Abstract

Shock is a life-threatening condition. It is essential to perform hemodynamic support on shocked patients to restore adequate circulation. Passive leg raising (PLR) is a critical act to assess fluid responsiveness which can provide significant information about fluid needs of shocked patients. The present study aimed to identify the implementation of passive leg raising in shocked patients. The scoping review was the method used in the present study. Relevant literatures were obtained from PubMed, CINAHL and Google Scholar Search engines from 2013 to 2023. The articles were manually extracted through tabulation and the data were thematically analyzed with an exploratory descriptive approach. Nine articles were found relevant and hence were included in the present study. The findings of the 9 articles revealed that the implementation of passive leg raising in shocked patients was an early strategy in assessing fluid responsiveness and fluid administration could be decreased after 48 hours of ICU admission. The findings of the current study suggested that the implementation of PLR significantly assessed fluid responsiveness which in the end guided in conducting fluid resuscitation in shocked patients and could reduce fluid administration in shocked patients for the first 48 hours of ICU admission. Therefore, health professionals working in critical care units including critical care nurses should consider performing PLR to shocked patients.

Abstract

This study investigates the impact of using warmed irrigation fluid over room-temperature fluid on patient body temperature during surgery, while also exploring the variables that influence its effectiveness. Systematic review following PRISMA guidelines. Comprehensive searches were conducted in PubMed/MEDLINE, Embase, Cochrane Central, and Web of Science (inception to March 2024) using PICO framework-derived search terms. Inclusion criteria: randomized controlled trials (RCTs) and systematic reviews comparing warmed irrigation fluids (≥36°C) versus room temperature in adult surgical patients. Quality assessment used Joanna Briggs Institute (JBI) critical appraisal tools. Data synthesis included descriptive analysis and meta-analysis where appropriate. Ten studies (eight randomized controlled trials and two systematic reviews/meta-analyses) met inclusion criteria, comprising 2,459 participants across various surgical procedures. Overall methodological quality was high (mean JBI score 10.2/11, 93%). Effectiveness analysis revealed that 67% of individual studies (6/9) demonstrated significant benefits, with clear procedure-specific patterns. Arthroscopic shoulder surgery showed the highest success rate (75%, 3/4 studies) with effect sizes of 67–80% reduction in hypothermia incidence when using 37°C irrigation. TURP procedures demonstrated conditional effectiveness (50% success rate) depending on anesthesia type, while laparoscopic surgery showed no benefit (0% success rate) due to competing heat loss mechanisms. Warmed irrigation fluids at 37°C demonstrate procedure-specific effectiveness, with arthroscopic shoulder surgery showing highest success rates (75%) and laparoscopic surgery showing no benefit.

Abstract

This study aims to determine the effectiveness of diabetic foot exercises on DPN values ​​in DMT-2 patients. The method used is a quasi-experiment with Pretest-Postest with a control group design. The study results showed that in the intervention group using Wilcoxon, 0.000 was obtained, and 0.001 in the control group. Still, if you look at the clinical results of a difference of less than 10, there is no significant difference in the degree of DPN before and after the intervention. The results of the study with the Mann-Whitney test showed that diabetic foot exercises are effective in reducing the degree of DPN (p = <0.05), where the intervention group had a lower degree of DPN (12.24) compared to the control group (30.76). There is a difference in the degree of DPN between the intervention and control groups, with a p = 0.001. In conclusion, foot exercises effectively reduce the degree of diabetic peripheral neuropathy in patients with type 2 diabetes mellitus at Dustira Hospital Cimahi. Abstract

