Abstract

Recent research has demonstrated that mesenchymal stem cells (MSCs) potentially benefit and enhance coronavirus disease (COVID-19) recovery. This benefit occurs via a mechanism that promotes viral clearance by phagocytes and macrophages. This action occurs through the innate (increase in IL-10 production and decrease in TNF-α and IL-12 production) and the adaptive immune system (decrease in IL-17 production, promote regulatory T cell proliferation and inhibit effectors T cell proliferation). MSCs are expected to act as an anti-inflammatory in the hyper-inflammatory state of COVID-19. MSCs enhance immune cell replacement that have been overwhelmed or have been lost due to cytokine storm. Although vaccines are the answer to this pandemic, MSCs can improve COVID-19 patients, especially in patients with chronic illnesses. The focus on keeping death-rates low is a great opportunity for MSCs-based therapy for severe or critically ill patients. MSCs and conditioned medium have the potential to serve as adjunctive therapy in preventing the body's overactive defense response or the so-called cytokine storm caused by COVID-19.Keywords: adjuvant therapy, COVID-19, mesenchymal stem cells, secretome

Abstract

The spread of coronavirus during the Covid-19 pandemic has resulted in an increased level of anxiety among pregnant women. Decrease in the level of anxiety in pregnant women can be achieved through non-pharmacological therapy, one of which is special massage. This study aims to analyze the difference in the effect of special massage on the anxiety level between primigravida pregnant women in the third-trimester and pregnant women without special massage in coping with childbirth during the Covid-19 pandemic. This was a quasi-experimental study with a Non-equivalent Control Group Design approach. The samples in this study were 80 pregnant women in the third-trimester. The sample size was determined using the formula for unpaired categorical analytical study to test the hypothesis between the two proportions. The test results showed a comparison of anxiety scores (pre and post) in the two study groups, with a decrease in the HARS score in the special massage group by 43.7percent. Meanwhile, the decrease in the HARS score in the group without special massage was -9.1percent. The statistical test results with the Mann-Whitney test obtained a p-value less than equals 0.001 (p less than 0.05) which indicated a significant difference. It can be concluded that special massage had a significant effect on a decrease in the HARS Score of primigravida pregnant women in the third-trimester pregnant women in coping with childbirth during the Covid-19 pandemic.

Abstract

Anaplastic meningioma is an uncommon neoplasm in childhood and adolescence. In contrast to the case in adults, pediatrics have a predominance of males over females. We report a case of a 17 years old with progressive scalp swelling on the right side. Patient underwent subtotal tumor resection at previous hospital three months ago. The operation was stopped due to extensive bleeding. Radiological investigations revealed a dura-based tumor with a cystic component. Following preoperative embolization, a craniotomy was done, and the tumor was almost completely removed. The tumor's histopathological analysis revealed characteristics of an anaplastic meningioma. The aggressive behavior of this tumor necessitates meticulous monitoring of therapeutic measures in such patients.

Abstract

Hyperinflammation in COVID-19 patients is one of the causes of the high mortality rate of COVID-19. An in vitro model mimicking the inflammatory responses in COVID-19 patients is important in the efforts of finding new drug candidates for this disease. Lipopolysaccharide (LPS) can increase the proinflammatory cytokine interleukin 8 in response to the presence of foreign substances. This preliminary study sought to explore the use of  the A549 cells as an in vitro inflammatory model. This study was conducted from August to November 2022 at the stem cell research and development laboratory of Bio Farma Indonesia. The exposure of 100, 500, and 1000 g/mL doses of LPS administered for 24, 72, and 120 hours on the A549 cells was analyzed for cell viability, population doubling time (PDT), and the presence of  proinflammatory cytokine IL-8. The group differences were examined using one- and two-way analysis of variance in IBM SPSS Statistics Version 29, with a p-value of 0.05 considered significant. Cells exposed to a dose of 1000 g/mL LPS had a lower viability and a higher proliferation rate (p<0.05) based on the viability and PDT. Viability, PDT, and pro-inflammatory cytokines showed concentration- and time-dependent responses. Therefore, increased levels of the proinflammatory cytokine IL-8 in cells exposed to LPS at a dose of 1000 g/mL for 24 hours can be used as a mimic to study hyperinflammation in COVID-19 patients.

