaitu 350 responden dengan menggunakan teknik total sampling. Data dikumpulkan menggunakan kuesioner tingkat pengetahuan dan sikap masyarakat. Uji normalitas Kolmogorov-Smirnov menunjukkan data tidak berdistribusi normal sehingga uji statistik yang digunakan yaitu Spearman’s Rho. Hasil: Hasil penelitian menunjukkan bahwa mayoritas responden memiliki tingkat pengetahuan baik sebanyak 299 responden (85,4%) dan sebagian besar memiliki sikap cukup dalam mengikuti jalur evakuasi sebanyak 206 responden (58,9%). Terdapat hubungan yang signifikan antara tingkat pengetahuan dengan sikap masyarakat (p-value < 0,001) dengan koefisien korelasi r =0,346 yang menunjukkan hubungan positif dengan kekuatan hubungan sedang. Kesimpulan: Terdapat hubungan yang signifikan antara tingkat pengetahuan dengan sikap masyarakat dalam mengikuti jalur evakuasi saat terjadi bencana erupsi Gunung Agung di Desa Tianyar.

Abstract

Enhancing family support in community-based palliative care is a growing global priority given the family's role as the primary caregiver for long-term palliative care patients in the community. This is a systematic literature review that aimed to synthesize recent evidence on the roles and contributions of families in caring for palliative patients in the community. A literature search was conducted through PubMed and ScienceDirect databases using the keywords “family role” OR “family support” AND “palliative” OR “palliative care.” The PEO framework guided the selection process, focusing on Population: families of palliative patients; Exposure: family caregiving roles; and Outcome: fulfillment of palliative care needs. Nine articles published between 2020 and 2025 met the inclusion criteria. The review identified diverse and interrelated family roles, including providing emotional and social support, delivering direct care, coordinating with healthcare professionals, engaging in decision-making and communication, and facilitating health education. Families also carried significant social and cultural responsibilities while adopting protective and adaptive coping strategies. The findings highlight that families are not merely companions but act as care managers, communicators, and educators who critically influence the quality of home-based palliative care. A unique insight from this review is the emphasis on the transitional phase from hospital to home as a crucial period requiring enhanced family capacity, structured coordination, and psychosocial support. Strengthening family education, telemedicine-based coordination, and community linkages emerges as a vital strategy to ensure holistic and sustainable palliative care delivery within community settings. Community nurses need to involve and meet the needs of families as clients in a nursing care plan that does not only focus on the needs of palliative patients

Abstract

Latar Belakang: Perawatan paliatif merupakan perawatan yang diberikan secara menyeluruh kepada pasien maupun keluarga pasien yang menderita penyakit mengancam jiwa. Permasalahan yang dijumpai adalah pengetahuan dan sikap mahasiswa keperawatan tentang perawatan paliatif ditemukan masih kurang. Pengetahuan dan sikap mahasiswa keperawatan yang kurang dikhawatirkan akan berdampak pada ketidakmampuan melakukan perawatan paliatif. Tujuan: Untuk mengetahui pengetahuan dan sikap mahasiswa keperawatan tentang perawatan paliatif pada anak dengan kanker dan non kanker di Denpasar. Metedologi: Desain comparative study dengan metode cross-sectional. Populasi pada penelitian ini adalah mahasiswa program studi keperawatan tingkat IV dengan jumlah sampel 238 responden yang diambil dengan teknik total sampling. Alat pengumpulan data yang digunakan adalah kuesioner yang disebarkan secara online dalam bentuk google formulir. Analisa data yang digunakan yaitu analisa univariat dan analisa uji Wilcoxon Signed Ranks Test. Hasil: Dari 238 responden, sebanyak 50,4% dari total responden memiliki pengetahuan kurang terkait perawatan paliatif pada anak kanker dan sebanyak 63,4% memiliki pengetahuan kurang terkait perawatan paliatif pada anak non kanker. Sikap mahasiswa keperawatan sebanyak 86,6% dari total responden memiliki sikap cukup terkait perawatan paliatif pada anak kanker dan sebanyak 87,4% memiliki sikap cukup terkait perawatan paliatif pada anak non kanker. Hasil uji Wilcoxon Signed Ranks Test, didapatkan terdapat perbedaan antara pengetahuan kanker dengan non kanker serta sikap kanker dengan non kanker (p-value < 0,05). Kesimpulan: Responden mayoritas memiliki pengetahuan kurang dan sikap yang cukup tentang perawatan paliatif pada anak dengan kanker dan non kanker. Terdapat perbedaaan pengetahuan dan sikap terkait perawatan paliatif pada anak dengan kanker dan non kanker.