Background: Ventilator-associated pneumonia (VAP) remains a leading cause of morbidity and mortality among patients in intensive care units (ICUs). Nurses play a central role in preventing VAP through consistent implementation of evidence-based prevention bundles. Although VAP bundles are widely recommended, suboptimal adherence among nurses continues to contribute to high VAP incidence rates, highlighting the need for strategies that improve compliance. Objective: This scoping review aimed to identify and synthesize interventions designed to enhance nurse compliance with VAP bundle implementation in ICU settings. Methods: A scoping review was conducted using the Population–Concept–Context (PCC) framework, focusing on nurses, interventions to improve VAP bundle compliance, and ICU settings. Electronic databases—including PubMed, Cochrane Library, ProQuest, EBSCOhost, ScienceDirect, ClinicalKey, SAGE Journals, and Google Scholar—were searched for full-text articles published in English between 2016 and 2021. Study selection followed the PRISMA framework. Data were extracted, analyzed, and synthesized narratively. Results: From 945 identified records, eight studies met the inclusion criteria. The reviewed literature identified educational and training programs, experiential learning strategies, interprofessional collaboration, and real-time compliance monitoring dashboards as effective interventions to improve nurse adherence to VAP bundles. Conclusion: Multifaceted interventions, particularly continuous education supported by supervision and monitoring, are effective in enhancing nurse compliance with VAP bundle implementation. Sustained organizational support is essential to improve adherence and reduce VAP incidence in ICU settings.

Abstract

Background: The incidence of cardiac arrest or Sudden Cardiac Arrest (SCA) in the community that cannot be saved increases every year. This is because they do not get the right and fast action due to the community's ignorance and inability to assist. Objective: This study aimed to evaluate health education on increasing knowledge and readiness of respondents in helping cardiac arrest victims. Methods: This study used a pre-experimental design with a one-group pre-post test approach to 52 respondents, namely the general public. The research sample was recruited using convenience sampling techniques. The sample was given health education on basic life support (BLS) and cardiac arrest management directly with several interactive sessions using visual aids such as posters and presentation slides. Data collection used a valid and reliable questionnaire (Knowledge and Readiness to Help). Data were analyzed univariately through frequency distribution, central tendency, mean difference, and bivariate tests using paired sample t-tests. Results: There was an increase in respondents' BLS knowledge from 6.87 to 11 (p<0.001) and readiness to help, with an average score increasing from 60.4 to 63.3 (p=0.002). These results indicate that health education can increase knowledge about BLS and readiness to help respondents. Research results show that readiness to act in emergencies depends on more than theoretical understanding; it also requires practical training, self-confidence, and direct experience. Conclusion: Health education significantly improved public knowledge and readiness to provide first aid for cardiac arrest victims. These findings indicate that structured Basic Life Support (BLS) education is effective in strengthening community preparedness and timely emergency response. Integrating regular and practical BLS training into community health programs may further enhance public capacity to respond to cardiac arrest events.