Abstract

Tujuan: Untuk menilai nekrosis tumor, penyengatan zat kontras, dan edema peritumoral pada MRI pre-operasi sebagai prediktor rerata waktu kesintasan pasien glioma derajat rendah dan tinggi. Metode: Penelitian ini menggunakan desain potong lintang retrospektif yang melibatkan 26 pasien glioma derajat rendah (n=11) dan derajat tinggi (n=15) berdasarkan hasil histopatologi pasca operasi. Gambaran radiologi MRI pre-operatif dinilai menggunakan parameter nekrosis, penyengatan zat kontras dan edema peritumoral. Selain itu juga dihitung waktu kesintasan pasien pasca operasi untuk kemudian dianalisis secara statistik angka rerata kesintasan menggunakan analisis Kaplan-Meier. Hasil: Gambaran nekrosis tumor pada MRI pre-operatif memiliki pengaruh yang secara statistik bermakna terhadap rerata waktu kesintasan pasien glioma (p=0,021). Selain itu Karnofsky performance score (KPS) pasca operasi (p=0,050) dan lokasi tumor (p=0,036) juga berpengaruh pada rerata waktu kesintasan pasien glioma. Pasien glioma dalam penelitian ini memiliki rerata waktu kesintasan yang lebih pendek jika memiliki gambran nekrosis derajat II pada MRI pre operatif (14 bulan); rerata waktu kesintasan lebih panjang jika lokasi tumor di lobus oksipital (38 bulan) dan KPS pasca operasi ?70 (28,67 bulan). Kesimpulan: Pemeriksaan radiologis pre-operatif dapat berkontribusi dalam penentuan prognosis pasien glioma. Meski demikian derajat histopatologis dan pemeriksaan molekuler tetap berperan penting dalam penentuan pilihan terapi lanjutan dan prognostik pasien glioma.

Abstract

BACKGROUND: Stroke is one of many leading causes of death and disability worldwide. Despite decades of research, treatment for stroke recovery is still inadequate. Recently, mesenchymal stem cell (MSC) therapy is considered as one of the potential treatments for stroke due to their capabilities to repair or regenerate damaged tissues, as well as immunomodulators in inflammatory conditions. Many pre-clinical and clinical trials have shown MSC therapy feasibility, safety, and efficacy in treating stroke. However, evidence regarding the optimal treatment plan and factors that can improve stem cell functional outcomes in treating stroke needs to be further explored. Therefore, a systematic review was conducted.METHODS: Recent literatures from 2015-2022 regarding stem cell therapies, specifically MSC on ischemic stroke, were collected from two reliable databases: PubMed and PubMed Central. Collected literatures were properly applied to inclusion-exclusion criteria and appraised critically. Keyword strategies on databases were employed, including both medical subject headings (MeSH).RESULTS: Five literatures from 726 were identified and used for systematic review. All animal model in the literatures were prepared to have middle cerebral artery occlusion. All studies indicated that MSC therapy is a safe and reliable procedure despite the variety of transplantation routes. No report of toxicity, rejection reaction, nor infection on MSC treated groups.CONCLUSION: Stem cell sources, dosages, and delivery routes could be resourceful for future translational studies to ensure the safety and efficacy of MSC therapy for ischemic stroke.KEYWORDS: ischemic stroke, mesenchymal stem cells, regenerative medicine, immunomodulator

Abstract

Background: The amniotic membrane (AM), a versatile biomaterial with inherent stem cells and extracellular matrix, has shown promise in various tissue engineering applications. Its potential as a dural substitute, particularly in addressing dural defects and preventing cerebrospinal fluid (CSF) leakage, has garnered increasing interest. However, a comprehensive understanding of the mechanical properties of non-human primate (NHP) AM, especially in relation to human AM, remains elusive. This knowledge gap hinders the optimal utilization of NHP models, such as Macaca fascicularis, in translational research for dural repair. This study aimed to characterize the mechanical properties of fresh Macaca fascicularis AM and dura mater and to investigate the influence of fetal gender, gestational age, and parity on AM mechanics. Methods: Sixteen fresh preparation amniotic membranes of Macaca fascicularis were obtained at the elective caesarean section that was already free of several infections, and three fresh preparation of dura mater of the same species were studied. The membranes were cut in specific sizes and then loaded at the Flavigraph (Textechno, Herbert Stein GmbH & Co.KG, Moenchengladbach, Germany) machine. The Young’s modulus, ultimate tensile strength, elongation at break, maximum elongation, and toughness of the amniotic membrane and dura mater were recorded and compared based on the fetal gender, gestational ages, and frequency of pregnancy. Results: This is the first report of mechanical properties of Macaca fascicularis amniotic membrane and dura mater. There are no statistically significant differences in mechanical properties of the amniotic membrane between the fetal gender, gestational age, and the frequency of pregnancy in fresh preparation amniotic membrane. The elasticity of the dura mater is seven times stiffer than the AM and the tensile strength of the dura mater is three times bigger than the AM, and the dura mater toughness is eight times bigger than the amniotic membrane. Conclusion: Our findings have shown the mechanical properties of Mf AM are not dependent on factors of fetal gender, gestational age, and frequency of pregnancy. This work provides an explanation of the physical properties of fresh preparation AM as the consideration to be used as allograft biomaterial in the dura mater substitution procedure.