Abstract

ABSTRACT Natural disasters and health crises have experienced a significant increase in the last decade. Nurses have a role in disaster mitigation and response, including providing holistic care to victims in every phase of response. Previous studies reported that nurses are often faced with various situations and challenges in every phase of emergency response such as limited resources and low preparedness. Previous studies have focused more on evaluating disaster preparedness training programs or the effectiveness of standard protocol-based nursing interventions, but have not sufficiently explored how nurses' direct experiences shape their perceptions of preparedness and the challenges they face. This study aims to explore nurses' perceptions of preparedness and challenges in caring for disaster survivors. This study method uses a scoping review approach with the Arksey O'Malley methodology framework. Literature searches were conducted on the Scopus, PubMed, EBSCOhost, and ScienceDirect databases for the journal period 2015 - 2025 resulting in 791 articles. In the process of screening and selecting articles, a systematic approach followed the PRISMA protocol. The process of selecting studies and reporting results followed the PRISMA-ScR guidelines. Of the 791 articles found, 10 final articles met all inclusion criteria. This study identified three nurses' perceptions of preparedness and challenges in caring for disaster survivors, namely perceptions of lack of preparedness, perceptions of limited resources and coordination, and perceptions of emotional distress during the disaster response process. Conclusions: Nurses face psychosocial and technical barriers when caring for disaster survivors. Resource support plays a role in supporting nurses' preparedness in the field. These results emphasize the need for increased disaster training programs and policy development to strengthen nurses' preparedness in dealing with crisis situations. Keywords: Nurse Preparedness, Disaster Survivors, Nurse Perception, Disaster Response, Nursing Challenges.  ABSTRAK Bencana alam dan krisis kesehatan mengalami proses peningkatan yang signifikan dalam dekade terakhir, Perawat mempunyai peran dalam mitigasi dan respon bencana, termasuk memberikan perawatan holistik pada korban dalam setiap fase penanggulangan. Studi terdahulu melaporkan bahwa perawat sering dihadapi berbagai situasi dan tantangan dalam setiap fase tanggap darurat seperti keterbatasan sumberdaya dan kesiapsiagaan yang rendah. Penelitian terdahulu lebih menitikberatkan pada evaluasi program pelatihan kesiapsiagaan bencana atau efektivitas intervensi keperawatan berbasis protokol standar, namun belum cukup menggali bagaimana pengalaman langsung perawat membentuk persepsi mereka terhadap kesiapsiagaan dan tantangan yang dihadapi. Penelitian ini bertujuan untuk mengeksplorasi persepsi perawat terhadap kesiapsiagaan dan tantangan dalam merawat penyintas bencana. Metode kajian ini menggunakan pendekatan scoping review dengan kerangka metodologi Arksey O’Malley. Pencarian literatur dilakukan pada basis data Scopus, PubMed, EBSCOhost, dan ScienceDirect untuk tahun jurnal periode 2015 - 2025 menghasilkan 791 artikel. Dalam proses screening dan seleksi artikel, pendekatan sistematis mengikuti protokol PRISMA.  Proses seleksi studi dan pelaporan hasil mengikuti pedoman PRISMA-ScR. Dari 791 artikel yang ditemukan, ada 10 artikel final yang memenuhi semua kriteria inklusi. Penelitian ini mengidentifikasi tiga persepsi perawat terhadap kesiapsiagaan dan tantangan dalam merawat penyintas bencana yaitu ppersepsi tentang kurangnya kesiapsiagaan, persepsi tentang keterbatasan sumber daya dan koordinasi, serta persepsi tentang tekanan emosional selama proses tanggap bencana. Perawat menghadapi hambatan dari aspek psikososial dan teknis saat merawat penyintas bencana. Dukungan sumber daya berperan dalam mendukung kesiapsiagaan perawat di lapangan. Hasil ini menegaskan perlu adanya peningkatan program pelatihan bencana dan pengembangan kebijakan guna memperkuat kesiapan perawat dalam menghadapi situasi krisis. Kata Kunci: Kesiapsiagaan Perawat, Penyintas Bencana, Persepsi Perawat, Tanggap Bencana, Tantangan Keperawatan.

Abstract

Cardiovascular disease remains the leading cause of global morbidity and mortality. Phase III cardiac rehabilitation plays a crucial role in long-term maintenance and secondary prevention; however, patient adherence and program sustainability remain suboptimal due to limited access, geographical barriers, and resource constraints. Phase III cardiac telerehabilitation has been developed as a digital-based alternative to support patient engagement and continuity of rehabilitation. This study aimed to analyze the effectiveness of phase III cardiac telerehabilitation on functional capacity, adherence, rehospitalization, and quality of life compared with conventional cardiac rehabilitation or standard care. This study was a systematic review and meta-analysis of 15 randomized controlled trials published between 2015 and 2025. The study population included adult patients with cardiovascular disease who had completed phase II cardiac rehabilitation and participated in phase III cardiac telerehabilitation programs. Outcomes assessed included functional capacity (VO₂peak, Six-Minute Walk Test, and metabolic equivalents), program adherence, rehospitalization, and quality of life. Data were analyzed using Review Manager (RevMan) version 5.3 with fixed-effect or random-effect models according to heterogeneity levels. The meta-analysis demonstrated that phase III cardiac telerehabilitation significantly improved functional capacity measured by VO₂peak with low heterogeneity (I² = 0%), as well as quality of life (SMD = 0.16; 95% CI 0.04–0.28; p = 0.01) and patient adherence to rehabilitation programs. However, no significant differences were observed in the Six-Minute Walk Test, metabolic equivalents, or rehospitalization compared with control groups. Based on these findings, phase III cardiac telerehabilitation is recommended as an effective and safe strategy to support the long-term sustainability of cardiac rehabilitation and improve patients’ quality of life.