Abstract

Introduction : Cerebral tuberculoma, a seldom encountered and severe manifestation of tuberculosis (TB), arises from the dissemination of Mycobacterium tuberculosis through the bloodstream. Its symptoms and radiological characteristics lack specificity, often resulting in diagnostic errors. Management predominantly involves medical intervention, with the treatment duration for cerebral tuberculoma ranging from 6 to 36 months. In specific instances, surgical intervention may be advised. Case presentation: We reported two cases of cerebral tuberculoma occurring in patients who presented with seizures, with space-occupying lesions evident on magnetic resonance imaging of the brain. There were no symptoms of concurrent extra cranial TB. Surgery was performed on both of the cases and anti-TB treatment began as soon as the diagnosis was made with corticosteroid as adjuvant treatment. Conclusion: : A combination of clinical, radiological, and histopathological examination is needed to confirm the diagnosis and determine the appropriate therapy. If ICP is increased as a result of the lesion and medical therapy has failed, surgical excision is required.

Abstract

Dendritic cell (DC) vaccines, as immunotherapy agents, can gather up and transport cancer-related antigens to T lymphocytes, activating anti-tumor effector responses. After being activated by DC, cytotoxic T lymphocyte cells (CTL) will secrete the cytolytic granzyme B that can effectively induce rapid apoptosis of target cells. On the other hand, DC also secrete several cytokines and a large number of exosomes, which together operate as a whole antigen-presenting entity. The efficacy of the vaccine’s treatment may be affected by the sources used for DC vaccines. Umbilical cord blood (UCB) from healthy donors can be employed when autologous cancer patient’s peripheral blood (PB) cannot be used as a source for isolating DC due to genetic abnormalities. Comparing UCB to other sources, there is a painless method of collecting sources as opposed to PB, which necessitates a venipuncture or leukapheresis procedure to isolate the blood. Many studies related to the use of PB-DC have been carried out, but research on potential comparisons between PB-DC and UCB-DC is still very limited. In this review, the potential of PB- and UCB-derived DC and their secretomes for cancer will be discussed.Keywords: dendritic cells, vaccines, umbilical cord blood, peripheral blood

Abstract

Mesenchymal Stem Cell (MSC) secretome has potential as a neuroprotective and neuroregenerative agent. It can have effects due to its paracrine factors, such as Brain Derived Neurotrophic Factor (BDNF) and Stromal-Cell Derived Factor-1 (SDF-1) which can be induced with hypoxia preconditioning. This compound may play a role in the treatment of neurological diseases. Stroke has become a neurological disease that contributes to high rates of mortality and morbidity worldwide. There have been several pre-clinical trials on animal stroke models using MSC secretomes from rats and humans, but no studies have been conducted on Non-Human Primate, such as Macaca fascicularis. This species has been widely used in biomedical research and part of it can be utilized for such studies which will reduce the cost of using human MSC. The results of this study, Umbilical Cord (UC)-MSCs of Macaca fascicularis have been successfully cultured and characterized in terms of phenotypic and differentiation. Hypoxia precondition was able to induce BDNF secretion up to 264 pg/mL and SDF-1 up to 666 pg/mL in the UCMSC secretome. Hypoxic preconditioning with 3% oxygen can induce the most optimal BDNF and SDF-1 secretion, compared to 1% and 5% hypoxia.