Abstract

Postoperative hypothermia remains a common and clinically significant complication among patients undergoing surgery with general anesthesia. This study aimed to identify determinants associated with postoperative hypothermia in patients receiving general anesthesia at RSUD Majalengka, Indonesia. A quantitative analytic observational design with a cross-sectional approach was applied to 91 postoperative patients selected using total sampling. Data were collected from medical records and direct observation, including age, body mass index (BMI), duration of surgery, and postoperative body temperature. Statistical analyses were conducted using univariate, bivariate (Chi-square), and multivariate logistic regression tests. Results showed that 48.4% of patients experienced postoperative hypothermia. Significant associations were found between hypothermia and BMI (p=0.034), duration of surgery (p=0.002), and age (p=0.009). Multivariate analysis revealed that duration of surgery was the most dominant predictor (OR=3.922; p=0.005), indicating patients undergoing longer procedures had nearly four times higher risk of hypothermia. These findings highlight the importance of intraoperative temperature management, particularly in patients undergoing prolonged surgical procedures.

Abstract

Introduction: The Blue Code system is one of the emergency procedure codes that should be activated immediately if a person is found and the condition of cardiac respiratory arrest. Success in the treatment of cardiac arrest depends on SDM, means and facilities that are supported by the code blue system.Purpose: The research is aimed at identifying barriers to the implementation of the Code Blue system in hospitals. The researchMethod: used is scoping review in documented using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), as well as PRISMA flow diagrams for detailed quantity of literature identified.Result: The research finds that there are still limitations in the implementation of the code blue system, which is an obstacle to the handling of code blue in hospitals.Conclusion: Based on the scopeing review research carried out, found obstacles to the implementation of the code blue system, including SDM both lack of SDM and which also contains team improvement in performing resuscitation starting from identification of patients with pulmonary heart failure, activation of code blue systems, response code blue team, implementation by the team, evaluation and documentation, means and insufficient prasarana, which in the end correlates in response team code blue in dealing with cardiac arrest.

Abstract

The use of mobile applications in child protection has increased in recent years, including their potential role in preventing child sexual abuse (CSA). However, the extent and nature of evidence supporting mobile application–based interventions for CSA prevention remain unclear. This scoping review aimed to identify, synthesize, and map existing evidence on the development and use of mobile applications for the prevention of child sexual abuse. Following the Arksey and O’Malley framework, a scoping review was conducted using systematic searches in ScienceDirect, CINAHL (EBSCO), and Scopus. Search terms included variations of mobile applications (including mobile and smartphone apps), prevention, and child sexual abuse. Eligible studies were quantitative intervention or mixed-methods studies that evaluated mobile application–based interventions targeting CSA prevention, were available in full text, published in English, and released between 2017 and 2024. Data were extracted manually and synthesized descriptively using thematic analysis. From 3,896 records, four studies met the inclusion criteria. Thematic analysis identified four key themes: (1) improved children’s awareness and protective skills; (2) promotion of self-protective behaviours; (3) enhanced parental knowledge; and (4) usability and accessibility of mobile applications. The available evidence on mobile applications for CSA prevention remains limited and heterogeneous. While existing studies suggest that mobile applications may support awareness-building and short-term preventive outcomes, the current evidence is insufficient to establish effectiveness. Further theory-informed and methodologically rigorous research is needed to evaluate sustained behavioural change, long-term outcomes, and equitable implementation across diverse contexts.
